Malignancies of the biliary tree, particularly the extrahepatic bile ducts, remain difficult clinical problems. Because experience with these uncommon tumors has been limited to a small number of centers, meaningful clinical trials have been difficult to perform. Complete resection remains the most effective therapy, but is usually possible in the minority of patients. Palliating the effects of biliary obstruction is thus often the primary therapeutic goal. Authors collected three cases of neuromas situated in the wall of extrahepatic bile ducts, which occluded lumen and caused jaundice. In two cases without any evidence of previous surgery or cholelithiasis, neuroma was localized in ductus choledochus. After excision of ductus choledochus and cholecystectomy, we performed hepaticojejunostomy Roux--en Y. In the last patient, 7 years after cholecystectomy, neuroma localized in hepatic duct was treated by T-tube. The results of reoperations on the extrahepatic bile ducts in 82 patient s performed, mainly, for undetected and uncorrigated changes in the extrahepatic bile ducts caused by diagnostic, tactical and technical errors made during the first operation are presented.

An elongated solid lesion observed on ultrasonography and CT in the biliary tree causing a smooth filling defect on cholangiography was observed in 2 patients. No tumor was observed in the liver parenchyma either on radiography or at operation. Histopathology showed hepatocellular carcinoma. After removal of the intraductal tumors, recurrence was observed in 2 and 6 months, respectively.

Thirty-seven patients with carcinoma of the extrahepatic bile ducts (EHBD), diagnosed over 14 years from 1974 to 1987, were studied retrospectively. The most common presenting complaint was jaundice, followed by abdominal pain, nausea and vomiting, and weight loss. The majority of EHBD carcinomas were located at the middle third of the bile duct (43.2%), followed by upper third bile duct (37.8%), and lower third bile duct (18.9%).
