Multifocal Balamuthia mandrillaris infection in a dog in Australia.

A 6-year-old male golden retriever, with an 8-month history of seizures and a clinical diagnosis of lymphoma in the central nervous system, was (at the owner's request) euthanized after signs of respiratory distress and shock developed. Upon postmortem examination, the diagnoses of meningoencephalitis and pneumonia were made. A histological examination of selected tissues from both the lung and central nervous system revealed a severe, acute, multifocal, amoebic, embolic pneumonia and a severe, chronic, multifocal, nonsuppurative, amoebic meningoencephalitis. Indirect immunofluorescence analys

Diagnosing Balamuthia mandrillaris Encephalitis With Metagenomic Deep Sequencing.

OBJECTIVE: Identification of a particular cause of meningoencephalitis can be challenging owing to the myriad bacteria, viruses, fungi, and parasites that can produce overlapping clinical phenotypes, frequently delaying diagnosis and therapy. Metagenomic deep sequencing (MDS) approaches to infectious disease diagnostics are known for their ability to identify unusual or novel viruses and thus are well suited for investigating possible etiologies of meningoencephalitis. METHODS: We present the case of a 74-year-old woman with endophthalmitis followed by meningoencephalitis. MDS of her cerebros

Balamuthia mandrillaris transmitted through organ transplantation --- Mississippi, 2009.

On December 14, 2009, a physician in Mississippi contacted CDC to report possible transplant-transmitted encephalitis in two kidney transplant recipients who shared the same organ donor. Histopathologic testing of donor autopsy brain tissue at CDC showed amebae, and subsequent testing of specimens from the donor and the two kidney recipients confirmed transmission by transplantation of Balamuthia granulomatous amebic encephalitis (GAE), a rare disease caused by Balamuthia mandrillaris, a free-living ameba found in soil. One kidney recipient, a woman aged 31 years, died; the other recipient, a

Fatal granulomatous amoebic meningoencephalitis due to Balamuthia mandrillaris.

Amoebic infections of the central nervous system (CNS) are very rare and usually fatal. A 33-year-old Bolivian male injured his elbow 18 months ago in an accident and, months later, developed multiple skin lesions. He was admitted with confusion, and brain images showed large multifocal lesions with mass effect. Review of the skin biopsy revealed the presence of amoebic trophozoites within a granulomatous inflammation. Despite treatment, he continued to deteriorate and died 17 days after admission. The brain was swollen with prominent uncal herniation, and the leptomeninges showed patchy exuda

Epidemiology and Clinical Features of Balamuthia mandrillaris Infection, China.

Balamuthia mandrillaris is a highly lethal free-living ameba that primarily affects the skin and central nervous system, manifesting clinically as chronic granulomatous lesions and granulomatous encephalitis. Once the central nervous system is involved, the mortality rate exceeds 90%. No standardized treatment regimen has yet been established. In this review, we summarized 66 cases reported from China. The median patient age was 36 years (range 10 months-77 years); 62.12% patients were male and 37.88% female. Fifty-five (83.33%) patients were immunocompetent. For 42 (63.64%) patients, initial

[Balamuthia mandrillaris amebic encephalitis in children: a case report].

Balamuthia mandrillaris amebic encephalitis is a rare but highly fatal parasitic disease in the central nervous system caused by amebae infections. This disease is characterized by complex, diverse and non-specific clinical manifestations and high difficulty in diagnosis, resulting in a high likelihood of missing diagnosis and misdiagnosis. This article presents the diagnosis and treatment of a child with definitive diagnosis B. mandrillaris amebic encephalitis as revealed by metagenomic next-generation sequencing of cerebrospinal fluids, so as to provide insights into clinical diagnosis and t

Laboratory testing of clinically approved drugs against Balamuthia mandrillaris.

Balamuthia mandrillaris is a free-living protist pathogen that can cause life-threatening granulomatous amoebic encephalitis. Given the lack of effective available drugs against B. mandrillaris encephalitis with a mortality rate of more than 90%, here we screened drugs, targeting vital cellular receptors and biochemical pathways, that are already in approved clinical use for their potential clinical usefulness. Amoebicidal assays were performed by incubating B. mandrillaris with drugs (3 × 10(5) cells/0.5 mL/well) in phosphate buffered saline for 24 h and viability was determined using Trypan

Balamuthia mandrillaris meningoencephalitis in an immunocompromised patient. Case report.

Balamuthia mandrillaris is a rare but increasingly recognized cause of amebic encephalitis, yet it remains poorly understood. The condition is almost universally fatal, and due to diagnostic difficulty, most cases are identified postmortem. The authors report a case of Balamuthia amebic encephalitis in a patient with combined variable immunodeficiency in which a rare antemortem diagnosis was made via brain biopsy. Despite broad-spectrum antimicrobial therapy, the outcome was fatal. Such presentations are challenging, and definitive diagnosis may require biopsy in consultation with a skilled ne

Balamuthia mandrillaris meningoencephalitis: the first case in southeast Asia.

We present a case of 23-year-old man with acute meningoencephalitis, accompanied by inflammation of a nasal ulcer. He had been healthy until six months prior to admission to the hospital when he had a motorcycle accident. A star-shaped wound at his nose was incurred after falling into a swamp. A computed tomogram of the brain showed two nonenhancing hypodense lesions at the left caudate nucleus and the right parietal lobe, ependymitis and leptomeningeal enhancement. A skin biopsy showed chronic noncaseous granulomatous inflammation without demonstrated microorganisms. The patient did not respo

A devastating case of a Balamuthia mandrillaris pediatric brain infection.

Balamuthia mandrillaris is an amoeba that causes an uncommon but deadly encephalitis, referred to as granulomatous amoebic encephalitis (GAE). The highest incidence reported worldwide has occurred in America, and within the United States, it has been highest in the Southwest affecting predominantly children and young men of Hispanic ethnicity. Clinical presentation of GAE includes fever, headache, nausea, vomiting, lethargy, irritability, stiff neck, hallucinations, photophobia, and seizures. Our patient was a Hispanic male child living in Arizona. The patient presented at 3 years of age for s

Balamuthia mandrillaris - a brief overview of the parasite causing encephalitis.

Balamuthia mandrillaris is a unicellular protist, known to cause infection of the central nervous system. The life cycle of B. mandrillaris is divided into vegetative, infectious trophozoites and dormant cysts. The ability of B. mandrillaris to produce a rare but fatal infection has resulted in considerable interest in this parasite by the scientific community. However, the pathogenic mechanisms remain incompletely understood. Additionally, symptoms vary from patient to patient, hence diagnosis is often delayed. This delay in diagnosis is compounded further with limited availability of experti

Neurosurgical intervention in the diagnosis and treatment of Balamuthia mandrillaris encephalitis.

The authors describe the unique presentation of Balamuthia mandrillaris encephalitis in a kidney donor and two recipients. All three patients suffered acute clinical deterioration, with radiological or clinical evidence of intracranial hypertension. Brain autopsy in the donor and an urgent brain biopsy in a recipient established the diagnosis. First-tier therapy, with mannitol and CSF drainage, successfully treated the intracranial hypertension in both recipients, while administration of a combination of antiamebic drugs was associated with survival in the second recipient. For both recipients

Balamuthia mandrillaris Encephalitis: Slow but Lethal March Towards the Brain.

Balamuthia mandrillaris causes granulomatous amebic encephalitis. It is puzzling that unlike many blood-borne pathogens that can invade the central nervous system within hours, B. mandrillaris typically takes weeks to months before culminating in encephalitis. Here, we discuss basis of Balamuthia 's slow pathogenesis with an eye for improving early recognition, therapeutic interventions and identifying novel targets for intervention.

Balamuthia mandrillaris: An opportunistic, free-living ameba - An updated review.

Balamuthia mandrillaris is an opportunistic, free-living ameba that is pathogenic to humans. It has a worldwide distribution but is mainly detected in warmer regions. Balamuthia infections are rare but have been reported in both immunocompetent and immunocompromised individuals of all ages. B. mandrillaris can enter through wounds on the skin or the nose and cause cutaneous lesions and the usually fatal Balamuthia amebic encephalitis (BAE). Infection usually spreads from the lungs or through nerve fibers, and attacks the central nervous system, forming granulomatous lesions and necrosis in the
