Probability of sporadic lymphangioleiomyomatosis in women presenting with spontaneous pneumothorax.

BACKGROUND: Sporadic lymphangioleiomyomatosis (S-LAM) is a rare low-grade neoplasm of young women characterized by multiple pulmonary cysts leading to progressive dyspnea and recurrent spontaneous pneumothorax (SP). The diagnosis of S-LAM may be delayed by several years. To reduce this delay, chest computed tomography (CT) screening has been proposed to uncover cystic lung disease in women presenting with SP. However, the probability to discover S-LAM in this population has not been determined precisely. The aim of this study was to calculate the probability of finding S-LAM in women presentin

Towards more reliable gambling cost estimates, a population-based study with register-linkage.

BACKGROUND: Although past research has shown a strong association between gambling participation and harms, relatively few studies have attempted to quantify the cost of these harms to society. The need to quantify costs has been identified in several countries, however, no consensus exists in the field of gambling studies on how one should estimate them. METHODS: Three methods were selected for costs calculations: Causality adjustment factors (with two variations: CAF 80%/ CAF 50%), Excess costs, and a method based on Bayes theorem. Our purpose was not to examine the overall costs of gamblin

A Primer on Bayesian Decision Analysis With an Application to a Kidney Transplant Decision.

A clinical decision support system (CDSS) is a computer program, which is designed to assist health care professionals with decision making tasks. A well-developed CDSS weighs the benefits of therapy versus the cost in terms of loss of quality of life and financial loss and recommends the decision that can be expected to provide maximum overall benefit. This article provides an introduction to developing CDSSs using Bayesian networks, such CDSS can help with the often complex decisions involving transplants. First, we review Bayes theorem in the context of medical decision making. Then, we int

Genetic risk estimation by healthcare professionals.

OBJECTIVES: To assess whether healthcare professionals correctly incorporate the relevance of a favourable test outcome in a close relative when determining the level of risk for individuals at risk for Huntington's disease. DESIGN AND SETTING: Survey of clinical geneticists and genetic counsellors from 12 centres of clinical genetics (United Kingdom, 6; The Netherlands, 4; Italy, 1; Australia, 1) in May-June 2002. Participants were asked to assess risk of specific individuals in 10 pedigrees, three of which required use of Bayes' theorem. PARTICIPANTS: 71 clinical geneticists and 41 other h

[Clinical use of a minicomputer in the evaluation of chemico-clinical parameters in the diagnosis of hepato-biliary-pancreatic diseases of surgical significance. 3 methods compared].

Bayes theorem; multivariance analysis, and the discriminating function of Mahalanobis applied to sixteen laboratory examinations employed in the diagnosis of diseases of the extrahepatic bile ducts and pancreas gave the following results: 1) the weight of each test was first measured in general terms, and then with regard to different levels of differentiation, with a classification in decreasing order in each case; 2) little redundancy was noted, and few examinations were of relatively negligible importance; furthermore, combinations of several tests did not lead to positive results; 3) recla

Context-aware monitoring: rethinking comprehensive screening in the era of AI.

Applying advanced measurement technologies proactively in asymptomatic populations predictably yields false positives, as a consequence of Bayes’ Theorem. Yet the same Bayesian arithmetic suggests a remedy: adding context. Serial and multimodal measurements, integrated using context-aware AI that prioritizes within-person change over population norms, can reduce false positive rates while preserving sensitivity. We describe early illustrations from imaging and molecular diagnostics and discuss challenges including cost, anxiety, liability, and equity.

Applied Bayesian Methods in the Rheumatic Diseases.

The use of applied Bayesian methods is increasing in rheumatology. Using the Bayes theorem, past evidence is updated with new data. Preexisting data are expressed as a prior probability distribution or prior. New observations are expressed as a likelihood. Through explicit incorporation of preexisting data and new data, this process informs how this new information should change the way we think. In this article, the authors highlight the use of applied Bayesian methods in the study of rheumatic diseases.

Adjusting the risk for trisomy 21 by a simple ultrasound method using fetal long-bone biometry.

OBJECTIVE: To establish the efficacy of second-trimester fetal long-bone biometry (femur, humerus, tibia, and fibula length) in detecting trisomy 21 and to generate tables for adjusting the risk of trisomy 21 according to long-bone biometry. METHODS: Four long-bones--femur, humerus, tibia, and fibula--were measured ultrasonically in singleton fetuses before genetic amniocentesis. Fetuses with normal karyotypes were used to derive regression equations describing predicted lengths on the basis of the biparietal diameter measurement. The efficacy of each abnormally short bone, alone and in combi

Decision-making in the diagnosis of tuberculous meningitis.

Tuberculous meningitis (TBM) is the most devastating form of tuberculosis (TB) but diagnosis is difficult and delays in initiating therapy increase mortality. All currently available tests are imperfect; culture of Mycobacterium tuberculosis from the cerebrospinal fluid (CSF) is considered the most accurate test but is often negative, even when disease is present, and takes too long to be useful for immediate decision making. Rapid tests that are frequently used are conventional Ziehl-Neelsen staining and nucleic acid amplification tests such as Xpert MTB/RIF and Xpert MTB/RIF Ultra. While pos

Determining clinical decision thresholds for HIV-positive patients suspected of having tuberculosis.

Clinical decision thresholds may aid the evaluation of diagnostic tests but have rarely been determined for tuberculosis (TB). We presented clinicians with six web-based clinical scenarios, describing patients with HIV and possible TB at various sites and with a range of clinical stability. The probability of disease was varied randomly and clinicians asked to make treatment decisions; threshold curves and therapeutic thresholds were calculated. Test and treatment thresholds were calculated using Bayes theorem and the diagnostic accuracy of Xpert MTB/RIF. We received 165 replies to our survey.

The use of probability trees in genetic counselling.

Calculation of genetic risks of persons who may be carriers of X-linked recessive conditions or autosomal dominant diseases with incomplete or delayed penetrance often requires the use of Bayes' theorem. Available methods of computing such risks are often too difficult or too time-consuming for clinicians to use routinely. Probability trees provide a rapid and simple graphical means of estimating genetic risks even in complex clinical situations. The use of probability trees is easily learned because the logic upon which they are based is inherent in their structure.

Decision making on ambiguous stimuli such as prosody by subjects suffering from paranoid schizophrenia, alcohol dependence, and without psychiatric diagnosis.

The aim of this study is the empirical verification of the Bayesian approach applied to the description of the decision-making process with regard to prosodic stimuli in different psychopathological states. Using the Bayesian formalism, the interpretation of a disturbance in internal representation of the contextual information in schizophrenia was given. The results obtained satisfied the formula derived from Bayes' theorem in all tested except a schizophrenic group. Results were interpreted as reflecting cognitive flexibility, and discussed in the context of social adaptation. Although the i

Does endoscopy improve prediction of the prognosis in upper gastrointestinal bleeding?

To evaluate the prognostic value of the endoscopic examination, we included 189 consecutive patients with upper gastrointestinal bleeding, in a prospective study. Rebleeding was more frequent in patients with endoscopic stigmata of hemorrhage. The prognostic prediction given by the physician was slightly, but not significantly, improved after the result of the endoscopy was available. However, when the clinical data were evaluated by computer, using the Bayes' theorem, the endoscopic findings provided no additional prognostic value. We conclude that the endoscopic findings have intrinsic progn

Prevalence of Alternative Diagnoses and Implications for Management in Idiopathic Normal Pressure Hydrocephalus Patients.

BACKGROUND: Following Bayes theorem, ventriculomegaly and ataxia confer only a 30% chance of idiopathic Normal Pressure Hydrocephalus (NPH). When coupled with positive responses to best diagnostic testing (extended lumbar drainage), 70% of patients recommended for shunting will not actually have NPH. This is inadequate clinical care. OBJECTIVE: To determine the proportion of alternative and treatable diagnoses in patients referred to a multidisciplinary NPH clinic. METHODS: Patients without previously diagnosed NPH were queried from prospectively collected data. At least 1 neurosurgeon, cogn

Breast cancer risk assessments to barrier contraception exposure. A new approach.

(Full text is available at http://www.manu.edu.mk/prilozi). The risk prediction models for breast cancer remain unsatisfactory. The existing models of breast cancer risk assessment have failed to consider (calculate) the exposure to condom use, defined as the major risk factor of breast cancer. All the models, including the NCI-Gail model, are based on the so-called "known" breast cancer risk factors, such as, menarche, age at first birth, parity, OC pills, diet, physical activity, age at menopause, number of breast biopsies, family history, ethnicity (race), age and other. The commonest predi

[Importance of germ cell mosaic for genetic counseling of families with Duchenne and Becker muscular dystrophy].

Germ cell mosaics were demonstrated in Duchenne's and Becker's muscular dystrophy by molecular genetic methods. These findings affect risk estimates in these x-chromosomal muscular diseases. The mutation-selection equilibrium for x-chromosomally lethal inheritance for determining the a priori probability for risk estimate according to the Bayes theorem must therefore be redefined to take the germ cell mosaic problem into account. Taking Duchenne's and Becker's progressive muscular dystrophy as an example, the article explains how the estimation of the heterozygote risk in women seeking advice

Validity of the Dissociative Experiences Scale in screening for multiple personality disorder: a multicenter study.

OBJECTIVE: The Dissociative Experiences Scale has proved a reliable and valid instrument to measure dissociation in many groups, but its capacity to distinguish patients with multiple personality disorder from patients with other psychiatric disorders has not yet been conclusively tested. METHOD: A discriminant analysis was performed to classify 1,051 subjects as having or not having multiple personality disorder. Another discriminant analysis was performed on a subgroup of 883 subjects more closely representing patients in a typical psychiatric facility in terms of base rates of dissociative

Bayesian analysis and risk assessment in genetic counseling and testing.

Risk assessment is an essential component of genetic counseling and testing, and Bayesian analysis plays a central role in genetic risk assessment. Bayesian analysis allows calculation of the probability of a particular hypothesis, either disease or carrier status, based on family information and/or genetic test results. Genetic risk should be assessed as accurately as possible for family decision making. Additional information, from the pedigree and/or from genetic testing, can often dramatically improve the accuracy of genetic risk assessment. We illustrate herein the application of Bayes' t

[Dyspareunia in women is common, particularly in younger women. Pain history and the women's age provide valuable clues for diagnosis].

Very little is known about the causes of dyspareunia. The aim of this study was to investigate the diagnoses in a non-patient population of women after a short interview and a careful pelvic examination, and to analyze the predictive value of age and different pain characteristics according to Bayesian analysis and Bayes' Theorem. Sixty-four women with severe and prolonged dyspareunia were recruited from the Swedish national screening program for cervical cancer. It was shown that the diagnoses varied with age, and that age and various pain characteristics were very good predictors of the diff

[Prediction of rehabilitation outcome after surgical interventions based on biochemical parameters: a new concept in mediator research].

The perioperative changes in epinephrin-, norepinephrine-, histamin-, C5a- and interleukin-6-levels were studied in 40 patients undergoing cholecystectomy for the diagnosis of acute cholecystitis. All relevant mediator levels could be determined in 38 patients. The outcome was not optimal in 16 of them (42%). In order to evaluate the predictive value of the mediators under investigation for the quality of the patients' outcome, a model based on the Bayes' theorem was developed. Using this model the outcome (optimal vs. not optimal) could be correctly predicted in 30 (79%) of our study patients
