Demographics of the homeless in an urban burn unit.

There are few articles about the homeless in burn literature. We sought to determine the demographic characteristics of the homeless citizens admitted to an urban burn center. This was a retrospective review from March 1999 to May 2004. Statistical analysis included chi2 and one-way analysis of variance. There were 1615 burn admissions, and 73 (4.5%) of these patients were homeless. Although the %TBSA affected was similar for the homeless and domiciled patients, the mean (+/-SD) age of the homeless was 44 +/- 10 years and their length of stay was 15 +/- 15 days, compared with 31 +/- 22 years a

Vancomycin-resistant organisms on a burn unit.

The incidence of infections due to vancomycin-resistant organisms has significantly increased during the past several years. This is important because vancomycin has been the drug of choice for treatment of infections due to methicillin-resistant Staphylococcus aureus (MRSA). Enterococci resistant to vancomycin are now emerging, and MRSA organisms with intermediate resistance to vancomycin have been identified in some centers. Cross transfer of resistance will eventually lead to the widespread development of organisms that are more difficult to eradicate. In our burn unit, we have encountered

A 1-year prospective study of burns in an Irish paediatric burns unit.

Burns are the fourth leading cause of injury death in children in the USA, accounting for 1300 paediatric deaths annually. The majority of paediatric burns mortality and morbidity result from simple domestic accidents that are preventable. A prospective study of paediatric burns from 1 January 1992 to 1 January 1993 was undertaken at our burns unit to outline the profile of the Irish paediatric burns problem. A total of 336 burns were referred to our unit over the 12 months (80 per cent self-referrals, 15 per cent tertiary referrals from district hospitals and 5 per cent GP referrals). Sixteen

Pseudomonas aeruginosa burn wound infection in a dedicated paediatric burns unit.

BACKGROUND: Pseudomonas aeruginosa infection is a major cause of morbidity in burns patients. There is a paucity of publications dealing with this infection in the paediatric population. We describe the incidence, microbiology and impact of P. aeruginosa infection in a dedicated paediatric burns unit. METHODS: A retrospective review of patients with clinically significant P. aeruginosa infection between April 2007 and January 2010 in the burns unit at Red Cross War Memorial Children's Hospital in Cape Town, South Africa, was performed. RESULTS: During the 36-month study period, 2 632 patient

Using Cutaneous Functional Units (CFUs) to Understand Burn Characteristics Associated With Range of Motion at Hospital Discharge in Adult Burn Survivors.

BACKGROUND: Burn injuries often lead to the development of burn scar contractures (BSC) and subsequent motion loss, which significantly impact function and quality of life for burn survivors. Understanding the relationship between burn characteristics and motion outcomes is critical to improving patient care and outcomes. We used a Cutaneous Functional Units (CFU) framework to investigate acute burn features at or near the time of injury associated with a joint-level range of motion at hospital discharge. METHODS: This study is an exploratory analysis of data collected as part of a previous p

The Use of Oxandrolone in the Management of Severe Burns: A Multi-service Survey of Burns Centres and Units Across the United Kingdom.

Introduction Severe thermal burns are a catastrophic injury. Those surviving the initial insult are subject to life-long disability, prolonged hospital admission, nutritional issues and poor wound healing. Oxandrolone has been shown to reduce hospital duration and promote lean body mass. Despite not being licenced for use in burns trauma within the United Kingdom (UK), services across the country utilise Oxandrolone in the management of severe burns. We aim to analyse the use of Oxandrolone in major burns across burns services within the UK. Methods We conducted a survey across all burn centre

Angioinvasive mucormycosis in burn intensive care units: A case report and review of literature.

BACKGROUND: Mucormycosis is a rare, rapidly progressive and often fatal fungal infection. The rarity of the condition lends itself to unfamiliarity, delayed treatment, and poor outcomes. Diagnosis of fungal infections early enough to enable appropriate treatment occurs in less than half of affected patients. CASE SUMMARY: An 11-year-old girl with a history of 15% total body surface area scald burns involving both lower limbs progressed to develop angioinvasive mucormycosis. This further led to a thrombosis of the right external iliac artery and vein and rapidly progressive necrosis of surroun

A survey of airway and ventilator management strategies in North American pediatric burn units.

A survey was used to gather information regarding airway management patterns in thermally injured children. North American pediatric burn centers listed by the American Burn Association were sent a survey designed to examine patterns of pediatric airway management in children with acute respiratory failure. The sample population means for the number of patients ventilated more than 48 hours and the number of patients ventilated more than 48 hours with inhalation injury were used to separate centers into large and small pediatric burn centers. Small pediatric burn centers had less than 50 patie

Epilepsy as a Cause of Burns: Our Experience in Ain Shams University Burn Unit.

Lack of patient education, and unaffordable or inaccessible medications, especially in low- and middle-income countries such as Egypt, leave patients with epilepsy at a higher risk of suffering burn-related injuries, especially with the unpredictable nature of seizures, which leads to accidental burns. We present this study to investigate all epileptic patients whose seizures were involved in the burn injury who presented to the Ain Shams University Burn Unit from January 2019 to July 2022, examining the epidemiology and outcome. A total of 369 burn patients were admitted to out unit during th

Factors associated with delayed admission to the burn unit: A major burn center's experience.

BACKGROUND: Timely admission to the burn unit is crucial. Ideal burn care requires prompt interventions such as wound and body temperature management, infection control, and fluid resuscitation to prevent complications like burn progression and infection. In this study, we identify specific factors and outcomes associated with delayed admission to a regional burn center. METHODS: Patients admitted to a large urban burn center from January 2015 to December 2023 were retrospectively queried, with subsequent collection of demographic and outcome variables from chart review. Burn injuries among children from a region-wide paediatric burns unit.

Burns are known to be a cause of the most severe childhood injuries. The purpose of this retrospective study was to investigate socio-demographic and other factors involved in children being presented to a burns unit for treatment. This is the first reported comprehensive audit of burns admissions highlighting factors that may relate to the occurrence of burns in children. Raw data was obtained from the data service unit (DSU) and the ward registers of a paediatric burns unit. Of 1249 admissions, 1156 cases (92.5%) had clearly specified causes. The number of annual admissions ranged from 225 t

Epidemiology of childhood burns in the burn unit of Ain Shams University in Cairo, Egypt.

Childhood burns in Egypt are a significant problem, especially in families of low socioeconomic status. These families live in overcrowded flats, which lack proper hygiene and tend to use kerosene stoves, which lack any safety measures. Three hundred and five burned children presented to the burn unit of Ain Shams University over a 20 month period. Proportionately more boys than girls were injured. There was an increase in the incidence between the ages of 4 to 6 years. Scalds formed 56.7% of the cause of burns, while 38.6% were due to flame. In 3 and 1.6% the cause of burn was electrical and

Healthcare-Associated Infections in a Burn Treatment Unit.

INTRODUCTION: Severe burns significantly weaken the immune system and disrupt the skin's natural barrier, which increases the likelihood of healthcare-associated infections (HAIs) and raises the risk of mortality. METHODS: Based on CDC criteria, this study retrospectively examines the incidence of HAIs in burn patients hospitalized at a university hospital in São Paulo, Brazil, over five years from 2018 to 2022. RESULTS: 536 patients were treated during this time, with 130 HAIs recorded in 88 individuals. The average age of the patients was 41 years, and the mean total body surface area (TBS

Homelessness and Inpatient Burn Outcomes in the United States.

Burn injuries are common in the homeless population. Little is known regarding whether homeless patients experience different outcomes when admitted for burns. We aim to 1) characterize the admitted homeless burn population, and 2) investigate differences in inpatient outcomes between the homeless and non-homeless populations. A retrospective cohort study was performed utilizing the Nationwide Inpatient Sample. Adult patients with complete data for burn characteristics were extracted. Variables included demographic, burn, and facility characteristics. Homelessness was identified with Internati

Wound management in burn centres in the United Kingdom.

Despite advances in the use of topical and parenteral antimicrobial therapy, and the practice of early tangential excision, infection of the burn wound remains a leading cause of morbidity and mortality. The aim of this study was to survey and compare wound management and antibiotic usage in burn surgery in all UK burn centres. A postal questionnaire was used, followed up by telephone. Answers were obtained from all 39 units treating burns in the UK. A written policy on antibiotic usage was used by 13 centres. Excisions were covered routinely by prophylactic antibiotic therapy in 18 units, and

An Unusual Presentation to the Burns Unit-A Cautionary Tale.

The authors report a case of a 34-year-old female with systemic lupus erythematosus (SLE) and lupus nephritis who was referred to the regional burns service with a suspected contact burn to the left flank and hypogastrium from a hot water bottle. The patient had been applying a hot water bottle to the area at night for pain relief and after 3 days she noticed a localized area of erythema which became larger and began to blister over a period of 5 days. The blistered areas were erythematous and wet; however, the capillary refill time was sluggish. The lesion was initially diagnosed and treated

Use of speech pathology services in the burn unit.

Speech-language pathologists (SLPs) provide intervention for communication challenges and swallowing disorders (dysphagia) that are common in cases of severe burns because of ventilator use, prolonged or multiple intubations, and tracheostomy tubes. To identify current inclusion of SLP services in burn care management, a survey was sent to 139 burn units across the United States. Results revealed limited inclusion of SLPs in burn care despite pertinent areas of SLP expertise. Education of burn care teams on the benefits of SLP inclusion is needed. Potential benefits in pain and medical managem
