Identification of Concussion Subtypes Based on Intrinsic Brain Activity.

IMPORTANCE: The identification of brain activity-based concussion subtypes at time of injury has the potential to advance the understanding of concussion pathophysiology and to optimize treatment planning and outcomes. OBJECTIVE: To investigate the presence of intrinsic brain activity-based concussion subtypes, defined as distinct resting state quantitative electroencephalography (qEEG) profiles, at the time of injury. DESIGN, SETTING, AND PARTICIPANTS: In this retrospective, multicenter (9 US universities and high schools and 4 US clinical sites) cohort study, participants aged 13 to 70 yea

Concussion: Physical Therapy Treatment After Mild Traumatic Brain Injury.

A concussion is a mild traumatic brain injury caused by a sudden blow or jolt to the head. Any excessive force to the head is considered a "concussive event," even when a concussion hasn't been diagnosed. These concussive events can happen during sport or recreation, as a result of a fall or accident, or from an act of violence. Many people recover from these concussive events with rest and gradually get back to normal activities; for others, though, symptoms persist. The good news is that these symptoms may respond well to physical therapy. To ensure the best possible outcome, getting a thoro

Headache management in concussion and mild traumatic brain injury.

Headache is one of the most common symptoms after traumatic brain injury (TBI), and posttraumatic headache (PTH) may be part of a constellation of symptoms that is seen in the postconcussive syndrome. PTH has no defining clinical features; currently it is classified as a secondary headache based on its close temporal relationship to the injury. A growing number of studies are characterizing PTH by using primary headache classifications. Moderate to severe PTH that is often disabling may be classified as migraine or probable migraine and is found in substantial numbers of individuals. Recent da

Localization and Identification of Brain Microstructural Abnormalities in Paediatric Concussion.

In the United States, approximately 2.53 million people sustain a concussion each year. Relative to adults, youth show greater cognitive deficits following concussion and a longer recovery. An accurate and reliable imaging method is needed to determine injury severity and symptom resolution. The primary objective of this study was to characterize concussions with diffusion tensor imaging (DTI). This was performed through a normative Z-scoring analysis of DTI metrics, fractional anisotropy (FA), axial diffusivity (AD), and radial diffusivity (RD), to quantify patient-specific injuries and ident

Concussive brain injury in the military: September 2001 to the present.

Since the terrorist attacks of September 11, 2001, 1,348,405 citizens have been deployed to combat in Operation Iraqi Freedom (OIF), Operation New Dawn in Iraq, and Operation Enduring Freedom in Afghanistan (OEF). During this same period 266,810 (20%) of these individuals have been diagnosed with a traumatic brain injury (TBI). The majority of these were Army soldiers, with 155,282 (58%) receiving the diagnosis. Mild TBI comprised 82% of the total, with the remainder being moderate to severe. Over this same period the Department of Defense (DoD) has invested $374.9 million to enhance access an

Task and driving performance of patients with a severe concussion of the brain.

Patients who had sustained a severe concussion of the brain more than 2 years earlier were tested in choice reaction time tasks in the laboratory in order to investigate their impaired reactive capacity. The focus of investigation was whether specific stages in the chain of the information process are affected by the injury. In addition, driving skills were measured in an instrumented car to determine if reaction time performance is predictive of car driving. In general, patients were much slower than control subjects, both in the reaction time and in the driving tasks. The results obtained in

A review of brain regions and associated post-concussion symptoms.

The human brain is an exceptionally complex organ that is comprised of billions of neurons. Therefore, when a traumatic event such as a concussion occurs, somatic, cognitive, behavioral, and sleep impairments are the common outcome. Each concussion is unique in the sense that the magnitude of biomechanical forces and the direction, rotation, and source of those forces are different for each concussive event. This helps to explain the unpredictable nature of post-concussion symptoms that can arise and resolve. The purpose of this narrative review is to connect the anatomical location, healthy f

Concussions and Brain Injuries in Children: United States, 2020.

A concussion is a mild type of traumatic brain injury (1). Previous studies using national data have shown that concussion prevalence can differ by survey methodology and question wording (2). The 2020 National Health Interview Survey (NHIS) included questions on concussion to measure both symptoms and diagnosis from a health care provider to provide a more complete understanding of the public health burden, because children with mild injuries may not see a doctor or receive a diagnosis. This report presents national estimates of lifetime symptomatology and health care professional diagnoses o

A common neural signature of brain injury in concussion and subconcussion.

The midbrain is biomechanically susceptible to force loading from repetitive subconcussive head impacts (RSHI), is a site of tauopathy in chronic traumatic encephalopathy (CTE), and regulates functions (e.g., eye movements) often disrupted in concussion. In a prospective longitudinal design, we demonstrate there are reductions in midbrain white matter integrity due to a single season of collegiate football, and that the amount of reduction in midbrain white matter integrity is related to the amount of rotational acceleration to which players' brains are exposed. We then replicate the observati

Chronic post-traumatic headache: associations with mild traumatic brain injury, concussion, and post-concussive disorder.

Chronic post-traumatic headache (PTH) always arises from some sort of head injury such as mild traumatic brain injury or concussion. The terminology and definitions for these injuries remain controversial and continue to evolve. This article reviews and updates current terminology and definitions for these head injuries, along with current concepts of pathophysiology. The second portion of this article considers the persistence of the other symptoms associated with chronic PTH, which constitute the post-concussive disorder. PTH is the most common symptom after a head injury, but post-concussiv

Sex and gender differences in mild traumatic brain injury/concussion.

The high incidence of concussions/mild traumatic brain injury and the significant number of people with persisting concussion symptoms as well as the concern for delayed, neurodegenerative effects of concussions makes them a major public health concern. There is much to learn on concussions with respect to pathophysiology as well as vulnerability and resiliency factors. The heterogeneity in outcome after a concussion warrants a more personalized approach to better understand the biological and psychosocial factors that may affect outcome. In this chapter we address biological sex and gender as

Brain function associated with reaction time after sport-related concussion.

Concussion is associated with significant functional disturbances in the first week post-injury. Computerized neurocognitive testing tools have become widely adopted in concussion management, to identify specific domains of impairment and obtain more objective measures of recovery. Reaction time (RT) slowing is a common sequela of concussion, however, the functional brain networks that underlie RT performance remain under-studied in both healthy and concussed athletic cohorts. This study used blood-oxygenation-level-dependent function magnetic resonance imaging (BOLD fMRI) to evaluate resting

Post-concussion syndrome. Brain damage, constitutional characteristics and environmental reactions.

A 9-year-old boy was referred for child-psychiatric examination because of failure in school, restlessness, impulsiveness and concentration difficulties. The symptomatology was ascribed to a previous severe head injury and defined as a post-concussion syndrome. During the examination it was found that he had an identical twin brother without a history of brain-damage. A comparison of the patient and his twin brother with the same examination methods at the age of 9 and 18 gave similar results. These indicate that factors other than brain damage also could explain a clinical picture with school

Responsiveness of the Post-Concussion Symptom Scale to Monitor Clinical Recovery After Concussion or Mild Traumatic Brain Injury.

BACKGROUND: The Post-Concussion Symptom Scale (PCSS) is used to assess the number and intensity of symptoms after a concussion/mild traumatic brain injury. However, its responsiveness to monitor clinical recovery has yet to be determined. PURPOSE: To evaluate the responsiveness of the PCSS to change and longitudinal validity in patients with persistent postconcussive symptoms as well as to explore the responsiveness of other clinical outcome measures to monitor recovery of physical symptoms in patients with persistent postconcussive symptoms. STUDY DESIGN: Cohort study (diagnosis); Level of

Mechanical disruption of the blood-brain barrier following experimental concussion.

Although concussion is now recognized as a major health issue, its non-lethal nature has limited characterization of the underlying pathophysiology. In particular, potential neuropathological changes have typically been inferred from non-invasive techniques or post-mortem examinations of severe traumatic brain injury (TBI). Here, we used a swine model of head rotational acceleration based on human concussion to examine blood-brain barrier (BBB) integrity after injury in association with diffuse axonal injury and glial responses. We then determined the potential clinical relevance of the swine
