BACKGROUND: Black widow spiders are distributed worldwide and, although rarely fatal, account for significant morbidity. Diagnosis can be challenging, and children are at risk of increased morbidity due to their small size.

Because there are few recent studies of black widow spider (Latrodectus Mactans) envenomation, a six-year retrospective study was conducted by patients admitted to or discharged from our hospital with this diagnosis. Priapism in children after a black widow spider bite is a rare phenomenon with only a few case reports noted in the literature. Black widow bites are commonly associated with pain, muscle cramping, hypertension, and tachycardia. The syndrome, latrodectism, is characterized by painful muscle rigidity and autonomic disturbances such as tachycardia, hypertension, and diaphoresis. Symptoms typically last for 1-3 days. Highly purified alpha-latrotoxin from the black widow spider venom (alpha-LTX) consists of two polypeptides with mol. wts of 130,000 and 8000 (LMWP). We have isolated two low mol. wt proteins LMWP and LMWP2 from the low mol. wt fraction of this venom. The chemical properties of these proteins and partial amino acid sequence of novel protein LMWP2 were studied. Electrical activities of thin hippocampal sections of the guinea pig were studied in vitro during and after administration of black widow spider venom (BSV). In CA3 neurons, BSV induced spontaneous discharges which were suppressed by Co2+ or Mn2+. During BSV administration, trains of potential changes resembling excitatory postsynaptic potentials (EPSPs) were recorded intracellularly. The EPSP-like potentials were blocked by TTX. BSV seems to induce impulses in presynaptic fibers or terminals and thereby facilitates release of neurotransmitters.

The symptomatology of envenomation by the Mediterranean Black Widow Spider, Latrodectus tredecimguttatus, is reviewed. The results confirm the hypothesis that the homeopathic remedy, Tarentula hispanica, is derived from this spider, not from the Wolf spider, Lycosa tarentula.

The most severe cases of arachnidism are those due to Latrodectus tredecimguttatus spider-bite. The Mediterranean area is the habitat of the L. tredecimguttatus species. In the last few years no series of patients with latrodectism has been reported in Spain. The poison ivy does not normally occur in Slovakia, like in the Czech Republic, but it can be introduced. The venom of the black widow spider is an effective weapon for capturing prey. It is a mixture of various active substances containing a protein neurotoxin called α-latrotoxin (α-LTX).
