13C-breath tests in the study of microsomal liver function.

Conventional liver tests can be used to estimate a mixture of injury and function but none of these may be regarded as a reliable marker either to quantify functional hepatic reserve or to reflect life-threatening complications of acute and chronic liver diseases. To overcome this limit, many dynamic tests have been developed in order to evaluate the "hepatic functional mass". Among these tests we can include breath tests with 13C-labeled substrates undergoing different metabolic pathways. As concerning the evaluation of microsomal function, two main categories of breath tests have been develo

The use of breath tests in the study of malabsorption.

Breath analysis has evolved into a technique that can support nicely the evaluation of patients with diarrhoea and/or weight loss and in whom nutrient malabsorption is a possibility. The lactose and sucrose breath hydrogen tests offer direct (and probably the most sensitive) documentation of the malabsorption of carbohydrate and are currently widely used for clinical analysis. The 14C-xylose breath test is the most sensitive and specific breath test for detecting bacterial overgrowth; it may be used in combination with the labelled bile acid breath test to make the latter a sensitive and speci

Hydrogen and methane breath tests for evaluation of resistant carbohydrates.

This review considers in detail the background, principles, techniques, limitations and advantages of the hydrogen and methane breath tests. Resistant food carbohydrates, defined as dietary carbohydrates partly or totally escaping small intestinal assimilation, are fermented in the human colon. This results in production of H2, CH4 and volatile fatty acids. Increased colonic H2 production is a sensitive index of increased carbohydrate fermentation, and a rather constant fraction of the colonic H2 production is excreted by the lungs. It is therefore possible to assess mouth-to-caecum transit ti

Developments in novel breath tests for bacterial and fungal pulmonary infection.

PURPOSE OF REVIEW: Breath testing has developed over the last 20 years. New techniques that can identify fingerprints for specific diseases and specific markers of respiratory pathogens have been applied to breath analysis. This review discusses the recent advances in breath analysis for the diagnosis of bacterial and fungal lower respiratory tract infections. RECENT FINDINGS: The current techniques continue to develop rapidly, but preconcentration techniques are needed to analyse many target volatile organic compounds for most systems. Breath testing with an electronic nose is promising for

Abnormal fatty acid breath tests in cancer patients, with nutritional correlation.

In a group of cancer patients selected to exclude common causes of abnormal lipid absorption, we have examined this function using a breath test. Abnormal breath tests of fatty acid absorption (FABT) are found in most cancer patients, as has been previously claimed. These tests are abnormal in both quality (delayed peak in nine of ten patients, and quantity (reduced maximum peak in five of ten patients) of fatty acid absorption. In many patients abnormal FABT is not due to chemotherapy. Our preliminary results indicate no definite malabsorption mechanism but more stringent tests of absorption

Practical application of breath tests in disorders of gut-brain interaction.

Frequently occurring diseases of disordered gut-brain interactions are the irritable bowel syndrome and functional dyspepsia. Breath tests are noninvasive and are used to monitor a variety of gastrointestinal functions or conditions. Their general principle is the oral application of a test substance, the metabolism of which results in a substrate that can be measured in expiratory air. Clinically used breath tests use carbohydrates or stable 13C-enriched substrates. This review will focus on two questions, which breath tests are relevant for initiating treatments and which breath tests are us

Acidic colonic microclimate--possible reason for false negative hydrogen breath tests.

About 5% of normal subjects fail to produce increased hydrogen breath concentration after ingestion of the non-digestible carbohydrate lactulose (low hydrogen producers). The existence of low hydrogen producers limits the diagnostic use of hydrogen (H2) breath tests. We studied the effects of lactulose and of magnesium sulphate (MgSO4) pretreatment on stool-pH and on hydrogen exhalation after oral loading with lactulose or lactose in 17 hydrogen producers and 12 low hydrogen producers. In seven hydrogen producers acidification of stool pH by lactulose pretreatment (20 g tid) decreased hydrogen

[Hydrogen (H2) breath tests in the diagnosis of small intestine diseases].

We have applied the ion sensitive analysis of hydrogen (H2) in expired air in intervals of five minutes for five hours to determine carbohydrate malabsorption. In 19 patients with lactose malabsorption the increase in hydrogen was prior (45, SE 13 min) to healthy volunteers (142, SD 71 min, p less than 0.01). In comparison to blood glucose with the breath test no false positive results were observed. D-xylose absorption measured by breath test and renal excretion for five hours was determined in 24 volunteers and 12 patients with various intestinal diseases. There was a good correlation betwee

Diagnosis by Microbial Culture, Breath Tests and Urinary Excretion Tests, and Treatments of Small Intestinal Bacterial Overgrowth.

Small intestinal bacterial overgrowth (SIBO) is characterized as the increase in the number and/or alteration in the type of bacteria in the upper gastrointestinal tract and accompanies various bowel symptoms such as abdominal pain, bloating, gases, diarrhea, and so on. Clinically, SIBO is diagnosed by microbial culture in duodenum/jejunum fluid aspirates and/or the breath tests (BT) of hydrogen/methane gases after ingestion of carbohydrates such as glucose. The cultural analysis of aspirates is regarded as the golden standard for the diagnosis of SIBO; however, this is invasive and is not wit

Reevaluating our understanding of lactulose breath tests by incorporating hydrogen sulfide measurements.

BACKGROUND AND AIM: Breath testing has become a commonly used tool in gastroenterology to evaluate changes in the fermentation pattern of the gut microbiome. Currently, hydrogen and methane gas concentrations are measured in breath testing and evaluated against specific cut-off values for interpretation as normal or abnormal. However, microbial gas kinetics is a complex process that is not currently fully considered when interpreting breath gas results. Gas exchange between hydrogen producers and hydrogen consumers (methanogens and sulfate-reducing bacteria) is a process whereby hydrogen avail

Diffuse gastrointestinal dysmotility by ultrasonography, manometry and breath tests in colonic inertia.

Patients with colonic inertia, the most severe form of chronic functional constipation, may present with a more diffuse panenteric motility disorder. We describe a case of a woman with severe longstanding colonic inertia associated with chronic functional dyspepsia and defective gastric, gallbladder and small intestinal motility, as confirmed by several diagnostic tests including two breath tests with the 13C-stable isotope. A subclinical form of authonomic sympathetic neuropathy was diagnosed, providing a possible pathophysiological explanation for the presence of simultaneous multiple motili

Diagnostic Utility of Carbohydrate Breath Tests for SIBO, Fructose, and Lactose Intolerance.

BACKGROUND: Unexplained bloating, gas, and pain are common symptoms. If routine tests are negative, such patients are often labeled as irritable bowel syndrome. AIMS: To determine the diagnostic utility of breath tests that assess for small intestinal bacterial overgrowth (SIBO), fructose or lactose intolerance, and the predictive value of symptoms. METHODS: Patients with gas, bloating, diarrhea, abdominal pain (≥ 6 months), and negative endoscopy and radiology tests were assessed with symptom questionnaires, glucose (75 g), fructose (25 g), or lactose (25 g) breath tests. Breath tests were

Breath tests with stable isotopes: have they a role in liver transplantation?

Evaluation of liver function is crucial in the overall management of patients with liver disease. In particular, patients with end-stage liver disease need accurate prognostic indicators to plan liver transplantation, and in this case, to manage their presence in the waiting list. Availability of predictors of clinical outcome is further essential after liver transplant, mainly to correctly diagnose and adequately treat complications, such as acute rejection, drug toxicity, liver dysfunction. Breath tests using labelled substrates selectively metabolized within the liver may represent an accur

[13C-urea breath test].

A 13C-urea breath test(13C-UBT) is known to be a good diagnostic method for detecting H. pylori infection since it is non-invasive, simple and accurate. Its diagnostic capability in determining H. pylori eradication has been studied using 153 patients (who underwent the H. pylori eradication therapy) as subjects at two times: 6-8 weeks after the end of administration (at the time of determination of eradication) and 6 months afterwards. When determination was made, the sensitivity and specificity of 13C-UBT compared to culture and histology were 100% and 95.1%, respectively. Seven inconsistent

Limitations of the triolein breath test.

Patients being investigated for intestinal absorptive capacity were classified as normals or malabsorbers on the basis of three fat absorption tests. Malabsorbers were further classified as mild, moderate, severe or gross according to severity of malabsorption. Using this classification system the triolein breath test was evaluated in 53 patients. Seventeen patients were excluded because their graph of percentage breath [14C]carbon dioxide versus time was exponential indicating that the peak [14C]-carbon dioxide may be occurring later than the six hour duration of the test. Hydrogen breath test in schoolchildren.

The frequency of negative hydrogen breath tests due to colonic bacterial flora which are unable to produce hydrogen was determined after oral lactulose challenge in 98 healthy Dutch schoolchildren. There was a negative result in 9.2%. The probability of a false normal lactose breath test (1:77) was calculated from these results together with those from a separate group of children with lactose malabsorption (also determined by hydrogen breath test). A study of siblings and mothers of subjects with a negative breath test did not show familial clustering of this condition. Faecal incubation test
