Fluid Management as a Risk Factor for Intra-abdominal Compartment Syndrome in Burn Patients: A Total Body Surface Area-Independent Multicenter Trial Part I.

Fluid management is one of the anticipated risk factors for intra-abdominal compartment syndrome (ACS). Since fluid requirements depend on the burned total body surface area (TBSA), an independent analysis is necessary to adapt resuscitation protocols and prevent this life-threatening complication. A retrospective multicenter study with matched-pair analysis was conducted in four German burn centers, including 38 burn patients with ACS who underwent decompressive laparotomy. Potential risk factors were analyzed, such as resuscitation volume, total fluid intake, mean fluid administration per da

Which infusate do I need? Physiological basis of fluid therapy.

A variety of infusates are used, and it is important that the consequences of overinfusion are understood. Infusates are used to expand the intravascular volume or to influence solute composition of body fluids. Choice of infusate depends upon the underlying physiological disturbances. The basis of infusate choice, the actions of frequently-used infusates and the consequences are outlined.

Changes in the red cell, plasma and inulin spaces and in the total water contents of rat femurs in cortisone induced osteoporosis.

Measurements of the bone vascular space by use of 99mTc-labelled red cells and 131I-serum albumin, and of the 3H-inulin spaces and total water contents were carried out in female rats treated for four weeks by daily subcutaneous injections of 15 mg cortisone acetate. Significant decreases in bone vascularity, inulin space and water contents were noted as compared to controls. These changes were not associated with differences in plasma calcium values, or parathyroid gland weights. They were found to be present together with significant decreases in bone density and ash contents of the femoral

Importance of lean body mass in the oncologic patient.

Loss of lean body develops from an imbalance in protein synthesis and catabolism and is associated with a variety of different disease and nondisease states, including severe malnutrition, cachexia, and physiologic age-related loss (sarcopenia). Loss of lean body mass is prevalent among a significant proportion of the elderly population and has been associated with increased adverse clinical outcomes. Recognition of individuals at risk for low lean body mass may be difficult due to unequal distribution of losses across muscle and adipose compartments, and individuals who are both obese and sar

Application of in vivo activation analysis to the compartmental study of sodium in the hand.

Sodium kinetics were studied in the hands of 14 subjects by local in vivo neutron activation analysis. Hands were irradiated with 252 Cf sources giving absorbed doses of 8 cGy. The variation in 24Na radioactivity was plotted against time, and each curve fitted to a function that was the sum of two exponentials. Two pools were identified, one corresponding to fast turnover, with a half-life of about one hour, the other to a slower turnover process with a half-life averaging 79 h if the calcium concentration of the hand bones is normal, and 35 h if it is subnormal. The mean ratio of calcium to s

Markers of cartilage metabolism in arthrosis. A review.

The mechanisms involved in the disease process in arthrosis are largely unknown, with genetics, joint malalignment, overload or trauma, obesity, and aging as some of the known or suspected contributing factors. Even less well known is how these general factors are translated into disease mechanisms at the cell and tissue levels. However, it may be argued that degradation of cartilage matrix is a key event at some time in the development of arthrosis. During this process, fragments of matrix molecules and other chondrocyte products are released into the joint fluid and eventually into other bod

Pharmacology of colloids and crystalloids.

We have attempted to review body fluid distribution by compartments so that the reader understands the physiology of ICF and ECF, and the relationship between interstitial and intravascular fluids. Crystalloids such as NS and RL are distributed to the ECF, whereas colloids primarily remain intravascular for longer periods. Although effective, crystalloids tend to require larger volumes for infusion, and edema remains a problem. Colloids as a group are extremely effective volume expanders, but none is ideal. Albumin, hetastarch, dextran, and the less commonly used colloids each have significant

Homeostasis--the key concept to physiological control.

Water in the body forms the fluid compartments inside and outside cells, but it is more than just a space filler. A continuous exchange of water between the compartments partly facilitates regulation of the compartment volume but also permits the exchange of substances dissolved within it. Some of these latter are ions that form the electrolyte environment in which cells must operate. Body fluid physiology is concerned with the actions and regulation of the major electrolyte constituents. The use of infusions provides clinicians with the means of supporting this physiology when homeostatic mec

Segmental Bioelectrical Impedance Spectroscopy to Monitor Fluid Status in Heart Failure.

Bioelectrical impedance spectroscopy (BIS) measures body composition, including fluid status. Acute decompensated heart failure (ADHF) is associated with fluid overload in different body compartments. This investigation aimed to evaluate the feasibility of measuring and monitoring fluid accumulation in patients with ADHF using BIS. The extracellular impedance as a surrogate marker for fluid accumulation was measured in 67 participants (25 healthy reference volunteers and 42 patients admitted with ADHF) using BIS in the "transthoracic", "foot-to-foot", "whole-body" and "hand-to-hand" segments.

Composition of postnatal weight loss and subsequent weight gain in small for dates newborn infants.

Using a sucrose and deuterium oxide dilution technique body water compartments and solids were serially determined in small for dates newborn infants at birth, at the moment of maximum postnatal weight loss and on recovery of birth weight. Compositions of weight loss and subsequent weight gain were calculated from the differences in body water compartments and solids between the first and the second and the second and the third study, respectively. Birth weight of the infants was 1.55 +/- 0.46 kg (mean +/- SD) (N = 7), gestational age was 35.7 +/- 3.1 weeks. Results show that despite changes i

[Liquid shifts in the lung under fenoterol and verapamil. Experimental study on the origin of pulmonary oedema under tocolysis. III (author's transl)].

The question of the causal relationship between tocolysis and pulmonary edema has been unresolved up till now. Eight mongrel dogs were given infusions of Fenoterol (0.06 mcg/kg/min) and later of Fenoterol (0.06 mcg/kg/min) and Verapamil (2 mcg/kg/min). In the case of infusion of Fenoterol alone measurements by the thermo-dye-technique showed massive shifts of lung fluids from the intravascular to the extravascular space corresponding to interstitial edema. The addition of Verapamil induced no further changes. The intrapulmonary haemodynamical overload by increased cardiac output and pulmonary

The relationship between nocturnal polyuria and the distribution of body fluid: assessment by bioelectric impedance analysis.

PURPOSE: Increased nocturnal urinary volume is closely associated with nocturia. We investigated the relationship between nocturnal polyuria and the variation of body fluid distribution during the daytime using bioelectric impedance analysis. MATERIALS AND METHODS: A total of 34 men older than 60 years were enrolled in this study. A frequency volume chart was recorded. Nocturnal polyuria was defined as a nocturnal urine volume per 24-hour production of greater than 0.35 (the nocturnal polyuria index). Bioelectric impedance analysis was performed 4 times daily at 8 and 11 a.m., and 5 and 9 p.m

Predictive modeling of fluid status in hemodialysis: model development and internal validation using the MONitoring dialysis outcomes (MONDO) global database.

BACKGROUND: Optimized fluid management is crucial in dialysis care because extracellular volume overload drives adverse cardiovascular outcomes. At the same time, comorbidities such as inflammation and protein energy wasting lead to decreased muscle mass and intracellular water. Accurate assessment of total body water (TBW) and its extracellular water (ECW) and intracellular water (ICW) compartments is therefore essential to guide ultrafiltration, evaluate dialysis adequacy, and monitor patient risk. METHOD: Using adult patients from the MONitoring Dialysis Outcomes (MONDO) 2012 cohort, we de

Disposition of intravenous potassium in anuric man: a kinetic analysis.

The disposition of a constant-rate (0.3 mEq/kg/hr) intravenous potassium load was studied in anuric, chronic hemodialysis patients. Changes in plasma potassium concentration could be adequately described by a two-compartment kinetic model under both isohydric conditions and during acute metabolic alkalosis. From 63 to 92% of the infused potassium left the extracellular fluid (ECF) in isohydric studies, and from 73 to 97% left the ECF in alkalanizing studies. Cellular uptake of the infused potassium was less when the plasma potassium concentration was higher. Plasma aldosterone levels rose but

Effect of antigen-induced synovitis on the extensor tendons within the retinacular compartment of the dog.

Antigen-induced proliferative synovitis was produced within the extensor retinacular compartment of one forepaw of young littermate dogs. The extensor tendons within the synovitis compartment were compared with the tendons in control compartments of the opposite forepaw. At 7 days following challenge, the tendons from the proliferative synovitis compartment showed increased uptake of [3H]proline at 5 min after pulse labeling. At 7, 14, and 28 days following challenge, the tendons within the proliferative synovitis compartment showed a significant increase in metabolic activity compared with co

Blood-labyrinth barrier and fluid dynamics of the inner ear.

Under normal conditions, the inner ear possesses remarkably stable homeostatic mechanisms for the maintenance of functional integrity of the inner ear fluid. The inner ear fluid maintains its homeostasis by a variety of regulatory mechanisms such as an ion transport system, a blood-labyrinth barrier, and a constant blood supply. Highly regulated transport of ions into and out of the inner ear provides for the maintenance of inner ear fluid composition necessary for auditory transduction. Any disturbance in one of these mechanisms can disrupt homeostasis expressed by ionic, osmotic, or metaboli

Perioperative bioelectrical impedence analysis in neurosurgery.

The use of bioelectrical impedence (BI) measurement to assess body composition has recently attracted the attention of anesthesiologists. Analysis of BI provides a non-invasive method to quantify fluid distribution in different body compartments. This study was designed to assess whether BI analysis reflects fluid depletion in neurosurgical patients with moderate blood loss. Six adult male patients scheduled for elective craniotomy under general anesthesia were studied. Exclusion criteria included patients with cardio-respiratory disease. BI analysis was performed at three stages, A, day befor
