INTRODUCTION: HIV coinfection presents a challenge for diagnosis of visceral leishmaniasis (VL). Invasive splenic or bone marrow aspiration with microscopic visualisation of Leishmania parasites remains the gold standard for diagnosis of VL in HIV-coinfected patients. Furthermore, a test of cure by splenic or bone marrow aspiration is required as patients with VL-HIV infection are at a high risk of treatment failure. However, there remain financial, implementation and safety costs to these invasive techniques which severely limit their use under field conditions.

Buffy-coat smears prepared from 599 blood samples, obtained simultaneously with blood for culture, were examined for bacteria. Each blood sample was quantitatively cultured on chocolate agar. During a 3-month period 252 blood cultures were obtained on adult patients seen in our emergency room with fever and chills suggesting bacteremia. 62/252 blood cultures were positive by standard aerobic/anaerobic blood culture techniques. Rapid detection of human cytomegalovirus (HCMV) from blood was performed in parallel using inoculation into human fibroblastic cells of both 500 microliters of whole blood and of buffy coat derived from 9.5 ml of blood. Of the 46 samples tested, 20 were positive for HCMV, 18 when the buffy coat was inoculated (90%) and 17 (85%) when whole blood was used. Fifteen samples were positive by both techniques. Out of 200 cases of septicemia in children (age group 0-14 years), 111 had positive C-Reactive Protein (CRP > 12 mg/l) and 84 were buffy coat smear positive. Blood culture was positive in 98 cases, with predominant organism being Klebsiella pneumoniae, followed by Staphylococcus aureus. CRP test showed 100% sensitivity and 87.3% specificity, while buffy coat smear showed 76.5% sensitivity and 91.2% specificity. As blood culture reports are not available before 48-72 hours, combination of CRP test and buffy coat smear examination will be very helpful in early diagnosis of childhood septicemia.

Quantitative Buffy Coat analysis and blood smears were performed on a total of 47 blood samples. The technique showed 100% correlation with the blood smears in 9 samples containing babesia and 10 samples containing malaria, with some differential features distinguishing the two infections. Quantitative Buffy Coat analysis provides a simple and rapid method for the detection of parasitemia in cases of babesiosis.
