Behavioral Health Care Delivery Through Street Medicine Programs in California.

Mental health and substance use disorders are prevalent among people experiencing homelessness. Street Medicine can reach unhoused people who face barriers to accessing healthcare in more traditional medical settings including shelter-based clinics. However, there is little guidance on best practices for mental health and substance use treatment through Street Medicine. The aim of the study was to describe behavioral health care through Street Medicine by analyzing data from the California Street Medicine Landscape survey and follow-up qualitative interviews. Most street medicine programs util

Behavioral addictions in addiction medicine: from mechanisms to practical considerations.

Recent progress has been made in our understanding of nonsubstance or "behavioral" addictions, although these conditions and their most appropriate classification remain debated and the knowledge basis for understanding the pathophysiology of and treatments for these conditions includes important gaps. Recent developments include the classification of gambling disorder as a "Substance-Related and Addictive Disorder" in the 5th edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) and proposed diagnostic criteria for Internet Gaming Disorder in Section 3 of DSM-5. Behavioral medicine in the prevention and treatment of cardiovascular disease.

Cardiac behavioral medicine is the application of behavioral and psychosocial principles to the prevention and treatment of heart disease. Most biomedical cardiovascular risk factors (e.g., high blood lipids, high blood pressure, diabetes) require behavioral and medical interventions. Other risks, including obesity, high-fat eating pattern, smoking, and inactivity, clearly require lifestyle change. Behavioral medicine screening and intervention have been applied to psychosocial risk factors such as depression, hostility, and social isolation. Appropriate assessment of risk factors is essential

Contributions of behavioral primatology to veterinary science and comparative medicine.

Behavioral primatology is a subdiscipline of the research area referred to as primatology. Like primatology, behavioral primatology is an eclectic field of study made up of researchers from diverse basic disciplines having very different historical roots and employing extremely different methodologies biased by emphases and assumptions dictated by their histories. Psychologists, zoologists, anthropologists, and psychiatrists make up the majority of those currently active in behavioral primatology, but others, including those in veterinary science, are active in research in the area. [Implementation of a Behavioral Medicine Oriented Concept in Cardiological Rehabilitation].

This study examined the implementation of a behavioral medicine oriented rehabilitation concept in a cardiological rehabilitation clinic. Psychotherapeutic interventions were based on an Acceptance and Commitment Therapy framework. Participants were treated in a behavioral cardiac rehabilitation program (BCR; n=149), or in a conventional cardiac rehabilitation program (CCR, n=100). We tested and compared the reach of the targeted group, the fidelity of the concept implementation in the BCR, as well as the completeness of the intervention (dose delivered) and the patient-reported behavioral med

Behavioral Sleep Medicine Services for Hypersomnia Disorders: A Survey Study.

Patients with hypersomnia disorders (HD) suffer from debilitating symptoms that result in reduced functioning, depression, anxiety, and overall worse quality of life. Little is known about the need and desire of this population to utilize behavioral sleep medicine (BSM) interventions that focus on psychosocial functioning and quality of life, and there have been limited attempts to develop such interventions. The purpose of this survey study was to gather patient-centered data on engagement in pharmacological and nonpharmacological interventions, the psychosocial impact of HD symptoms on quali

Evaluation of a behavioral medicine consultative treatment for chronic, ruminative vomiting.

This case study evaluated a multicomponent behavioral medicine treatment package for chronic ruminating and vomiting in a 13-year-old child with multiple disabilities. Treatment incorporated dietary and medication manipulations combined with behavioral intervention. The treatment package was designed and evaluated via a consultative model in a residential-care setting. The child's ruminative vomiting decreased steadily with implementation of treatment and reduced rates were maintained 2 years later. This case illustrates collaboration between physicians and psychologists in developing comprehe

Integrating Behavioral Sleep Medicine with Behavioral Parent Training for Preschool ADHD: Stakeholder Insights for an Early Childhood Intervention.

OBJECTIVES: Behavioral Parenting Training (BPT), the gold-standard treatment for preschool attention-deficit/hyperactivity disorder (ADHD), does not consistently improve core symptoms of inattention and hyperactivity/impulsivity. Pairing BPT with complementary preventative therapies targeting biological mechanisms, such as sleep dysregulation, may improve treatment outcomes. This qualitative study with key stakeholders aims to inform the development of parent-based interventions that incorporate behavioral sleep medicine (BSM) strategies to improve ADHD-related outcomes in preschoolers. Can attention control conditions have detrimental effects on behavioral medicine randomized trials?

OBJECTIVE: Attention control (AC) conditions are used to balance nonspecific attention in randomized trials of behavioral interventions. Very little guidance about which behavioral interventions and outcomes merit AC is available in the literature. The primary aim of the present study is to demonstrate a scenario in which use of AC in a behavioral randomized trial was unnecessary and possibly detrimental. METHODS: Exploratory analyses were performed in a randomized controlled trial that tested whether a patient-centered counseling intervention reduced low-density lipoprotein cholesterol level

New solutions for treating chronic insomnia: an introduction to behavioral sleep medicine.

Insomnia is one of the most frequent complaints brought to primary care physicians and research suggests insomnia's prevalence is on the rise. Insomnia evaluation and treatment can be a time-intensive process that puts significant demands on a busy medical practice. To date, hypnotic medications are the most frequently prescribed treatment for insomnia and have been demonstrated to be efficacious for the treatment of acute insomnia. Cognitive-behavioral treatment (CBT) has been found to be just as effective as hypnotics for the treatment of acute insomnia and more effective for the treatment o

Precision medicine in substance use disorders: Integrating behavioral, environmental, and biological insights.

Substance use disorders (SUD) are chronic, relapsing conditions marked by high variability in treatment response and frequent relapse. This variability arises from complex interactions among behavioral, environmental, and biological factors unique to each individual. Precision medicine, which tailors treatment to patient-specific characteristics, offers a promising avenue to address these challenges. This review explores key factors influencing SUD, including severity, comorbidities, drug use motives, polysubstance use, cognitive impairments, and biological and environmental influences. Advanc

Advancing our understanding of religion and spirituality in the context of behavioral medicine.

Recognizing and understanding the potentially powerful roles that religiousness and spirituality (RS) may serve in the prevention and amelioration of disease, as well as symptom management and health related quality of life, significantly enhances research and clinical efforts across many areas of behavioral medicine. This article examines the knowledge established to date and suggests advances that remain to be made. We begin with a brief summary of the current knowledge regarding RS as related to three exemplary health conditions: (a) cardiovascular disease; (b) cancer; and, (c) substance ab

Optimizing pain management through collaborations with behavioral and addiction medicine in primary care.

Chronic noncancer pain (CNCP) affects many primary care patients, and carries a large human and economic burden. In response to the widespread perception that pain is underdiagnosed and undertreated, regulatory bodies have encouraged more comprehensive services to address pain syndromes. Significant hurdles exist in treating CNCP in primary care settings, and interventional therapies and pharmacotherapy often do not provide complete symptomatic relief. This article describes a multidimensional and interdisciplinary approach to the treatment of CNCP. The utility of collaborations with behaviora

Is there a continued need for behavioral pharmacology in the discovery and development of medicines?

Since their introduction in the 1950s by Peter Dews, behavioral pharmacology laboratories have collected experimental data for discovery, approval, and regulation of many new and improved therapeutic agents across a host of therapeutic domains. The ethical use of animals in research requires evaluation of the value of animal data to justify their use. The present commentary provides a discussion of the risk-benefit analysis of the use of behavioral methods in drug discovery and drug development using antidepressants as an example. It is concluded that there are some misconceptions both within

Sleep, suicide behaviors, and the protective role of sleep medicine.

OBJECTIVE/BACKGROUND: Sleep disturbance is associated with suicidal thoughts and behaviors. The relationship of specific sleep disorders to suicide attempts is less well established. Whether treating sleep disorders reduces suicide attempts remains controversial. METHODS: Suicide attempts, treatment utilization, and psychiatric diagnoses were extracted from electronic medical records and a suicide attempt database from the U.S. Department of Veterans Affairs. The sample (N = 60,102) consisted of patients with any record of suicide attempt in FY13-14 and a 1:1 case-control of patients with no

Assessing Value of Behavioral Interventions: Designing Cost-Effectiveness Analyses for Food-Is-Medicine Interventions.

Food-is-Medicine (FIM) interventions, including medically tailored meals, medically tailored groceries, and produce prescriptions, represent a promising approach to prevent, manage, and treat diet-related diseases. As the evidence base for FIM interventions expands through large-scale randomized trials, standardized value assessment frameworks will be essential to inform coverage and reimbursement decisions. Using FIM interventions as examples, this article provides guidance on economic evaluation methods, particularly cost-effectiveness analysis. We further discuss emerging methodological app
