Innovations in blood donor screening and blood collection.

Blood donor screening and blood collection are two of the donor-focused activities inherent in the process of maintaining an adequate blood supply. Donor screening represents procedures to provide safety for both donors as well as recipients. Donor medical history questions are so cumbersome that self-administered manual, computer-assisted, or on-line donor questions are under consideration in order to reduce time involved in the screening process. Blood collection procedures remain primarily based on whole blood donations, but apheresis donations are increasing steadily. The yield of multiple

Update of data from the world apheresis association (WAA) registry.

The WAA registry has been active since 2002. It allows bed side registration of safety and efficacy data. The data each center enters is accessible for its own use but also used for merged analysis. Most types of procedures are represented. Treatments of many severe diseases as well as the collection of autologous and donor cells for therapeutic use especially in oncologic diseases are recorded. Previous reports have shown a successive reduction in adverse events (AE) over the years. The aim of the present report is to update data of the risk for AE during the years from 2013 to Oct 2024. The Italian registry of therapeutic apheresis in SISTRA: Year of activity 2023.

The Italian Registry of Therapeutic Apheresis (IRTA) collects clinical data on patients undergoing therapeutic apheresis procedures throughout the national territory, with the main objective of improving the quality and safety of the care provided to the patient. Given the importance of centralizing the collection and analysis of information on therapeutic apheresis, the National Blood Center (NBC), at the request of the Italian Scientific Society of Hemapheresis and Cellular Manipulation (SIdEM), has included IRTA in the Information System of Transfusion Services (SISTRA), which is the inform

Apheresis for lupus erythematosus: state of the art.

PP is a safe, expensive, labor-intensive procedure. Its absolute SLE indications include hyperviscosity, cryoglobulinemia, pulmonary hemorrhage and TTP. PP may be useful in cyclophosphamide-resistant, serious, organ-threatening disease. It may be potentially useful in the antiphospholipid syndrome or mothers or children at risk for congenital heart block. Refinements in apheresis technology may expand the indications for PP.

Clostridium perfringens in apheresis platelets: an unusual contaminant underscores the importance of clinical vigilance for septic transfusion reactions (CME).

BACKGROUND: Posttransfusion sepsis is typically caused by aerobic bacteria in apheresis platelets (PLTs) that escape detection by routine quality control cultures performed on every donation before components are distributed. We report the first case to implicate an anaerobic isolate, Clostridium perfringens, in apheresis PLTs and investigate its detection in vitro by approved tests. STUDY DESIGN AND METHODS: The C. perfringens strain was inoculated at high (10-100 colony-forming units [CFUs]/mL) or low (1-10 CFUs/mL) concentrations into apheresis PLTs and evaluated for growth over 5 to 7 day

Powdered sorbent liver dialysis and pheresis in treatment of hepatic failure.

The Liver Dialysis Unit (the Unit) is a liver-assist device that employs hemodiabsorption (dialysis of blood against powdered sorbents) to selectively remove numerous small molecular weight toxins of hepatic failure. The Unit has been cleared by the Food and Drug Administration and is indicated and marketed for treatment of acute hepatic encephalopathy (AHE) due to decompensation of chronic liver disease (A-on-C) or fulminant hepatic failure (FHF). Controlled, prospective, and randomized studies of liver dialysis were conducted at several centers, enrolling 56 patients with AHE, grades II-IV w

Transfusion medicine: A research agenda for the coming years.

The important scientific and clinical advances of the last century in transfusion medicine include methods for avoiding hemolytic transfusion reactions and preventing transmission of viral infectious diseases. The next great clinical advances will require improving the efficacy and safety of transfusions, as well as acknowledgement of the now proven serious complications of transfusion, including nosocomial infection, thrombosis, inflammation and multi-organ failure. Possible strategies include (1) universal leukoreduction to mitigate transfusion immunomodulation effects and improve storage co

Complications of blood donation reported to haemovigilance systems: analysis of eleven years of international surveillance.

BACKGROUND AND OBJECTIVES: The International Haemovigilance Network collects aggregate data on complications of blood donation from member haemovigilance systems (HVS). We analysed the data collected in 2006-2016 in order to learn from it and consider future improvements. MATERIALS AND METHODS: National HVS entered annual data on donation complications and on annual whole blood and apheresis donations in the 'ISTARE' (International Surveillance of Transfusion Adverse Reactions and Events) online database. We calculated national and aggregate donation complication rates. RESULTS: Twenty-four

An Annual Review of Important Apheresis Articles in 2025 From the American Society for Apheresis Attending Physician Subcommittee.

Apheresis medicine is a continually evolving field with numerous studies published each year. To help apheresis practitioners stay up to date with the current literature, members of the American Society for Apheresis (ASFA) Attending Physician Subcommittee identified and summarized 10 seminal articles published in 2025, from the field of apheresis medicine. PubMed was used to identify articles published in four topics including donor apheresis, therapeutic apheresis, apheresis education, and apheresis collection for cellular therapy. These articles met at least one of the following criteria: n

What's in a Name: Your PLEX or PEX Is Our TPE.

In the American Society for Apheresis (ASFA) guidelines, the abbreviation for therapeutic plasma exchange is standardized as TPE. However, the term PLEX (derived from PLasma EXchange), among others, is not infrequently encountered in clinical practice and in publications. Search queries were performed in PubMed using therapeutic plasma exchange, plasma exchange, "therapeutic plasma exchange," "plasma exchange," TPE plasma, PLEX plasma, PEX plasma, TPE, PLEX, and PEX. As compared to TPE, the term PLEX was more commonly used in articles related to neurology, nephrology, and rheumatology, while t

The use of apheresis in immune renal disorders.

Rapidly progressive glomerulonephritis (RPGN) is often associated with the presence of autoantibodies. Included in this group are the glomerulonephritides associated with anti-GBM antibody (Goodpasture's syndrome). IgA mesangial deposition (the renal component of Henoch-Schönlein purpura), lupus erythematosus, cryoglobulinemia and the antineutrophil cytoplasmic antibody (ANCA)-associated pauci-immune group. In each of these cases, apheresis may provide a therapeutically useful option. Apheresis has also been found useful in certain types of antibody-mediated transplant rejection and in lowerin

Access in therapeutic apheresis.

Various modes may be used to perform apheresis, such as plasma exchange, plasmapheresis, immunoadsorption, and blood perfusion. The vascular access used for acute procedures may be sufficiently supplied by two peripheral veins or catheters placed in the femoral vein or the right internal jugular vein. For chronic treatment it might be necessary to place an arteriovenous fistula or graft. The risks involved when using the various accesses is discussed, as is the insertion technique of the femoral vein catheter. The insertion of catheters into larger vessels is preferably controlled by ultrasoun

Universal leukocyte reduction.

Leukocyte reduction of blood components, in the United States, is generally reserved for conditions in which a clinical indication has been documented. There is no evidence that either Creutzfeldt-Jakob disease or variant Creutzfeldt-Jakob disease are transmitted by transfusion in humans or that leukocyte reduction of blood components could reduce their transmission. A number of adverse outcomes following transfusion are alleged to be the result of white blood cells. At this point in time, there are insufficient clinical data to justify the universal leukocyte reduction of blood components.

In vivo recovery and in vitro quality assessment of apheresis red blood cells stored for 42 days in non-DEHP disposable sets.

BACKGROUND AND OBJECTIVES: Di(2-ethylhexyl) phthalate (DEHP) has been used in blood bag manufacturing for decades, but concern about its potential toxicity has prompted regulatory efforts to eliminate it from medical devices, including blood bags. As manufacturers transition to non-DEHP materials, evaluation is required to ensure blood component quality in modified bag systems. Investigational Trima Accel disposables were developed using plasticizers approved as DEHP alternatives per the European Pharmacopoeia. Red blood cells (RBCs) were collected, and two consecutive studies evaluated in vit

Prion safety of transfusion plasma and plasma-derivatives typically used for prophylactic treatment.

Reports about transfusion-related transmissions of variant Creutzfeldt-Jakob disease have urged the need for more information regarding the risk for prion contaminated units in the blood supply and the safety of transfusion plasma and biopharmaceuticals derived from this precious raw material. According to a possible epidemiological model, the risk in many European countries is the same or lower than that of human immunodeficiency virus. Comprehensive investigations have shown that the prion safety margin of both single-donor and pooled solvent/detergent treated transfusion plasma is high. Fur

Predicting Non-Target Neutrophil Collection During HPC(A): Implications for Research Protocols in Apheresis.

Neutrophils are unintentionally co-collected during HPC(A) procedures using mononuclear cell collection protocols. Although non-target granulocyte co-collection is generally clinically acceptable, excessive neutrophil content may compromise certain translational research applications. This study aimed to develop a predictive model for neutrophil yield during routine HPC(A). A retrospective single-center analysis of 377 HPC(A) procedures performed between January 2023 and April 2026 was conducted. Donor demographics, hematological parameters, procedural characteristics, and collection efficienc
