The classic two-stage masquelet technique is an effective procedure for the treatment of large bone defects. Our group recently showed that one surgery could be saved by using a decellularized dermis membrane (DCD, Epiflex, DIZG). In addition, studies with bone substitute materials for defect filling show that it also appears possible to dispense with the removal of syngeneic cancellous bone (SCB), which is fraught with complications. The focus of this work was to clarify whether the SCB can be replaced by the granular demineralized bone matrix (g-DBM) or fibrous demineralized bone matrix (f-D

Fresh compact bone, the candidate graft material for bone regeneration, is usually grafted for horizontal bone augmentation. However, the dense calcified structure inhibits the release of growth factors and limits cellular and vascular perfusion. We aimed to create mechano-chemically altered dense skull bone by ultrasonic treatment, along with partial demineralization using commercially available acidic electrolyzed water (AEW). The parietal skull bone of an 11-month-old Wistar rat was exposed and continuously treated with a piezoelectric ultrasonic scaler tip for 1 min, using AEW (pH 2.3) or

OBJECTIVE: The objective of this study was the review of 11 patients with two different treatment methods used historically for aneurysmal bone cyst in the sacrum. The outcome of both procedures is reported. In addition, the treatment technique of CT-guided percutaneous injections of demineralized bone matrix mixed with bone marrow concentrate is described.
MATERIALS AND METHODS: From 1997 to 2008, 11 patients with sacral aneurysmal bone cyst were treated at the Rizzoli Institute, Bologna, Italy. The first seven patients had surgical curettage without bone grafting, chemical adjuvants, or art

Bone grafting with demineralized bone matrix (DBM) is useful in reconstructive surgery to ultimately provide anatomic alignment, restore function, or augment/change the biomechanics of the foot and ankle. DBM should be used in conjunction with a transplant or implant displaying mechanical strength. DBM can augment cortical grafts used for bridging gaps or defects and lengthening procedures by increasing the connectivity of the structural graft with the host bone. Another useful application is providing a biologic boost to patients who have less-than-ideal physiology. Because DBM has higher con

The mechanisms by which the osteoclast attaches to and resorbs bone are not fully understood. Morphologic techniques have primarily been used to examine these mechanisms, but many studies have been based on decalcified material. In this study, the attachment of the osteoclast to alveolar bone and its relationship to the mineral component during the active resorption associated with tooth eruption in the rat was examined using transmission electron microscopy and techniques designed to minimize demineralization effects during processing. Large conglomerates of bone mineral were found within bot

Mandibular distraction osteogenesis is a major modality in treating micrognathia associated with severe class II malocclusion. Shortcomings associated with mandibular distraction are often secondary to lack of adequate bone at the level of the osteotomies, either from insufficient original bone stock or paucity of bone secondary to previous surgical procedures. To address the challenge of limited bone availability at the distraction site, our institution has implemented a stepwise protocol in the patients with severe class II malocclusion and insufficient bone stock at the level of the planned

BACKGROUND: Dentin is a permeable tubular composite and complex structure, and in weight, it is composed of 20% organic matrix, 10% water, and 70% hydroxyapatite crystalline matrix. Demineralization of dentin with gradient concentrations of ethylene diamine tetraacetic acid, 0.6 N hydrochloric acid, or 2% nitric acid removes a major part of the crystalline apatite and maintains a majority of collagen type I and non-collagenous proteins, which creates an osteoinductive scaffold containing numerous matrix elements and growth factors. Therefore, demineralized dentin should be considered as an exc

The treatment of through-and-through human periapical lesions by barrier membrane techniques has not been reported in the periodontal literature. However, periodontists frequently perform this treatment. The authors present a case that demonstrates radiopacity after regeneration surgery of a human periapical transosseous mandibular defect, suggesting local bone regeneration. The surgical technique performed in this case was based on the principles of guided bone regeneration using a demineralized freeze-dried bone membrane (DFDBM). The bone membrane acted as an efficient barrier that excluded

This report describes a new technique for treatment of a segmental defect in long bones that uses a cylindrical titanium mesh cage, in combination with cancellous bone allograft and demineralized bone matrix putty (Grafton), stabilized with a statically locked intramedullary nail. Two clinical cases of tibia defects treated with this technique are presented. At the one-year follow-up, radiographically both cases demonstrated excellent limb alignment, stability, and bony healing. Immediate full weight-bearing was initiated in each case, and early limb functional recovery was achieved. Prelimina

Defining the ideal material for frontal sinus obliteration remains controversial. Autogenous cancellous bone is effective because of its biological properties: it undergoes fast revascularization acting as an active scaffold for bone healing, but is linked to additional donor site morbidity. Bone dust harvesting from the skull surface produces no sequelae but availability is limited. Many efforts have been made to overcome these drawbacks, and an ideal bone substitute sought. Demineralized Bone Matrix (DBX; Musculoskeletal Transplant Foundation, Edison, NJ, USA) is a commercially available pro

Xeroradiography is a new and simple radiodiagnostic technique that makes it possible to analyse on one positive film not only the bone tissue but also the neighbouring structures (ligaments, muscles, fatty tissue, the vessels, and the skin). On the basis of about 100 examinations, the authors indicate the value and the limitations of the technique in rheumatological practice. Because of the technical aspects of the technique, the dorsolumbar spine and the facial massif are poorly visualized. The swamping of the contrast also limits its use for cases of demineralization (osteoporosis). On the o

Control of biomechanical properties of demineralized diaphyseal bone allografts is required for their clinical application. Therefore, the changes in flexural rigidity in human fibulae were investigated as a function of the demineralization depth using a nondestructive bending test. Starting at the facies medialis, the flexural rigidity was determined in 24 planes at 15 degrees sequential angular increments, which allowed data collection around the circumference of the bone. Test bones included 4 pairs of left and right human fibulae and 15 single fibulae. The elliptical distribution of the fl

Reconstruction of craniofacial defects in children presents several challenges that are not encountered in the adult population. Autologous bone grafts have long been the criterion standard for repairing these defects. Recently, several new materials and techniques have expanded our arsenal of reconstructive options. In this clinical report, we describe the use of both particulate bone grafting and demineralized bone matrix together to repair craniofacial defects encountered in pediatric patients.

Autologous iliac crest bone graft remains the gold standard for lumbar fusion. The potential for complications has led to the development of alternative bone graft materials and enhancers, including autologous growth factors, demineralized bone matrix products, osteoinductive agents, and ceramic products. The current literature centers mainly on preclinical studies, which, further complicating the situation, evaluate these products in different clinical scenarios or surgical techniques. Autologous growth factors and demineralized bone matrix products have had promising results in preclinical s

Proximal femoral fracture in elderly subjects is a major event that is life-threatening in the medium-to-long term. Advanced age, male gender and number of comorbidities largely account for high mortality and require geriatric expertise. Protein-energy malnutrition and bone demineralization increase mortality. Mortality can, on the other hand, be reduced by acting on two variables accessible to medical intervention: daily activities and nutritional status. Functional and neurocognitive assessment allow the risk of dependency to be evaluated, and global geriatric work-up can prevent sudden brea

Biochemical applications are increasingly utilized in paleontological studies, especially for detecting ancient proteins in fossil samples. Histopathological staining techniques have been applied, but they have yet to specifically target type I collagen, the primary bone matrix protein and the most significant protein of interest in paleoproteomic research. Moreover, these staining methods are often applied to demineralized fossils, which remove the original microstructure of the bone matrix and increase the risk of contamination. To address these limitations, this study aimed to test the effe

We investigated the expression of tumor necrosis factor-alpha (TNF alpha) gene in the area of implanted calf cancellous bone with different calcium content and the influence of calcium salts on local cellular immunity. The particles of mineralized and partially demineralized bone were implanted in the mouse's muscle pouch, and removed 3, 5, 10 and 20 days after implantation of the bone particles. The specimens were processed for determining the expression of mRNA encoding TNF alpha, which was performed by a nonradioactive in situ hybridization technique. The expression of TNF alpha mRNA was ma

BACKGROUND: Techniques to improve and accelerate tendon-bone healing could be advantageous in anterior cruciate ligament (ACL) reconstruction. Effects of demineralized bone matrix (DBM) on intra-articular tendon-bone healing have not been examined.
HYPOTHESIS: Demineralized bone matrix has the potential to convey osteoinductive growth proteins to the site of healing at the tendon-bone interface. We hypothesized that the presence of DBM will result in more bone formation and hasten tendon-bone healing.
STUDY DESIGN: Controlled laboratory study.
METHODS: Fifty-six female athymic rnu/rnu (nude
