[First use of botulinum toxin type B in ENT patients with secondary therapy failure of botulinum toxin type A].

The first botulinum toxin type B (BT-B) formulation, NeuroBloc, has been licensed in Germany for the treatment of cervical dystonia since March 2001. This allows the treatment of patients with secondary failure of botulinum type A (BT-A) injections for the first time. Three patients with such a secondary failure were successfully treated with BT-B. A total of 1,000 mouse units (MU) NeuroBloc per eye were injected in a patient with blepharospasm. The treatment was effective for 4 months. In a patient with spasmodic dysphonia, 250 MU were injected into each vocal cord. This was effective for 3.5

Further studies using higher doses of botulinum toxin type F for torticollis resistant to botulinum toxin type A.

OBJECTIVE: A previous study of botulinum toxin type F (BTX-F) treatment for torticollis had shown a dose of 520 MU to be effective, but for a much shorter duration than is usual with botulinum toxin type A (BTX-A). The objective was to assess the effect of a higher dose of BTX-F. METHODS: Four of the previously treated patients, plus an additional patient, were treated with a higher dose of 780 MU BTX-F. All were secondary nonresponders to BTX-A due to neutralising antibodies. A test injection of 40 MU BTX-F was also given into the extensor digitorum brevis muscle (EDB), to examine the time c

Native botulinum toxin type A vs. redesigned botulinum toxins in pain: What did we learn so far?

Driven by the clinical success of botulinum toxin serotype A (BoNT/A) and the need for improved chronic pain management, researchers attempted to develop re-designed botulinum toxin (BoNT)-based molecules as novel analgesics. Various recombinant protein expression strategies including retargeted binding domains, and chimeric toxins combining different serotypes were tested to improve BoNT/A therapeutic safety margin and expand its efficacy. The aim of this review is to re-evaluate the current design strategies for recombinant BoNT-based molecules for pain treatment, compares their analgesic pr

Botulinum toxin therapy of migraine and tension-type headache: comparing different botulinum toxin preparations.

Most of the initial reports on botulinum toxin in tension-type headache (TTH) and in migraine were positive. Unfortunately, these results were not reproduced in well-designed, randomized controlled trials. So far, doses from 20 U (Botox) to 500 U (Dysport) have been studied in patients with chronic TTH, and doses from 16 to 200 U (Botox) in patients with migraine. Overall, there is no evidence for a beneficial effect of botulinum toxin, although trends favoring botulinum toxin were reported. Experience with botulinum toxin type B (Myobloc/NeuroBloc) is limited and similar to the experience wit

Oral toxicities of Clostridium botulinum type A and B toxins from different strains.

The production and the oral toxicity for mice of Clostridium botulinum type A and B toxins of different strains were studied. All five type B strains produced both 16S (large or L) and 12S (medium or M) toxins, although the relative amounts varied with the strains. The culture supernatant of type B Okra strain was the most potent in oral toxicity. The L toxin of this culture was about 700 times more toxic in feeding tests with mice than the L toxin from type B strain NH-2, whereas the M toxins of the two strains had the same oral toxicity. These results indicate that the oral toxicity of type

Botulinum toxin type a for great auricular neuralgia.

great auricular neuralgia is a rare disorder with only 18 cases described in the literature. Since it's a rare disorder, there are no evidence-based therapeutic recommendations but only case reports to guide physicians. We report a case of great auricular neuralgia treated with botulinum toxin type A subcutaneous injection with significant remission of pain. Botulinum toxin type A could be an effective and safe treatment in this setting; however, more studies are needed to confirm our results.

Aesthetic medicine is witnessing an increasing exploitation of all the procedures. The demand has never been higher than it is today. The number of practitioners is also increasing year by year. Consequently complications and other kinds of related troubles and procedures are also rising. Never like today is fundamental, in case of troubles, how to properly manage with the most frequent issues. In aesthetic medicine field, botulinum toxin procedures are the safest. Lot of patients are worried about botulinum toxin despite the available scientific literature. Rare short-term complications are o

Botulinum toxin type A in the treatment of Raynaud's phenomenon.

Raynaud's phenomenon is a vasospastic disorder clinically characterized by cold or stress-induced discoloration of the skin, pain and ulcers of the fingers or toes. Although this phenomenon might be self-limiting, there is a subgroup of patients requiring a therapeutic approach. The majority of patients do well on conservative measures; however, there is also a subgroup requiring systemic treatment. The efficacy of these systemic treatments is currently not thoroughly investigated. Furthermore, no uniform guidelines exist regarding the choice for a treatment option. In the past several years,

Botulinum toxin type A therapy for hemifacial spasm.

BACKGROUND: This is an update of a Cochrane Review, first published in 2005. Hemifacial spasm (HFS) is characterised by unilateral, involuntary contractions of the muscles innervated by the facial nerve. It is a chronic disorder, and spontaneous recovery is very rare. The two treatments routinely available are microvascular decompression and intramuscular injections with botulinum toxin type A (BtA). OBJECTIVES: To compare the efficacy, safety, and tolerability of BtA versus placebo in people with HFS. SEARCH METHODS: We searched CENTRAL, MEDLINE, Embase, reference lists of articles, and con

Mechanisms of Botulinum Toxin Type A Action on Pain.

Already a well-established treatment for different autonomic and movement disorders, the use of botulinum toxin type A (BoNT/A) in pain conditions is now continuously expanding. Currently, the only approved use of BoNT/A in relation to pain is the treatment of chronic migraines. However, controlled clinical studies show promising results in neuropathic and other chronic pain disorders. In comparison with other conventional and non-conventional analgesic drugs, the greatest advantages of BoNT/A use are its sustained effect after a single application and its safety. Its efficacy in certain thera

Application of Botulinum toxin type A: an arsenal in dentistry.

An extremely effective way of preventing damage to and enhancing treatment of dental hard tissues and restorations would be to ''de-programme'' the muscles responsible for excessive destructive forces and other gnathological-related diseases. The new paradigm is the intramuscular injection of Botulinum toxin type A (BOTOX) into the affected muscles. It is a natural protein produced by anaerobic bacterium, Clostridium botulinum. The toxin inhibits the release of acetylcholine (ACH), a neurotransmitter responsible for the activation of muscle contraction and glandular secretion, and its administ

Xeomin: an innovative new botulinum toxin type A.

Botulinum toxin type A (BoNT/A) is a well-established treatment for conditions characterized by muscle and autonomic nerve terminal overactivity, such as cervical dystonia and blepharospasm, and hyperhidrosis, respectively. BoNT/A is not digested in the gastrointestinal tract as it forms a complex with several proteins that protect and stabilize the neurotoxin. However, the pure neurotoxin is solely responsible for the therapeutic effect, and the complexing proteins have been shown to exhibit immunostimulating activity. The complexing proteins are not required for the stabilization of the neur

Botulinum toxin type A for treatment of refractory gastroparesis.

PURPOSE: The clinical efficacy and adverse reactions of botulinum toxin type A for refractory gastroparesis are discussed. SUMMARY: Botulinum toxin type A does not have an FDA-labeled indication for the treatment of gastroparesis, but it has been evaluated for this indication in several small case studies and open-label trials. The dose of botulinum toxin type A ranged from 80 to 200 units, injected directly into the pylorus in patients refractory to standard management. One study found both clinical and scintigraphic improvements in patients with diabetic gastroparesis that were maintained f

The treatment of masseter hypertrophy with botulinum toxin type A.

Masseter hypertrophy is a benign condition with variable causative factors, such as bruxism, temporomandibular disorders, malocclusion and others, but has an unclear etiology in the majority of cases. Surgical masseteric resection was the conventional method of treatment for the asymmetric swellings over the ramus and angle of the mandible on one or both sides, in addition to the generally unsuccessful treatments of occlusal adjustment, splint therapy, tranquilizers or others. An effective alternative was the local injection of very small doses of botulinum toxin type A into the masseter. The
