Machine-learning prediction models for any blood component transfusion in hospitalized dengue patients.

BACKGROUND: Blood component transfusions are a common and often necessary medical practice during the epidemics of dengue. Transfusions are required for patients when they developed severe dengue fever or thrombocytopenia of 10×109/L or less. This study therefore investigated the risk factors, performance and effectiveness of eight different machine-learning algorithms to predict blood component transfusion requirements in confirmed dengue cases admitted to hospital. The objective was to study the risk factors that can help to predict blood component transfusion needs. METHODS: Eight predicti

Methodological considerations for linked blood donor-component-recipient analyses in transfusion medicine research.

In recent years, there has been a concerted effort to improve our understanding of the quality and effectiveness of transfused blood components. The expanding use of large datasets built from electronic health records allows the investigation of potential benefits or adverse outcomes associated with transfusion therapy. Together with data collected on blood donors and components, these datasets permit an evaluation of associations between donor or blood component factors and transfusion recipient outcomes. Large linked donor-component recipient datasets provide the power to study exposures rel

[Indications of the different components of blood and outcome of transfusion practices in postpartum hemorrhage].

Despite a high incidence of mortality associated with post-partum haemorrhage, indications for transfusion of blood and its components in obstetrics remain ill defined. Transfusion is often massive if needed. Transfusion management depends upon appreciation of blood loss, with systematic research of associated disseminated intravascular coagulopathy (DIC) or other preexisting coagulation disorders. If DIC is ruled out, the classic rules of volemic substitution apply, besides certain qualitative characteristics intrinsic to the blood components or products used. Treatment of DIC calls for fresh

The specifications of blood components for neonatal transfusion: a European study and critical review.

BACKGROUND: Blood component safety measures implemented by Blood Establishments (BE) specifically for neonates are frequently precautionary rather than evidence based. We conducted a European survey on specifications for neonatal red blood cells, platelet, and plasma transfusions. METHODS: The survey was sent to transfusion experts in BEs in 38 European countries between May 2023 and January 2024. We received responses from 20/38 countries. RESULTS: Many BEs provide neonatal components with additional safety features not routinely applied for adults, and we found evidence of significant vari

Infectious risks associated with the transfusion of blood components and pathogen inactivation in Japan.

Even after the implementation of the nucleic acid amplification testing (NAT) system, there remains a residual risk of viral transmission through blood transfusion because of the limited sensitivity of the reagents used and the pooling strategy of the current NAT system. From the calculation using NAT yield and the length of the window period, we presume that we will obtain 0.75 donations for human immunodeficiency virus and 0.58 donations for hepatitis C virus annually that are individual donation-NAT positive but 50-individual pool-NAT negative, figures that are comparable with those in othe

[Indications of blood components and outcome of transfusion practices in hemorrhage of multiple trauma].

Bleeding occurring in a patient with multiple trauma has an unpredictable evolution; blood losses are often very important and their origins mostly unclear. These problems should not prevent the use of a strategy for optimal use of blood components. Indications for packed red cells, fresh frozen plasma, platelets and coagulation factors are discussed. Indications for packed red cells are discussed in relation with the patient's conditions (myocardial, cerebral and pulmonary functions) and the clinical status. If the patient is shocked, transfusion must be ordered to maintain a haemoglobin leve

Blood donor and component management strategies to prevent transfusion-related acute lung injury (TRALI).

OBJECTIVE: Discuss the pros and cons of using donor and blood product-management strategies to prevent transfusion-related acute lung injury (TRALI). DATA SOURCE: A review of the literature was performed. RESULTS: Despite therapeutic advances in pulmonary and critical care medicine, TRALI is now considered to be one of the leading causes of transfusion-associated mortality, and thus determining how to prevent TRALI is extremely important. Donor and product-management strategies to prevent this life-threatening condition have been suggested, but because of gaps in our understanding of TRALI,

Transfusion-transmitted cytomegalovirus: behaviour of cell-free virus during blood component processing. A study on the safety of labile blood components in Switzerland.

BACKGROUND: Nowadays, most blood products are leukocyte-reduced. After this procedure, the residual risk for transfusion transmitted cytomegalovirus (TT-CMV) is mostly attributed to cell-free viruses in the plasma of blood donors following primary infection or viral reactivation. Here, objectives are: 1) to study the behaviour of cell-free CMV through the blood component processing; 2) to determine the anti-CMV seroprevalence, the level of viremia, the window-period in blood donor population; and 3) to identify cases of TT-CMV in bone marrow transplant (BMT) recipients. Blood Transfusion Components Inducing Severe Allergic Reactions: The First Case of Kounis Syndrome Induced by Platelet Transfusion.

Kounis syndrome is a multisystem and multidisciplinary disease affecting the circulatory system that can be manifested as spasm and thrombosis. It can occur as allergic, hypersensitivity, anaphylactic, or anaphylactoid reactions associated with the release of inflammatory mediators from mast cells and from other interrelated and interacting inflammatory cells, including macrophages and lymphocytes. A platelet subset with high- and low-affinity IgE surface receptors is also involved in this process. Whereas the heart, and particularly the coronary arteries, constitute the primary targets of inf

Association of blood component transfusions with neurodevelopmental impairment in preterm infants by gestational age.

BACKGROUND: Transfusion is frequently prescribed for preterm infants and known to be associated with adverse outcomes. We evaluated the association between transfusions of red blood cells (RBCs), platelets, and plasma in preterm infants and neurodevelopmental impairment (NDI). METHODS: This retrospective cohort study included 706 preterm infants. The primary outcome was severe NDI diagnosed at 18-30 months of corrected age. After adjusting, weighted logistic regression was performed to evaluate the associations between each transfusion type, the number of transfusions, and severe NDI. RESULT

Storage and transfusion of infected autologous blood or components: a survey of North American laboratories.

CONTEXT: Many patients request that autologous blood or components be collected and available for use during scheduled surgical or invasive medical procedures to avoid exposure to human immunodeficiency virus (HIV), hepatitis B virus (HBV), and hepatitis C virus (HCV) from allogeneic transfusions. Some patients from whom autologous blood is collected are themselves infected with HIV, HBV, or HCV. However, unlike HIV-, HBV-, or HCV-infected allogeneic blood and components, which must be excluded from the community blood supply, infected autologous blood and components are allowed to be stored i

Effect of donor, component, and recipient characteristics on hemoglobin increments following red blood cell transfusion.

Significant research has focused individually on blood donors, product preparation and storage, and optimal transfusion practice. To better understand the interplay between these factors on measures of red blood cell (RBC) transfusion efficacy, we conducted a linked analysis of blood donor and component data with patients who received single-unit RBC transfusions between 2008 and 2016. Hemoglobin levels before and after RBC transfusions and at 24- and 48-hour intervals after transfusion were analyzed. Generalized estimating equation linear regression models were fit to examine hemoglobin incre

Activated clotting time of thrombelastography (T-ACT) predicts early postinjury blood component transfusion beyond plasma.

INTRODUCTION: Rapid thrombelastography (rTEG) has been advocated as a point-of-care test to manage trauma-induced coagulopathy. rTEG activated clotting time (T-ACT) results become available much sooner than other rTEG values, thus offering an attractive tool to guide blood component transfusion in a hemorrhagic shock. We hypothesize that patients with a prolonged T-ACT require replacement of platelets (Plts) and cryoprecipitate (Cryo) in addition to plasma to correct trauma-induced coagulopathy. METHODS: A prospective trauma registry was reviewed for patients with an r-TEG available within 3

The impact of blood component transfusion practices on patient survival after abdominal aortic aneurysm surgery.

BACKGROUND: The aim of the present study was to investigate the blood transfusion practice in patients operated for abdominal aortic aneurysm (AAA) with special emphasis on massive transfusion in cases with rupture. MATERIAL AND METHODS: From a database, 504 patients operated for AAA were stratified into 2 groups; an early transfusion period (1992-1999) and a late transfusion period (2000-2008) to evaluate the changes in transfusion practices over the course of time. RESULTS: Patients operated for nonruptured AAA (n = 330) showed a decreased mortality rate from 4% (early transfusion period)

Factors Influencing Blood Component Availability: The Views of Transfusion Service Leaders in Riyadh, Saudi Arabia.

BACKGROUND: Maintaining the availability of blood components remains a major challenge for transfusion services and healthcare institutions. Blood shortages can disrupt clinical care, complicate prioritization decisions, and place substantial operational pressure on transfusion services. Limited attention has been given to the perspectives of transfusion service personnel directly involved in shortage management, particularly within tertiary healthcare settings in Riyadh, Saudi Arabia. METHODS: A cross-sectional questionnaire-based study was conducted using a structured survey designed to ass
