Complex fragmentation of the articular surface of the distal radius: management with small Kirschner wires and bone graft.

Severe intra-articular fractures of the distal radius with comminuted, displaced, and malrotated fragments are the most challenging fracture patterns. Reconstruction faces 3 major problems: fixation of fragments that are too small for stabilization by standard plates or screws, restoration of substantial cartilage loss in the articular surfaces, and extended metaphyseal/subchondral bony defects. For addressing these problems, a strategy is reported, by applying small, subchondrally placed Kirschner wires for the realignment of the articular surfaces, temporary inlay of a silicone foil in case

Management of scaphoid nonunion with iliac crest bone graft and K-wire fixation.

PURPOSE: Scaphoid nonunion can occur in both non-operative and operatively treated scaphoid fractures. Without treatment, this can lead to a predictable pattern of carpal collapse and degenerative arthritic change and patients can experience both pain and functional loss in the early and late phases of progression. An operative technique with a high success rate for union is important. This paper describes a technique for treatment of scaphoid nonunion with K-wire fixation and iliac crest cancellous bone graft. METHODS: A retrospective review from 1996 to 2010 was performed on a single senior

[Sequestered bone infection after Kirschner wire osteosynthesis of humeral head fracture].

Percutaneous stabilization of fractures of the humeral head by K-wires is a proved therapy. This proceeding provides enough stability for early moving of the joint and has a low incidence for complications. In the case described here the special is the cause for local infection: local osteonecrosis induced by drilling-holes too close together.

Tension band wiring-bone grafting for spondylolysis and spondylolisthesis. A clinical and biomechanical study.

Patients with spondylolysis or spondylolisthesis with persistent symptoms often require surgical treatment. The purpose of this article is to present a new surgical technique and clinical results of 13 patients with symptomatic spondylolysis/spondylolisthesis who were treated with tension band wiring (intra- or intersegmental) and bone grafting techniques. This article also presents the results of biomechanical effects of these tension band wiring methods on canine lumbar spines with experimental spondylolytic defects. Thirteen adult patients, three with spondylolysis, and ten with spondylolyt

Treatment of ununited fractures of the scaphoid by iliac bone grafts and Kirschner-wire fixation.

Of 151 ununited fractures of the scaphoid that were treated with iliac bone grafts and Kirschner-wire fixation through a volar approach, all but four (97 per cent) healed in an average of seventeen weeks, Three of the four failures resulted from obvious technical errors. Neither the preoperative existence of necrosis of the proximal fragment nor the location of the fracture affected the results. When there was mild radiocarpal arthritis preoperatively, it did not progress postoperatively; if there was moderate radiocarpal arthritis preoperatively, progression seldom was seen if a radial styloi

A comparative study of wire osteosynthesis versus bone screws in the treatment of mandibular prognathism.

In sagittal split osteotomies as well as in vertical osteotomies for reduction of the mandible, wire and bone screw osteosynthesis has been used in the Department of Oral and Maxillofacial Surgery of the University of Erlangen-Nuremberg within the last few years in a total of 221 cases of mandibular prognathism. In about one half of these cases a conventional wire osteosynthesis was performed, and in the other half a stable osteosynthesis with bone screws was used. One hundred twenty-one cases could be included in a follow-up study, and a true comparison could be made between the two methods.

Design of osteosynthesis plate for detecting bone union using wire natural frequency.

We have developed a novel osteosynthesis plate with bone union detection using a wire's natural frequency (BUDWF) to provide the quantitative result of bone union detection. The concept for detecting bone union is measuring the rate of frequency change. The frequency is measured from sound generated from the wire attached to a modified plate. The plate is modified from a Syncera ADLER B0409.10 and attached with 0.3 mm diameter 316L stainless steel wire. The sound generation mechanism was created by PEEK and installed on the plate to generate the sound. The preliminary experiments were conducte

Kirschner wire placement in scaphoid bones using intraoperative CT-guided stereotaxy.

BACKGROUND: Minimally invasive ostesynthesis of scaphoid fractures may reduce the risk of fracture non-union and shorten the duration of illness. The aim of this study was to analyze the technical feasibility and targeting accuracy of computed tomography (CT) - guided stereotactic Kirschner (K)-wire positioning in the scaphoid. METHODS: Nineteen Formalin preserved cadaveric upper extremities (10 right, 9 left) were fixed in 90 degree dorsal extension for percutaneous access from palmar. An ideal central position of the K-wire was planned on the computer adapted from intraoperative CT data. A

An intermaxillary fixation screw traction wire: an aid for facial bone fracture repair.

We have devised a new technique to improve stabilization of fractured facial bone fractures (frontal sinus fractures, zygomatic fractures, mandibular condyle fractures) by intermaxillary fixation screw traction wires (stainless steel wires through intermaxillary fixation screws). A retrospective study evaluating intermaxillary fixation screw traction wires was performed. We have used this technique for 3 cases of frontal sinus fractures, 9 cases of zygomatic fractures, and 7 cases of mandibular condyle fractures. After dissection of a fractured site, a hole is drilled on the fractured bone whe

Reinforcement of PMMA bone cement with a continuous wire coil--a canine femur study.

The hoop and axial strains present due to loading after femoral stem implantation in canine femurs, implanted with and without a wire coil surrounding the distal tip were investigated. One stem served as the control without wire coil while the other was experimental reinforced with a wire coil. Both stems were implanted by identical methods. Ideally, the wire coil should serve to reduce the hoop and axial strains present in the distal tip of the arthroplasty. The strains were measured using 90 degrees rosette strain gages. Though the coil's position was altered slightly during implantation usa

Effect of a figure-of-eight cerclage wire with two Kirschner wires on fixation strength for transverse metacarpal shaft fractures: an in vitro study with artificial bone.

BACKGROUND: Metacarpal shaft fractures are a common type of hand fracture. Despite providing strong fixation strength, plate fixation has numerous shortcomings. Concerning internal fixation with Kirschner wires (K-wires), although this approach is frequently used to treat metacarpal shaft fractures, the lack of functional stability may result in fixation failure. OBJECTIVE: To evaluate the effect of figure-of-eight cerclage wire on fixation for transverse metacarpal shaft fractures using two K-wires. MATERIALS AND METHODS: We used a saw blade to create transverse metacarpal shaft fractures i

Osteosynthesis of carpal scaphoid nonunion with interpositional bone graft and Kirschner wires: a 3- to 6-year follow-up.

BACKGROUND: Malreduced or neglected unstable carpal scaphoid fractures may lead to nonunion, which is often difficult to treat and carries the risk of carpal instability and degenerative arthrosis. Here, we describe a new "sandwich" method, which takes advantage of minimal dissection and has promising results. METHODS: From 1989 through 1992, we treated 39 patients with scaphoid nonunion by using the sandwich method that consisted of a wedge corticocancellous strut graft and numerous cancellous bone chips. Divergent Kirschner wires were used for fixation of reduction. We retrospectively revie

Intramedullary bone fragment obstructing passage of reaming guide wire with iatrogenic fractured tibia.

Reamed interlocking intramedullary fixation is the treatment of choice for displaced tibial shaft fractures in adults. In most cases it can be performed without difficulty; however, technical difficulties may be encountered during nailing in some cases. This article describes a case of closed nailing for a tibial shaft fracture in which intramedullary guide wire was obstructed by a small intramedullary bone fragment in the distal fracture segment. Forceful reaming and insertion of the nail led to a break in the cortex of the distal fragment and bending of guide wire. Finally, open reduction an

Repair of the defect in spondylolysis or minimal degrees of spondylolisthesis by segmental wire fixation and bone grafting.

Twenty-two patients with spondylolysis and minimal degrees of spondylolisthesis have been treated with repair of the defect by segmental wire fixation and bone grafting. Twenty-one patients are available for follow-up. Eighty percent of the patients obtained good to excellent results, and 90% obtained solid fusion of the pars defect. The technique would appear to have the greatest use in patients less than 30 years of age with minimal degrees of displacement.
