Birth asphyxia in a resource-limited setting: Challenges and implications for neonatal care in Nepal.

Birth asphyxia is a common problem especially in low resource developing countries like Nepal during the delivery. Birth asphyxia remains a major cause of mortality and morbidity worldwide and specially in developing countries like Nepal. Despite the advancement in medical science birth asphyxia still remains a major problem in newborn and preventing the risk factors after evaluating remains the center of reducing the prevalence. Proper healthcare protocols, early intervention and appropriate management is important for preventing long term sequalae and mortality. This study aims to assess the

Mortality among very low birth weight infants after hospital discharge in a low resource setting.

BACKGROUND: Early discharge of very low birth weight infant (VLBW) in low resource settings is inevitable but to minimize mortality of these infants after discharge we need to identify the death attributes. METHOD: A prospective cohort was conducted among 190 VLBW infants discharged from Mulago Special Care Baby Unit (SCBU) with discharge weight of < 1500 g over an 8 months period. These infants were followed up with the aims of determining the proportion dead 3 months after discharge, identifying factors associated and possible causes of death. Relevant data were captured, transferred in to

Safely Supporting the Establishment of Breastfeeding in the Setting of Fentanyl Use Before the Birth Hospitalization.

The increasing prevalence of nonprescribed fentanyl use during pregnancy presents a clinical dilemma for lactation counseling, because pharmacokinetic data on fentanyl transfer into breast milk remain limited. For families who choose it, human milk for newborns and infants is the gold standard; clinicians' deviation from this recommendation requires a clear rationale and supporting evidence. The 2023 Academy of Breastfeeding Medicine's clinical protocol, "Breastfeeding in the Setting of Substance Use and Substance Use Disorder" summarizes available evidence regarding substance use and breastfe

Recognition of and Response to neonatal intrapartum-related complications in home-birth settings in Bangladesh.

Intrapartum-related complications (previously called 'birth asphyxia') are a significant contributor to deaths of newborns in Bangladesh. This study describes some of the perceived signs, causes, and treatments for this condition as described by new mothers, female relatives, traditional birth attendants, and village doctors in three sites in Bangladesh. Informants were asked to name characteristics of a healthy newborn and a newborn with difficulty in breathing at birth and about the perceived causes, consequences, and treatments for breathing difficulties. Across all three sites 'no movement

Birth outcomes for women planning Vaginal Birth after Caesarean (VBAC) in midwifery led settings: A systematic review and meta-analysis.

PROBLEM: There is a limited knowledge base available to midwives, obstetricians and women planning vaginal birth after caesarean (VBAC), impeding their ability to make informed choices regarding planned place of birth. BACKGROUND: A VBAC is associated with fewer complications for both mother and baby, but little is known on the safety and success of planning a VBAC in midwifery led settings such as birth centres and home birth, compared to obstetric led settings. AIM: To synthesise the findings of published studies regarding maternal and neonatal outcomes with planned VBAC in midwifery setti

Adapting the preterm birth phenotyping framework to a low-resource, rural setting and applying it to births from Migori County in western Kenya.

BACKGROUND: Preterm birth is the leading cause of neonatal and under-five mortality worldwide. It is a complex syndrome characterized by numerous etiologic pathways shaped by both maternal and fetal factors. To better understand preterm birth trends, the Global Alliance to Prevent Prematurity and Stillbirth published the preterm birth phenotyping framework in 2012 followed by an application of the model to a global dataset in 2015 by Barros, et al. Our objective was to adapt the preterm birth phenotyping framework to retrospective data from a low-resource, rural setting and then apply the adap

The emergence of birth territory theory: Understanding the experience of giving birth in the home versus the hospital setting in Baringo and Nakuru counties of Kenya.

A comprehensive, qualitative approach to research was utilized to explore the ways in which birth territory theory can illuminate the birthing experiences of women living in Baringo and Nakuru counties, and how birth territory theory can be decentered through an African case study. Research was conducted using a combination of qualitative methods including participant observation, semi-structured interviews, and focus groups with adult women living in Baringo or Nakuru counties in Kenya who have previously given birth. Results suggest that multiple societal factors contribute to the experience

Amniotic Fluid Proteasome and Immunoproteasome in the Setting of Intra-Amniotic Infection, Inflammation, and Preterm Birth.

Preterm birth is an important determinant of neonatal morbidity and mortality and intra-amniotic infection (IAI) and inflammation play a causative role. The constitutive proteasome and immunoproteasome are key players in maintenance of proteostasis and their alteration outside pregnancy has been linked to pathogenesis of numerous inflammatory diseases. Our goal was to evaluate the levels, activities, and potential origin of amniotic fluid (AF) proteasome in women with preterm birth induced by infection and/or inflammation. Total proteasome and immunoproteasome concentrations were measured in A

Outcomes by birth setting and caregiver for low risk women in Indonesia: a systematic literature review.

BACKGROUND: Care for women during pregnancy, labour, birth and the postpartum period is essential to reducing maternal and neonatal mortality and morbidity, however the ideal place and organisation of care provision has not been established. The World Health Organization recommends a two-tier maternity care system involving first-level care in community facilities, with backup obstetric hospital care. However, evidence from high-income countries is increasingly showing benefits for low risk women birthing outside of hospital with skilled birth assistance and access to backup care, including lo

Core temperature after birth in babies with neonatal encephalopathy in a sub-Saharan African hospital setting.

KEY POINTS: Therapeutic hypothermia (HT) to 33.0-34.0°C for 72 h provides optimal therapy for infants with neonatal encephalopathy (NE) in high-resource settings. HT is not universally implemented in low- and middle-income countries as a result of both limited resources and evidence. Facilitated passive cooling, comprising infants being allowed to passively lower their body temperature in the days after birth, is an emerging practice in some West African neonatal units. In this observational study, we demonstrate that infants undergoing facilitated passive cooling in a neonatal unit in Accra,

TLR-4-dependent and -independent mechanisms of fetal brain injury in the setting of preterm birth.

In this study, we sought to assess how essential activation of toll-like receptor 4 (TLR-4) is to fetal brain injury from intrauterine inflammation. Both wild-type and TLR-4 mutant fetal central nervous system cells were exposed to inflammation using lipopolysaccharide in vivo or in vitro. Inflammation could not induce neuronal injury in the absence of glial cells, in either wild-type or TLR-4 mutant neurons. However, injured neurons could induce injury in other neurons regardless of TLR-4 competency. Our results indicate that initiation of neuronal injury is a TLR-4-dependent event, while pro

Obstetric Violence Through the Perspectives of Women and Midwives in Hospital Birth Settings: A Qualitative Study.

Background/Objectives: Obstetric violence is increasingly recognised as a violation of women's autonomy, dignity, and right to respectful maternity care, yet its interpretation may differ between women receiving care and midwives providing care within hospital systems. This study explored obstetric violence from the perspectives of women who perceived their childbirth as traumatic and midwives working in hospital birth settings. Methods: This qualitative study was conducted in one university hospital and two public hospitals in Türkiye. Data were collected through individual semi-structured in

The emergence of the vertical birth in Ecuador: an analysis of agenda setting and policy windows for intercultural health.

Maternal mortality continues to claim the lives of thousands of women in Latin America despite the availability of effective treatments to avert maternal death. In the past, efforts to acknowledge cultural diversity in birth practices had not been clearly integrated into policy. However, in Otavalo (Ecuador) a local hospital pioneered the implementation of the 'Vertical Birth'-a practical manifestation of an intercultural health policy aimed at increasing indigenous women's access to maternity care. Drawing on agenda-setting theory, this qualitative research explores how the vertical birth pra

Fear causes tears - perineal injuries in home birth settings. A Swedish interview study.

BACKGROUND: Perineal injury is a serious complication of vaginal delivery that has a severe impact on the quality of life of healthy women. The prevalence of perineal injuries among women who give birth in hospital has increased over the last decade, while it is lower among women who give birth at home. The aim of this study was to describe the practice of midwives in home birth settings with the focus on the occurrence of perineal injuries. METHODS: Twenty midwives who had assisted home births for between one and 29 years were interviewed using an interview guide. The midwives also had exper
