Tuberculin and histoplasmin sensitivity in six solomon islands populations.

Skin testing with antigens from Histoplasma capsulatum, Mycobacterium tuberculosis (PPD-S), and atypical mycobacteria (PPD-B, PPD-G, PPD-Y, and PPD-platy) was carried out among six population groups in the Solomon Islands between 1968 and 1972. There was no positive reaction to histoplasmin among any of the groups, suggesting that histoplasmosis is not endemic in the Solomon Islands. There were significant numbers of tuberculin reactors among each group. Largest mean reactions to PPD-S were present among the Lau and Ulawa, in whom reactions to PPD-S were larger than those to any other antigen

[The pathogens of atypical respiratory infections and asthma].

The pathogens of atypical respiratory infections form a big heterogeneous group of different microorganisms. The classical ones belong to bacterial species of Mycoplasma pneumonaie, Chlamydia pneumoniae and Legionella pneumophila. This category comprises also some viruses and rickettsia. In many cases they are responsible for the asthma course exacerbation. Their contribution in the asthma development is suspected, too. Potential protecting effect against atopy and asthma is a subject of speculations. The authors discuss all three aspects, reporting own observations. A precise determination of

[Cervicofacial actinomycosis. Apropos of 2 atypical locations].

Cervico-facial actinomycosis is a chronic bacterial disease caused by actinomycetes. The diagnosis may be made on the specific anatomo-pathological appearance of the granulomas and be confirmed by anaerobic culture of the germs. The authors report two atypical cases: the first is a case of a thyroglossal pseudocyst, the second that of adenoidal vegetations in an adult. Treatment with Penicillin G or V for six weeks produced a favourable response. The various clinical forms of the condition described in the literature, should encourage the physician to include bacteriological testing for actino

Cutaneous diphtheria in a child returning from visiting friends & relatives in Burkina Faso: a case report.

We present a case report of an 8-year-old African origin, Italian born girl, fully immunized for Diphtheria, with multiple ulcers caused by Corynebacterium diphtheria. The clinical event was preceded by a recent trip to Burkina Faso in a rural area. The atypical presentation with ulcers spread to the entire body and particularly to one leg, led to a bacterial swab, from whose culture the pathogen emerged. The girl was initially empirically treated with amoxicillin/clavulanate and then azithromycin. Cutaneous forms are the most common of all forms and have a relevant spreading role. This case i

Demystifying pleomorphic forms in persistence and expression of disease: Are they bacteria, and is peptidoglycan the solution?

There is considerable circumstantial evidence linking tissue pleomorphic forms of unknown origin with idiopathic chronic inflammatory, collagen, lymphoproliferative, nephro-urological (including interstitial cystitis and prostatodynia), and neoplastic diseases. Although these forms have been observed in stained tissue histopathologic specimens for many decades, most are ignored and generally regarded as diagnostically insignificant staining artifacts or debris. It is hypothesized that these pleomorphic forms are not staining artifacts/cellular debris, but instead represent various stages in th

Multifocal erythema migrans - rare presentation in a 2-year-old child.

Ticks are a vector of infectious diseases, both bacterial and viral. Among ticks, Lyme disease is considered to be the most common vector-borne disease in North America and Europe. Many countries in Europe are endemic for Lyme disease, especially the central and north-eastern regions. Lyme disease is characterized by numerous signs and symptoms from various organ systems including the skin, joints, heart and nervous systems. Early stage manifesting with skin symptoms occurs in most infected people. Erythema multiforme appears at the site of a tick bite usually within one month. Typical skin le

Chronic infectious keratitis: A review of etiopathogenesis, clinical presentation and management challenges.

Chronic infectious keratitis represents a distinct clinical entity characterized by indolent progression, diagnostic difficulty, and prolonged therapeutic courses. Unlike acute bacterial keratitis, chronic infections often present with subtle inflammatory signs, recurrent episodes, or poor response to empirical antimicrobial therapy. Among the most important causes are herpetic keratitis, acanthamoeba keratitis, Microsporidial stromal keratitis, and atypical (non-tuberculous) mycobacterial keratitis. Herpes simplex virus keratitis may manifest as recurrent epithelial disease or immune-mediated

Bacteraemia, urinary tract infection and malaria in hospitalised febrile children in Nairobi: is there an association?

BACKGROUND: There is laboratory evidence of altered immune function in children with malaria. Bacterial infections have been documented to complicate severe forms of malaria. However, it remains unclear whether such infections are attributable to the malaria, other risk factors, or are coincidental. OBJECTIVE: To determine the prevalence of bacteraemia and urinary tract infections (UTI) in febrile hospitalised children with and without malaria. DESIGN: A cross-sectional survey. SETTING: General paediatric wards, Kenyatta National Hospital, Nairobi. SUBJECTS: Children aged between three mon

Tinea incognito skin lesions worsen after antifungal treatment: Atypical tinea appearing twice in a case: A case report.

RATIONALE: Atypical tinea lesions can exhibit diverse manifestations. Positive fungal microscopy results confirmed the diagnosis of tinea incognito and other forms of atypical tinea. Patients may concurrently suffer from fungal and bacterial coinfections, requiring physicians to carefully observe, accurately assess, and implement treatments that target both fungi and bacteria. PATIENT CONCERNS: After a 10-month period of misdiagnosis, fungal elements were identified upon examination, suggesting a possible breakthrough in diagnosis. However, the worsening of skin lesions following antifungal t

[Otitis externa, a self-inflicted disease?].

Otitis externa, an inflammation of the external auditory canal is the most frequent cause of ENT consultation all over the world. It can be caused by viral, bacterial or fungal infections. In most of the cases it is provoked by anatomical causes and, even more frequently inadvertently, by the patient himself. Typically, otitis externa is an isolated episode of short duration, easily treated with topical medication. It evolves rarely to the atypical forms of chronic otitis externa and malignant otitis externa. It is important to search for an allergy, eczema or immunodepression in cases of chro

Case studies in current drug development: 'glycylcyclines'.

Glycylcyclines represent a new class of tetracycline antibiotics with potent antibacterial activities against resistant pathogens. One of the glycylcyclines, Tygacil, was selected for further development and has been approved by the FDA. It has an expanded broad-spectrum of antibacterial activity both in vitro and in vivo. It is active against a wide range of clinically relevant pathogens including Gram-positive, Gram-negative, atypical, and anaerobic bacteria and bacterial strains carrying either or both of the two major forms of tetracycline resistance (efflux and ribosomal protection). [Erysipelas].

Erysipelas is an acute dermo-hypodermal infection (non necrotizing) of bacterial origin, mainly group A beta-haemolytic streptococcus. The lower limbs are affected in more than 80% of the cases and the identified risk factors are disruption of cutaneous barrier, lymphoedema and obesity. Diagnosis is clinical and based upon the association of an acute inflammatory plaque with fever, lymphangitis, adenopathy and leukocytosis. Bacteriology is usually not helpful because of low sensitivity or delayed positivity. In the atypical forms erysipelas must be distinguished from necrotizing fasciitis and

Possible misidentification of Bacteroides sp., probably B. ureolyticus as Taylorella equigenitalis: implications for the laboratory diagnosis of CEM.

A wild-type isolate with similar morphological and phenotypic properties to Taylorella equigenitalis, the causative bacterial agent of contagious equine metritis (CEM), was referred for molecular identification by PCR amplification of the 16S rRNA gene. A species-specific PCR failed to yield a product compatible with that of T. equigenitalis. The direct sequencing of the universal 16S rRNA PCR amplicon suggested the presence of a Bacteroides sp., probably Bacteroides ureolyticus, with no consequent effects on the movement and transportation of the animal. Adoption of such a molecular means of

Changing pattern of osteomyelitis in infants and children.

A retrospective analysis of 332 children with osteomyelitis (OM), managed from 1966 to 1996, was undertaken to evaluate etiology, clinical course and treatment results. In 64% of all patients positive bacterial cultures were obtained, Staphylococcus aureus, streptococci, pneumococci, and Haemophilus influenzae were the most frequently cultured pathogens. In two-thirds of the cases long bones (femur, tibia, humerus) were affected. Osteoarthritis or suppurative arthritis was evident in 27%; 32 of 170 (19%) re-evaluated patients had moderate or severe sequelae. Risk factors for an unfavorable cou

Atypical pneumonia (Review).

Atypical pneumonia encompasses diverse pathogens, such as Chlamydia pneumoniae, Mycoplasma pneumoniae and Legionella species, which differ from typical bacterial pneumonia in their extrapulmonary manifestations. Clinical differentiation relies on systemic involvement rather than on standalone symptoms. Despite challenges in distinct diagnosis, syndromic approaches and weighted point systems aid in accurate presumptive diagnoses. Antibiotic treatment, often non-β-lactams due to the unique cell structures of atypical pathogens, targets intracellular processes. Macrolides, tetracyclines, quinolon

Cervical intraepithelial neoplasia (CIN) and Trichomonas vaginalis infection as revealed by polymerase chain reaction.

The impact of sexually transmitted diseases (STDs) on the development of cervical intraepithelial neoplasia (CIN) has been increasingly recognized over the last 25 years. The role of bacterial vaginosis (BV) is the focus of several current investigations, not only in the genesis of CIN but also in the development of other gynecological and obstetric complications. Out of 150 women with vaginitis, 46 were diagnosed as T. vaginalis infection by different methods: wet mount, In-Pouch TV culture and PCR. Pap smear showed that 10 smears had atypical squamous cells of undetermined significance (ASCU

Dermatological Opportunistic Infections in HIV Seropositive Patients: An Observational Study.

Background and objective In developing countries, the dermatological manifestation of the human immunodeficiency virus (HIV) has a high prevalence. Apart from the systemic infection that ensues HIV, skin manifestations form a major part of the disease burden. They can present with atypical forms, and necessary tools for diagnosis may not be available in rural and remote parts of the country. Hence, they can stay misdiagnosed or undiagnosed, contributing to the morbidity of the patients. We attempted to enumerate the dermatologic opportunistic infections (OIs) in Rajkot city, Gujarat, India, in
