Current management of aneurysmal bone cysts.

Aneurysmal bone cysts (ABCs) are benign bone lesions arising predominantly in the pediatric population that can cause local pain, swelling, and pathologic fracture. Primary lesions, which constitute roughly two thirds of all ABCs, are thought to be neoplastic in nature, with one third of ABCs arising secondary to other tumors. Diagnosis is made with various imaging modalities, which exhibit characteristic features such as "fluid-fluid levels," although biopsy is critical, as telangiectatic osteosarcoma cannot be excluded based on imaging alone. Currently, the standard of care and most widely e

Aneurysmal bone cysts of the spine.

PURPOSE: Aneurysmal bone cyst is a benign, relatively uncommon lesion, representing 1.4 % of primary bone tumors. The vertebral column is involved in 3-30 % of cases. This report describes clinical characteristics and treatment results of 18 patients with aneurysmal bone cyst of the spine. METHODS: Between 1991 and 2008, 18 patients with aneurysmal bone cyst of the spine were surgically treated in our department. The clinical records, radiographs, histologic sections, and operative reports were analyzed. RESULTS: There were 11 male and 7 female patients; mean age was 22.1 years (range 7-46 y

Aneurysmal bone cysts of the spine.

Aneurysmal bone cysts are uncommon bony lesions of the spine. Two such patients are reported in the study. The first patient had involvement of first and second thoracic vertebrae, and a preoperative diagnosis was made possible due to the typical magnetic resonance imaging appearances. A total removal was achieved in two stages. In the second patient the C2 spinous process was involved, and this patient presented as atantoaxial dislocation. Removal of the abnormal tissue and posterior fusion was done. Both the patients showed neurologic improvement. The relevant literature is reviewed.

Aneurysmal bone cysts of the spine.

Two cases of aneurysmal bone cyst of the spine are reported. These rather rare tumours may pose initial diagnostic problems. If, however, the clinical and radiological features are studied, diagnosis should be easy and the appropriate treatment will follow. Computed tomography provides an additional diagnostic aid. Management appears to be primarily surgical, but the role of radiotherapy has not yet been clearly defined.

Aneurysmal bone cysts of the jaws.

Two cases of aneurysmal bone cyst are recorded. In the first there is clinical evidence to suggest that the lesion had been present for an unusually long period while incomplete clinical records of the other also indicate an unusual history. Histologically both lesions show a paucity of giant cells. It is not possible to make significant observations on aetiology other than support the theory that the lesion arises as a result of altered haemodynamics.

Spinal Aneurysmal Bone Cysts (ABCs): Optimal Management.

AIM: To review the published literature on the treatment of aneurysmal bone cysts (ABCs). METHOD: A systematic review of the English literature to April 2019 for all articles, with a minimum of three patients and 2-year follow-up, reporting on the treatment of spinal ABCs. The various treatment options were compared for the rates of recurrence, complications and mortality. RESULTS: Twenty-one articles and 272 patients (mean age 16.9 years, range 3-67) were included in this review. The overall recurrence rate for ABCs following all treatments is 12.8%. This is highest in those lesions describ

Bisphosphonate therapy for spinal aneurysmal bone cysts.

PURPOSE: To assess the efficacy of bisphosphonate therapy in the management of spinal aneurysmal bone cysts (ABCs). METHODS: A prospective study of six consecutive patients aged between 7 and 22 years with spinal ABCs treated with pamidronate (1 mg/kg) or zoledronate (4 mg). A visual analogue scale (VAS) for pain and radiological (contrast-enhanced MRI and CT scan at 3 and 6 months, then yearly X-rays) follow-up was continued for a minimum of 6 years. RESULTS: One patient with an unstable C2/3 failed to respond to a single dose of bisphosphonate and required surgical resection and stabilisat

[Aneurysmal bone cysts of the spinal canal].

Aneurysmal bone cysts (ABC) are rare bone tumors of controversial pathogenesis. In 15% of cases, they are localized in the vertebrae. Despite the absence of anaplasia, their volume and localization are often responsible for incomplete surgical resection. The authors present 5 cases of primary vertebral ABC, 4 females and one male, localized in cervical (3 cases), dorsal (1 case) and lumbar (1 case) spine. Symptoms included rachialgia and torticollis. Radiological and histopathological examination disclosed the typical features of ABC. Pre-operative embolization (in 3 cases) and surgery (comple

Aneurysmal bone cysts of the dorsal spine.

Aneurysmal bone cyst is an uncommon but interesting condition, more so when it involves the vertebrae. Three cases of aneurysmal bone cysts of the dorsal spine, suspected on clinicoradiological findings and confirmed by histopathology, are reported in this paper. The authors believe that when the disease affects the pedicles or transverse processes of a vertebra, the plain roentgenographic findings are quite typical of the disease, and a confident radiological diagnosis is possible in the majority of cases.

Selective arterial embolisation in aneurysmal bone cysts.

The treatment of three cases of aneurysmal bone cyst by selective arterial embolism is described. Two cases had involvement of the pelvis and one of the knee. The 2-year follow up has been promising. Radiologically the lesion has been arrested with increased density of the cyst wall and intra-cystic trabecular new bone formation. Pain relief has been complete and up to now no surgical treatment has been required.

Current treatments of primary aneurysmal bone cysts.

The management of aneurysmal bone cyst depends on the age of the patient, the location, extent, aggressiveness and the size of the lesion. In the light of their experience and a review of the literature of 1256 aneurysmal bone cysts, the authors analyzed various treatment modalities. Inactive lesions can heal with biopsy or curettage alone. In active or aggressive lesions, elective treatment usually consists of curettage, whether associated or not with bone grafting and local adjuvants. Aneurysmal bone cyst in young children do not seem more aggressive than in older children. In pelvic locatio

Diagnosis and Current Treatment of Aneurysmal Bone Cysts.

The purpose of this review is to increase awareness about the evolution and development of current trends in the diagnosis and treatment of aneurysmal bone cysts (ABCs). ABCs are benign, but locally aggressive bone tumors that mainly affect children. ABCs comprise 1% of all primary bone tumors and occur most frequently during the first two decades of life. The diagnosis is made using a variety of imaging modalities and has the characteristic features of an expansile, radiolucent lesion that is often seen in the metaphyseal region of the bone and has fluid-fluid levels that are apparent on MRI.

Multifocal polyostotic aneurysmal bone cysts in a llama.

A 3.3-year-old 125-kg castrated male llama was evaluated because of acute non-weight-bearing lameness on the left hind limb. Physical examination revealed crepitus in the midportion of the femur. On radiographs, a comminuted middiaphyseal fracture was seen. There was also a region of bone lysis with cortical thinning and expansion in the distal metaphysis and epiphysis of the left femur. Multiple small circular lesions were observed in the proximal metaphysis of the left femur, and the proximal portion of the left tibia appeared irregular. The owner elected to pursue treatment, and the fractur

[Echography for the diagnosis of aneurysmal bone cysts].

Aneurysmal bone cysts are benign tumors which are likely to recur. While growing, they cause severe osteolysis, able to balloon the bone and to thin the cortex. The latter is reduced to a thin bone shell, and sometimes completely interrupted. Tumor mass can spread into the surrounding soft tissues as in other bone tumors, especially malignant ones, which are often misdiagnosed. Whether the cortex is interrupted or only thinned, an acoustic window is obtained which allows an easy study of tumor mass with US. Seven patients were examined, who presented lytic lesions of different bones, produced

Aneurysmal bone cysts: MR imaging at 1.5 T.

Two patients with aneurysmal bone cysts of the pelvis were imaged using a 1.5-T magnetic resonance imaging device. Findings included multiple internal septations, cysts with fluid-fluid levels of varying intensity, and an intact rim of low-intensity signal completely surrounding the lesion. These findings allow a specific diagnosis of aneurysmal bone cyst to be made.

Denosumab for treating aneurysmal bone cysts in children.

BACKGROUND: Aneurysmal bone cyst (ABC) is a benign tumour whose progression involves the RANK/RANKL signalling pathway. Surgery is the reference standard treatment but carries risks that vary with the site of the tumour. Denosumab is a human monoclonal IgG2 antibody that targets the RANK/RANKL pathway and may therefore hold promise for inhibiting ABC progression. The objective of this study was to evaluate denosumab use in paediatric patients (younger than 18 years) with ABC and to describe the clinical and radiological outcomes, as well as the side effect profile. HYPOTHESIS: Denosumab is a
