Correlation between self-reported low back pain and morphological changes in abdominal wall muscles.

BACKGROUND: Morphological changes in the abdominal wall muscle in diastasis recti abdominis (DRA) patients with low back pain (LBP) symptoms and whether there is a wider interrectus distance (IRD) remain unclear. OBJECTIVE: The aim of this study was to determine the correlation between DRA severity and LBP and the morphological differences in abdominal wall muscles between patients who presented with symptoms of LBP and those who were asymptomatic. METHODS: A total of 57 postpartum DRA patients were enrolled, including 21 without LBP, 36 with LBP, and 30 healthy nulliparous women. The numeri

Structural Changes of Lumbar Muscles in Non-specific Low Back Pain: A Systematic Review.

BACKGROUND: Lumbar muscle dysfunction due to pain might be related to altered lumbar muscle structure. Macroscopically, muscle degeneration in low back pain (LBP) is characterized by a decrease in cross-sectional area and an increase in fat infiltration in the lumbar paraspinal muscles. In addition microscopic changes, such as changes in fiber distribution, might occur. Inconsistencies in results from different studies make it difficult to draw firm conclusions on which structural changes are present in the different types of non-specific LBP. Insights regarding structural muscle alterations i

Molecular evidence for local denervation of paraspinal muscles in failed-back surgery/postdiscotomy syndrome.

A study was performed to analyze whether local denervation of the medial branch of the dorsal ramus of the lumbar spinal nerve occurs in a patient with postoperative failed-back surgery syndrome/postdiscotomy syndrome (FBSS/PDS). We investigated the effect of the loss of innervation of the multifidus muscle on neuronal nitrite oxide synthetase (n-NOS) and endothelial nitrite oxide synthetase (e-NOS) applying realtime RT-PCR and immunohistochemistry. Our study demonstrates a substantial reduction of n-NOS expression, supporting the view that local denervation of the multifidus is involved in th

The relation between the transversus abdominis muscles, sacroiliac joint mechanics, and low back pain.

STUDY DESIGN: Two abdominal muscle patterns were tested in the same group of individuals, and their effects were compared in relation to sacroiliac joint laxity. One pattern was contraction of the transversus abdominis, independently of the other abdominals; the other was a bracing action that used all the lateral abdominal muscles. OBJECTIVES: To demonstrate the biomechanical effect of the exercise for the transversus abdominis known to be effective in low back pain. SUMMARY OF BACKGROUND DATA: Drawing in the abdominal wall is a specific exercise for the transversus abdominis muscle (in coc

Vertebral muscles of the back and tail of the albino rat (Rattus norvegicus albinus).

The dorsal and ventral vertebral muscles of the back and the tail of the albino Norway rat are described and discussed. These muscles were analyzed because they are undoubtably used during the sexual posturing, lordosis, of the female rat, as well as participating in a variety of other behaviors. The muscles are described by region (thoracic-lumbar or sacrocaudal), and the classifications of Vallois are followed where possible. Of the epaxial (dorsal) muscles, the three longitudinal systems of muscles, the transversospinalis, the longissimus, and the iliocostalis systems, can be identified in

Associations of Lumber Disc Degeneration With Paraspinal Muscles Myosteatosis in Discogenic Low Back Pain.

Accompanied with intervertebral disc (IVD) degeneration, increasing fat infiltration of paraspinal muscles may be related to discogenic low back pain (DLBP), but their relationship is still unclear and the classical animal models are not completely applicable. The purpose of this study was to assess the paraspinal muscle fat infiltration in patients with DLBP by quantitative MRI, and to develop a novel DLBP rat model to explore the potential relationship between DLBP paraspinal muscle fat infiltration and TNF-α levels. We measured the proton density fat fraction (PDFF) of the multifidus and er

Activation imbalances in lumbar spine muscles in the presence of chronic low back pain.

Paraspinal electromyographic (EMG) activity was recorded bilaterally from three lumbar levels during 30-s isometric trunk extensions [40 and 80% of maximum voluntary contraction (MVC)] in 20 healthy men and 14 chronic low back pain patients in pain. EMG parameters indicating neuromuscular fatigue and contralateral imbalances in EMG root-mean-square amplitude and median frequency were analyzed. Patients in pain showed less fatigue than controls at both contraction levels and produced only 55% of their MVC. Patients in pain likely did not produce a "true" maximum effort. A low MVC estimate would

Associations of back muscle endurance with occupational back muscle activity and spinal loading among subsistence farmers and office workers in Rwanda.

Over the course of the physical activity transition, machines have largely replaced skeletal muscle as the source of work for locomotion and other forms of occupational physical activity in industrial environments. To better characterize this transition and its effect on back muscles and the spine, we tested to what extent typical occupational activities of rural subsistence farmers demand higher magnitudes and increased variability of back muscle activity and spinal loading compared to occupational activities of urban office workers in Rwanda, and whether these differences were associated wit

The Association between Imaging Parameters of the Paraspinal Muscles, Spinal Degeneration, and Low Back Pain.

This narrative review investigated imaging parameters of the paraspinal muscles and their association with spinal degenerative features and low back pain (LBP) found in the literature. Three principal signs of muscle degeneration were detected on imaging: decreased muscle size, decreased radiographic density, and increased fat deposits. Men have a higher density of paraspinal muscles than women, younger individuals have a higher density than older ones, and lean individuals have a higher density than those with an increased body mass index. Fatty infiltration appears to be a late stage of musc

MRI assessment of paraspinal muscles in patients with acute and chronic unilateral low back pain.

OBJECTIVE: To investigate the changes in paraspinal muscle cross-sectional area (CSA) and composition, using the digital data from lumbar spine MRIs of patients with acute and chronic low back pain (LBP). METHODS: In total, 178 patients with unilateral LBP who had lumbar MRI examination were recruited. The data were obtained by a retrospective documentation audit. The CSAs and mean signal intensities of the bilateral paraspinal muscles [psoas major (PM), quadratus lumborum, multifidus (MF) and erector spinae (ES)] were measured, and the percentage of fat infiltration was calculated. The data

Trunk muscles strength as a risk factor for nonspecific low back pain: a pilot study.

OBJECTIVE: To investigate the effects of asymptomatic back muscle weakness and spinal deformity on low back pain (LBP). METHODS: Sixty healthy subjects without LBP participated in this study. Radiography and an isokinetic/isometric dynamometer were used to respectively measure spinal scoliosis/lordosis and the strength of the trunk flexors/extensors. After 2 years, 48 subjects visited the hospital again and LBP episodes, its severity and the Korean version of the Oswestry Disability Index were assessed. Differences between the group with LBP and the group without LBP were evaluated and the as

Radiation-induced tissue damage is caused by ionizing radiation mainly affecting the skin, vascular, neuronal or muscle tissue. Early damages occur within weeks and months while late damages may occur months or even decades after radiation.Radiation-induced paresis of the spine or the trunk muscles with camptocormia or dropped-head syndrome are rare but have already been described as long-term sequelae after treatment of Hodgkin's lymphoma. The differential diagnosis includes limb-girdle muscular dystrophy, fascioscapulohumeral muscular dystrophy (FSHD) or lysosomal storage diseases (e. g. Aci

[Importance of the back muscles in rehabilitation of postoperative segmental lumbar instability--a biomechanical analysis].

In the present study, a biomechanical in-vitro experimental design for assessing lumbar instability following discectomy had been complemented by computer simulation of the muscular influence. The most impressive changes of the instability parameters chosen, i.e., mobility (flexibility) and neutral zone, occurred in respect of the study movement directions Ty, Tz and Mx (lumbar disc height, anterior/posterior dislocation, and flexion/extension). The influence of the multifidus muscle had enabled the flexion/extension neutral zone changes to be offset. Flexibility (+/- Rx) was found to have bee

Evaluation of the thickness of the transverse muscles of the abdomen and the internal oblique muscles using ultrasound imaging in women with back pain.

INTRODUCTION: It has been estimated that chronic low back pain (LBP) affects 18% of young people, and 28-42% of people in middle age, with women being more affected than men. Chronic LBP can arise from a variety of factors, among which are biomechanical changes in core muscles, such as in their thickness, which can impair spinal stability. OBJECTIVE: To analyze the thickness of the transversus abdominis (TrA) and internal oblique (IO) and muscles on the right side in different postures. METHODS: Sedentary women with a normal Body Mass Index were recruited. Participants with a history of chro

Specificity of back muscle response to submaximal fatiguing contractions.

A fatigue-related decrease in quality and accuracy of the proprioceptive feedback from muscle spindles in the back (e.g. stretch reflexes) may lead to a deterioration of spinal stability, thereby making the spine more vulnerable to external perturbations. Endurance time, tissue oxygenation, reflex latency and blood pressure response were studied in twenty subjects during a submaximal endurance test of the back extensors (20 % MVC) performed as either a position task (supporting a weight stack while maintaining trunk position) or a force task (exerting a force against a dynamometer). Mean endur

Clear cell sarcoma of the upper thoracic back muscle.

Clear cell sarcoma (CCS), also called malignant melanoma of soft parts, is a rare malignant soft tissue tumor and is often associated with tendons or aponeuroses. Most of CCS involve extremities, especially lower extremities, but a tumor occurring in the trunk is rare. We report an extremely rare case of CCS originated in the upper thoracic back muscle. To our knowledge, this case is the second report of CCS of the back muscle.

The Role of Back Muscle Dysfunctions in Chronic Low Back Pain: State-of-the-Art and Clinical Implications.

Changes in back muscle function and structure are highly prevalent in patients with chronic low back pain (CLBP). Since large heterogeneity in clinical presentation and back muscle dysfunctions exists within this population, the potential role of back muscle dysfunctions in the persistence of low back pain differs between individuals. Consequently, interventions should be tailored to the individual patient and be based on a thorough clinical examination taking into account the multidimensional nature of CLBP. Considering the complexity of this process, we will provide a state-of-the-art update
