A preliminary report is given for total joint replacement arthroplasty utilizing a silicone-polyester prosthesis, in selected cases of destroyed metatarsophalangeal joints of the great toe. Fifty-three prostheses were implanted over a period from March 1971 to March 1977, with a minimum follow-up of one year. Overall results were good from the standpoint of pain relief and cosmetic correction, in severely destroyed and deformed joints. The essential aspects for successful prosthetic arthroplasty include proper seating of the prosthesis and complete realignment of the first ray, paralleling the

Bunions occur in athletes. Often, the bunion is compensated and has a congruous joint, needing minimal treatment. If the bunion rapidly progresses, the sesamoids sublux, and the joint becomes incongruous; surgery becomes necessary. Surgery for decompensated bunions and secondary problems is discussed.

BACKGROUND: Hallux valgus is a common condition with a complex etiology resulting in numerous treatment options. Recurrence of the deformity can occur following correction. Surgical technique and possibly also postoperative care play a role in reducing recurrence rates. This article highlights a postoperative surgical dressing technique which allows for semirigid support during the immediate postoperative period.
METHODS: A wooden tongue depressor placed along the medial border of the hallux comprises the primary support for the dressing. The rigidity of the tongue depressor allows for the ha

The results of Mitchell osteotomy for repair of hallux valgus/bunion deformity are presented. Twenty-five consecutive patients on whom 33 first metatarsal osteotomy/bunionectomy procedures of the Mitchell-Hawkins variety demonstrate satisfactory and durable reduction of deformity. These was an average decrease in the first intermetatarsal angle of 6.1 degrees and an average decrease in the hallux abductus angle of 18.4 degrees. This procedure successfully corrects an elevated intermetatarsal angle and an elevated proximal articular set angle at a single osteotomy site. Nonunion of the osteotom

Evolution of the terminology applied to the bunion deformity has progressed in parallel with our changing understanding of the deformity itself. Along this path of progression have been multiple terms, sometimes with multiple meanings. Hallux valgus and metatarsus primus varus are 2 of the most common terms for the deformity. Although commonly used, these descriptors can have multiple meanings, and inconsistencies in interpretation can lead to confusion. We propose a more detailed terminology to provide a more accurate description of the entire bunion deformity in 3 planes and for both the hal

Bunion formation in adults is an example of bone growth that occurs after physis closure. Bone is laid down secondary to mechanical irritation caused by foot deformity. It is a mechanism of ectopic bone formation unrelated to physeal growth. In this article, bone formation is analyzed using immunohistochemical and cell culture techniques. Using markers specific for mesenchymal cells (collagen type IIa and fibroblast growth factor receptor 3), a cell population is defined in the soft tissues that overlie the bunion and is isolated from explant cultures. The cells do not produce bone matrix in c

Pes planus has been suggested as a factor in the development of bunions in adolescents. This study objectively evaluated the radiographs of 36 adolescents with 66 hallux valgus deformities for the presence of associated flatfeet. The bunion deformity in these adolescents was also characterized. Only 26.6% of these adolescents had an increased intermetatarsal angle and 8.1% an abnormal metatarsus varus angle as compared with the published norms. Measurements of pes planus including talar pitch, calcaneal plantar angle, dorsoplantar talonavicular angle, and lateral talocalcaneal angle showed an

A large exostosis was the source of a bunion deformity in a 60-year-old woman. Its unusual clinical and radiographic features were suggestive of a bizarre parosteal osteochondromatous proliferation. However, histologic features were most consistent with a benign osteocartilaginous exostosis.

There have been many surgical approaches described for the surgical treatment of tailor's bunions. The authors have been using a procedure that addresses both an elevated fourth to fifth intermetatarsal angle and an elevated lateral deviation angle, for the past 3 years. The metaphyseal osteotomy provides reliable correction of these two abnormalities and allows for uncomplicated rigid fixation.

Twelve patients who had a total of 18 Mitchell bunionectomies were reviewed to assess the long-term results of the procedure. Although metatarsus varus correction was maintained in all cases, hallux valgus recurred in 11 of the 18 cases. Sixty-seven percent reported complete relief or improvement of preoperative pain. Although lateral metatarsalgia did occur, the most common area of persistent pain remained the first metatarsal. Six of 18 procedures had marked loss of active joint motion, associated with pain and an unsatisfactory result. Of 18 procedures, 11 (61%) were satisfied with the resu

Current practice and controversies in UK bunion surgery are discussed. Patients tend to be offered a distally based metatarsal osteotomy, such as a chevron or scarf osteotomy, for mild to moderate symptomatic bunions. Greater deformities are managed with a more extreme scarf, supplemented with a proximal phalangeal osteotomy. A proximal fusion in the form of the Lapidus-type procedure is still reserved for the most severe, hypermobile, or arthritic cases. Minimally invasive techniques for bunions have failed to disseminate into common UK practice. The trends in the United Kingdom regarding lit

The results of bunion surgery on 50 feet in 31 adolescents are reviewed. Average follow-up was 3 years 2 months. The failure rate was 36%. Fifty-one percent of the children had hypermobile flatfeet, and 32% had a long first ray. The recurrence rate in these groups was 56 and 50%, respectively. There were 20 reoperations for either recurrence (12) or hardware removal (eight). Elective bunion surgery in adolescents should only be performed in the face of progressive, painful deformity where both the patient and the patient's parents fully understand the goals and risks of surgery.

BACKGROUND: Hallux valgus is classified as an abnormal deviation of the great toe (hallux) towards the midline of the foot.
OBJECTIVES: To identify and evaluate the evidence from randomised trials of interventions used to correct hallux valgus.
SEARCH STRATEGY: We searched the Cochrane Musculoskeletal Injuries Group trials register (2003/1), the Cochrane Central Register of Controlled Trials (The Cochrane Library issue 1, 2003), MEDLINE (January 1966 to March 2003) and EMBASE (1980 to January 2003). Pathologic conditions of the metatarsophalangeal joints comprise the majority of painful conditions of the forefoot. Clinical manifestations commonly include hallux valgus (bunion), bunionette ("tailor's bunion"), and metatarsalgia. Conservative treatment remains the hallmark of initial management of these disorders. Conservative measures emphasize the adequate accommodation of the forefoot within the toe box of the shoe and the even distribution of plantar pressures by various metatarsal pads or bars and soft insoles. Numerous and sometimes controversial surgical procedures have been develope

The modified Lapidus procedure with rigid internal fixation was used to correct adolescent bunion deformities in 20 patients (32 feet). All patients were female and skeletally mature at surgery. Their average age was 18 years. Preoperatively, 100% had bunion pain and trouble with shoe wear, and 93% had a hypermobile first ray. Sixteen patients (24 feet) were available for follow-up examination and radiographs. The average follow-up time for the entire group was 24.3 months (range 3-72 months). Excellent or good results were obtained in 91% of patients. One recurrence required reoperation with
