Selecting pharmacies for COVID-19 testing to ensure access.

Rapid diagnostic testing for COVID-19 is key to guiding social distancing orders and containing emerging disease clusters by contact tracing and isolation. However, communities throughout the US do not yet have adequate access to tests. Pharmacies are already engaged in testing, but there is capacity to greatly increase coverage. Using a facility location optimization model and willingness-to-travel estimates from US National Household Travel Survey data, we find that if COVID-19 testing became available in all US pharmacies, an estimated 94% of the US population would be willing to travel to

COVID-19 cases and testing in 53 prison systems.

BACKGROUND: COVID-19 has entered United States prison systems at alarming rates. Disparities in social and structural determinants of health disproportionately affect those experiencing incarceration, making them more vulnerable to COVID-19. Additionally, prisons are sites of congregate living, making it impossible to practice social distancing, and most prisons have relied only on incremental measures to reduce risk and spread of COVID-19. To more fully understand the impact that COVID-19 is having on incarcerated populations, it is critical to have systematic data on testing, test positivity

Overview of COVID-19 testing and implications for otolaryngologists.

BACKGROUND: Testing for SARS-CoV-2 is important for decision making prior to surgery in otolaryngology. An understanding of current and developing testing methods is important for interpreting test results. METHODS: We performed a literature review of current evidence surrounding SARS-CoV-2 diagnostic testing highlighting its utility, limitations, and implications for otolaryngologists. RESULTS: The currently accepted RT-PCR test for SARS-CoV-2 has varying sensitivity according to which subsite of the aerodigestive tract is sampled. Nasal swab sensitivities appear to be about 70%. Chest CT i

Geographical access to COVID-19 testing centers in India.

BACKGROUND: Geographic accessibility to testing constitutes an important part of pandemic preparedness. However, there is no data on access to COVID-19 testing centers in India. Our study looks at the current geographical accessibility to these testing centers across 36 states and union territories (UTs) and 735 districts. METHODS: We acquired the testing center geocodes from the Indian Council of Medical Research. We took motorized and walking travel-time friction surface rasters from the Malaria Atlas Project 2019 and high-resolution population estimates from WorldPop 2020. Using these, we e

Extraordinary Complication of COVID-19 Swab Testing: CSF Rhinorrhea.

Coronavirus disease 2019 (COVID-19) is a highly contagious disease that has reached pandemic status. The diagnosis of COVID-19 is made using the polymerase chain reaction test to evaluate a swab taken from the naso/oro-pharyngeal area. Since COVID-19 is still a pandemic, it is thought that the use of this diagnostic test will be on the agenda for a while. A patient with COVID-19 symptoms presented with unilateral cerebrospinal fluid rhinorrhea after a nasopharyngeal swab test. Although nasopharyngeal swabs are taken very frequently, the complication rate is very low. However, life-threatening

Increasing the COVID-19 immunization rate through allergy testing.

BACKGROUND: Vaccination of the population is required to combat the COVID-19 pandemic. Allergy testing could reduce anxiety towards COVID-19 vaccination and thereby may increase vaccination rate, however, its effectiveness remains unclear. METHODS: One hundred and thirty prospective real-life patients in need of but not daring to get vaccinated asked for allergy workup for COVID-19 vaccine hypersensitivity in 2021/2022. Characterization of patients, identification of anxieties, decrease of patient's anxiety levels, overall vaccination rate and adverse reactions after vaccination were assessed

Implementing a Covid-19 Testing Intervention in African American Testing Deserts in Chicago: an Academic-Community Partnership to Promote Covid-19 Health Equity.

PURPOSE: To design, implement, and evaluate a church-based community intervention to provide access to COVID-19 testing in African American neighborhoods in Chicago identified as testing deserts. METHODS: In collaboration with the Alive Faith Network (AFN), we created a network of church-based COVID-19 testing sites in underserved African American Chicago neighborhoods identified as testing deserts. Sites offered free SARS-CoV-2 polymerase chain reaction-based testing, culturally tailored education, and linkage to care and social resources. Over time, we transitioned to community sites due to

Salivary testing of COVID-19: evaluation of serological testing following positive salivary results.

BACKGROUND: Salivary detection of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has been proposed as an alternative to nasopharyngeal or oropharyngeal swab testing. Our group previously published a study demonstrating that both testing methods identified SARS-CoV-2 using polymerase chain reaction (PCR)-based detection methodology. We therefore conducted a follow-up study using antibody testing to evaluate the accuracy of saliva versus swabs for COVID-19 detection and the durability of antibody response. METHODS: Venous blood samples were collected from consenting participants a

Pneumocystis pneumonia, a COVID-19 mimic, reminds us of the importance of HIV testing in COVID-19.

While clinical environments are highly focused on COVID-19, reports of missed or delayed treatment for conditions that imitate COVID-19, such as pneumonia caused by the fungus Pneumocystis jirovecii, are emerging. Given the uncertain spectrum of COVID-19 presentations and variable sensitivity of laboratory tests for SARS-CoV-2, there is a risk that, without a high index of suspicion, alternative aetiologies may be overlooked while pursuing a diagnosis of COVID-19. The British HIV Association has been calling for the inclusion of HIV testing in all patients admitted to hospital with suspected C

Testing in two pandemics: community perspectives on HIV testing during COVID-19 in NYC.

The COVID-19 pandemic significantly disrupted healthcare systems, including access to HIV testing services. In 2021, community organizations participating in a NYC HIV testing initiative reported a substantial decline in HIV testing between 2019 and 2020. To examine how providers adapted during this period, Health Department staff conducted stakeholder interviews and focus groups to identify the underlying causes of this testing decline. Providers described both enabling and obstructive factors in delivering services during the pandemic. Telehealth, cross-training, service bundling and flexibl

COVID-19 testing decisions and behaviours in two Australian cities.

ISSUE ADDRESSED: High levels of testing are crucial for minimising the spread of COVID-19. The aim of this study is to investigate what prevents people from getting a COVID-19 test when they are experiencing respiratory symptoms. METHODS: Semi-structured, qualitative interviews were conducted with 14 purposively sampled adults between 20 November 2020 and 3 March 2021 in two capital cities of Australia and analysed thematically. The analysis included people who reported having respiratory symptoms but who did not undergo a COVID-19 test. RESULTS: Participants appraised risks of having COVID-

Trust, efficacy and ethicacy when testing prisoners for COVID-19.

PURPOSE: The outbreak of the severe acute respiratory syndrome coronavirus 2 virus and subsequent COVID-19 illness has had a major impact on all levels of society internationally. The extent of the impact of COVID-19 on prison staff and prisoners in England and Wales is unknown. Testing for COVID-19 both asymptomatic and symptomatic, as well as for antibodies, to date, has been minimal. The purpose of this paper is to explore the widespread testing of COVID-19 in prisons poses philosophical and ethical questions around trust, efficacy and ethicacy. COVID-19 testing and diagnosis: A comparison of current approaches.

The severe acute respiratory syndrome coronavirus 2 is a novel coronavirus that causes the coronavirus disease 2019 (COVID-19). COVID-19 has been declared a pandemic by the World Health Organisation since March 2020. To date, the number of confirmed COVID-19 cases has exceeded 47 million and more than 1.2 million people have lost their lives to the disease. The disease is spreading at an exponential rate with no signs of slowing down. COVID-19 testing and early diagnosis play a crucial role in not just patient management, but also the prevention of the further spread of the disease. Various di

An experimental test of the nicotinic hypothesis of COVID-19.

The pathophysiological mechanisms underlying the constellation of symptoms that characterize COVID-19 are only incompletely understood. In an effort to fill these gaps, a "nicotinic hypothesis," which posits that nicotinic acetylcholine receptors (AChRs) act as additional severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) receptors, has recently been put forth. A key feature of the proposal (with potential clinical ramifications) is the suggested competition between the virus' spike protein and small-molecule cholinergic ligands for the receptor's orthosteric binding sites. This noti

Nonserologic test for COVID-19: How to manage?

BACKGROUND: Diagnosis of Severe Acute Respiratory Coranavirus-2 (SARS-CoV-2) infection is currently based on real-time PCR (RT-PCR) performed on either nasopharyngeal (NPS) or oropharyngeal (OPS) swabs; saliva specimen collection can be used, too. Diagnostic accuracy of these procedures is suboptimal, and some procedural mistakes may account for it. METHODS AND RESULTS: The video shows how to properly collect secretions from the upper airways for nonserologic diagnosis of COVID-19 by nasopharyngeal swab (NPS), oropharyngeal swab (OPS), and deep saliva collection after throat-cleaning maneuver
