Development and Characterization of a Biomimetic Totally Implantable Artificial Basilar Membrane System.

Cochlear implants (CIs) have become the standard treatment for severe-to-profound sensorineural hearing loss. Conventional CIs have some challenges, such as the use of extracorporeal devices, and high power consumption for frequency analysis. To overcome these, artificial basilar membranes (ABMs) made of piezoelectric materials have been studied. This study aimed to verify the conceptual idea of a totally implantable ABM system. A prototype of the totally implantable system composed of the ABM developed in previous research, an electronic module (EM) for the amplification of electrical output

Nanomechanical mapping reveals localized stiffening of the basilar membrane after cochlear implantation.

Cochlear implantation leads to many structural changes within the cochlea which can impair residual hearing. In patients with preserved low-frequency hearing, a delayed hearing loss can occur weeks-to-years post-implantation. We explore whether stiffening of the basilar membrane (BM) may be a contributory factor in an animal model. Our objective is to map changes in morphology and Young's modulus of basal and apical areas of the BM after cochlear implantation, using quantitative nanomechanical atomic force microscopy (QNM-AFM) after cochlear implant surgery. Cochlear implantation was undertake

Noise-induced hearing losses. Can they be explained by basilar membrane movement?

The development of noise-induced hearing losses must in some way be related to the basilar membrane movement. Our analysis of this relationship is based on Mössbauer effect measurements of the basilar membrane movement in human temporal bone preparations. At a single basilar membrane position in each of seven preparations, the displacement frequency response was measured for a given sound pressure level at the ear drum. The measurements covered the place range 2.2--6.2 kHz. Despite the inadequacy of the experimental data, there seems to be no doubt that low frequency components contribute subs

Behavioural measurement of level-dependent shifts in the vibration pattern on the basilar membrane.

Physiological data suggest that the travelling wave on the basilar membrane evoked by a sinusoid of fixed frequency moves towards the base with increasing level. We describe two psychoacoustic experiments that attempted to provide evidence for and quantify the extent of such a shift in humans. In experiment 1, masking patterns were measured in forward masking using a fixed 6-kHz tone presented at 65 or 85 dB sound pressure level. The threshold for detecting a brief sinusoidal signal was measured as a function of signal frequency for several time delays of the signal relative to the end of the

A finite element model to predict the consequences of endolymphatic hydrops in the basilar membrane.

Ménière's disease is an inner ear disorder, associated with episodes of vertigo, fluctuant hearing loss, tinnitus, and aural fullness. Ménière's disease is associated with endolymphatic hydrops. Clinical evidences show that this disease is often incapacitating, negatively affecting the patients' everyday life. The pathogenesis of Ménière's disease is still not fully understood and remains unclear. Previous numerical studies available in the literature related with endolymphatic hydrops, are very scarce. The present work applies the finite element method to investigate the consequences of endol

The mechanical waveform of the basilar membrane. II. From data to models--and back.

Mechanical responses in the basal turn of the guinea-pig cochlea are measured with low-level broad-band noise as the acoustical stimulus [for details see de Boer and Nuttall, J. Acoust. Soc. Am. 101, 3583-3592 (1997)]. Results are interpreted within the framework of a classical three-dimensional model of the cochlea that belongs to a very wide class of nonlinear models. The use of linear-systems analysis for this class of nonlinear models has been justified earlier [de Boer, Audit. Neurosci. 3, 377-388 (1997)]. The data are subjected to inverse analysis with the aim to recover the "effective b

Frequency tuning of basilar membrane and auditory nerve fibers in the same cochleae.

Responses to tones of a basilar membrane site and of auditory nerve fibers innervating neighboring inner hair cells were recorded in the same cochleae in chinchillas. At near-threshold stimulus levels, the frequency tuning of auditory nerve fibers closely paralleled that of basilar membrane displacement modified by high-pass filtering, indicating that only relatively minor signal transformations intervene between mechanical vibration and auditory nerve excitation. This finding establishes that cochlear frequency selectivity in chinchillas (and probably in mammals in general) is fully expressed

Speech recognition in noise: estimating effects of compressive nonlinearities in the basilar-membrane response.

OBJECTIVES: This experiment was designed to estimate effects of cochlear nonlinearities on tonal and speech masking for individuals with normal hearing who have a range of quiet thresholds. Physiological and psychophysical evidence indicates that for signals close to the characteristic frequency (CF) of a place on the basilar membrane, the normal growth of response of the basilar membrane is linear at lower stimulus levels and compressed at medium to higher stimulus levels. In contrast, at moderate to high CFs, the basilar membrane responds more linearly to stimuli at frequencies well below th

Otoacoustic emission estimates of human basilar membrane impulse response duration and cochlear filter tuning.

This study describes a method based on temporal suppression of click-evoked otoacoustic emissions (CEOAEs) to estimate the time course and duration of human basilar membrane impulse responses (BM IRs). This was achieved by tracing the suppression of dominant peaks in the CEOAE spectrum as a function of the temporal separation between two equal-level stimulus clicks. The relationship between the suppression pattern and underlying BM IR duration near the generation site of the CEOAE frequency was established using model simulations. To relate BM IR duration estimates to cochlear filter tuning (Q

Fibronectin-like immunoreactivity of the basilar membrane of celloidin-embedded human temporal bone sections.

Dysfunction of the mechanical properties of the basilar membrane is a potential cause of presbycusis. In cases of minimal sensorineural or strial degeneration it is believed to play a major role. The membrane has been shown to be partly composed of fibronectin. Fibronectin immunoreactivity is diminished in aged rats. Mesothelial cell line the perilymphatic surface of the membrane and are reduced in number in the aged rat cochlea. Fibronectin immunoreactivity was examined in human temporal bone sections (6 months to 92 years old). Hematoxylin and eosin stained section (17 to 97 years) were immu

Effectiveness of active middle ear implant placement methods in pathological conditions: basilar membrane vibration simulation.

Active middle ear implants (AMEI) amplify mechanical vibrations in the middle ear and transmit them to the cochlea. The AMEI includes a floating mass transducer (FMT) that can be placed using two different surgical approaches: "oval window (OW) vibroplasty" and "round window (RW) vibroplasty." The OW and RW are windows located on the cochlea. Normally, sound stimulus is transmitted from the middle ear to cochlea via the OW. RW vibroplasty has been suggested as an alternative method due to the difficulty of applying OW vibroplasty in patients with ossicle dysfunction. Several reports compare th

A self-mixing laser-diode interferometer for measuring basilar membrane vibrations without opening the cochlea.

A laser-diode forms the basis of a displacement sensitive homodyne interferometer suitable for measurements from poorly reflective surfaces. The compact and cost-effective interferometer utilizes the self-mixing effect when laser light reflected from a moving target re-enters the laser cavity and causes phase dependent changes of the lasing intensity. A piezo positioner was used to displace the interferometer with known frequency and amplitude as a basis for real-time calibration of the interferometer's sensitivity. The signal-processing algorithm is described that allows measurements in prese

Basilar-membrane motion in the alligator lizard: its relation to tonotopic organization and frequency selectivity.

In the alligator lizard the entire basilar membrane is accessible for measurements of its velocity by the Mössbauer method. Tests of the method indicate (1) the Mössbauer source can be placed on the basilar membrane without altering the signal-transmission properties of the cochlea, and (2) the source adheres to the basilar membrane. Isovelocity curves (IVCs) were constructed by plotting (as a function of tone frequency) the sound-pressure level at the tympanic membrane required to produce a specified velocity amplitude. IVCs from 21 lizards for source locations spanning the length of the basi

Reflections on the role of a traveling wave along the basilar membrane in view of clinical and experimental findings.

Air conduction (AC) is accompanied by displacements of the two cochlear windows, bulk fluid flow between them, a pressure difference across the basilar membrane, leading to a passive traveling wave along the membrane, which activates the cochlear amplifier and enhances the displacements. AC interacts with bone conduction (BC) stimulation, so that it has been assumed that BC stimulation also involves a passive traveling wave. However, several clinical conditions and experimental manipulations provide evidence that a passive traveling wave may not be involved in BC stimulation at low intensities

A human temporal bone study of changes in the basilar membrane of the apical turn in endolymphatic hydrops.

We observed that some temporal bones with endolymphatic hydrops (EH) showed varying degrees of basalward displacement (towards the scala tympani) of the basilar membrane (BM) in the apical turn of the cochlea. In some, the BM was adherent to the bony wall of the scala tympani (i.e., the interscalar septum). Such mechanical distortion of the BM could conceivably alter cochlear mechanics and lead to sensorineural hearing loss. The results of a systematic evaluation of 234 temporal bones to characterize, quantify and determine the functional significance of this observation are presented. Four gr

A study of the vibration of the basilar membrane in human temporal bone preparations by the use of the Mössbauer effect.

Using the Mössbauer technique, we have studied the vibration of the human basilar membrane and malleus head in the sound frequency range 0.2--9.0 kHz and at a sound pressure of 100 dB at the tympanic membrane. The displacement frequency response curves for the basilar membrane and the malleus head have similar shapes, with a maximum at about 1 kHz. Below and above 1 kHz the curves have a slope of about 10 dB/octave and--100 dB/octave, respectively. In addition, the basilar membrane has a maximum displacement at a frequency dependent distance from the stapes. A simple hydrodynamic model for the

Behavioral estimates of basilar-membrane compression: additivity of forward masking in noise-masked normal-hearing listeners.

Cochlear hearing loss is often associated with a loss of basilar-membrane (BM) compression, which in turn may contribute to degraded processing of suprathreshold stimuli. Behavioral estimates of compression may therefore be useful as long as they are valid over a wide range of levels and frequencies. Additivity of forward masking (AFM) may provide such a measure, but research to date lacks normative data from normal-hearing (NH) listeners at high sound levels, which is necessary to evaluate data from hearing-impaired (HI) listeners. The present study measured AFM in six NH listeners for signal

Distinctive mechanical response characteristics of the guinea pig apical cochlear organ of Corti and basilar membrane.

Upon acoustic stimulation of the mammalian cochlea, a travelling wave will move in a base-to-apex direction, peaking close to the resonant place of the basilar membrane. A gradient of different resonances along the basilar membrane gives rise to frequency-place coding, or tonotopicity, throughout the majority of the cochlea. Per our previous findings, the organ of Corti in the guinea pig cochlear apex does not display tonotopicity. The behavior of the basilar membrane has not been explored throughout the cochlear apex, and so hypothetically tonotopicity is conserved at the level of this struct

Extracorporeal membrane oxygenation states basilar artery thrombectomy and left posterior cerebral artery stent thrombectomy: A case report.

BACKGROUND: Extracorporeal membrane oxygenation (ECMO) is a new type of extracorporeal respiratory and circulatory assistance device. It can drain venous blood out of the body and inject it into veins or arteries after being oxygenated by an oxygenator (membrane lung) to replace lung and heart functions in a short time. ECMO can provide tissue blood perfusion and gas exchange almost equivalent to cardiac output and extend the effective treatment time window for patients with acute circulatory failure to restore cardiopulmonary function. CASE SUMMARY: We report a case of an 81-year-old woman w
