Thoracic epidural vs balanced anesthesia in morbid obesity: an intraoperative and postoperative hemodynamic study.

Thirty-eight morbidity obese patients undergoing gastric bypass were divided into two groups. All patients received general endotracheal anesthesia with muscle relaxation and controlled respiration with N2O-O2 mixture. In addition, group I, 17 patients, received balanced anesthesia, while the remaining 21 patients, group II, received thoracic (T-5) epidural analgesia. Postoperative analgesia was achieved with morphine intravenously in group I and with 0.5% bupivacaine epidurally in group II. Circulatory function was measured and calculated using radial artery cannulation and pulmonary artery c

Balancing Anesthesia in a Child With Mitochondrial Disease: A Case Report.

Anesthetic management of patients with mitochondrial disease requires an in-depth knowledge and understanding of its pathophysiology to ensure the safe conduct of anesthesia. Our case report illustrates the importance of careful anesthetic planning and execution for a four-year-old undergoing full mouth rehabilitation. It is essential to avoid factors that increase metabolic stress, such as prolonged fasting, hypoglycemia, postoperative nausea, hypothermia, acidosis, hypovolemia, and ischemic or hypoxic events. Balanced general anesthesia was achieved by using incremental doses of anesthetics,

Use of Ketamine or Xylazine to Provide Balanced Anesthesia with Isoflurane in C57BL/6J Mice.

Balanced anesthesia-the use of a combination of drugs to achieve a desired anesthetic plane-offers many benefits, including smoother induction and recovery and fewer adverse effects than occur with individual drugs. Although premedication prior to inhalant anesthesia is routine in other species, mice are commonly induced with gas anesthesia alone. The hypothesis of this study was that premedication with ketamine or xylazine would safely reduce the stress of isoflurane induction and lower the minimum alveolar concentration (MAC) of isoflurane. Young adult male and female C57BL/6J mice were prem

Balanced anesthesia and ventilation strategies for an alpaca (Lama pacos) with an increased anesthetic-risk.

We report the use of a balanced anesthetic technique in a three-year-old, female Huacaya alpaca with an increased anesthetic risk that underwent an extensive dental surgery. Anesthesia was provided with an infusion of midazolam, fentanyl, S-ketamine and low concentrations of isoflurane in oxygen. The mandibular alveolar nerve was desensitized with a lidocaine-bupivacaine combination. The alpaca showed signs of hypoxemia fifteen minutes after anesthesia induction and arterial blood gases confirmed severe venous admixture. Application of positive end expiratory pressure (PEEP) of 6-9 cm H2O impr

Comparison of distal radius fracture plating surgery under wide-awake local anesthesia no tourniquet technique and balanced anesthesia: a retrospective cohort study.

BACKGROUND: Distal radius fractures (DRF) are frequently treated with internal fixation under general anesthesia or a brachial plexus block. Recently, the wide-awake local anesthesia with no tourniquet (WALANT) technique has been suggested as a method that results in higher patient satisfaction. This study aimed to evaluate the functional outcomes, complications, and patient-reported outcomes of DRF plating surgery under both the WALANT and balanced anesthesia (BA). METHODS: Ninety-three patients with DRFs who underwent open reduction and plating were included. Regarding the anesthetic techni

Continuous intravenous infusion of rocuronium (ORG 9426) in patients receiving balanced, enflurane, or isoflurane anesthesia.

BACKGROUND: Rocuronium (ORG 9426) is a new nondepolarizing neuromuscular blocking agent with a rapid onset and an intermediate duration of action. This study obtains the infusion requirements of rocuronium in 30 patients in whom anesthesia was maintained with barbiturate-nitrous oxide-opioid, nitrous oxide and enflurane, or nitrous oxide and isoflurane. METHODS: For all 30 patients, anesthesia was induced with intravenous thiopental and fentanyl, followed by 0.45 mg/kg rocuronium. Patients were randomly allocated to receive either: 1) nitrous oxide in 40% oxygen supplemented with fentanyl, th

Dexmedetomidine as part of balanced anesthesia care in children with malignant hyperthermia risk and egg allergy.

Malignant hyperthermia is an acute hypermetabolic crisis triggered in susceptible patients by the administration of succinylcholine or a volatile anesthetic agent. When anesthesia care is provided to malignant hyperthermia-susceptible patients, a total intravenous anesthesia technique with propofol is frequently chosen. However, coexisting allergies to egg and soybeans may contraindicate the use of propofol. We present our experience with the use of dexmedetomidine as part of the anesthesia regimen in 3 patients with family histories of malignant hyperthermia and personal histories of egg or s

Comparison of Cardiorespiratory Effects of Two Balanced Anesthesia Protocols in Baboons (Papio hamadryas) Undergoing Laparoscopic Salpingectomy.

Laparoscopic salpingectomy is a minimally invasive surgery that requires careful anesthetic management due to the effects of intra-abdominal gas insufflation. In this retrospective study, baboons (Papio hamadryas) were treated with two induction protocols: medetomidine-ketamine (MK; n = 16) and medetomidine-tiletamine-zolazepam (MZ; n = 12) via intramuscular injection. A laryngeal mask (LMA) was used for airway management and anesthesia was maintained with isoflurane in 100% oxygen. For statistical analysis, the following parameters were analyzed via two-way ANOVA: heart rate (HR), respiratory

Comparison of Ultrasound-Guided Thoracic Paravertebral Block Using Ropivacaine and Balanced General Anesthesia in Breast Surgeries.

BACKGROUND AND AIMS: Despite the latest advances in breast surgery, the procedure is frequently associated with postoperative pain, nausea, and vomiting, which leads not only to increased patient's suffering but also to a prolongation of hospital stays and related costs. Thoracic paravertebral block (TPVB) has been successfully used to provide analgesia for multiple thoracic and abdominal procedures in both children and adults. METHODS: Forty patients were allocated for this observational, comparative study and divided into two groups of 20 each, namely thoracic paravertebral group (Group P)

Sustainable anesthesia and environmental stewardship: balancing ecological responsibility with patient safety.

PURPOSE OF REVIEW: This narrative review aims to summarize and assess recent research in sustainable anesthesia and patient safety. RECENT FINDINGS: Recent evidence characterizing volatiles as per- and polyfluoroalkyl substances, supports transitioning towards intravenous anesthesia where clinically appropriate. Potential ecotoxicity caused by propofol is minimal, and total intravenous anesthetic techniques are safe and effective. Reusable anesthetic equipment, such as laryngeal mask airways, breathing circuits, and patient warming devices, reduces greenhouse gas emissions and does not impact

To intubate or not? Balancing anesthesia in rodent fMRI: strategies to mitigate confounding effects.

More than a decade ago, the introduction of intubation and mechanical ventilation for performing blood oxygen level-dependent functional MRI studies in the rodent brain allowed an improved control over the physiological conditions during scanning sessions. An accurate understanding of respiratory parameters permits to respect the 3Rs in animal research, improves significantly the fMRI outcome, and promises improved translational studies. Developments also prompted a better comprehension on anesthetics and their impact on rodent brain physiology and function, bringing new insights on the buildu

Balancing Risks in Obstetrics: Anesthesia Management in Facioscapulohumeral Muscular Dystrophy with Scoliosis.

Facioscapulohumeral muscular dystrophy (FSHD), the third most common form of muscular dystrophy, is associated with a normal life expectancy, resulting in more pregnant women presenting with the condition. Pregnancy often exacerbates muscle weakness in patients with FSHD, posing unique perioperative challenges. Careful evaluation of systemic involvement is essential to minimize maternal and neonatal risks. Perioperative management is often challenging because both general and neuraxial anesthesia carry significant risks. Spinal deformities further complicate anesthesia management, requiring tr

Balanced Opioid-Free Anesthesia on Chronic Postsurgical Pain After Video-Assisted Thoracoscopic Surgery: A Randomized Controlled Trial Protocol.

BACKGROUND: Opioids are widely used for anesthesia and postoperative analgesia; however, their use is related to increased risks of untoward effects including hyperalgesia and chronic postsurgical pain (CPSP). We aim to compare opioid-free anesthesia (OFA) with opioid-based anesthesia (OBA) on the incidence of CPSP after video-assisted thoracoscopic surgery (VATS). METHODS: This randomized controlled clinical trial was approved by the Medical Ethics Committee of the First Affiliated Hospital of Soochow University, Suzhou, China. A total of 180 adult patients undergoing VATS lung resection wil

A balanced anesthesia with a combination of xylazine, ketamine and butorphanol and its antagonism by yohimbine in pigs.

The effects of intramuscular injections of xylazine (2 mg/kg)-ketamine (15 mg/kg) [X-K15], and xylazine (2 mg/kg)-ketamine (5 mg/kg)-butorphanol (0.22 mg/kg) [X-K5-B] were compared in atropinized (0.05 mg/kg) miniature pigs (pigs). Both combinations induced the anesthesia for more than 1 hr, however X-K5-B induced the more potent and well balanced anesthesia as compared with X-K15, although the amount of ketamine was reduced to one third. The duration of loss of pedal reflex, an indicator of surgical anesthesia, in X-K5-B (62 +/- 13 min) was significantly (P less than 0.05) longer than in X-K1

Comparison of the Quality of Recovery Using QoR-40 Questionnaire between Total Intravenous Anesthesia and Balanced Anesthesia in Patients Undergoing Elective Supratentorial Craniotomy, A Randomized Controlled Trial.

BACKGROUND: The quality of recovery following neurosurgery is of utmost significance, and the type of anesthesia used directly impacts it. Previous research on total intravenous anesthesia (TIVA) and inhalation anesthesia yielded mixed results. We used the Quality of Recovery-40 questionnaire (QoR-40) to prove equivalence in quality of recovery between TIVA and balanced anesthesia in patients undergoing elective supratentorial tumor resection. METHODS: After Institutional Review Board approval, this prospective, single-blinded, single-center, randomized equivalence trial was conducted at a te

Measuring the dynamic balance of integration and segregation underlying consciousness, anesthesia, and sleep in humans.

Consciousness requires a dynamic balance of integration and segregation in brain networks. We report an fMRI-based metric, the integration-segregation difference (ISD), which captures two key network properties: network efficiency (integration) and clustering (segregation). With this metric, we quantify brain state transitions from conscious wakefulness to unresponsiveness induced by the anesthetic propofol. The observed changes in ISD suggest a profound shift towards the segregation of brain networks during anesthesia. A common unimodal-transmodal sequence of disintegration and reintegration
