The purpose of this report is to describe four cases of nocardiosis observed over an eight-year period in medical units of Principal Hospital in Dakar, Senegal. It is a rare infection occurring mainly in people with weakened immune systems. Pulmonary forms are predominate and clinical and laboratory presentation can mimic pulmonary tuberculosis. Diagnosis should be suspected in patients presenting pulmonary infections and negative sputum bacilloscopy. Nocardia bacteria should be identified before starting antibiotic treatment. Patients require long-term antibiotic treatment with third generati

OBJECTIVES: We investigated the prevalence of anti-phenolic glycolipid-I (PGL-I) IgM antibodies among temporarily unfit blood donors at the Pará State Blood Bank (HEMOPA), located in the Amazon region of northern Brazil. Using an arbitrary high cutoff for optical density (OD ≥0.750) in ELISA, a subset of donors was invited for clinical evaluation for leprosy.
METHODS: Temporarily unfit individuals were invited to participate, and blood samples were collected for anti-PGL-I IgM titration by ELISA. Donors with high OD values were referred for clinical examination, slit skin smear (SSS) bacillos

Ziehl-Neelsen (ZN) smear microscopy remains central to tuberculosis (TB) diagnosis and treatment monitoring; however, its sensitivity is limited by incomplete recovery of Mycobacterium tuberculosis during pre-analytical processing. This study evaluated whether modifying centrifugation force and duration improves bacillary recovery and ZN smear microscopy performance. Laboratory experiments were conducted using M. tuberculosis H37Rv suspensions and clinical sputum specimens. Following NALC-NaOH treatment, samples were centrifuged at 2,000, 3,000, and 6,000 × g for 40 min. The effect of centrifu

INTRODUCTION: Tuberculosis continues to be a global public health problem, the extrapulmonary form being estimated to occur in 10-20% of immunocompetent individuals, increasing in patients who are carriers of the human immunodeficiency virus (HIV); its diagnosis is difficult with conventional methods due to the paucibacillary nature of samples. The Xpert® MTB/RIF test represents an important development in the molecular detection of Mycobacterium tuberculosis and has been used with a variety of non-respiratory clinical samples.
OBJECTIVE: To determine the effectiveness of Xpert® MTB/RIF in th

Leprosy is a chronic infectious disease caused by Mycobacterium leprae and, depending on the host immune status, presents different clinical forms. This report describes the case of a 46-year-old man who had hypoesthetic lesions in the infrahyoid region for 30 days. The bacilloscopy was negative. The anatomopathological examination showed alterations corresponding to the tuberculoid pole (epithelioid histiocytes) and virchowian pole (foamy histiocytes), compatible with borderline-virchowian leprosy (Ridley and Jopling classification). Rapid tests for HIV I, II, and syphilis were positive, with

Approximately 30% of the world population presents with tuberculosis. In developed countries, genitourinary manifestation is responsible for over 40% of extrapulmonary cases. Genitourinary tuberculosis is often diagnosed in the later stage due to the fact that the symptoms are non-specific and technical difficulties to isolate the tubercle bacillus through bacilloscopy or culture in specific medium are many. We present a case of a 47-year-old African-American woman with relapsing urinary infection and sterile pyuria. After a 4 four-year evolution, the patient developed functional exclusion of

Neural leprosy, which is characterized by nerve involvement without visible skin lesions, presents a diagnostic challenge. This case report examined the significance of diverse diagnostic modalities in the identification of pure neural leprosy. A 28-year-old patient with symptoms of edema, pain, paresthesia, and diminished sensitivity in the lower limbs underwent various tests. A stilt skin smear yielded negative results on bacilloscopy, whereas a Fast ML Flow leprosy test and electroneuromyography supported the diagnosis. This discussion highlights the importance of accessible methods for ear

The authors studied the Mitsuda reaction in 37 leprosy patients (18 reactional tuberculoid, 19 reactional borderline cases) and compared the results with clinical findings, histopathology and bacilloscopy. Evaluation of the Mitsuda reaction was carried out on days 30, 60, 90 and 120. Most of the reactional tuberculoid patients showed a Mitsuda reaction of +3 in opposition to the reactional borderline patients who showed only +. Bacilloscopic analysis revealed that in 75% of the reactional tuberculoid cases there were rare or no bacilli; bacilli were present in 95% of the reactional borderline

Lucio's phenomenon represents a serious cutaneous necrotizing reaction, which can occur with Lucio's leprosy and also in other forms of lepromatous leprosy. The authors discuss the case of a 63-year-old male patient presenting with ulcers and necrotic cutaneous lesions on the limbs, torso and face, associated with recurring sinusitis, hoarseness and a weight loss of 25 kilos over the last year. Bacilloscopy of intradermal scrapings showed a bacilloscopic index of 5 and the histopathological findings were compatible with the diagnosis of Lucio's phenomenon. The chosen treatment was exclusively

Manual acid-fast staining often results in specimen slippage, inaccurate positioning, and increased biosafety risks. This study evaluated a modified staining rack incorporating a slide-positioning device and an adjustable tilt function to improve the stability and safety of manual acid-fast staining. A total of 142 clinical specimens (70 sputum, 30 bronchoalveolar lavage fluid, 32 pleural effusion and 10 stool specimens) were stained using the modified rack, a conventional rack, and a BSZ-GT116 fully automated stainer. The single-batch staining time with the modified rack was 935.33 ± 20.91 s,

Tuberculosis (TB) is the leading cause of infection-related mortality in the world, and cases in the United States are increasing. Prompt diagnosis, isolation, and treatment improve patient outcomes and reduce onward transmission. Screening of contacts and high-risk persons for asymptomatic TB infection and treatment of those testing positive reduces progression to disease. TB disease should be suspected in persons with compatible symptoms, risk factors, and consistent imaging. Microbiological confirmation requires an acid-fast smear, nucleic acid amplification test, and culture. Treatment, gu

INTRODUCTION: Timely diagnosis and prompt initiation of treatment are crucial for the effective management of tuberculosis (TB), particularly in People living with HIV (PLHIV). A delay in diagnosing TB,in this special population, not only postpones the start of appropriate treatment but also increases the risk of poor clinical outcomes and TB transmission. In resource-limited settings, where advanced diagnostic tests such as Xpert-MTB assay and mycobacterium culture may be unavailable, there is a notable lag in diagnosing smear-negative pulmonary TB among PLHIV. Empiric anti-TB treatment is a

This study was opened and conducted from colligated patient folders during 5 years (from January 1999 to December 2003). The main objective was the description of epidemiological, diagnostically and prise and prise of charge aspects of microscopy negative pulmonary tuberculosis. Patients were enrolled in this study in accordance to WHO criteria. During these 5 years, pulmonary tuberculosis has constituted 72.2% on all froms of tuberculosis with 20.2% of microscopy negative pulmonary tuberculosis. The BCG vaccine scar has been observed in 50.8% of cases. The IDR was negative in 21.3% of cases;

The aim of this study is to evaluate the investigation actions of home contacts of tuberculosis (TB) index cases in the Tuberculosis Control Program (TCP) of Pelotas/RS - Brazil. This cross-sectional descriptive study was based on data from the records of index cases. The sample consisted of 163 contacts of 68 subjects with pulmonary TB in treatment in the TCP between June and August, 2009. These contacts were predominantly female, over 10 years of age and the daughters of index cases. The requested examination was a chest radiography and bacilloscopy. Active TB was found in 1.8 % of contacts.

The isolation of mycobacteria in abdominal specimens during a 10 years period is presented. Twenty-three clinical cases have been reviewed; patients were divided in three groups: 1) Peritoneal and intestinal tuberculosis. 2) Pulmonary tuberculosis with isolation of M. tuberculosis in feces, and 3) Miliary tuberculosis. We emphasize the low yielding of bacilloscopy, the low number of colonies in cultures and the importance of the microbiological study of abdominal specimens in the confirmatory diagnosis. The predominant symptoms of peritoneal tuberculosis were abdominal pain and distention and

BACKGROUND: The aim of this study was to evaluate the clinical importance of a single isolation of Mycobacterium kansasii in order to determine whether a single isolation of nontuberculous mycobacteria is sufficient to diagnose mycobacteriosis or whether multiple isolations are required, in which case, it would be better to wait until a second isolation is produced before undertaking identification of all nontuberculous mycobacteria.
PATIENTS AND METHODS: Between 1992-1998, we studied the characteristics of 77 isolates of M. kansasii because it is the nontuberculous mycobacterium most frequen

INTRODUCTION: Lesotho has among the world's highest TB prevalences and the national program finding only 50% of incident cases. Lesotho underwent its first TB prevalence study in 2019 in reviewing this study, the authors recognized that the standard measures for screening and diagnosis TB often failed to detect the presence of culture positive pulmonary TB. This review seeks to determine if the systematic under detection of culture positive TB by standard methods is true among prevalence surveys in Africa.
METHODS: We conducted a review to identify TB prevalence surveys conducted in Africa fr
