Clinical Reasoning: A 24-Year-Old Woman With Penetrating Neck Injury From a Needlefish.

Evaluating patients with a traumatic spinal cord injury can be complicated by other injuries. In this case, a 24-year-old woman injured by a needlefish presented with combined motor and sensory defects, cranial nerve deficits, and a blunt vascular injury. This case highlights the importance of neurologic and vascular localizations and an understanding of spinal cord injuries involving various ascending and descending tracts. Appreciation of these anatomical considerations through this case illustrates the diagnostic approach to neurologic evaluation. While we present a traumatic etiology for m

A stitch in time: unrecognized retained foreign bodies after a needlefish injury.

We present a case report of a traveller injured by a needlefish in the Caribbean. The needlefish leapt from the ocean and struck the traveller's face at high speed, causing a seemingly superficial puncture wound on his nose. Later, it became apparent that multiple fish bones had broken off and lodged in his nasal cavity, very narrowly missing his cribriform plate. Some bones were discharged spontaneously through his nose over the next 3 months, and one required surgical removal. Our report highlights the importance of urgent radiological examination in patients injured by needlefish, even if t

Two cases of penetrating abdominal injury from needlefish impalement.

BACKGROUND: Needlefish injuries to humans are extremely rare. An extensive review of the medical literature reveals only 22 cases previously reported. Most injuries reported have occurred in the Indo-Pacific region, but there are scattered reports worldwide. OBJECTIVE: To report two cases of life-threatening penetrating abdominal injuries from needlefish impalement. CASE REPORT: A 19-year-old man and a 40-year-old woman were both struck in the abdomen by a needlefish and required emergent exploratory laparotomy for instability. Their injuries were a laceration of a branch of the left portal

[Needlefish jaw in the orbit].

We report a case of unsuspected penetrating trauma with intraorbital foreign body, namely a needlefish jaw. A 44-year-old fisherman presented with vertical diplopia and discrete swelling of the upper lid near the medial canthus after being hit by a fish. He was unaware of any penetrating lesion or foreign body. There was no entry wound. CT-scan showed a foreign body between the globe and the medial orbital wall. Surgical exploration found that it was a 4.5cm long needlefish jaw. Removal resulted in complete resolution of symptoms. Needlefish can be very dangerous. This is the first reported ca

Infestation of the Parasitic Isopod Mothocya parvostis on Juveniles of the Black Sea Bream Acanthopagrus schlegelii as an Optional Intermediate Host in Hiroshima Bay.

In Hiroshima Bay, parasitic isopods of the genus Mothocya infest the black sea bream Acanthopagrus schlegelii (Bleeker, 1854) and the Japanese halfbeak Hyporhamphus sajori (Temminck and Schlegel, 1846), two fish species that are abundant and commercially important in the Seto Inland Sea of Japan. Immature and mature Mothocya individuals can infect both juveniles and adults of H. sajori, while immature Mothocya are known to parasitize juveniles of A. schlegelii; i.e., no Mothocya parasites are found in adult A. schlegelii. The identification of the immature Mothocya parasitizing juveniles of A.

Traumatic tympanic membrane perforation by a needlefish: case report.

OBJECTIVES: We report a unique case of traumatic tympanic membrane perforation caused by a needlefish beak. We describe the mechanism of injury, the clinical findings and the treatment. CASE REPORT: An 11-year-old boy presented with otorrhoea and hearing loss secondary to a traumatic tympanic membrane perforation by a needlefish. The perforation was repaired by performing a myringoplasty, with satisfactory post-operative audiological results. CONCLUSION: To our knowledge, this is the first reported case of its kind. It is recommended that careful examination of the middle-ear space should al

Penetrating injury of the maxilla by needlefish jaws.

INTRODUCTION: Needlefish penetrating injuries have become a worldwide problem, inflicting critical morbidities and even mortalities. This is the first published case of needlefish injury in the Mediterranean basin. CASE REPORT: A 29 year old man was admitted to Meir Medical Centre in Israel with a penetrating facial wound caused by elongated needlefish jaws. The severity of the wound contrasted greatly with the expected injury from collision with a fish inflicting a small penetration lesion. The rigid jaws penetrated the maxilla transversely and obliquely from the left canine-fossae, through

Three new species of Prosorhynchoides (Digenea: Bucephalidae) from Tylosurus gavialoides (Belonidae) in Moreton Bay, Queensland, Australia.

We surveyed 30 individuals of Tylosurus gavialoides (Castelnau) (Belonidae) collected from Moreton Bay, Queensland, Australia, and describe three new species of Prosorhynchoides Dollfus, 1929 from them. The new species are morphologically distinct from existing Prosorhynchoides spp. and 28S and ITS-2 ribosomal DNA data further supports our morphological findings. We also conduct the first mitochondrial DNA analysis of species of Prosorhynchoides. The new species from T. gavialoides form a strongly supported clade on the basis of the two ribosomal markers, further supporting the emerging hypoth

Seven-Year Follow-Up of a Patient With a Retained Needlefish Jaw Fragment Abutting Against His Left Internal Carotid Artery.

A 51-year-old man presented to the ophthalmology service with binocular diplopia and facial numbness. The patient was returning from a trip to Mexico. He reported having been hit in the left periocular region by a fish while swimming. Local doctors repaired a laceration in the left lateral canthus shortly after the incident. Orbital imaging revealed 2 needle-like foreign bodies corresponding to retained pieces of a needlefish jaw in the left orbit. Given the location of the foreign bodies, observation with repeat imaging was deemed more appropriate than surgical exploration. Subsequent imaging

Nonsurgical Management of Retained Needlefish Jaw.

While scuba diving, the left medial canthus of a 53-year-old man was pierced by a needlefish. He immediately lost vision in his left eye. An orbital computed tomographic scan showed the needlefish jaw in the left optic canal. The left medial orbit was explored surgically but no foreign object was removed. One month later, MRI confirmed the presence of the retained needlefish jaw. A conservative approach was taken and the patient remained stable over 3 months of follow-up.

Ratfish (Chimaera) spine injuries in fishermen.

An occupational hazard peculiar to fishermen, is an injury from a sharp fish spine. Such spines can cause envenomation injury, infectious sequelae or trauma to anatomical structures. The management of two fishermen with penetrating ratfish (Chimaera) spine injuries to the lower limb is described. Both were managed by removal of the spine under general anaesthesia. In the second patient, the spine was embedded adjacent to the left femoral artery, highlighting the potential for major haemorrhage and supporting the use of surgical wound exploration when important structures may be involved. Herei

Metazoan parasite fauna of migrating common garfish, Belone belone (L.), in the Baltic Sea.

A total of 35 common garfish, Belone belone (Linnaeus, 1761), were studied for metazoan parasites on their spawning grounds in the western Baltic Sea. Nine parasite species were found, and six new locality records could be established for German coastal waters (Axine belones, Monogenea; Proteocephalus sp., Cestoda; Anisakis simplex (s.s.), Contracaecum rudolphii A and Hysterothylacium aduncum, Nematoda; Echinorhynchus gadi, Acanthocephala). For the first time, the marine ectoparasite A. belones was recorded from the gills of garfish inside the Baltic Sea, indicating its ability to survive the

Penetrating Orbital Injury From a Needlefish.

Orbital penetrating injuries are an unfortunately common occurrence; however, those from marine animals are rare. Injuries from marine animals can be quite profound and there are no known reports of complete visual recovery after an orbital penetrating injury. Complications range from secondary infection to a carotid-cavernous fistula, which can complicate the management of these patients. We report a case of penetrating orbital injury from a needlefish with complete return of visual function after surgical removal of foreign debris and an extended course of antibiotics.
