Inserting pedicle screws in lumbar spondylolisthesis - The easy bone conserving way.

BACKGROUND: Pedicle screw fixation in high grade lumbar listhetic vertebral body has been nightmare for Orthopaedic and spine surgeons. This is because of abnormally positioned listhetic pedicles and non-visualization of pedicle in conventional image intensifier (C-Arm). This results into increased surgical time, more blood loss, radiation exposure and more chances of infection. To overcome this problem, we have devised a new Technique of putting of pedicle screw fixation in listhetic vertebrae. METHODS: Total 20 patients of average age of 42 (25-56) were included during 2010 to 2015. Listhes

Pulmonary cement embolism after augmentation of pedicle screws with bone cement.

The authors present the very rare complication of widespread pulmonary artery cement embolism after pedicle screw augmentation with bone cement for multilevel spine fusion. A 69-year-old woman with severe osteoporosis underwent a posterior T12S1fusion because of lumbar scoliosis. After two months the superior pedicle screws loosened, and a revision spondylodesis T8L1 was performed with bone cement augmentation of the pedicle screws. Although cement leakage was seen paravertebrally, the patient was asymptomatic and reported distinct pain relief, so that no further investigations were initiated.

[The juvenile bone cyst: treatment with continuous decompression using cannulated screws].

BACKGROUND: A unicameral bone cyst (UBC) may be caused by an intraosseous venous obstruction with the accumulation of interstitial fluid. Therefore, continuous decompression of the fluid using cannulated screws would represent a causal therapeutic approach. MATERIALS AND METHODS: The clinical and radiographic follow-up of 17 patients with UBCs treated by curettage and mechanical decompression using cannulated screws was evaluated retrospectively. RESULTS: Radiographic examination revealed the following results: complete healing of the lesion (n=2), persistent defect after healing (n=6), pers

Dural reconstruction with fascia, titanium mesh, and bone screws: technical note.

OBJECTIVE: After the resection of cranial base tumors, there may not be enough free dural margin left for reconstruction after involved bone and dura have been removed. In such a situation, dural reconstruction becomes a problem. We propose a new technique of dural closure in such cases. METHODS: A fascial graft is prepared from either fascia lata, abdominal fascia, pericranium, or temporal fascia and is trimmed to a size slightly larger than that of the dural defect. The fascial graft is placed over the dural defect and affixed to the underlying bone with a piece of titanium mesh, titanium s

Effect of bone quality on the forces generated by compression screws.

Internal fixation of the fractured scaphoid bone is used to promote union between bone fragments and to decrease wrist immobilization. Headless screws are commonly used because they minimize interference with articular surfaces and reduce tissue irritation and immobilization. In the present experiment, compressive force was measured as a function of bone quality for two headless screw types, the Herbert and the Acutrak. Forty-seven cylindrical samples of cancellous bone were prepared from fresh, previously frozen human cadaveric distal femora. The compressive forces generated as the screws wer

[Compression osteosynthesis with percutaneous bone screws--clinical aspects and radiographic findings].

In a previous paper (Jacobs 1976) the development of an apparatus was described which achieved a compression osteosynthesis through the use of percutaneous bone screws without encroaching on the area of the fracture. As indication for this procedure we consider the condition which is described under the synonyms "post-traumatic osteomyelitis", "osteitis" and "fracture infection". Clinical experience is described with consideration of technical measurements - for the first time in vivo measurements of interfragmental compression in the patient and X-ray findings.

Bone density optimized pedicle screw insertion.

Background: Spinal fusion is the most common surgical treatment for the management of degenerative spinal disease. However, complications such as screw loosening lead to painful pseudoarthrosis, and are a common reason for revision. Optimization of screw trajectories to increase implant resistance to mechanical loading is essential. A recent optimization method has shown potential for determining optimal screw position and size based on areas of high bone elastic modulus (E-modulus). Aim: The aim of this biomechanical study was to verify the optimization algorithm for pedicle screw placement i

Alendronate improves screw fixation in osteoporotic bone.

BACKGROUND: Animal studies have demonstrated the efficacy of the use of bisphosphonates to enhance screw fixation in bone. In this prospective, randomized study of pertrochanteric fractures treated with external fixation, we tested whether systemic administration of bisphosphonates would improve the fixation of hydroxyapatite-coated screws implanted in osteoporotic bone. METHODS: Sixteen consecutive patients with a pertrochanteric fracture were selected. Inclusion criteria were female gender, an age of at least sixty-five years, and a bone mineral density T-score of less than -2.5 standard de

The Herbert bone screw: a ten year perspective.

It is now over ten years since the Herbert bone screw was first released in Australia. In 1984, Herbert and Fisher first published their experience with the use of this new bone screw in the management of scaphoid fractures. Since that time, there has been a growing interest in the technique and over 60 articles have appeared in the English literature. The purpose of this paper is to review and comment on the relevant publications as they relate to the biomechanical properties of the screw, to its use in the management of scaphoid fractures and to other applications in surgery of the hand.

Use of the Herbert bone screw for scaphoid nonunions.

The Herbert bone screw was used to treat 22 selected patients with established scaphoid nonunions. The indications for its use included evidence of avascular necrosis, proximal third fracture fragment, previous bone graft, fracture angulation or displacement, and a bipartite scaphoid. Treatment resulted in radiographic and clinical evidence of healing in 16 patients; 4 patients were symptom-free, but had incomplete healing as evidenced by radiograph; and 2 patients had persistent symptoms with radiographic evidence of nonunion.

[Percutaneous Herbert screw osteosynthesis of the scaphoid bone].

Scaphoid fractures are the most common carpal fractures. Conservative treatment is long, and non-union is frequent after immobilization in a cast. The Herbert procedure is an improvement, but access to the scaphoid is through an extensive open exposure that damages the blood vessels and the anterior radiocarpal ligaments. In order to solve this problem we used the Herbert screw for percutaneous internal fixation resulting in minimal operative trauma. This procedure requires intraoperative x ray guidance and the use of the accessories of the Herbert set except the "Jick". We operated 23 patient

Use of the Herbert bone screw in surgery of the wrist.

The Herbert bone screw is a new device, designed to produce compression and rigid fixation of small cancellous bone fragments. Being completely buried within the bone, it may be introduced through the articular surface of the joint. A second operation to remove the implant is not required. Although originally developed for fixation of scaphoid fractures, its use has been extended to other carpal bones and small joint fusions in the hand and wrist. The author's experience with this technique shows that excellent results may be achieved when rigid internal fixation allows for early mobilization

Lateral migration resistance of screw is essential in evaluating bone screw stability of plate fixation.

Conventional evaluation of the stability of bone screws focuses on pullout strength, while neglecting lateral migration resistance. We measured pullout strength and lateral migration resistance of bone screws and determined how these characteristics relate to screw stability of locking plate (LP) and dynamic compression plate (DCP) fixation. Pullout strength and lateral migration resistance of individual bone screws with buttress, square, and triangular thread designs were evaluated in polyurethane foam blocks. The screw types with superior performance in each of these characteristics were sel

The head to foot screw fixation. A new technique of percutaneous screw fixation of the scaphoid bone.

OBJECTIVES: Many techniques have been described for screw fixation of the scaphoid bone. The approach is either proximal or dorsal. A new percutaneous technique is presented by the authors called the head to foot screw fixation. Indications and results are evaluated. METHODS: It is a percutaneous technique with fixation of the scaphoid bone by two screws introduced in an opposite direction: a proximal screw and a distal screw. No postoperative immobilization was necessary. A prospective study was conducted in 40 patients over a period of three years. The average age was 25 years with extremes
