A widespread dissemination of Bacillus licheniformis in a tertiary hospital: an outbreak or pseudo-outbreak?

OBJECTIVE: Bacillus licheniformis is a Gram-positive bacterium commonly found in soil and water. There have been very few reports on the pathogenicity of B. licheniformis in humans. In this prospective study, the emergence of cases affected by B. licheniformis during a period of 38 days was reported together with investigations into the sources of spread to hospitalized patients in a tertiary hospital. METHODS: Blood cultures of 45 patients grew Bacillus spp. in October and November, 2021. To identify the source and prevent further dissemination of the pathogen, all commonly used materials we

Bacterial Colonization of Mobile Phones: Myth or Reality.

Introduction Bacteria tend to persist on mobile phones for longer durations causing hospital-acquired infections. This is primarily because mobile phones have become an extended hand to healthcare workers due to their unavoidable utilization and the lack of sanitization after use in wards. Methods A questionnaire was used to assess the usage and disinfection practices of mobile phones among medical students regularly attending the wards of a teaching hospital. Culture was done to assess the presence of bacteria and their resistance to antibiotics. Three sterile cotton swabs were performed for

[The management of catheter related blood stream infection].

Catheter-related bloodstream infection causes severe complications and high mortality. CDC revised the guideline for the prevention of intravascular catheter-related infection in 2011, and various anti-infection devices were introduced. However, the most important deterrents of infection are basic procedures such as hand-hygiene and maximal barrier precautions. I would like to describe the points involving early detection, and its medical treatment and prevention.

[A Case of Delayed Vascular Injury as a Complication Related to Implanted Central Venous Port Catheter].

A 74-year-old woman with advanced gastric cancer was admitted to our hospital. A central venous (CV) port catheter was implanted into the right subclavian vein for preoperative chemotherapy and parenteral nutritional management. On the 35th day after implantation, she complained of diarrhea, fever and dyspnea. The chest radiograph showed a right-sided massive pleural effusion. As the patient progressively fell into severe respiratory distress, endotracheal intubation was performed for management of respiration by mechanical ventilation. Initially, given the patient's symptoms, she was diagnose

Periorbital cellulitis caused by Bacillus thuringiensis.

PURPOSE: Periorbital cellulitis is an inflammation of the lids and periorbital tissues without signs of true involvement such as proptosis or limitation of eye movement. METHODS: Bacillus thuringiensis is a Gram-positive, spore-forming soil bacterium with the ability to produce insecticidal crystal proteins. B thuringiensis is an extremely rare causative organism of orbital and periorbital infections. However, it was rarely seen isolated in pediatric cases with preseptal cellulites. Ocular infections of this bacteria quickly progress, within 12-48 hours from inoculation, leading to endophthal

Massive horizontal gene transfer, strictly vertical inheritance and ancient duplications differentially shape the evolution of Bacillus cereus enterotoxin operons hbl, cytK and nhe.

BACKGROUND: Bacillus cereus sensu lato comprises eight closely related species including the human pathogens Bacillus anthracis and Bacillus cereus. Within B. cereus sensu lato, chromosomally and plasmid-encoded toxins exist. While plasmid-mediated horizontal gene transfer of the emetic toxin, anthrax and insecticidal toxins is known, evolution of enterotoxin genes within the group has not been studied. RESULTS: We report draft genome assemblies of 25 strains, a phylogenetic network of 142 strains based on ANI derived from genome sequences and a phylogeny based on whole-genome SNP analysis. T

Ventricular shunt infection and meningitis due to Bacillus cereus.

Non-anthrax Bacillus species are usually considered to be contaminants if found in clinical specimens. Only a few patients with systemic infections due to Bacillus cereus are reported. We present the case of a 18-month old boy with a primitive neuroectodermal tumor (PNET) in the brainstem and obstructive hydrocephalus that required an outlying and subsequently a ventriculoperitoneal drain. Following contamination at the site of entry of the external drain, shunt infection and meningitis with Bacillus cereus developed. Antibiotic treatment with vancomycin failed to eliminate the bacterium from

Genomics and phenotypic profiling of Bacillus paralicheniformis MB647 from an orthopaedic implant infection.

A rod-shaped, endospore-forming, Gram-positive, facultatively anaerobic bacterium was isolated from an infected orthopaedic implant in Islamabad, Pakistan. The strain, designated MB647, is motile with optimal growth at 37 °C and pH 7.0. Whole genome sequencing followed by genome-based phylogenetic analysis placed MB647 within the family Bacillaceae and confirmed its species identity as Bacillus paralicheniformis, supported by OrthoANI values exceeding 99% relative to reference genomes. B. paralicheniformis is predominantly regarded as an environmental or food-associated species and has only ra

[The detection of Peptostreptococcus asaccharolyticus and Bacillus cereus in clinical materials from a child who died of sepsis with a fulminant clinical course].

A case of an acute disease with a rapid clinical course and a fatal outcome in the presence of irreversible toxicoinfectious shock, appearing in two children after the consumption of sheep kidneys, is described. The post mortem examination of the children revealed the presence of hemorrhagic, erosive and necrotic areas on the mucous membrane of the stomach, the duodenum and the upper section of the small intestine. From the material obtained by probing the stomach of one of the children 6 hours before death P.asaccharolyticus and B.cereus were isolated. Hemorrhage, erosions and necrosis were a

Rare case of bacteremia due to Lysinibacillus sphaericus in a person living with HIV.

Lysinibacillus sphaericus, as an insect pathogen, is a ubiquitous Gram-positive bacterium present in the environment. It is often considered to be contaminating bacteria. L. sphaericus has been reported to cause infectious diseases in humans relatively rarely. We report a rare case of bacteremia due to L. sphaericus in a person living with HIV, which is also the first reported case of bacteremia caused by L. sphaericus in China. L. sphaericus easily causes infection in immunocompromised individuals. We found that L. sphaericus and Lysinibacillus fusiformis could not be distinguished by their 1

Bacillus cereus Sepsis in a Patient with Severe Alcoholic Hepatitis.

A 57-year-old woman with a liver injury was referred to our hospital. She had a history of heavy alcohol consumption and had developed severe alcoholic hepatitis. Blood cultures revealed bacteremia caused by Bacillus cereus. The patient was treated with short-term steroid therapy for liver injury and vancomycin administration for B. cereus sepsis, which led to recovery. The findings in the present case suggest the need for empirical therapy with vancomycin in patients with severe alcoholic hepatitis and suspected B. cereus infection.

Diversity of Bacillus cereus group strains is reflected in their broad range of pathogenicity and diverse ecological lifestyles.

Bacillus cereus comprises a highly versatile group of bacteria, which are of particular interest because of their capacity to cause disease. Emetic food poisoning is caused by the toxin cereulide produced during the growth of emetic B. cereus in food, while diarrhoeal food poisoning is the result of enterotoxin production by viable vegetative B. cereus cells in the small intestine, probably in the mucus layer and/or attached to the host's intestinal epithelium. The numbers of B. cereus causing disease are highly variable, depending on diverse factors linked to the host (age, diet, physiology a

[A sepsis case caused by a rare opportunistic pathogen: Bacillus pumilus].

The high prevalence of Bacillus species in nature and the detection of these bacteria as contaminant in cultures may lead diagnostic dilemma, however they should still be considered as a pathogen particularly in case of repeated positive cultures from patients with risk factors. Bacillus pumilus is a bacteria, though rarely, been reported as the causative agent of various infections such as sepsis, endocarditis, skin infections and food poisoning in human. In this report, a sepsis case in an immunocompetent patient caused by B.pumilus was presented. A 38-year-old female patient was admitted to

Necrotizing gastritis due to Bacillus cereus in an immunocompromised patient.

Bacillus cereus is increasingly being acknowledged as a serious bacterial pathogen in immunocompromised patients. We present a case of acute necrotizing gastritis caused by B. cereus in a 37-year-old woman with acute myeloblastic leukemia, who recovered following total parenteral nutrition and treatment with imipenem and vancomycin. B. cereus was isolated from gastric mucosa and blood cultures. Up to now, no case of acute necrotizing gastritis due to this organism has been reported.

Systemic Bacillus species infection mimicking listeriosis of pregnancy.

Bacillus species are increasingly recognized as agents of infection in humans. These organisms are ubiquitous in nature and can cause clinical illness ranging from transient bacteremia to serious systemic infection. We describe a pregnant intravenous drug abuser with fever, constitutional symptoms, and premature labor. Her blood cultures yielded gram-positive bacilli, and her clinical course was consistent with systemic listeriosis of pregnancy. Pathological examination of the placenta revealed acute villitis, and Bacillus species grew from cultures of both placenta and blood. Through biochemi

Results in the management of Bacillus endophthalmitis.

Eighteen cases of culture-proven Bacillus endophthalmitis were reviewed. The majority (13/18) were secondary to penetrating ocular trauma. Others were secondary to corneal ulcer (1), postoperative (1), and endogenous (3). Bacillus cereus was the most commonly identified species (12/18). Antibiotic sensitivities demonstrated uniform sensitivity to vancomycin and the aminoglycosides. Visual prognosis was generally poor; only two patients maintained 20/200 or better in the affected eye. Fourteen eyes were enucleated or eviscerated. The remaining two eyes had light perception vision at the last fo

Identification of Lysinibacillus sphaericus Binary toxin binding proteins in a malarial mosquito cell line by proteomics: A novel approach towards improving mosquito control.

Bacterial insecticidal proteins, such as the Bin toxin from Lysinibacillus sphaericus, could be used more extensively to control insecticide resistant mosquitoes. This study was aimed at identification of mosquito cell proteins binding Bin toxin. Results showed that purified toxin was toxic to Anopheles gambiae larvae and Ag55 cultured cells. Clathrin heavy chain (an endocytosis protein) and glycolytic enzymes such as pyruvate kinase, enolase and dihydrolipoamide dehydrogenase were identified as binders of Bin toxin. The viability of Ag55 cells in the presence of endocytosis inhibitor, pitstop
