The value of blood glucose testing over urine sugar testing is emphasized, and techniques of self-monitoring of blood glucose in children are presented. Test strips and reflectance meters currently on the market are described and evaluated. Useful suggestions are made for introducing blood glucose testing to children, stressing flexibility in the frequency of testing, at least initially, and proper education in the interpretation of testing results.

The association between obesity and diabetes supports an endocrine role for the adipocyte in maintaining glucose homeostasis. Here we report that mice lacking the adipocyte hormone resistin exhibit low blood glucose levels after fasting, due to reduced hepatic glucose production. This is partly mediated by activation of adenosine monophosphate-activated protein kinase and decreased expression of gluconeogenic enzymes in the liver. Hyperglycemia can be a significant problem in the trauma population and has been shown to be associated with increased morbidity and mortality. Hyperglycemia in the trauma patient, as in other critically ill patients, is caused by a hypermetabolic response to stress and seems to be an entity of its own rather than simply a marker.
