Disposition of cervical vertebrae, atlanto-axial joint, hyoid and mandible during x-ray laryngoscopy.

Ten healthy volunteers underwent direct laryngoscopy using topical anaesthesia and a curved Macintosh laryngoscope blade. A lateral x-ray was performed during laryngoscopy in a standard intubating position. In this position the lower neck was relatively straight and increasing curvature occurred from the mid cervical spine upwards. Extension at the atlanto-axial joint was probably near maximum; this has implications for conditions associated with laxity of this joint. The position of the mandible was influenced by the direction of pull on the laryngoscope handle. The hyoid was drawn forward an

Congenital deformation of the posterior arch of the atlas: Subluxation of the atlanto-axial joint with temporary quadriplegia.

Instabilities of the craniocervical junction can be of rheumatic, traumatic, or congenital origin. The reported patient has a congenital malformation of the cervical spine, which is frequently observed in patients with Klippel-Feil syndrome. Her posterior arch of the atlas (C1) is hypoplastic and a chronic subluxation of the atlanto-axial joint would be possible. Although most common fusions in Klippel-Feil syndrome patients exist at C2/3, the majority of studies about Klippel-Feil syndrome deal with pediatric or adolescent individuals. Through extreme flexion of her neck, there was a compress

Osteoarthrosis of the atlanto-axial joints. Long-term follow-up after treatment with arthrodesis.

We evaluated the results for fifteen patients in whom symptomatic osteoarthrosis of the atlanto-axial joints had been treated with an atlanto-axial or occipitocervical arthrodesis between 1973 and 1990. Thirteen patients had long-term follow-up (average duration, seven years and two months; range, four years and two months to nineteen years and two months). The two remaining patients had died: one, four days postoperatively, from a cardiopulmonary arrest, and the other, one year and eight months postoperatively, from complications related to bladder cancer. Preoperatively, all fifteen patients

Characteristics of patients with cervical pain originating from the lateral atlanto-axial joint (C1-C2).

BACKGROUND: The lateral C1-C2 joint is an understudied source of upper cervical pain and is typically investigated only after negative work-up at C2-C3 and C3-C4. The clinical profile of patients with presumed C1-C2 pain remains poorly defined, and the relationship between diagnostic block response and medium-term corticosteroid benefit is unclear. OBJECTIVE: To characterize the clinical profile of patients with persistent upper cervical pain after negative C2-C3 and C3-C4 work-up who underwent C1-C2 intra-articular injection, and to identify factors associated with immediate diagnostic block

[Arthritis of the atlanto-axial joint with inflammatory neck pain as a primary manifestation of seronegative rheumatoid arthritis].

HISTORY AND ADMISSION FINDINGS: A 68-year-old woman with known degenerative joint disease suffered from increasing neck pain. Physical examination revealed painfully restricted movement of the cervical spine. INVESTIGATIONS: Erythrocyte sedimentation rate and C-reactive protein were increased. Tests for rheumatoid factors, antinuclear, anti-citrullinated protein and anti-neutrophil cytoplasmic antibody were negative. Cervical spine x-ray showed osteochondrosis with partially bridging spondylosis at C5/C6, but there was no atlanto-axial dislocation. Magnetic resonance imaging (MRI) revealed bo

The lateral atlanto-axial joints and their synovial folds: an in vitro study of soft tissue injuries and fractures.

Thirty victims of blunt cervical trauma, who did not survive (acute death group), were studied at autopsy as were 11 patients who survived trauma for more than 1 month and ten controls without any known cervical trauma. All specimens were sagitally sectioned in 2-mm slices and carefully examined for injuries to the lateral parts of the atlanto-axial joints. In the acute death group, five cases showed a rupture of the joint capsule, three of these with a massive hemarthrosis. In 17 cases there was bruising around the second cervical spinal nerve. Bruising of the intra-articular synovial fold wa

Nontraumatic subluxation of the atlanto-axial joint as rare form of acquired torticollis: diagnosis and clinical features of the Grisel's syndrome.

STUDY DESIGN: Case report and review of the literature. OBJECTIVE: We report a case of Grisel's syndrome with a delayed diagnosis. The patient's first presentation in our pediatric orthopedics department was 2 month after surgery (cochlea implantation) with a persistent torticollis. Radiographs revealed a subluxated atlantoaxial joint. We treated our patient with manual repositioning and calculated antibiotics, which lead to a restitutio ad integrum within a short time. SUMMARY OF BACKGROUND DATA: Grisel's syndrome is synonymous with rare nontraumatic, rotational subluxation of the atlantoax

Magnetic resonance imaging features of bilateral multiloculated extraneural ganglion cysts of the occipito-atlanto-axial joint causing hypoglossal nerve paralysis in a dog.

A 14-year-old spayed female Miniature Pinscher presented with tongue curling, dysphagia, hypersalivation, and sublingual gland swelling. Comprehensive evaluation, including neurologic and musculoskeletal examinations, blood work, and urinalysis, revealed no abnormalities other than tongue-related signs. Magnetic resonance imaging (MRI) revealed a multilobed cystic structure in the occipito-atlanto-axial joint, compressing the right hypoglossal canal. The lesion appeared cerebrospinal fluid (CSF)-like on T1-weighted and T2-weighted images, and hyperintense compared with CSF on fluid-attenuated

[Atlanto-axial rotation and distance in small children. A postmortem study].

The aetiology of atlanto-axial rotatory subluxation is obscure. Therefore, a post mortem investigation was designed in order to evaluate the C 1-2 rotation and translation mobility and to clear the borderline between mobility and instability and to clear the role of the atlanto-axial joints in atlanto-axial rotatory subluxation. C 1-2 specimen of 2 months, 9 months and 53 months old children were used. In the intact specimen and after sequential decision of the atlanto-axial joint capsule, the atlanto-axial membrane, the transverse ligament of the atlas, and eventually the alar ligaments, the

Osteoarthritis of the alanto-axial joint.

Thirty-one patients suffering from osteoarthritis of the atlanto-axial joint are described, suggesting that this disease is not particularly uncommon. The most characteristic radiological signs are narrowing of the joint space from loss of articular cartilage, marginal cortical thickening and osteophyte formation. Histological examination in the cadaver has shown thinning and fibrillation of articular cartilage and intra-cartilaginous ossification. The radiological appearances of the atlanto-axial joint are classified in three types, lateral atlanto-axial, atlanto-odontoid and mixed. Peak age

[Changes in the cervical spine in juvenile rheumatoid arthritis (author's transl)].

Changes in the cervical spine in juvenile rheumatoid arthritis can be shown by A.P., lateral and functional views with maximal flexion and extension, and particularly by tomography. The latter is essential for proper evaluation of the atlanto-occipital and atlanto-axial joints and of the odontoid. Atlanto-axial subluxation was found in eight out of eighty-four patients. Destruction of the odontoid was found in twelve out of fifty-three patients examined by tomography. The earliest tomographic changes at the atlanto-occipital joint consists of isolated joint narrowing, erosions and fusion and w

Atlanto axial rotatory fixation--a cause of torticollis.

Atlanto-axial rotatory fixation is a rare but important cause of persistent torticollis. The diagnosis is confirmed by careful radiological examination of the atlanto-axial joint, also employing tomography if necessary. Treatment is skull traction in an attempt to improve the position followed by posterior atlanto-axial fusion to prevent further, and potentially disastrous, displacement. Atlanto-axial dislocation or subluxation should be looked for in all cases of acute torticollis, when reduction by traction will most likely be successful and prevent the development of atlanto-axial rotatory

Craniovertebral Junction Abnormalities in Surgical Patients With Congenital Muscular Torticollis.

Our clinical experience led us to realize that craniovertebral junction (CVJ) abnormalities were common in surgical patients with congenital muscular torticollis (CMT). This study aimed to report the concurrence rate of CVJ abnormalities in surgical patients with CMT, along with comprehensive evaluation of type of concurrent CVJ abnormalities. This was a retrospective cohort study in a tertiary hospital, including 41 subjects who underwent surgical release for CMT at the mean age of 8.38 years. The presence of CVJ abnormalities was analyzed, using craniofacial 3-dimensional computed tomography

[Transoral atlanto-axial plate fixation in the treatment of a malunited dens fracture and secondary atlanto-axial instability].

INTRODUCTION: A case of a 22-year-old patient with a malunited dens fracture and secondary atlanto-axial instability is presented. The significant narrowing of the spinal canal due to the atlanto-axial instability was associated with anterior myelon compression and neurological deficit. METHODS: A transoral approach with odontoid resection and anterior atlanto-axial plate fixation was performed. With this technique the atlanto-axial subluxation was reduced and the myelon was decompressed. RESULTS: The postoperative course was uneventful. The follow-up showed a complete remission of the neuro
