Body Surface Potential Mapping during Ventricular Depolarization in Athletes with Prolonged PQ Interval after Exercise.

BACKGROUND: Prolongation of the PQ interval, generally associated with an atrioventricular conduction delay, may be related to changes in intraventricular impulse spreading. OBJECTIVE: To assess, using body surface potential mapping (BSPM), the process of ventricular depolarization in athletes with prolonged PQ intervals at rest and after exercise. METHODS: The study included 7 cross-country skiers with a PQ interval of more than 200 ms (Prolonged-PQ group) and 7 with a PQ interval of less than 200 ms (Normal-PQ group). The BSPM from 64 unipolar torso leads was performed before (Pre-Ex) and

Prediction of atrial fibrillation using the recurrence complex network of body surface potential mapping signals.

OBJECTIVE: Atrial fibrillation (AF) is the most common type of persistent arrhythmia. Early diagnosis and intervention of AF is essential to avert the further fatality. The technique of noninvasive electrical mapping, especially the body surface potential mapping (BSPM), has a more practical application in the study of predicting AF, when compared with the invasive electrical mapping methods such as the epicardial mapping and interventional catheter mapping. However, the prediction of AF with noninvasive signals has been inadequately studied. Thus, the aim of this paper was to analyze the prop

Body surface activation mapping of electrical dyssynchrony in cardiac resynchronization therapy patients: Potential for optimization.

BACKGROUND: Electrical synchronization is likely improved by cardiac resynchronization therapy (CRT), but is difficult to quantify with 12-lead ECG. We aimed to quantify changes in electrical synchrony and potential for optimization with CRT using a body-surface activation mapping (BSAM) system. METHODS: Standard deviation of activation times (SDAT) was calculated in 94 patients using BSAM at baseline CRT (CRTbl), native, and different CRT configurations. RESULTS: SDAT decreased 20% from native to CRTbl (p<0.01) and an additional 26% (p<0.01) at optimal CRT (CRTopt), the minimal SDAT setting

[Rhythm analysis of body surface potential mapping recordings from atrial fibrillation patients based on autocorrelation function].

The study of atrial fibrillation (AF) has been known as a hot topic of clinical concern. Body surface potential mapping (BSPM), a noninvasive electrical mapping technology, has been widely used in the study of AF. This study adopted 10 AF patients' preoperative and postoperative BSPM data (each patient's data contained 128 channels), and applied the autocorrelation function method to obtain the activation interval of the BSPM signals. The activation interval results were compared with that of manual counting method and the applicability of the autocorrelation function method was verified. Furt

Conduction abnormalities in the right ventricular outflow tract in Brugada syndrome detected body surface potential mapping.

Brugada syndrome (BrS) causes sudden death in patients with structurally normal hearts. Manifestation of BrS in the ECG is dynamical and most patients do not show unequivocal signs of the syndrome during ECG screening. We have obtained 67-lead body surface potential mapping recordings of 25 patients with BrS and analyzed their spatial distribution of surface potentials during ventricular activation. Six patients presented spontaneous type I ECGs during the recording. These patients showed non-dipolarities in isopotential maps at the right ventricular outflow tract (RVOT) region during the deve

Selection of optimal recording sites for limited lead body surface potential mapping: a sequential selection based approach.

BACKGROUND: In this study we propose the development of a new algorithm for selecting optimal recording sites for limited lead body surface potential mapping. The proposed algorithm differs from previously reported methods in that it is based upon a simple and intuitive data driven technique that does not make any presumptions about deterministic characteristics of the data. It uses a forward selection based search technique to find the best combination of electrocardiographic leads. METHODS: The study was conducted using a dataset consisting of body surface potential maps (BSPM) recorded fro

Constructing Bodily Emotion Maps Based on High-Density Body Surface Potentials for Psychophysiological Computing.

Emotion is a complex physiological and psychological activity, accompanied by subjective physiological sensations and objective physiological changes. The body sensation map describes the changes in body sensation associated with emotion in a topographic manner, but it relies on subjective evaluations from participants. Physiological signals are a more reliable measure of emotion, but most research focuses on the central nervous system, neglecting the importance of the peripheral nervous system. In this study, a body surface potential mapping (BSPM) system was constructed, and an experiment wa

Insights Into the Spatiotemporal Patterns of Complexity of Ventricular Fibrillation by Multilead Analysis of Body Surface Potential Maps.

BACKGROUND: Ventricular fibrillation (VF) is the main cause of sudden cardiac death, but its mechanisms are still unclear. We propose a noninvasive approach to describe the progression of VF complexity from body surface potential maps (BSPMs). METHODS: We mapped 252 VF episodes (16 ± 10 s) with a 252-electrode vest in 110 patients (89 male, 47 ± 18 years): 50 terminated spontaneously, otherwise by electrical cardioversion (DCC). Changes in complexity were assessed between the onset ("VF start") and the end ("VF end") of VF by the nondipolar component index (N D I B S P M ), measuring the frac

Characterization of the spatial distribution of late ventricular potentials by body surface mapping in patients with ventricular tachycardia.

Low-level activity at the end of the QRS complex was analyzed from 63 thoracic leads in 15 normal subjects and in 21 patients with ventricular tachycardia (VT). The latter had old myocardial infarction and no conduction disturbances and had not been receiving antiarrhythmic drugs. In both normal subjects and patients with VT, isopotential maps of the time-averaged and filtered (25 Hz high-pass) electrocardiograms during the terminal portion of the QRS were dipolar, i.e., they showed single positive and negative regions. For patients with VT, the extrema were either distant, with one over the p

Estimated body surface potential maps in emergency department patients with unrecognized transient myocardial ischemia.

BACKGROUND: We report on 5 patients who presented to the emergency department (ED) with chest pain, had negative serum troponin levels, and were discharged with a presumed noncardiac diagnosis. Thereafter, retrospective analysis of Holter monitoring data recorded for a clinical trial revealed ST events indicative of transient myocardial ischemia that was unrecognized clinically. STUDY AIM: The purpose of this analysis was to determine whether initial body surface potential maps estimated from optimal ischemia electrode sites estimated body surface potential map (EBSPM) showed signs of ischemi

[Spatial distribution of atrial late potentials on body surface mapping in patients with paroxysmal atrial fibrillation].

We investigated the spatial distribution of atrial late potentials (ALP) in patients with paroxysmal atrial fibrillation (Paf) by use of body surface signal-averaged ECG. The P wave-triggered signal-averaged ECG was recorded in 20 patients with Paf and 34 control patients from precordial 16 unipolar leads (standard V1-V6 and two intercostal spaces below and above V1, V2, V4-V6). The duration (Ad) and number of fragmented deflection (Nf) of filtered P wave were measured on each lead. % Area was also calculated by dividing the area for the last 20 msec by the total area of filtered P wave. Minimal configuration of body surface potential mapping for discrimination of left versus right dominant frequencies during atrial fibrillation.

BACKGROUND: Ablation of drivers maintaining atrial fibrillation (AF) has been demonstrated as an effective therapy. Drivers in the form of rapidly activated atrial regions can be noninvasively localized to either left or right atria (LA, RA) with body surface potential mapping (BSPM) systems. This study quantifies the accuracy of dominant frequency (DF) measurements from reduced-leads BSPM systems and assesses the minimal configuration required for ablation guidance. METHODS: Nine uniformly distributed lead sets of eight to 66 electrodes were evaluated. BSPM signals were registered simultaneo

Distinguising features of left anterior fascicular block and inferior myocardial infarction as presented by body surface potential mapping.

Total body surface maps obtained from 19 patients with previous inferior myocardial infarction (IMI) were compared with maps obtained from 19 patients with left anterior fascicular block (LAFB) and six more patients in whom electrocardiographic changes were indistinguishable between IMI and LAFB. Three distinguishing features were detected: 1) abnormal high anterior positivity developed both in IMI and LAFB, but its onset was earlier in LAFB; 2) a broad rim of abnormal right lower negativity was seen in both groups, but in IMI it was within the first 40 msec, whereas in LAFB it was found in th

Detection of the spatial distribution of late potentials by body surface mapping using forty-five unipolar, leads.

For evaluating the spatial location of late potentials (LPs), the authors designed a new system for the body surface mapping of signal-averaged, filtered ECGs using forty-five thoracic unipolar leads (5 x 9 array). The signals from patients with old myocardial infarction (MI, N = 7) and arrhythmogenic right ventricular dysplasia (ARVD, N = 1) were amplified and passed through a bandpass (100-300 Hz) filter. The departure maps, LP isopotential maps, and LP30 area maps were generated and superimposed. The LP30 duration was determined as the section between the filtered QRS endpoints and points t

Prognostic significance of ST potentials determined by body surface mapping in inferior wall acute myocardial infarction.

Electrocardiographic body surface mapping on admission to coronary care has been shown to predict prognosis in a previous study of 100 patients with inferior wall acute myocardial infarction (AMI). A further 98 patients with first inferior wall AMI were now studied by body surface mapping on admission to coronary care to confirm that both the spatial distribution or map pattern of ST-segment potentials and the precise measurement of the maxima and minima are of prognostic significance. Each ST-segment map was compared by correlation coefficient to the average map pattern of the 4 groups derive

[Eigenvector analysis in body surface potential maps for evaluating right ventricular hypertrophy in primary pulmonary hypertension].

This study related the eigenvectors derived from the QRS complex in body surface isopotential maps of normal subjects to regional myocardial excitation, with special emphasis on the right ventricle. The subjects consisted of 120 normal healthy adults and eight patients with primary pulmonary hypertension and right ventricular hypertrophy (PPH). According to the Karhunen-Loève theory, eigenvectors were derived from the normal group and the eigenvector coefficient of each subject was obtained against time for the first three components. The cumulative proportion of the first three eigenvectors w

Body surface potential maps in patients with pulmonic valvular and aortic valvular stenosis of mild to moderate severity.

Multiple-lead surface potential maps were compared throughout ventricular activation in nine patients with mild-to-moderate pulmonic stenosis and in 12 patients with mild-to-moderate aortic stenosis. Abnormal patterns of potential distribution were found in aortic stenosis, including three patients without electrocardiographic evidence of left ventricular enlargement. When related to the onset of depolarization, abnormal departures started later, peaked later, lasted longer, and were more intense, more uniform and discrete in aortic stenosis. In pulmonic stenosis, abnormal departures started e

Repercussions of cardiac resynchronization therapy on the ventricular repolarization of heart failure patients as assessed by body surface potential mapping.

OBJECTIVE: Cardiac resynchronization therapy (CRT) is an adjunct treatment for heart failure (HF) which associates with left bundle-branch block (LBBB) and is refractory to medical therapy. However, nearly 1/3 of the patients still do not respond, the reasons for which have yet to be determined. Additionally, experimental studies proved that epicardial left ventricle (LV) pacing yields reversed electrical activation sequence, increasing QT interval duration and dispersion of the ventricular repolarization, and leaving patients at greater risk for ventricular arrhythmias. METHODS: In this stud

Using computerised surface wound mapping to compare the potential medical effectiveness of Enhanced Protection Under Body Armour Combat Shirt collar designs.

INTRODUCTION: Protecting the neck from explosively propelled fragments has traditionally been achieved through a collar attached to the ballistic vest. An Enhanced Protection Under Body Armour Combat Shirt (EP-UBACS) collar has been identified as an additional method of providing neck protection but limited evidence as to its potential medical effectiveness exists to justify its procurement. METHOD: Entry wound locations and resultant medical outcomes were determined using Abbreviated Injury Scale (AIS) for all fragmentation neck wounds sustained by UK soldiers between 01 January 2010 and 31
