Acrylic bone cements with bismuth salicylate: Behavior in simulated physiological conditions.

In a previous work, we reported the development of acrylic bone cement formulations for application in percutaneous vertebroplasty, by using bismuth salicylate (BS) as the radiopaque agent. Our objective was to obtain high radiopacity along with a therapeutical effect produced by the release of salicylic acid in situ. To follow that study, the setting kinetics and static and dynamical mechanical properties of the BS cements were studied in simulated physiological conditions. Moreover, radiopacity after various times of immersion in saline and the wettability of the cements surfaces were determ

Investigation of Antibiotic-Releasing Biodegradable Composite Bone Cements for Treating Experimental Chronic Maxillofacial Bone Infection.

Chronic osteomyelitis of the maxillofacial bones (i.e., jaw bones) is a persistent infection that requires effective treatment. Because systemic antibiotics seldom reach necrotic areas to remove bone infection, local antibiotic carriers such as antibiotic-loaded bone cement can be tried. It is critical to assess the biosafety and efficacy of two new antibiotic-loaded biodegradable composite bone cements for treating chronic mandibular osteomyelitis, and their drug eluting efficiency and other relevant aspects prior to clinical trial. The physico-mechanical properties, and drug release capacity

Bone cement implantation syndrome induced by antibiotic-loaded bone cement covering the infected bone surface: A case report.

INTRODUCTION AND IMPORTANCE: Bone cement implantation syndrome (BCIS) typically occurs during bone cementation and prosthesis insertion. Almost all of the models used to explain BCIS are based on studies related to hip arthroplasty. BCIS induced by bone cement implantation in superficial wounds has not been reported. CASE PRESENTATION: We report the case of a 37-year-old man with chronic osteomyelitis of the left tibia (Cierny - Mader type II), who developed BCIS after covering the infected bone surface with antibiotic-loaded bone cement. BCIS presented as acute massive pulmonary exudation an

Acrylic Bone Cements Modified with Graphene Oxide: Mechanical, Physical, and Antibacterial Properties.

Bacterial infections are a common complication after total joint replacements (TJRs), the treatment of which is usually based on the application of antibiotic-loaded cements; however, owing to the increase in antibiotic-resistant microorganisms, the possibility of studying new antibacterial agents in acrylic bone cements (ABCs) is open. In this study, the antibacterial effect of formulations of ABCs loaded with graphene oxide (GO) between 0 and 0.5 wt.% was evaluated against Gram-positive bacteria: Bacillus cereus and Staphylococcus aureus, and Gram-negative ones: Salmonella enterica and Esche

Strontium modified calcium phosphate cements - approaches towards targeted stimulation of bone turnover.

Making use of the potential of calcium phosphates to host a variety of ions in their crystal lattice, ion substitution of calcium phosphate bone cements has become the subject of intense investigations in the last few years, since this approach allows one to stabilize a bone defect and to locally deliver therapeutic ions into a specific defect site at the same time. In this respect significant attention has been given to strontium ions (Sr2+) lately. Strontium possesses the unique potential to both stimulate new bone formation and inhibit cell-driven bone resorption and thus has been used succ

Design and evaluation of a nanoenhanced anti-infective calcium phosphate bone cements.

Post-operative complications due to infections are the most common problems that occur following dental and orthopedic implant surgeries and bone repair procedures. Preventing post-surgical infections is therefore a critical need that current polymethylmethacrylate (PMMA) bone cement fail to address. Calcium phosphate cements (CPCs) are unique in their ability to crystallize calcium and phosphate salts into hydroxyapatite (HA) and hence is naturally osteoconductive. Due to its low mechanical strength its use in implant fixation and bone repair is limited to non-load bearing applications. The p

An in vitro biomechanical evaluation of bone cements used in percutaneous vertebroplasty.

The purpose of this study was to determine the strength and stiffness of osteoporotic vertebral bodies (VBs) subjected to compression fractures and subsequently treated with bipedicular injections of various polymethylmethacrylate cements. Ten spines were harvested from nonembalmed female cadavers (age 68.6 +/- 13.7 years) and evaluated for bone mineral density using the dual energy X-ray absorptiometry method (t-score = -2.3 +/- 2.4). The 50 VBs (L1-L5) were disarticulated, compressed in a materials testing machine to determine initial strength and stiffness, and then assigned to one of six g

Pulmonary cement embolism after augmentation of pedicle screws with bone cement.

The authors present the very rare complication of widespread pulmonary artery cement embolism after pedicle screw augmentation with bone cement for multilevel spine fusion. A 69-year-old woman with severe osteoporosis underwent a posterior T12S1fusion because of lumbar scoliosis. After two months the superior pedicle screws loosened, and a revision spondylodesis T8L1 was performed with bone cement augmentation of the pedicle screws. Although cement leakage was seen paravertebrally, the patient was asymptomatic and reported distinct pain relief, so that no further investigations were initiated.

Bone cement implantation syndrome in cemented hip hemiarthroplasty-a persistent risk.

BACKGROUND: Every year, ~ 210,000 initial implantations of hip endoprostheses are carried out in Germany alone. The "bone cement implantation syndrome" (BCIS) is considered a severe peri- and early-postoperative complication when implanting cemented prostheses. The origin of the BCIS and its impact on the clinical outcome are still uncertain. This study investigates the clinical progression after BCIS cases in patients with cemented hemiarthroplasty. Risk factors for the occurrence of BCIS are evaluated. MATERIAL AND METHODS: Clinical data of all patients with a proximal femur fracture and wh

Bone cement treatment for aneurysmal bone cyst in a dog.

An eighteen month old female Doberman pinscher dog was referred to teaching hospital of Adnan Menderes University, Faculty of Veterinary Medicine with the complaint of right forelimb lameness for a month. On the basis of clinical, radiographical, scintigraphical, computed tomographical and histopathological findings, aneurysmal bone cyst was diagnosed. Surgical curettage and bone cement treatment were applied. The patient recovered after 12 months. This case proves that aneurysmal bone cyst, without osteolysis and/or damages to the surrounding tissues, may result in a good prognosis if curetta

Calcium phosphate-based anti-infective bone cements: recent trends and future perspectives.

Bone infection remains a challenging condition to fully eradicate due to its intricate nature. Traditional treatment strategies, involving long-term and high-dose systemic antibiotic administration, often encounter difficulties in achieving therapeutic drug concentrations locally and may lead to antibiotic resistance. Bone cement, serving as a local drug delivery matrix, has emerged as an effective anti-infective approach validated in clinical settings. Calcium phosphate cements (CPCs) have garnered widespread attention and application in the local management of bone infections due to their in

Prophylactic Antibiofilm Activity of Antibiotic-Loaded Bone Cements against Gram-Negative Bacteria.

Gram-negative bacilli can be responsible for prosthetic joint infection (PJI) even if staphylococci are the main involved pathogens. Gram-negative PJIs (GN-PJI) are considered difficult-to-treat infections due to the increase in antimicrobial resistance and biofilm formation. To minimize the risk of infection in cases of arthroplasties with cemented prosthesis, bone cement can be loaded with antibiotics, especially gentamicin. In this study, we aimed to compare the prophylactic antibiofilm activity of ready-to-use antibiotic-loaded bone cements (ALBC), already commercialized or new prototypes.

Influence of Selected Surfactants on Physicochemical Properties of Calcium Phosphate Bone Cements.

The influence of the three nonionic surface active agents such as Tween 20, Tween 80, and Tetronic 90R4 on hydrolysis, setting reaction, microstructure, and mechanical properties of alpha tricalcium phosphate (α-TCP) based materials was determined. The study revealed that the addition of any of the surfactants mentioned above slightly prolonged the setting time of the tested cements (up to 5 min). On the other hand, it was found that surfactants influence the long-term hydrolysis reaction. The addition of surfactants also affected the microstructure of the final materials, especially after inc

Evaluation of elution and mechanical properties of two injectable chemotherapeutic bone cements.

Chemotherapeutic bone cements can both stabilize the bone fractures as well as deliver chemotherapy agents directly to the bone metastatic site and adjacent soft tissue tumors. This study evaluated the in vitro elution and flexural properties of Vertebroplastic™ and Confidence Ultra™ bone cements (Depuy Spine Inc., Raynham, Mass., USA) containing methotrexate. In vitro elution was measured by placing bone cement specimens containing 4 different methotrexate amounts in 20 ml saline, and the methotrexate elution was measured at regular intervals for 672 h. The flexural properties of bone cement

A rat osteoporotic spine model for the evaluation of bioresorbable bone cements.

BACKGROUND CONTEXT: As the aging population increases, the rising prevalence of osteoporosis-related spine fractures will have a dramatic impact on health care. At present, mainstay treatment relies on systemic medications intended to prevent diminishing bone mineral density (BMD) and bone mass. However, an adjunctive treatment strategy is to target specific areas of the skeletal system that are prone to clinically significant osteoporotic fractures. We term this strategy the "local treatment of osteoporosis" or osteoplasty. Potential use of osteoplasty involves the percutaneous injection of b

The behavior of novel hydrophilic composite bone cements in simulated body fluids.

Composite bone cements were formulated with bioactive glass (MgO--SiO(2)--3CaO. P(2)O(5)) as the filler and hydrophilic matrix. The matrix was composed of a starch/cellulose acetate blend (SCA) as the solid component and a mixture of methylmethacrylate/acrylic acid (MMA/AA) as the liquid component. The curing parameters, mechanical properties, and bioactive behavior of these composite cements were determined. The addition of up to 30 wt % of glass improved both compressive modulus and yield strength and kept the maximum curing temperature at the same value presented by a typical acrylic-based

Dangers with cementation under low-viscosity state: Cement arterio-venogram and bone cement implantation syndrome.

Cement arterio-venogram is a rare event with cement extrusion into femoral nutrient vessels. In literature it is known to be benign with no significant clinical sequelae. It is postulated that it is due to high cement implantation pressure, that results in optimal cement filling quality. All previously reported cases were female patients, and it is thought to be a female only phenomenon due to the relatively narrow femoral canal leading to higher pressures during cementation. In this case series we report 3 cases different to existing literature. All 3 patients showed a cement arterio-venogram

PMMA Bone Cement in Interventional Oncology.

Polymethylmethacrylate (PMMA) bone cement is increasingly being used for percutaneous minimally invasive treatments of patients suffering from bone malignancies. PMMA is composed of a polymeric powder and a monomeric liquid. Once mixed, the polymerization process begins and leads to a viscous fluid that can be injected through a bone trocar. Cement progressively hardens within the bone, leading to a viscoelastic solid material. PMMA interacts with the surrounding cancellous bone through mechanical interlocking via interdigitations in trabecular bone. It can also bond with hardware, such as tit
