Generation of enzyme-converted type O whole blood from type A whole blood: A translational study using swine whole blood.

BACKGROUND: Low-titer O whole blood (LTOWB) is increasingly used to improve trauma outcomes, but maintaining supplies remains challenging, particularly in austere environments. This study evaluated EnzAO1 (FpGalNAcDeAc) and EnzAO2 (FpGalNase) to generate enzyme-converted O type whole blood (ECO-WB) from A type swine whole blood (WB). STUDY DESIGN AND METHODS: WB from 16 A type and 4 O type swine were collected in Citrate Phosphate Dextrose Adenine-1 (CPDA-1). Type A WB was randomized to enzymatic conversion at either room temperature or 4°C. Each unit was divided into 45 mL aliquots and treat

In vitro compatibility of type-B feline blood with canine blood of different DEA 1 phenotypes and with type-A feline blood.

Canine xenotransfusion may be a temporary emergency option for type-B cats when compatible feline blood is unavailable, but the in vitro compatibility of type-B feline blood with canine blood has not been fully characterized. This study evaluated and compared the slide-agglutination crossmatch compatibility of type-B feline blood with canine blood with different dog erythrocyte antigen (DEA) 1 phenotypes and with type-A feline blood. Blood samples were obtained from 70 type-B cats, 17 type-A cats, 26 DEA 1-positive dogs, and 18 DEA 1-negative dogs. Among 300 major crossmatches, incompatibility

Updates on practical ABC blood compatibility testing in cats.

In feline practice, blood groups were considered unimportant until the 1980s. Since then much has been learned. The most important blood group system in cats is the AB (renamed here as ABC) blood group system consisting of blood types A, B and AB (better referred to as C). Type B cats have strong anti-A alloantibodies potentially leading to incompatibility reactions during A-B mismatched transfusions or neonatal isoerythrolysis (NI) in type A and C (AB) kittens born to type B queens. Acute hemolytic transfusion reactions as well as NI have been clinically well documented in cats. Immunological

The risk of an overt hemolytic transfusion reaction following the use of an immediate spin crossmatch.

The major crossmatch must include an anti-human globulin test, unless the transfusion recipient has no apparent significant unexpected antibodies, in which case the use of only an immediate spin crossmatch method is considered acceptable. However, a minority of laboratories utilize only an immediate spin crossmatch as their routine major crossmatch, possibly because contemporary antibody screening tests occasionally miss detecting some unexpected antibodies, and these missed antibodies are more often detected by the anti-human globulin crossmatch than by the immediate spin crossmatch. In the p

Molecular insights into blood groups and implications for blood transfusion.

In recent years technologic advances have led to the understanding of the molecular genetics of many blood group antigens. This review highlights the molecular basis of selected antigens and describes implications for transfusion medicine. The knowledge can be used in the following clinical settings to genotype a patient: 1) to identify a fetus at risk for hemolytic disease of the newborn 2) to genotype patients who have been recently transfused or whose erythrocytes have a positive direct antiglobulin test 3) to determine which phenotypically Fy(b-) patients have the FY*B gene, 4) to genotype

Alloimmunization of a dog erythrocyte antigen 1- dog transfused with weakly dog erythrocyte antigen 1+ blood.

BACKGROUND: Acute hemolytic transfusion reactions because of dog erythrocyte antigen (DEA) 1 sensitization after mismatched transfusions are serious complications. Dog erythrocyte antigen 1 expression varies from negative to weakly to strongly positive. OBJECTIVES: To assess alloimmunization after transfusion of weakly DEA 1+ blood to a DEA 1- dog. ANIMALS: One DEA 1- recipient and 1 weakly DEA 1+ donor, and 106 control dogs. METHODS: Long-term follow-up study. Matched for DEA 3, 4, 5, and 7, Dal, and Kai 1 and 2, weakly DEA 1+ donor packed red blood cells (RBCs) were transfused 3 times (0.

[When A is not A anymore: problems and pitfalls with blood group typing].

Correct blood group typing is a prerequisite for transfusion. In most cases blood group determination is without problems; however, in individual cases various factors can complicate blood group determination and sometimes lead to confusing findings. For a better understanding the clinician should have basic knowledge of blood typing. Blood group determination usually covers the AB0 blood groups, Rhesus and Kell systems; in addition, a direct Coombs test and an antibody screening test for the detection of irregular antibodies in the recipient are performed. Confusion of patients, blood samples

Molecular analysis and transfusion management in a rare case of cis-AB blood group: A report from India.

Molecular characterization of a rare cis-AB blood group has not been done in the Indian subcontinent. Herein, we report a case of A2B3 blood group in an Indian patient which was subsequently confirmed to be a case of cis-AB phenotype. Blood grouping was performed by the column agglutination technique (CAT), conventional tube technique (CTT) and subsequently, whole exome sequencing for molecular analysis. The patient was initially typed as AB, RhD positive in forward grouping. However, serum grouping showed agglutination (2+) with the B red cells in CAT. In CTT, an extra reaction was observed w

[New crossmatch procedure: computer crossmatch experience in a blood service division].

We established a computer crossmatch system in our hospital to commence 24-hour-a-day blood services. About 97% of red blood products were shipped using this system. Among them, only one patient developed a mild delayed hemolysis. ABO-mismatched transfusion or release of incompatible blood components was not reported at all. Since the turnaround time is shortened, cost reductions were achieved by reductions in blood units tied up in the crossmatched blood inventory, leading to reduced blood ordering.

Phenotyping of minor blood groups (C, c, E, e, and Mia) using a paper-based device and image-based high-throughput detection.

Testing ABO and D (Rh) are priorities before blood transfusion, and the minor blood group antigen should be matched to reduce alloimmunization. In a recent study, a paper-based device (PAD) was developed for C, E, c, e, Mia phenotyping, combined with image-based high-throughput detection. A total of 148 ethylenediamine tetra acetic acid (EDTA) blood samples were used to evaluate and create an optimal criterion using OpenCV for high-throughput interpretation. Results revealed that anti-C, -c, -E, -e, and -Mia were successful for blood group phenotyping with area under the receiver operating cha

Limitations of the crossmatch for detection of incompatibility between A2B red blood cells and B patient sera.

A recent evaluation of the immediate-spin crossmatch has revealed limitations when used for the detection of ABO incompatibilities in tests between group B patient sera and group A2B donor red blood cells. Presented with the option of deleting the anti-globulin crossmatch, the study reported here was conducted to determine whether the anti-globulin crossmatch would detect the ABO incompatibilities missed by the immediate spin. Results of 1,000 crossmatches between group B patient sera and group A2B red blood cells showed that the anti-globulin phase was capable of detecting a higher percentage

Impact of haemoglobin vesicles on pretransfusion testing and mitigation strategies.

BACKGROUND AND OBJECTIVES: Haemoglobin-based oxygen carriers (HBOCs) are intended to treat major bleeding, particularly in emergencies. While haemoglobin vesicles (HbVs)-liposome-encapsulated haemoglobin with a long shelf life-and other HBOCs can cause colour-related interference in chemistry testing, their effect on standardized pretransfusion assays remains unclear. MATERIALS AND METHODS: We evaluated the impact of HbVs in blood on routine pretransfusion testing, including ABO blood group/Rhesus D (RhD) typing, antibody screening and cross-matching, using tube and column agglutination techn

Subgrouping of A and AB blood groups in Indian blood centres: is it required?

Anti A1 antibody in the serum of A2 and A2B individuals is rare but when present can have laboratory and clinical significance. Routine subgrouping of all A and AB blood groups in blood centres in India is difficult due to economic constraints and has always been a point of debate. This study thus brings out the prevalence of anti A1 antibody and the clinical significance related to its presence. The results of the study showed a low prevalence of anti A1 antibody and when present, it had a low thermal amplitude and titre. Further, no blood group discrepancy or problems during compatibility te

[Genotyping of blood group systems at the CNRGS. I: FY, JK, MNS systems].

AIM OF THE STUDY: Determination of blood group antigens from data obtained by using molecular methods (genotyping) has become an indispensable tool in the specialized immunohematology laboratories. The French National Reference Centre for Blood group typing (CNRGS) routinely performs genotyping of the FY, JK and MNS system (common genotyping), providing a phenotype deduced from genotyping data for FY1, FY2, JK1, JK2, MNS3 and MNS4 antigens. PATIENTS AND METHODS: We performed a study to evaluate the common genotyping prescriptions referred to the CNRGS over the last three years. RESULTS: Betw

A rare case of clinically significant naturally occurring anti-Cw reported in a healthy male donor.

Anti-Cw antibody is an immunoglobulin against the red cell antigen Cw which is a low frequency red cell antigen that belongs to the Rh antigen system. It is a clinically significant antibody and may cause haemolysis on exposure to antigen positive red cells. Due to its low frequency, it is not included in routine antibody screening panels. A 32 years healthy male donor with no history of transfusion donated whole blood at the department of Transfusion Medicine & Blood Centre of our institute. As a part of routine pre-transfusion testing, the donor's samples were subjected to automated blood gr

Bridging the gap: Exploring the microbial influence on forensic ABO typing discrepancies for enhanced investigative accuracy.

Forensic serology has long been a cornerstone in forensic investigations, utilizing multidisciplinary approaches to identify and individualize biological evidence at crime scenes. However, the field faces challenges such as the potential for blood group changes in individuals affected by diseases or medical treatments, leading to false outcomes in prolonged legal cases. Additionally, the passive adsorption of bacterial products by red blood cells can result in blood group switching, emphasizing the need for careful analysis, especially in cases of concurrent infections. Furthermore, post-morte
