Male Infertility in Humans: An Update on Non-obstructive Azoospermia (NOA) and Obstructive Azoospermia (OA).

Non-obstructive azoospermia (NOA) and obstructive azoospermia (OA) are two common causes of infertility that affect a considerable number of men. However, few studies were performed to understand the molecular etiology of these disorders. Studies based on bioinformatics and genetic analyses in recent years, however, have yielded insightful information and have identified a number of genes that are involved in these disorders. In this review, we briefly summarize and evaluate these findings. We also discuss findings based on epigenetic modifications of sperm DNAs that affect a number of genes p

Azoospermia due to spermatogenic failure.

This article summarizes the current literature regarding azoospermia caused by spermatogenic failure. The causes and genetic contributions to spermatogenic failure are reviewed. Medical therapies including use of hormonal manipulation, whether guided by a specific abnormality or empiric, to induce spermatogenesis are discussed. The role of surgical therapy, including a discussion of varicocelectomy in men with spermatogenic failure, as well as an in-depth review of surgical sperm retrieval with testicular sperm extraction and microdissection testicular sperm extraction, is provided. Finally, f

Tuberculous azoospermia - a case report.

This a case report of an immunocompetent (HIV negative) 54 years old man who had been diagnosed with epididymo-testicular tuberculosis. This happened 13 years after he had suffered from pulmonary tuberculosis. The diagnosis was made after a testicular biopsy for infertility. The outcome of anti-tuberculosis treatment was good although the azoospermia lasted. The aim of this report is to emphasize that the genital tuberculosis still exists in our sub-region and that high index of suspicion is required with effective anti-tuberculous therapy in order to achieve good outcome.

Varicocele repair for nonobstructive azoospermia.

PURPOSE OF REVIEW: To evaluate the role of varicocelectomy in the management of patients with varicoceles and nonobstructive azoospermia and to review predictors of successful outcomes. RECENT FINDINGS: Several small, retrospective, noncontrolled studies have documented return of sperm to the ejaculate in up to 56% of men with nonobstructive azoospermia (NOA) following varicocele repair. Additionally, a recent meta-analysis has reported a 6% spontaneous pregnancy rate in amongst NOA patients who underwent varicocele repair, regardless of surgical technique. Although these observations are pro

[Azoospermia factor and male infertility].

Azoospermia factor (AZF) microdeletions of the Y chromosome, which occur in 1 - 55% of infertile men, are closely associated with severe spermatogenic failure and represent the most frequent molecular genetic causes of azoospermia and severe oligozoospermia. Researches on AZF and its related genes, approaching the mechanisms of spermatogenic failure at the molecular level, are of great significance for the diagnosis, treatment and prognosis of male infertility. The detection of AZF microdeletions can provide scientific basis for correct diagnosis and reasonable therapy. This article outlines t

Azoospermia with Klippel-Feil anomaly.

We present a case of azoospermia with Klippel-Feil anomaly. Klippel-Feil anomaly is characterized by the fusion of two or more cervical vertebrae and a short neck, limitation of head movement, and low posterior hairline. The association of this anomaly with MURCS (Müllerian-duct aplasia, renal agenesis, and cervical somite dysplasia) is traditionally regarded as being limited to females, but it has been hypothesized that men displaying the combination of azoospermia, segmentation abnormalities of the cervicothoracic spine, and renal anomalies have a male analog of MURCS. Here we describe the f

[Microsurgical therapy of occlusive azoospermia].

From 5/88 to 8/93 148 patients were operated on for obstructive azoospermia using a microsurgical technique. In 68 patients we performed a vasectomy reversal by vasovasostomy (VT), 42 patients underwent a tubulovasostomy (TT), and 8 patients had a VT with contralateral TT. Two patients had implantation of an alloplastic spermatocele, 9 patients had microsurgical epididymal sperm aspiration (MESA), and 19 patients microsurgical exploration. Patency was achieved in 89% of patients after VT and in 32% after TT. A pregnancy occurred in 34% after VT and in 13% after TT. No pregnancy was achieved af

Surgical treatment of obstructive azoospermia.

The records of 325 azoospermic patients were reviewed. A total of 109 patients (33.5%) had obstructive azoospermia and 48 of them underwent surgical treatment. Three patients had vas deferens agenesia and underwent artificial spermatocele; 14 with epididymal obstruction were treated by an end-to-end microsurgical vasoepididymostomy; and 31 vasectomized patients were submitted to microsurgical reversal. There was no pregnancy in the spermatocele patients. In the vasoepididymostomy patients 60% became patent and 30% achieved pregnancy. In the reversal group 90% of patency and pregnancy rate of 8

[Chromosome anomalies in human azoospermia].

The anomalies of genome were found as a result of cytogenetic study of three azoospermic men. In two cases, the circular Y chromosome was revealed. Different methods of chromosome staining demonstrated complete loss of heterochromatic portion of the long arm of the Y chromosome in one case, and the absence of the euchromatic region in another. A balanced translocation among the chromosomes 1 and 15 was observed in the third case. A question concerning disturbances of spermatogenesis having chromosomal etiology is discussed.

Nonobstructive azoospermia: an etiologic review.

Azoospermia is the complete absence of spermatozoa in the ejaculate in two or more semen analyses after centrifugation. Nonobstructive azoospermia (NOA) represents the most severe form of male factor infertility accounting for 10%-15% of cases and stems from an impairment to spermatogenesis. Understanding of the hypothalamic-pituitary-testicular axis has allowed NOA to be subcategorized by anatomic and/or pathophysiologic level. The etiologies of NOA, and therefore, the differential diagnoses when considering NOA as a cause of male factor infertility, can be subcategorized and condensed into s

Sperm retrieval for obstructive azoospermia.

Advances in the treatment of male infertility now routinely allow men with obstructive azoospermia to have fertility treatment without microsurgical reconstruction. A variety of methods for retrieving sperm from men with obstructive azoospermia have been described. The goals of sperm retrieval are to obtain the best quality sperm possible, to retrieve adequate numbers of sperm for immediate use and for cryopreservation, and to minimize damage to the reproductive tract.

Accidental radiation exposure and azoospermia.

Seven Georgian male soldiers (19-25 years old) had accidentally been exposed to radiation by Cs-137 between April 1996 and May 1997. No information about the exact time and duration of exposure was available. All patients presented with the subacute stage of Cutaneous Radiation Syndrome with deep painful ulcers on different body sites, predominantly on the legs. Semen analyses showed complete azoospermia in 4 patients, with elevated follicle-stimulating hormone (FSH) in 3 and elevated luteinizing hormone (LH) in 2 of them. One patient had severe oligozoospermia of 7 million sperm per mL, with

[Therapeutic strategies in azoospermia: management].

ICSI allows use of epididymal or testicular sperm in case of azoospermia. The aim of this study is to propose an algorithm for the management of the different situations of azoospermia according to our personal experience and according to data of literature. The following points are discussed: epididymal or testicular origin of sperm, retrieval technique, contribution of freezing, interest of in vitro culture, side effects and need of a preliminary biopsy. So a therapeutic strategy adapted to both types of azoospermia obstructive and non obstructive can be proposed.

Varicocelectomy in men with nonobstructive azoospermia.

The management of nonobstructive azoospermia (NOA) is challenging. Even the most successful intervention for this condition, microsurgical testicular sperm extraction (microTESE) with in vitro fertilization (IVF)-intracytoplasmic sperm injection (ICSI), is not successful at least 40% of the time. Varicocele is present in 15% of young men, and varicocelectomy improves fertility outcomes in men with palpable varicoceles and abnormal semen analyses. Unfortunately, there are limited investigations into the effect of varicocelectomy in men with NOA. Uncontrolled cohort studies report the return of
