Breast density, benign breast disease, and risk of breast cancer over time.

OBJECTIVES: Assessing the combined effect of mammographic density and benign breast disease is of utmost importance to design personalized screening strategies. METHODS: We analyzed individual-level data from 294,943 women aged 50-69 years with at least one mammographic screening participation in any of four areas of the Spanish Breast Cancer Screening Program from 1995 to 2015, and followed up until 2017. We used partly conditional Cox models to assess the association between benign breast disease, breast density, and the risk of breast cancer. RESULTS: During a median follow-up of 8.0 year

Ipsilateral breast tumour recurrence in hereditary breast cancer following breast-conserving therapy.

The overall rate of an ipsilateral breast tumour recurrence (IBTR) after breast-conserving therapy (BCT) ranges from 1% to 2% per year. Risk factors include young age but data on the impact of BRCA1/2 mutations or a definite positive family history for breast cancer are scarce. We investigated IBTR after BCT in patients with hereditary breast cancer (HBC). Through our family cancer clinic we identified 87 HBC patients, including 26 BRCA1/2 carriers, who underwent BCT between 1980 and 1995 (cases). They were compared to 174 patients with sporadic breast cancer (controls) also treated with BCT,

Glycolytic enzymes in breast cancer, benign breast disease and normal breast tissue.

The activities of hexokinase, phosphofructokinase, aldolase, enolase and pyruvate kinase were studied in breast cancer tissues, in comparison to benign breast disease and normal breast tissues. The enzyme activities in breast cancer were significantly increased compared to normal and benign breast tissues (p less than 0.001). Also the increase in activity in benign disease compared to normal was statistically significant (p less than 0.001). Within the group of benign diseases, fibroadenomas could be distinguished from fibrocystic disease, the former generally showing higher activities compare

Mesothelioma presenting as a breast lump: when is a breast lump not a breast lesion?

A case of mesothelioma presenting as a breast lump. Relevant imaging is reviewed and the differential diagnoses discussed. The absence of the pectoralis muscle on mammogram is a potentially ominous sign which should not be assumed to be due to technical factors.

Family History of Breast Cancer, Breast Density, and Breast Cancer Risk in a U.S. Breast Cancer Screening Population.

Background: The utility of incorporating detailed family history into breast cancer risk prediction hinges on its independent contribution to breast cancer risk. We evaluated associations between detailed family history and breast cancer risk while accounting for breast density. Methods: We followed 222,019 participants ages 35 to 74 in the Breast Cancer Surveillance Consortium, of whom 2,456 developed invasive breast cancer. We calculated standardized breast cancer risks within joint strata of breast density and simple (1st-degree female relative) or detailed (first-degree, second-degree, or f

Breast density and risk of breast cancer.

Early studies reported a 4- to 6-fold risk of breast cancer between women with extremely dense and fatty breasts. As most early studies were case-control studies, we took advantage of a population-based screening program to study density and breast cancer incidence in a cohort design. In the Capital Region, Denmark, women aged 50 to 69 are invited to screening biennially. Women screened November 2012 to December 2017 were included, and classified by BI-RADS density code, version 4, at first screen after recruitment. Women were followed up for incident breast cancer, including ductal carcinoma

Breast asymmetry and predisposition to breast cancer.

INTRODUCTION: It has been shown in our previous work that breast asymmetry is related to several of the known risk factors for breast cancer, and that patients with diagnosed breast cancer have more breast volume asymmetry, as measured from mammograms, than age-matched healthy women. METHODS: In the present study, we compared the breast asymmetry of women who were free of breast disease at time of mammography, but who had subsequently developed breast cancer, with that of age-matched healthy controls who had remained disease-free to time of the present study. The study group consisted of 252

Breast-feeding and breast cancer in the offspring.

The causation of breast cancer in certain strains of mice by a virus that can be transmitted vertically, through the milk produced during lactation, has led to the hypothesis that a similar phenomenon could exist in humans. There have been laboratory-based studies in humans suggesting that a virus may be involved in the etiology of female breast cancer although other investigations did not support this hypothesis. Descriptive data and epidemiologic evidence of ecologic nature do not indicate a role of lactation in the causation of human breast cancer, but the hypothesis has not been adequately

Care of the breast and support of breast-feeding.

Primary care physicians can integrate care of the breasts through a woman's life cycle. Early and frequent nursing and careful attention to the infant's suckling position during the postpartum period can prevent several common problems. Neonatal jaundice, poor weight gain, mastitis, and candidiasis should be recognized and managed correctly. Outside the puerperium, fibrocystic change, nonlactational mastitis, and benign breast masses are encountered commonly. Problems related to silicone augmentation are discussed briefly.

[Tuberculosis of the breast misdiagnosed as breast cancer].

The authors report 15 patients with tuberculosis of the breast initially misdiagnosed as breast cancers. Criteria of differentiation of breast tuberculosis were: incidence (Tbc: cancer = 1: 245), lacking in constant site of involvement, possible concomitant involvement of other organs, eg. lung tuberculosis, youth of the patient, multi-para and history of breast feeding, in isolated cases, co-existence of tuberculosis and cancer is not impossible. The authors stress meticulous care in evaluating the history, symptoms, signs and supplementary examinations. Needle biopsy and pathology are indica

Breast cancer: management of the opposite breast.

Women who have or had cancer in one breast are at especially high risk for developing cancer in the contralateral breast. Other risk factors for second primary breast tumors include age, the use of radiation in treating the index cancer, and pathological characteristics of the original cancer. Management approaches range from close clinical and mammographic surveillance only to immediate prophylactic contralateral mastectomy. Routine and selective biopsy of the opposite breast at the time of treatment of the initial cancer have their proponents. The authors discuss these methods and the effect

Association of breast cysts and breast cancer.

A review of 915 consecutive patients with breast cancer and 812 with breast cysts showed that an association between the two is uncommon--5% of breast cancers were associated with cysts and 4% of cysts were associated with breast cancer. Four types of association were identified: (a) cystic cancers--easily diagnosed because of the characteristic features of the aspirate, failure of the mass to disappear and early recurrence in a patient whose age and menstrual status were not usually associated with cysts; (b) cancers occurring simultaneously with breast cysts--recognized because they did not

Management of the remaining breast in breast reconstruction.

Prophylactic subcutaneous mastectomy is performed only in high risk patients. In other cases, the breast is adapted if needed by conventional techniques. These are slightly modified to obtain a less conical shape of the breast and preserve the whole surface of the areola. In the majority of the cases the nipple-areolar complex will be used at a second stage as donor site for the reconstructed breast.

Breast Abscess Mimicking Breast Carcinoma in Male.

Male breast can show almost all pathological entities described in female breast. Inflammatory conditions of the breast in male are not common; however, occasionally, it can be encountered in the form of an abscess. Clinically, gynecomastia always presents as a symmetric unilateral or bilateral lump in the retroareolar region, and any irregular asymmetric lump raises a possibility of malignancy. Radiology should be used as a part of the triple assessment protocol for breast lump along with fine-needle aspiration cytology for definite diagnosis and proper management.

[Occult breast cancer discovered following breast reduction].

In plastic surgery, reduction mammaplasty is a common operation. The incidence of occult breast cancer detection is low but if diagnosed, consequences for the patient and the attending physician have a great impact. In this case, a 55-year-old woman complained of physical discomfort because of macromastia. Reduction mammaplasty was performed. Histopathological analysis revealed breast cancer of the breast reduction specimen. Once this diagnosis was made, the physicians looked more thoroughly into the patient's history, and a risk analysis focused on breast cancer. On a multidisciplinary level
