Tilted Endosseous Dental Implants in Atrophic Maxilla: A Clinical Prospective Study.

Background Atrophic edentulous maxillary arches present a significant surgical challenge for conventional implant placement owing to inadequate bone height, pneumatization of the maxillary sinus, and compromised bone quality in the posterior region. Traditional solutions, including sinus floor elevation, ridge augmentation, and bone grafting, carry inherent morbidity and procedural complexity. Objective This study aimed to evaluate the clinical and radiographic outcomes of tilted endosseous dental implants placed in atrophic maxillae, with specific assessment of osseointegration quality, crest

Bone grafting to the maxillary sinuses, nasal floor and anterior maxilla in the atrophic edentulous maxilla. A two-stage technique.

This study presents the results from 20 consecutive patients treated with an autogenous bone graft from the iliac crest. In ten patients the graft was placed in the maxillary sinuses and the floor of the nose (inlay group). Ten patients, in addition to the inlay graft, had a corticocancellous bone block secured with mini-screws to the anterior maxillary ridge (inlay/onlay group). Endosteal implants (Brånemark) were placed six months after surgery. A total of 136 implants were placed, of which eight failed to integrate during the six-month healing period. A further 15 implants were lost during

Avoiding Unwanted Exposure of Subperiosteal Implants in the Atrophic Maxilla: Technical Note.

The use of subperiosteal implants for the rehabilitation of atrophic maxillae has taken attention recently as a validated option in the treatment of severe atrophy. The systems have been improved by technological advancements and the analysis of variables affecting their prognosis. However, one of the most frequent issues to resolve is the unwanted exposure of a portion of the metal mesh close to the abutment and prosthetic connections, which can result in infections, hygiene problems, and, in some instances, complete failure. It seems that the surgical approach is a hot topic for the success

Managing the severely atrophic maxilla: Farewell to zygomatic implants and extensive augmentations?

BACKGROUND: Dental rehabilitation of severe atrophic upper jaws remains challenging. A new generation of subperiosteally placed and rigid multi-vector bone-anchored patient-specific implants proposes an innovative line extension in implant dentistry. This single-center retrospective study focused on treating severely atrophic maxillae using these implants. METHODS: All patients who were treated with a patient-specific implant (IPS Implant® Preprosthetic, KLS-Martin, Tuttlingen, Germany) at Hannover Medical School due to severe atrophy of the maxilla who had no history of malignancy, cleft lip

Pterygoid and tuberosity implants in the atrophic posterior maxilla: A retrospective cohort study.

STATEMENT OF PROBLEM: Rehabilitation of the partially or completely edentulous posterior maxilla using dental implants is a clinical challenge because of the presence of the maxillary sinus, as well as the low quality and quantity of bone in that region. In addition to bone augmentation procedures, posterior maxillary rehabilitation using implants includes their anchoring in bones such as the zygoma, pterygoid, and maxillary tuberosity, as well as in short implants. However, the performance of pterygoid and tuberosity implants in the atrophic posterior maxilla is unclear. PURPOSE: The purpose

Management of atrophic maxilla in severe osteoporosis treated with bisphosphonates: a case report.

Studies investigating whether osteoporosis is a risk factor for using implants have revealed no correlation between possible implant failure and the severity of osteoporosis. However, osteoporotic patients frequently require bone regeneration techniques, because they do not fulfil optimum conditions for implant placement owing to the diminished bone structure and rapid resorption of the alveolar bone. We present a successful case of advanced implant therapy using platelet-rich plasma in a patient with severe osteoporosis who had been previously treated with bisphosphonates. We are not aware th

Maxillary sinus lateral wall thickness and morphologic patterns in the atrophic posterior maxilla.

BACKGROUND: The aim of the present study is to examine the sinus lateral wall thickness (LWT) of atrophic posterior maxilla (<10 mm) of patients with complete and partial edentulism and determine the influence of residual ridge height (RH), sex, and age on maxillary LWT. METHODS: Four hundred fourteen measures were taken from 140 consecutive patients that met the inclusion criteria. On the selected sagittal section, a built-in digital caliper recorded in millimeters the RH and LWT (a perpendicular line at 3, 5, 7, 10, 13, and 15 mm from the lowest point of the sinus floor). Pre-prosthetic surgery options for fixed dental implant reconstruction of the atrophic maxilla.

Four different treatment scenarios exist when considering fixed dental implant restoration of the atrophic maxilla. This paper discusses the use of pre-prosthetic surgery for the increasingly large section of the edentulous population who demand a natural intra- and extra-oral post-operative appearance. Based on residual ridge position, resorption pattern and lip support the treatment modalities must be appropriate for each diagnosis. Sinus augmentation, onlay bone grafting and orthognathic surgical techniques can be employed singly or in combination to adequately address hard and soft tissue

Prosthetic rehabilitation of the atrophic maxilla using pre-implant surgery and endosseous implants.

Pre-implant surgery was carried out in 20 patients with advanced atrophy of the edentulous maxilla, using autogenous bone grafts. Endosseous implants were placed after the initial healing period and these were used to retain removable overdentures. Patients were observed for up to 5 years with 15 of the 105 implants placed being lost and two remaining as sleepers. The resulting implant success rate was 84%. Three patients had to return to conventional dentures while the remaining 17 expressed a high degree of satisfaction with their implant retained prostheses. Pre-implant surgery successfully

Prosthetic Rehabilitation of Atrophic Maxilla by Extrasinus Zygomatic Implants: An Illustrative Case Report.

Prosthetic rehabilitation of severely atrophied jaws remains a major challenge in implant dentistry. Zygomatic implants offer a viable alternative where conventional implants are not feasible. We present the first documented case of zygomatic implant rehabilitation using the extrasinus technique in Nepal. A 49-year-old male with severe maxillary atrophy was treated with two anterior conventional implants and two zygomatic implants, while the mandible received four implants. The patient was rehabilitated with a Malo bridge and hybrid prosthesis. The patient was followed up for 6 years; the impl

A 9-14 year follow-up of onlay bone grafting in the atrophic maxilla.

Treatment of the atrophic edentulous maxilla is challenging especially when bone graft procedures are necessary. In this study an onlay bone graft, a saddle or veneer, with or without maxillary sinus floor inlay graft, harvested from the anterior iliac crest, in combination with implants was used in the reconstruction of patients with extreme atrophy in their maxillae. The aim was to investigate treatment outcome, and the impact of gender and smoking, in 44 patients in a prospective, long-term, follow-up study concerning implant survival rate and marginal bone loss adjacent to the surfaces of

Interventions for replacing missing teeth: zygomatic implants for the rehabilitation of the severely atrophic edentulous maxilla.

RATIONALE: Dental implants are widely used to replace missing teeth but require sufficient bone for support. In people with severe maxillary (i.e. upper jaw) bone loss, zygomatic implants provide an alternative to bone augmentation by anchoring in the cheekbone instead of the upper jaw. Potential advantages include avoiding bone grafting and enabling immediate or early placement of a fixed prosthesis. However, the procedure is technically demanding and may be associated with serious complications. OBJECTIVES: To assess the effects of zygomatic implants for the rehabilitation of people with se

Inlay-onlay grafting for three-dimensional reconstruction of the posterior atrophic maxilla with mandibular bone.

This prospective study describes and evaluates a surgical approach for 3D reconstruction of the posterior maxilla with autogenous mandibular bone in 16 patients (mean age 51 years). Bone blocks were harvested from the mandible and used as lateral or vertical block grafts (onlay); they were also partially milled and used for sinus elevation (inlay). In 4 cases, anorganic bovine bone was added at the periphery of the blocks. 4 months after grafting, implants were placed in a second operation and loaded after 12 weeks. Lateral and vertical augmentations were measured immediately after grafting an

Utilization of a Subsequent Guide for Pterygomaxillary and Zygomatic Implants in the Immediate Restoration of a Severely Atrophic Maxilla.

The pterygomaxillary and zygomatic implants offer a viable alternative to maxillary sinus floor elevation in treating atrophic posterior maxilla, providing the benefit of immediate restoration. However, the limited visibility and the complex anatomy of the maxilla make the surgery more challenging. This brief clinical study describes a subsequent guide for placing pterygomaxillary and zygomatic implants and performing the osteotomy of the maxillary sinus bone window. This approach ensures precise implant placement and helps avoid intraoperative complications.
