Primary biliary cirrhosis among atomic bomb survivors in Nagasaki, Japan.

Despite rapid progress in methods for analyzing radiation effects, much remains to be learned about the mechanisms and processes of radiation-induced immunological dysfunction. Among 17,899 sera obtained from atomic bomb survivors in Nagasaki, Japan, sera from 484 participants who complied with a reexamination for alkaline phosphatase (ALP) were tested for antimitochondrial antibody (AMA) by indirect immunofluorescence, and autoantibodies against 2-oxo-acid dehydrogenase complex (2-OADC) by immunoblotting to investigate the prevalence of primary biliary cirrhosis (PBC). Of these 484 sera, 28 (…

Noncancer disease incidence in the atomic bomb survivors: 1958-1986.

Using the longitudinal data of the Adult Health Study (AHS) cohort collected during 1958-1986, we examined for the first time the relationship between exposure to ionizing radiation and the incidence of 19 nonmalignant disorders in the A-bomb survivors. Affected individuals were ascertained through the three-digit codes of the International Classification of Diseases which are encoded in the AHS database subsequent to diagnoses made on the basis of general laboratory tests, physical examinations, and history-taking conducted during biennial AHS examinations. The disease onset time was estimate

Cytogenetic and molecular changes in leukemia among atomic bomb survivors.

Seventy five radiation-related leukemia patients in Hiroshima including 16 patients exposed to more than one Gray were cytogenetically examined. Statistical analysis of data on the frequencies of chromosomal aberrations in the survivor groups according to bone marrow doses by DS86 estimation revealed that the heavily exposed group tended to have significantly higher aberration rates compared to the non-exposed group. Furthermore, the chromosomal aberrations in the survivors were observed to be of a more complex nature and had the characteristic findings of secondary leukemia. These observation

Association of Acute Radiation Syndrome and Rain after the Bombings in Atomic Bomb Survivors.

Acute radiation-induced symptoms reported in survivors after the atomic bombings in Hiroshima and Nagasaki have been suspected to be associated with rain that fell after the explosions, but this association has not been evaluated in an epidemiological study that considers the effects of the direct dose from the atomic bombs and other factors. The aim of this study was to evaluate this association using information from a fixed cohort, comprised of 93,741 members of the Life Span Study who were in the city at the time of the bombing. Information on acute symptoms and exposure to rain was collec

Unilateral multiple primary tumours in an atomic-bomb survivor.

We report here a case of an atomic-bomb survivor with sequential, unilateral, multiple-organ primary tumours after exposure to direct external radiation. This 67-year-old woman was 8 years old when she was exposed to radiation from the atomic bomb. At the time of the explosion, she was in an open area, but hiding behind a tree, which shielded her left side. Therefore, the right side of her body was exposed to radiation directly and primarily. Since then, she has been diagnosed sequentially with breast cancer, ovarian tumour, thyroid tumour, head skin cancer and lung cancer. In each case, the t

Circulating Hematopoietic Stem and Progenitor Cells in Aging Atomic Bomb Survivors.

It is not yet known whether hematopoietic stem and progenitor cells (HSPCs) are compromised in the aging population of atomic bomb (A-bomb) survivors after their exposure nearly 70 years ago. To address this, we evaluated age- and radiation-related changes in different subtypes of circulating HSPCs among the CD34-positive/lineage marker-negative (CD34(+)Lin(-)) cell population in 231 Hiroshima A-bomb survivors. We enumerated functional HSPC subtypes, including: cobblestone area-forming cells; long-term culture-initiating cells; erythroid burst-forming units; granulocyte and macrophage colony-f

Effect of radiation on age at menopause among atomic bomb survivors.

Exposure to ionizing radiation has been thought to induce ovarian failure and premature menopause. Proximally exposed female atomic bomb survivors were reported to experience menopause immediately after the exposure more often than those who were distally exposed. However, it remains unclear whether such effects were caused by physical injury and psychological trauma or by direct effects of radiation on the ovaries. The objective of this study was to see if there are any late health effects associated with the exposure to atomic bomb radiation in terms of age at menopause in a cohort of 21,259…

Skin cancer incidence among atomic bomb survivors from 1958 to 1996.

The radiation risk of skin cancer by histological types has been evaluated in the atomic bomb survivors. We examined 80,158 of the 120,321 cohort members who had their radiation dose estimated by the latest dosimetry system (DS02). Potential skin tumors diagnosed from 1958 to 1996 were reviewed by a panel of pathologists, and radiation risk of the first primary skin cancer was analyzed by histological types using a Poisson regression model. A significant excess relative risk (ERR) of basal cell carcinoma (BCC) (n = 123) was estimated at 1 Gy (0.74, 95% confidence interval (CI): 0.26, 1.6) for …

Cataracts in Atomic-bomb Survivors More than 70 Years after Radiation Exposure.

Previous ophthalmological studies among atomic-bomb survivors and other radiation-exposed populations have shown a significant association between radiation exposure and cataract, particularly posterior subcapsular cataract (PSC). However, reports of radiation effects on nuclear and cortical cataract are inconsistent. In the current study, we reexamined the issue of radiation effects on cataractogenesis using modern methods and well-defined diagnostic criteria. A questionnaire on ophthalmologic diseases was obtained from 2,439 atomic bomb survivors who were exposed at age 15 or younger and par…

Missing doses in the life span study of Japanese atomic bomb survivors.

The Life Span Study of atomic bomb survivors is an important source of risk estimates used to inform radiation protection and compensation. Interviews with survivors in the 1950s and 1960s provided information needed to estimate radiation doses for survivors proximal to ground zero. Because of a lack of interview or the complexity of shielding, doses are missing for 7,058 of the 68,119 proximal survivors. Recent analyses excluded people with missing doses, and despite the protracted collection of interview information necessary to estimate some survivors' doses, defined start of follow-up as O…

Salivary gland tumors in atomic bomb survivors, Hiroshima, Japan. I. Epidemiologic observations.

An epidemiologic investigation was undertaken in Hiroshima to study the relationship of salivary gland tumors to exposure to ionizing radiation. Two hundred and eleven cases examined between 1953-1971 were confirmed by a review of histologic specimens. Of these, 66 cases occurred in residents of Hiroshima City who were born before the A-bomb: 30 among A-bomb-exposed, and 36 in non-exposed individuals. The incidence of salivary gland tumors among the exposed individuals was significantly higher, 2.6 times greater than that among the nonexposed individuals for all tumors, and 10 times greater fo

Dedifferentiated liposarcoma in the retroperitoneum in an atomic bomb survivor: report of a case.

A 76-year-old Japanese man was admitted to Kosei-Nenkin Hospital (Osaka, Japan) in November 2006; his chief complaint was a 10-kg loss in body weight over 3 months prior to admission. Abdominal computed tomography (CT) and dynamic magnetic resonance imaging (MRI) showed three masses in the retroperitoneum. The patient subsequently underwent surgery. The final histopathological diagnosis of tumors 1 and 2 was malignant fibrous histiocytoma of the retroperitoneum, and tumor 3 was a well-differentiated liposarcoma. By the presence of the liposarcoma, tumor 1 and 2 were thought to be the dediffere

The importance of data from the atomic bomb survivors for international radiological protection.

Epidemiological studies using data from the lifetime experience of the survivors of the atomic bombings of Japan have been fundamental in shaping the international system of radiological protection since a clear excess risk of, first leukaemia and then other cancers, became apparent in the 1950s and 1960s. Cancer mortality and incidence data have been collected, collated and analysed by the Radiation Effects Research Foundation (RERF, and its predecessor, the Atomic Bomb Casualty Commission, ABCC) and the risk estimates so produced have been used subsequently by the International Commission on

Thyroid Dysfunction and Autoimmune Thyroid Diseases Among Atomic Bomb Survivors Exposed in Childhood.

CONTEXT: The risk of thyroid cancer increases and persists for decades among individuals exposed to ionizing radiation in childhood, although the long-term effects of childhood exposure to medium to low doses of radiation on thyroid dysfunction and autoimmune thyroid diseases have remained unclear. OBJECTIVE: To evaluate radiation dose responses for the prevalence of thyroid dysfunction and autoimmune thyroid disease among atomic bomb survivors exposed in childhood. DESIGN, SETTING, AND PARTICIPANTS: Hiroshima and Nagasaki atomic bomb survivors who were younger than 10 years old at exposure …

Organ dose conversions from ESR measurements using tooth enamel of atomic bomb survivors.

Dose conversions were studied for dosimetry of atomic bomb survivors based upon electron spin resonance (ESR) measurements of tooth enamel. Previously analysed data had clarified that the tooth enamel dose could be much larger than other organ doses from a low-energy photon exposure. The radiation doses to other organs or whole-body doses, however, are assumed to be near the tooth enamel dose for photon energies which are dominant in the leakage spectrum of the Hiroshima atomic bomb assumed in DS02. In addition, the thyroid can be a candidate for a surrogate organ in cases where the tooth enam…

Radiosensitivity of skin fibroblasts from atomic bomb survivors with and without breast cancer.

Fibroblasts were established in vitro from skin biopsies obtained from 55 women and 1 man with or without breast cancer and with or without exposure to radiation from the atomic bomb explosion in Hiroshima. The radiosensitivity of these cells was evaluated by clonogenic assays after exposure to X-rays or to fission neutrons from a 252Cf source. Data were fitted to a multitarget model, S/S0 = A [1 - (1 - ekD)N], for both X-ray and neutron dose-survival curves. A single hit model, S/S0 = AekD, fits the neutron dose-survival responses as well. There were no differences in the means or variances o

Association between radiation, glaucoma subtype, and retinal vessel diameter in atomic bomb survivors.

We examined the relationship between glaucoma subtype and retinal vascular caliber as markers of ocular circulation. Subjects were Japanese atomic bomb survivors in Hiroshima and Nagasaki. After a screening examination, potential cases were subjected to further definitive examination. The diameters of central retinal artery and vein equivalents (CRAE and CRVE) on digitized retinal photographs were measured using an established method. Generalized linear regression analyses were used to examine the associations among vessel diameters, radiation exposure, and prevalence of glaucoma subtypes amon…

Discussion: severe mental retardation and cancer among atomic bomb survivors exposed in utero.

When I was in medical school, Douglas Power Murphy, Professor of Obstetrics and Gynecology, told us of his inexpensive, simple study of "microcephaly" and mental retardation in newborn infants whose mothers had received therapeutic radiation early in pregnancy. His review of the literature and mail inquiry of other obstetrics centers in the United States revealed 14 published cases (Murphy, '28) and 16 unpublished (Goldstein and Murphy, '29). Here am I, 52 years later, still updating his findings.

Temporal analysis of a dose-response relationship: leukemia mortality in atomic bomb survivors.

A data analysis that incorporates time dependencies is demonstrated for the dose response of leukemia mortality in the atomic bomb survivors. The time dependencies are initially left unspecified and the data on leukemia mortality--up to the end of 1978--are used to infer them. Several findings based on T65 revised doses (T65DR) are obtained. First, it is shown that the fits to the data of time-dependent L (linear in gamma dose)-Q (quadratic in gamma dose)-L (linear in neutron dose), L-L, and Q-L dose-response models are significantly improved (P less than 0.001) by using the corresponding time…
