For the past three decades balloon embolectomy has been the treatment of choice for acute lower limb occlusion. However, although usually successful in emboli, results are often disappointing in thrombotic atherosclerotic vessels. Attempted dissolution of the clot is accordingly attractive, and has theoretically been possible since the introduction of streptokinase in 1933. This was initially used intravenously, with variable success rates, although intraarterial administration is currently the method of choice. Later thrombolytic drugs such as tissue plasminogen activator, urokinase and anist

Iliac vein aneurysms are rare, even among venous aneurysms. We report the first case of an external iliac vein aneurysm presenting with paradoxical embolism to bilateral lower extremities. Workup after embolectomy demonstrated a patent foramen ovale along with the aneurysm and an inferior vena cava obstruction, which was treated with balloon angioplasty and ultimately stenting. The patient then underwent interval lateral aneurysmectomy. At 4-year follow-up, she is free of recurrence.

Surgical cases of massive pulmonary embolism remain rare in Japan. To discuss the surgical problems, clinical courses of 4 patients who underwent pulmonary embolectomy under cardiopulmonary bypass at our hospital for the last six years were reviewed. There were 2 men and 2 women; ranging from 41 to 72 years (mean age, 63 years), 1 of whom had deep venous thrombosis of the lower extremity as a predisposing factor. The initial disease recurred in 2 patients. Shock occurred in 3 patients preoperatively, 2 of whom had sudden syncope. Cardiac arrest occurred before and during pulmonary arteriograph

Fourteen patients with complicated venous thrombosis or recurrent pulmonary embolism were treated by implantation of an endoluminal spiral prosthesis subsequent to balloon angioplasty, surgical thrombectomy or embolectomy, a combination of these, or, in 2 cases, no other treatment. The patients were divided into 2 groups, based on their primary diagnosis and the purpose of the prosthesis. Group I included 8 patients with extensive iliofemoral or caval thrombosis, caused by congenital caval stenosis (1 case) or extravascular compression or retraction (7 cases); 7 of these patients had had previ

Two hundred and forty eight peripheral arterial emboli occurring in 221 patients during a period of 20 years have been retrospectively reviewed. Best results are seen following early surgery with the balloon catheter but, in the clinical absence of tissue necrosis, late embolectomy is worthwhile. Atrial fibrillation and post-myocardial thrombus are the major sources of emboli, but tumour and septic emboli do occur and histological and bacteriological examination of retrieved material is essential. Patients who have sustained myocardial infarction have an increased risk of embolism in the first

OBJECTIVE: In this study, we investigated the benefits of using Fogarty balloon catheterization in the treatment of acute leg ischemia with respect to amputation, fasciotomy, and mortality in older patients.
METHODS: A total of 102 patients age >65 who had a Fogarty thrombo-embolectomy for acute thromboembolic limb ischemia were investigated retrospectively. Patients were evaluated based on Rutherford IIa and IIb criteria and duration of ischemia. Duration of ischemia was divided into 3 categories (as 0 to 4, 4 to 8, and >8 hours) to evaluate the effect of ischemia period on vital parameters

A 16-year-old, French saddlebred horse was referred for examination because of colic signs, diagnosed with incarceration of the jejunum in a mesoduodenic rent, and subsequently treated surgically (with an end-to-end anastomosis of the jejunum and an enterotomy of the pelvic flexure). The horse initially recovered without complications; however, on the following day, it exhibited moderate signs of endotoxemia and severe serosanguineous discharge from the abdominal wound. Abdominal ultrasonography revealed substantial peritoneal effusion, necessitating the placement of an abdominal drain. The bl

A subclavian artery dissection (SAD) is usually associated with coexisting aortic disease, and spontaneous SAD is extremely rare. This report presents the case of a spontaneous SAD patient who developed atypical clinical symptoms. A 41-year-old woman presented with bilateral ischemia of her lower limbs. An urgent bilateral femoral thrombo-embolectomy was performed using a balloon catheter. Postoperative enhanced computed tomography (CT) demonstrated a localized thrombus in the left subclavian artery extending toward the descending thoracic aorta, and a follow-up CT angiogram obtained 3 months

Acute upper-extremity arterial occlusion may be due to embolic phenomena or de novo thrombosis. If the occlusion is left untreated, claudication or ischemia necessitating amputation can occur. Operative Fogarty-balloon embolectomy has been the treatment of choice for this entity. In a 6-year period the authors used fibrinolysis on nine occasions in eight patients to treat acute upper-extremity arterial occlusions. Concomitant balloon angioplasty was helpful in four cases. Success, defined as a normal hand with at least one artery that was continuously patent to the wrist, was achieved in all p

The most efficient treatment for acute arterial embolism is operative embolectomy using Fogarty's balloon catheter, especially if a single large artery is involved. Unfortunately, although the early surgical success of arterial thromboembolectomy often seems acceptable, the early clinical outcome still remains unsatisfactory. This may be related to the incomplete restoration of perfusion (i.e. residual thrombus in distal vessels not reached by the balloon catheter thromboembolectomy), propagation of residual thrombi or presence of underlying steno-occlusive lesions. In such a situation a metic

The authors review the surgical management of nine complex paraclinoid aneurysms treated with the endovascular balloon catheter technique. With the patient under general anesthesia, the balloon catheter was guided into the feeding artery of the aneurysm by the Seldinger technique. After the aneurysm was exposed, the balloon was inflated temporarily to prevent premature rupture and to facilitate the dissection of the aneurysm. For the larger paraclinoid aneurysm, the double-lumen catheter was introduced into the cervical internal carotid artery (ICA). After temporarily trapping the aneurysm by

A young male patient presented at a tertiary care hospital with cold and bluish left upper limb accompanied with digital gangrene arousing suspicion of peripheral vascular disease. History did not reveal any high-risk behavior. Clinical examination and subsequent investigations lead to the diagnosis of acute infective endocarditis of native aortic valve along with peripheral embolism caused by methicillin-resistant Staphylococcus aureus. Fogarty's balloon embolectomy was done following which patient developed pseudoaneurysm of the left subclavian artery. These iatrogenic sequelae were managed

PURPOSE: The guiding role of the Fogarty catheter was investigated among patients suffering from limb ischemia due to acute femoropopliteal bypass graft occlusion.
METHODS: A total of 27 patients with a history of femoropopliteal bypass operation who was admitted with acute limb ischemia were enrolled in this retrospective study. In cases in which the Fogarty catheter could not be passed through the popliteal anastomosis, the popliteal region was explored and a new bypass or patch plasty was performed for the distal anastomosis. The cases in which the blood circulation was observed in the gra

BACKGROUND: Surgical treatments for trigeminal neuralgia may include percutaneous techniques including the balloon compression technique. We present here a simple, effective, and safe adaptation of the historical technique described by Mullan in 1978.
METHOD: Our procedure is performed in a bi-plane neuro-radiology room. During general anesthesia, 14-G needle is guided under radioscopy to foramen ovale. The 3-F embolectomy catheter is then inserted and inflated with contrast for a period of 2 min 15 s.
CONCLUSION: Our technique, performed entirely under bi-plane fluoroscopy, allows a quicker

The purpose of this study was to investigate the application of trial balloon occlusion for permanent closure of patent ductus arteriosus (PDA) with severe pulmonary hypertension (PH) in adults, and to assess its immediate and short-term results. From September 1999 to September 2005, a total of ten adults (two males, ages ranging from 20 to 54 years) with PDA who met the criterion for severe PH (basal pulmonary vascular resistance >8 Wood units) received trial balloon occlusion via an embolectomy balloon catheter. Post-occlusion hemodynamics, along with an overall clinical and hemodynamic ass

We present a modification of a previously reported endovascular technique where a large embolus was "pushed and parked" into a diseased artery. A saddle embolus at the bifurcation of the popliteal artery, which occurred as a complication after a percutaneous subintimal recanalization, was pushed and parked into the tibio-peroneal trunk. This was achieved using 2 balloon catheters, one to disengage the embolus from the anterior tibial artery, and the other to push the embolus into the tibio-peroneal trunk, thus establishing flow into the anterior tibial artery. Pushing and parking an embolus in

BACKGROUND CONTEXT: Healthy nerves are able to stretch and glide as responses to normal physiological movement. Injury to the nerve may alter the nerve's mechanical properties and result in neuropathy. Whether cauda equina compression alters the mechanical properties of the sciatic nerve is still unclear.
PURPOSE: The purpose of this study was to demonstrate the changes in excursions and strains of the sciatic nerve in vivo after acute cauda equina compression was induced by epidural balloon compression.
STUDY DESIGN: An animal comparative study with induced cauda equina compression was desi
