Minimizing the Translation Error in the Application of an Oblique Single-Cut Rotation Osteotomy: Where to Cut?

OBJECTIVE: An oblique single cut rotation osteotomy enables correcting angular bone alignment in the coronal, sagittal, and transverse planes, with just a single oblique osteotomy, and by rotating one bone segment in the osteotomy plane. However, translational malalignment is likely to exist if the bone is curved or deformed and the location of the oblique osteotomy is not obvious. METHODS: In this paper, we investigate how translational malalignment depends on the osteotomy location. We further propose and evaluate by simulation in 3-D, a method that minimizes translational malalignment by v

Precision of using contralateral lesser trochanter as a landmark to prevent rotational malalignment in fracture shaft of femur: a human cadaveric study.

BACKGROUND: Rotational Malalignment is abnormal rotational alignment between proximal and distal parts of femoral bone. If there is more than 15 degrees of rotational malalignment in femoral bone, the patient will have abnormality of gait and they will have difficulties to perform their daily life activities. The chances to have rotational malalignment in femoral fracture are increased after intramedullary nailing which is more than plate and screws fixation. In this study, we used contralateral Lesser Trochanter to be the reference for rotational determiner. MATERIAL AND METHOD: 17 pairs of

Pre-operative frontal plane malalignment predicts the extension gap asymmetry in knee osteoarthritis.

PURPOSE: In contrast to bone cuts, soft-tissue releases cannot be planned sufficiently prior to total knee arthroplasty (TKA). Intra-operative evaluation of the extension gap after tibial or femoral resection may result in an unnecessarily excessive bone resection. The present study examines whether extension gap asymmetry can be calculated from the pre-operative long standing X-ray. METHODS: In this retrospective study, 32 patients with navigated unconstrained TKA were included. The pre-operative malalignment was measured on long standing X-rays and compared with the extension gap asymmetry

Association between decreased bone mineral density and severity of distal radial fractures.

BACKGROUND: The role of osteoporosis and osteopenia in the etiology of fractures of the distal part of the radius is well established, but any link between osteoporosis and the severity of the distal radial fracture has not been extensively investigated. The aim of this study was to investigate the association between the degree of osteoporosis and the severity of distal radial fractures. METHODS: All patients over fifty-five years of age with a low-energy distal radial fracture were offered dual x-ray absorptiometry scanning of the hip. Data on the 137 consecutive patients were collected pro

Statistical analysis of axial deformity during distraction osteogenesis of the tibia.

In this study, we documented the prevalence of coronal axis malalignment in a series of 93 tibias (from 54 patients) lengthened with monolateral fixation. The average length obtained by distraction osteogenesis was 8.9 cm (range, 3.5-15.6) or 38% of the original bone length (range, 11-78%). Fifty (54%) of 93 tibias had documented valgus angulation of > or = 10 degrees or had fixator manipulation during the lengthening process for undesirable or progressive angulation; no cases of varus angulation were noted. Thirteen (14%) segments had later corrective osteotomy for unsatisfactory valgus malal

Lower quarter screening for skeletal malalignment--suggestions for orthotics and shoewear.

Skeletal malalignments of the lower quarter, deformities within a bone or at a joint, may be the primary cause of musculoskeletal patient problems. Skeletal malalignments also may sustain the presence of a musculoskeletal patient problem that has some other causal mechanism. A screening exam for skeletal alignment of the lower quarter may assist clinicians in identifying skeletal malalignments that are associated with a musculoskeletal complaint. The purposes of this paper are to: 1) describe components for a skeletal alignment screening exam, 2) analyze how lower quarter malalignments may inf

Positioning evaluation of corrective osteotomy for the malunited radius: 3-D CT versus 2-D radiographs.

The authors retrospectively investigated the postoperative position of the distal radius after a corrective osteotomy using 2-dimensional (2-D) and 3-dimensional (3-D) imaging techniques to determine whether malposition correlates with clinical outcome. Twenty-five patients who underwent a corrective osteotomy were available for follow-up. The residual positioning errors of the distal end were determined retrospectively using standard 2-D radiographs and 3-D computed tomography evaluations based on a scan of both forearms, with the contralateral healthy radius serving as reference. For 3-D ana

SPECT-CT compared with conventional imaging modalities for the assessment of the varus and valgus malaligned hindfoot.

The combined single-photon emission computed tomography and conventional computed tomography (SPECT/CT) technique has increased the sensitivity and specificity of bone scans. We examined the value of using SPECT/CT for the assessment of coronal plane hindfoot deformities. Twenty-seven patients with varus (11 patients) or valgus (16 patients) malalignment of the hindfoot were assessed using radiography, conventional CT, bone scintigraphy, and SPECT/CT. The amount of deformity, stage of osteoarthritis, and level of activation on bone scans and SPECT/CT were measured. Activation was assessed in 1

[X-ray diagnosis of foot deformities].

In order to analyze foot deformities, the foot is divided into three compartments. Their normal and pathological positions are defined by the alignment of the bones' axes. The various foot deformities can be put down to a malalignment of the particular compartments. X-ray analysis of the malalignment allows a diagnosis to be made. The most important congenital and acquired foot deformities are discussed.

Prospective investigation of the relationship between dorsal tilt, carpal malalignment, and capitate shift in distal radial fractures.

AIMS: Carpal malalignment after a distal radial fracture occurs due to loss of volar tilt. Several studies have shown that this has an adverse influence on function. We aimed to investigate the magnitude of dorsal tilt that leads to carpal malalignment, whether reduction of dorsal tilt will correct carpal malalignment, and which measure of carpal malalignment is the most useful. METHODS: Radiographs of patients with a distal radial fracture were prospectively collected and reviewed. Measurements of carpal malalignment were recorded on the initial radiograph, the radiograph following reduction

Pisiform malalignment associated with distal radius fractures.

BACKGROUND: The association of scaphoid or other carpal bone fractures with distal radius fractures is frequently reported, whereas few studies have described pisiform malalignment associated with distal radius fractures. The purpose of this study was to investigate the frequency and characteristics of pisiform malalignment associated with distal radius fractures. METHODS: We performed a retrospective study by reviewing the data of 152 consecutive patients with a mean age of 63 years who were treated surgically for distal radius fractures during a five-year period. We evaluated the pisotrique

Management of the Failed OCD.

PURPOSE OF THE REVIEW: Osteochondritis dissecans (OCD) is a pathologic condition of subchondral bone most frequently occurring in the medial femoral condyle of the knee in children and adolescents. Salvage techniques are necessary when either nonoperative or typical operative treatments fail, or the OCD presents in an unsalvageable state. The purpose of this review is to describe the evaluation and management of failed OCDs. RECENT FINDINGS: Thorough preoperative planning is essential to the treatment of failed OCDs. Radiographs and advanced imaging such as MRI and CT allow for a detailed ass

Global tilt angle in spinal alignment of asymptomatic older women with low bone mass.

This study investigated the relationship between sagittal spinal alignment and musculoskeletal health in older women, particularly those exhibiting diminished bone density without apparent symptoms. The study assessed the impact of global tilt (GT) and the presence of coronal malalignment on spinal health. The research involved 165 asymptomatic older women with an average age of 68.91 ± 7.25 years and average body mass index of 24.26 ± 3.66 kg/m². Comprehensive standing anteroposterior and lateral spine radiographs were used for assessment. Through multivariate linear regression analysis, the

Scapular Morphology and Posterior Shoulder Stability: Biomechanical Evidence From an Advanced Cadaveric Shoulder Simulator.

BACKGROUND: Static posterior shoulder subluxation (SPSL) is associated with both glenoid retroversion and altered acromial morphology. Although abnormal glenoid anatomy has been considered a crucial etiological factor, the biomechanical role of acromial anatomy remains incompletely understood. HYPOTHESIS: Combined acromial and glenoid malalignment would produce greater posterior humeral head translation than either deformity alone, and targeted corrections could restore posterior stability. STUDY DESIGN: Controlled laboratory study. METHODS: Six fresh-frozen cadaveric shoulders underwent te

Surgical strategies for atypical femoral fracture nonunion: Augmentative plating with or without exchange nailing.

INTRODUCTION: This study investigated the outcomes of augmentative plating, with or without exchange nailing, in treating AFF nonunion. METHODS: Ten female patients who underwent augmentative plating with or without exchange nailing for AFF nonunion were retrospectively analyzed. To measure bone remodeling suppression, preoperative bone turnover markers (BTMs) were evaluated. The key outcome measures were the effectiveness of screw fixation in plating, malalignment correction via neck-shaft angle (NSA) adjustments, and improvements in ambulatory function assessed by the Koval score. RESULTS:

Blocking screw (pin) technique to achieve an acceptable reduction in intramedullary of subtrochanteric femoral fractures: indications, techniques, and clinical outcomes.

INTRODUCTION: Subtrochanteric femur fractures treated with intramedullary nails present biomechanical benefits, early weight-bearing, and reduced soft tissue damage, promoting bone union. However, improper reduction due to muscular forces and fragmented proximal bone increases nonunion risk. This study evaluates the efficacy of the blocking screw technique in preventing malalignment during intramedullary nailing. MATERIAL AND METHODS: In cases of subtrochanteric femoral fractures where malalignment, such as varus and anterior angulation, was anticipated during intramedullary nailing, proper r

Malalignment in plate osteosynthesis.

The aim for this review is to present general considerations in relation to malalignment after osteosynthesis with plate fixation and its consequences after fractures in adults in each of the following anatomical locations: humerus, forearm, femur, tibia. Recommendations for accepted malalignment in humerus diaphyseal fracture is varus <20 degrees, valgus <15 degrees, sagittal deformity <5 degrees and rotation <30 degrees. Recommendations when treating fractures of the forearm is anatomical reduction. Varus of ulna leads to loss of pronation. Valgus of ulna leads to loss of both pronation and

Role of Coronal Plane Malalignment in Hallux Valgus Correction.

Coronal malalignment is an important deformity parameter in hallux valgus feet. Approximately 90% of patients with hallux valgus have some degree of metatarsal pronation. In operated hallux valgus, persistent metatarsal pronation is an independent deformity relapse factor. Coronal malalignment can be identified through an anteroposterior (AP) weight-bearing foot radiograph and a weight-bearing forefoot scan. The AP foot view can identify 3 levels of rotation: mild, moderate and severe metatarsal pronation. Regarding the treatment options, some techniques are capable of rotational correction, s

Stress Shielding Effects of Short Stem Alignment and Bone Density in Reverse Shoulder Arthroplasty.

Short-stem humeral implants in reverse total shoulder arthroplasty (rTSA) are increasingly adopted because they preserve bone stock and reduce stress shielding. However, their shorter length raises the risk of malalignment, and outcomes may be further affected by osteoporosis, which is common in rTSA patients. This study investigated how humeral stem alignment (neutral, varus, valgus) and bone mineral density simultaneously influence stress distribution in the proximal humerus using finite element (FE) analysis. FE models were developed from computed tomography scans of one normal and one oste
