Eighteen members of a family were investigated for cardiac conduction abnormalities following the discovery of a second-degree atrioventricular block in a fetus at 35 weeks of gestation. The conduction disturbance was diagnosed by ultrasonography. Seven of the family members were diagnosed as suffering from sinus node dysfunction and/or various degrees of atrioventricular block. Three of them were children aged 9 months to 6 years and all were asymptomatic. The symptomatic family members were two adults. One of them had a pacemaker inserted for a complete atrioventricular block and Adam-Stokes

Atrioventricular node reentry tachycardia is a common type of supraventricular tachycardia. Rarely is it incapacitating and refractory to drug therapy, but when it is, the only option in therapy until recently has been atrioventricular node ablation or antitachycardia pacemaker insertion. The purpose of this paper is to review the case histories of four patients in whom we have surgically abolished atrioventricular node reentrant tachycardia while intentionally preserving atrioventricular node conduction. All four patients had atrioventricular node reentrant tachycardia confirmed by electrophy

Atrioventricular (AV) conduction disturbances in patients (pts) with sick sinus syndrome (SSS) are well known, but sinus node function (SNF) in AV block is not well documented. We therefore performed electrophysiological tests to evaluate SNF in 30 pts with high-degree AV block (group 1) and AV conduction in 15 pts with SSS (group 1). These measurements were repeated after vagolysis with atropine in group 2. In group 1 ergometry was performed if the electrophysiological SNF tests were abnormal. Results (mean +/- 1 SD) for group 1 were: sinus node recovery time (SNRT) 1184 +/- 473 ms, corrected

We report a patient with incessant, exercise-limiting supraventricular tachycardia on the ventricular level and a constant 1:2 atrioventricular relation. Careful mapping of the AV nodal region revealed His alternans in the inferior AV nodal area and nonalternating His morphologies in the superior His region. Radiofrequency catheter ablation in the inferior AV node cured the patient (11-month follow-up). Constant dual ventricular activation, His alternans, distinct His morphologies in the superior and inferior His, and long-term suppression of the tachycardia by ablation in the so-called slow p

BACKGROUND: Selective radiofrequency (RF) ablation of the slow pathway is an effective treatment for atrioventricular (AV) nodal reentry tachycardia. A previous report showed that rapid junctional tachycardia (JT) caused by RF associated with loss of ventriculoatrial (VA) conduction is related to increased risk for AV block. However, this can be difficult to detect during energy delivery, and more importantly, it cannot be measured before the onset of RF energy delivery. The aim of our study was to determine whether measurements made from electrograms could be used to predict the risk of AV bl

A variety of electrocardiographic manifestations of dual AV nodal physiology have been reported. The specific subtype dual ventricular response is considered as a very rare phenomenon. We present the case of a 53 year old lady who suffered from paroxysmal regular tachycardias for more than seven years. In the last 6 months the symptomatology of the cardiac arrhythmia changed to more persistent und irregular rhythm disturbances. Treatment with class Ia antiarrhythmic drugs and beta-blocking agents failed. The latter even seemed to worsen her very disturbing palpitations. After examination of th

Functional longitudinal dissociation of the atrioventricular (AV) node exhibiting two discrete discontinuities in AV nodal conduction curves suggestive of triple AV nodal pathways has been described. The authors report here unusual electrophysiologic properties of the AV node in a patient with documented episodes of paroxysmal supraventricular tachycardia. Programmed atrial extrastimuli introduced at A1-A2 coupling intervals of 390 ms with a driven cycle length of 500 ms produced a sudden marked increase of 75 ms at the A2-H2 intervals suggesting failure of the fast pathway with conduction pro

The purpose of this study was to determine the charges for radiofrequency catheter modification of the atrioventricular (AV) node in 15 patients with symptomatic AV node reentrant tachycardia despite pharmacologic therapy and to compare these charges with the estimated charges for health care utilization by the same patients before the catheter procedure was performed. There were seven men and eight women with a mean age of 50 +/- 17 years. The mean duration and frequency of symptoms were 16 +/- 9 years and 4.5 +/- 6 episodes/month, respectively. Fourteen of the 15 patients required only one p

Primary cardiac tumors are uncommon clinical entities with an incidence of 0.0017% to 0.03% of all autopsies. Cystic tumor of the atrioventricular (AV) node, also known as mesothelioma of the AV node, is a benign congenital tumor that is located in the triangle of Koch in the AV nodal region of the atrial septum of the heart. It comprises of 2.7% of cardiac tumors and is the most common primary cardiac tumor causing sudden death. We herein review the etiology, presentation, differential diagnosis, diagnosis, management, surgical approaches and outcomes of this rare tumor.

UNLABELLED: Accelerated junctional rhythm (JR) is known as a response of the atrioventricular node to thermal injury and a common finding in radiofrequency ablation for AV node reentrant tachycardia. We studied JR during 1314 radiofrequency current deliveries in 172 patients with AV node reentrant tachycardia or paroxysmal atrial fibrillation in relation to the ablation result. JR in a successful RF delivery were characterized by cycle length and conduction.
RESULTS: Selective slow pathway ablation was performed in 100 patients (Group A), selective fast pathway ablation in 41 patients (Group

Catheter ablation of the atrioventricular node is now the treatment of choice for definitive management of some supraventricular tachyarrhythmias. Radiofrequency current is used as the energy source in most centers. We describe a 67-year-old woman who had rheumatic mitral valve disease and chronic atrial fibrillation with rapid ventricular response. She underwent successful radiofrequency catheter ablation of the atrioventricular node, followed by permanent pacemaker insertion. She required mitral valve replacement 16 months later and died postoperatively of left ventricular failure. At autops

In 7 patients conventional a-v nodal ablation was performed using electrical energy delivered from the cardioverter via a catheter-electrode positioned near His bundle area. In all patients indication for the ablation were rapid symptomatic supraventricular arrhythmias refractory to previous pharmacological treatment. During the ablation 1-2 discharges of 200-300 J energy were used, repeating the procedure in 2 patients after 24 hours. In all patients the complete a-v block was obtained and the injury of the postero-septal accessory pathway in 3 subjects. Patients were pacemaker dependent with

OBJECTIVES: The purpose of this study was to characterize anterograde and retrograde properties of the atrioventricular (AV) node in children and to determine the presence of ventriculoatrial (VA) conduction and dual AV node pathways.
BACKGROUND: Although AV node reentry is common in adults, it accounts for 13% of pediatric supraventricular tachycardia (SVT). The age-related changes in the AV node with development are poorly understood. The incidence of dual AV node pathways and VA conduction in the pediatric population is unknown.
METHODS: Electrophysiologic studies were performed in 79 pat

6 patients with reentrant supraventricular tachycardia were studied using the atrial extra-stimulus (S1-S2) technique. 3 patients had smooth curves of H1-H2 intervals plotted against A1-A2 intervals, and echoes occurred only in the relative refractory period of the AV node. These patients met traditional criteria for reentry within the AV node, but all 3 were found to have accessory pathways which functioned only in the retrograde direction. 3 other patients had discontinuous curves of H1-H2 intervals plotted against A1-A2 intervals, and the echo zone was confined to coupling intervals produci

This light microscopic study of the cardiac junctional tissues was based on 27 human embryos, fetuses and postnatal hearts. Evidence was presented that superficial and deep portions of the postnatal AV node were derived from two cellular primordia in the posterior wall of the common atrium at the 6-mm stage. The small right primordia was associated with the right venous valve and give rise to the loosely organized superficial AV node that extended posteriorly to the coronary sinus ostium. A larger left primordia formed the more compact deep subdivision of the AV node located against the anulus

Narrow QRS complex tachycardias originating from the atrioventricular (AV) node region primarily include atrioventricular nodal reentrant tachycardia (AVNRT) and junctional ectopic tachycardia (JET). While AVNRT, the most common paroxysmal supraventricular tachycardia, is based on a reentrant mechanism involving dual AV nodal physiology, JET predominantly arises from enhanced automaticity or triggered activity and frequently occurs either congenitally or postoperatively. Electrophysiological studies allow for a differentiated diagnosis of both entities, with maneuvers such as ventricular stimu

Our purpose was to describe a technique of atrioventricular (AV) node modification for patients with drug refractory AV nodal reentrant tachycardia (AVNRT). Nine patients (mean age, 45 +/- 20; range, 14-82) with recurrent drug refractory AVNRT (n = 8) or sudden cardiac death thought to be precipitated by AVNRT (n = 1) underwent a percutaneous catheter procedure to modify AV nodal function. The area between the electrode recording the maximal His-bundle electrogram and the ostium of the coronary sinus was divided into three zones. Perinodal direct current shocks of 100-300 J were delivered to o

BACKGROUND: The cystic tumor of the atrioventricular node (TAV) is a rare, congenital cardiac tumor, typically located at the base of the atrial septum. Histologically benign, this multicystic mass is a tumor of the conduction system and is considered the smallest tumor capable of causing sudden and unexpected death. TAV has shown a predilection for women with a mean age at presentation of 38 years. The majority of cases are diagnosed incidentally at autopsy, while antemortem surgical excision is rare, with ours being the fifth and sixth reported cases in the medical literature.
