Obstetrical brachial plexus palsy.

In this article, the authors review their approach to evaluation, operative management, and reconstructive technique. Brachial plexus injuries in the newborn are usually managed nonoperatively. The timing and indications for primary surgery vary significantly between institutions. The motor examination is used to determine which infants would benefit from operative management. Patients are selected based on established criteria, such as the Toronto Test Score, applied at age 3 months. However, some cases are initially less clear, and we may recommend delaying operative management until age 6 m

Pediatric brachial plexus reconstruction.

Obstetrical brachial plexus palsy is commonly attributed to excessive traction applied to the baby's neck during a difficult delivery. The majority of infants with brachial plexus palsy recover spontaneously within the first 3 months of life. However, in 10 to 30 percent of cases, the recovery is incomplete. Global palsy and the absence of biceps muscle function at 3 months of age have been adopted as the main indications for early brachial plexus microsurgery. In late cases or when primary reconstruction has not yielded satisfactory results, secondary reconstruction will intervene as an enhan

Obstetrical brachial plexus palsy.

Obstetrical brachial plexus palsy, one of the most complex peripheral nerve injuries, presents as an injury during the neonatal period. The majority of the children recover with either no deficit or a minor functional deficit, but it is almost certain that some will not regain adequate limb function. These few cases must be managed in an optimal way. Considerable medical and legal debate has surrounded the etiologic factors of this traumatic lesion, and obstetricians are often considered responsible for the injury. According to recent studies, spontaneous endogenous forces may contribute subst

Obstetrical brachial plexus palsy.

Since the days of Hippocrates, scripts have included descriptions of infants who were unable to move their arms. However, it was not until the mid-1700s that an obstetric cause for the paralysis was considered. In 1872, the term obstetrical brachial plexus palsy was coined when a correlation was made between excessive traction on the brachial plexus during delivery and the clinical finding of arm paralysis. Surgical intervention became the norm in the beginning of the 19th century and continued until 1930. Poor outcomes and spontaneous resolution of obstetrical brachial plexus palsy prompted a

Obstetric brachial plexus palsy.

In a retrospective survey of 46 conservatively treated patients with obstetric brachial plexus palsy (OBPP) the following conclusions were reached. An immediate examination of the neurological status of the neonate root by root is essential for proper analysis of the course of the recovery. Mild cases with C5-C6 root injury (Erb's palsy) have a good outcome and may be treated conservatively. Intermediate cases with C5-C7 root injury with additional drop hand require close observation as early as age 3 weeks when the first electromyographic (EMG) examination is indicated. The second EMG should

Obstetrical brachial plexus lesions.

The renewal of interest in brachial plexus surgery has been led by the success of microsurgical techniques in other peripheral nerve repairs. The results in adults remain poor, but in obstetrical palsy the regenerative capacity of babies allows for dramatic recovery after nerve repair or reconstruction. Obstetrical brachial plexus injuries represent a unique group with similar aetiology, age and prognosis.

Brachial plexus traction injuries.

Brachial plexus traction injuries most frequently occur following acute flexion or extension of the neck. The symptomatology following this injury may be defined clearly into the anatomic patterns of upper trunk, lower trunk, posterior cord, medial cord, and lateral cord radiations. This injury is the most common cause of the neurogenic form of thoracic outlet syndrome and is frequently seen in conjunction with cervical spine disease as well as peripheral entrapment syndromes of the ulnar, radial, and medial nerves.

Obstetrical Brachial Plexus Palsy.

Obstetrical brachial plexus palsy is a disorder of the peripheral nervous system and occurs in as many as 0.4% of infants born. It is associated with shoulder dystocia, use of mechanical extraction, and macrosomia; it occurs more frequently in infants born by vaginal delivery. The unilateral injury to the brachial plexus complex occurs during the delivery phase as lateral traction is applied to the head to permit shoulder clearance. The infant typically presents in the delivery room with decreased active movements of the affected arm and asymmetrical primitive reflex responses. The severity of

Obstetrical brachial plexus palsy.

Obstetrical brachial plexus palsy is considered to be the result of a trauma during the delivery, even if there remains some controversy surrounding the causes. Although most babies recover spontaneously in the first 3 months of life, a small number remains with poor recovery which requires surgical brachial plexus exploration. Surgical indications depend on the type of lesion (producing total or partial palsy) and particularly the nonrecovery of biceps function by the age of 3 months. In a global palsy, microsurgery will be mandatory and the strategy for restoration will focus first on hand r

Axillary brachial plexus block.

The axillary approach to brachial plexus blockade provides satisfactory anaesthesia for elbow, forearm, and hand surgery and also provides reliable cutaneous anaesthesia of the inner upper arm including the medial cutaneous nerve of arm and intercostobrachial nerve, areas often missed with other approaches. In addition, the axillary approach remains the safest of the four main options, as it does not risk blockade of the phrenic nerve, nor does it have the potential to cause pneumothorax, making it an ideal option for day case surgery. Historically, single-injection techniques have not provide

Obstetric brachial plexus injury.

Obstetric brachial plexus injury (OBPI), also known as birth brachial plexus injury (BBPI), is unfortunately a rather common injury in newborn children. Incidence varies between 0.15 and 3 per 1000 live births in various series and countries. Although spontaneous recovery is known, there is a large subset which does not recover and needs primary or secondary surgical intervention. An extensive review of peer-reviewed publications has been done in this study, including clinical papers, review articles and systematic review of the subject. In addition, the authors' experience of several hundred

Obstetric brachial plexus injuries.

Obstetric brachial plexus lesions (OBPLs) are typically caused by traction to the brachial plexus during labor. The incidence of OBPL is about 2 per 1000 births. Most commonly, the C5 and C6 spinal nerves are affected. The prognosis is generally considered to be good, but the percentage of children who have residual deficits may be as high as 20% to 30%. Surgery should be restricted to severe cases in which spontaneous restoration of function is not likely to occur (ie, in neurotmesis or root avulsions). In this article, the authors present an overview of our current understanding of the neuro
