PURPOSE OF REVIEW: The discovery of the extracellular calcium-sensing receptor, CasR has broadened our understanding of calcium homeostasis and led to the development of new pharmacological agents, calcimimetics, for treating hyperparathyroidism. In the present review, I discuss the function of CasR as well as provide evidence for the presence of additional calcium-sensing mechanisms in the skeleton and possibly other tissues.
RECENT FINDINGS: Inactivating and activating mutations of the CasR respectively cause hereditary hyperparathyroidism, and demonstrate the predominant role of the CasR i

The calcium-sensing receptor (CaSR) plays an important role in sensing extracellular calcium ions and regulating parathyroid hormone secretion by parathyroid gland cells, and the receptor is a suitable target for the treatment of hyperparathyroidism. Cinacalcet hydrochloride is a representative CaSR agonist which widely used for the hyperparathyroidism. However, it has several issues to clinical use, such as nausea/vomiting and strong inhibition of CYP2D6. We tried to improve these issues of cinacalcet for a new pharmaceutical agent as a preferable CaSR agonist. Optimization from cinacalcet re

Hypoparathyroidism is a rare endocrine disorder characterized by deficient secretion of parathyroid hormone (PTH), resulting in hypocalcemia, hyperphosphatemia, and impaired mineral homeostasis. Although most cases are postsurgical, the disease encompasses a heterogeneous group of etiologies, including genetic, autoimmune, and infiltrative causes. For much of its history, hypoparathyroidism was considered unique among endocrine deficiencies in that it was not treated with hormone replacement, owing to limited understanding of parathyroid physiology and the absence of safe and effective PTH-bas

RATIONALE: Hypocalciuric hypercalcemia is classified as acquired hypocalciuric hypercalcemia (AHH) and familial hypocalciuric hypercalcemia (FHH). While FHH is inherited as an autosomal dominant trait, AHH is one of the rare acquired diseases and is usually treated with prednisolone. Here, we report a case with relapsing AHH which was well controlled with cinacalcet therapy.
PATIENT CONCERN: A 68-year-old Japanese man was referred to our institution because of hypercalcemia. Despite such hypercalcemia, he was almost asymptomatic.
DIAGNOSTICS: We diagnosed him as AHH due to the following reas

Parathyroid hormone (PTH) has attracted considerable interest as a bone anabolic agent. PTH plays a central role in regulating calcium phosphate metabolism and its increases in production in response to low serum calcium levels. A continuous hypersecretion of PTH, as occurs in primary hyperparathyroidism, leads to bone resorption. In this study, the effect of different doses of parathyroid hormone (PTH) on bone mineral content (BMC) in rats was investigated by particle-induced X-ray emission (PIXE). This study will help in investigating further the toxicity of extremely high doses of PTH on BM

OBJECTIVE: While activation of the calcium (Ca) sensing receptor (CaSR) suppresses parathyroid hormone (PTH) secretion, calcitonin (CT) secretion is stimulated via CaSR. The aim of this study was to evaluate PTH and CT responses during a calcium infusion test (CIT) in patients with primary hyperparathyroidism (PHPT).
METHODS: This pivotal prospective study included 64 patients (44 PHPT patients vs. 20 healthy controls [HCs], median age 57 [25-79] vs. 56 [39-74] years). All PHPT patients underwent parathyroidectomy (PTX). A week before and 1 month after PTX, the CIT was performed (bolus infusi

General knowledge of the role of vitamin D3 in human physiology has been shaped by its discovery as a preventive agent of nutritional rickets, a defect in bone development due to inadequate uptake of dietary calcium. Studies on the function of the hormonal form of vitamin D3, 1alpha,25-dihydroxyvitamin D3, have been greatly accelerated by the molecular cloning and structural analysis of the vitamin D3 receptor, which is a ligand-activated regulator of gene transcription. Molecular genetic techniques including genomics have helped reveal that 1alpha,25-dihydroxyvitamin D3 can control more than

The calcium sensing receptor (CaSR) and fibroblast growth factor 23 (FGF-23) play central roles in the regulation of calcium and phosphorus metabolism, respectively. CaSR controls parathyroid hormone secretion and renal calcium reabsorption. Inactivating mutations of the CaSR result in conditions characterized by hypercalcemia and hypocalciuria, whereas activating lesions cause hypoparathyroidism and hypercalciuria. Calcimimetics are a group of agonists for the CaSR that have been shown to be powerful agents in the treatment of secondary hyperparathyroidism. FGF-23 acts on the kidney to inhibi

Recognizing the role of the extracellular calcium-sensing receptor (CaR) in mineral metabolism greatly improves our understanding of calcium homeostasis. The biology of the low affinity, G-protein-coupled CaR and the effects of its activation in various tissues are reviewed. Physiological roles include regulation of parathyroid hormone (PTH) secretion by small changes in ionized calcium (Ca2+) and control of urinary calcium excretion with small changes in blood Ca2+. The CaR also affects the renal handling of sodium, magnesium and water. Mutations affecting the CaR that make it either less or

Calreticulin is an ancient and highly conserved protein. It is intensively studied and has been assigned multiple functions, the scope and variety of which are exceptionally wide for a single protein. Subsequent to the description of its calcium binding properties, calreticulin has been characterized as a molecular chaperone, an extracellular lectin, an intracellular mediator of integrin function, an inhibitor of steroid hormone-regulated gene expression and a C1q-binding protein. That one protein can perform so many functions is at once intriguing and controversial and further investigation i

The antiepileptic drugs are known to alter the bone metabolism and result in various range of pathological conditions (osteoporosis, osteomalacia, vitamin D deficiency, secondary hyperparathyroidism). These clinical conditions developed either due to direct effect of these therapeutic agents on bone or due to alteration in the homeostasis of other regulators of bone metabolism like serum calcium, vitamin D, and parathyroid hormone. Long-term therapy with antiepileptic agents results in higher incidence of pathological fracture, which is a consequence of either osteoporotic or osteomalasic (sec

The calcium-sensing receptor (CaSR), a key member of the family C G protein-coupled receptors (GPCRs), plays a crucial role in regulating calcium homeostasis and parathyroid hormone (PTH) secretion. It responds to various physiological ligands, including calcium ions and amino acids, activating multiple signaling pathways through interactions with different G proteins and β-arrestin. This review focuses on the structural features of CaSR, emphasizing recent advances in understanding its activation mechanisms via agonists and allosteric modulators. CaSR holds significant therapeutic potential,

IMPORTANCE: Postoperative hypocalcemia is the most frequent complication after total thyroidectomy. Prophylactic supplementation with calcium and calcitriol (C+C) is widely used, but selective administration based on early postoperative parathyroid hormone (PTH) levels may reduce overtreatment.
OBJECTIVE: To compare the incidence of symptomatic and biochemical hypocalcemia with routine prophylactic C+C and selective supplementation guided by postoperative PTH levels.
DESIGN, SETTINGS, AND PARTICIPANTS: This was a multicenter, pragmatic, randomized clinical trial conducted from June 1, 2022,

Vitamin D was discovered as a preventive agent of nutritional rickets, a defect in bone development due to inadequate uptake of dietary calcium. However, a variety of studies over the last several years has revealed that Vitamin D controls much more than calcium homeostasis. In particular, there is widespread evidence that the hormonal form of Vitamin D, 1alpha,25-dihydroxyvitamin D(3) [1,25(OH)(2)D(3)], is an important regulator of cellular differentiation and proliferation. Direct genomic signaling by 1,25(OH)(2)D(3) occurs through the Vitamin D receptor, which is a nuclear receptor and a li

Mitochondria play a critical role in regulating cellular functions, such as redox signaling, calcium homeostasis, and apoptosis. Also, mitochondria are crucial for neurogenesis and neuronal functions. Melatonin is an indole analog hormone, which is generally produced by the pineal gland. It plays a vital role in circadian rhythm and act as a powerful antioxidant by scavenging free radicals, immunomodulators, and anticancer agents. Schizophrenia and mood disorders are the two major psychiatric disorders. Disturbances of sleep and circadian rhythms are well-known symptoms of schizophrenia and mo

Pre-eclampsia is a multisystem pregnancy disorder characterised by variable degrees of placental malperfusion, with release of soluble factors into the circulation. These factors cause maternal vascular endothelial injury, which leads to hypertension and multi-organ injury. The placental disease can cause fetal growth restriction and stillbirth. Pre-eclampsia is a major cause of maternal and perinatal mortality and morbidity, especially in low-income and middle-income countries. Prophylactic low-dose aspirin can reduce the risk of preterm pre-eclampsia, but once pre-eclampsia has been diagnose

AIMS/HYPOTHESIS: Nutrient homeostasis requires integration of signals generated by glucose metabolism and hormones. Expression of the calcium-stimulated adenylyl cyclase ADCY8 is regulated by glucose and the enzyme is capable of integrating signals from multiple pathways. It may thus have an important role in glucose-induced signalling and glucose homeostasis.
METHODS: We used pharmacological and genetic approaches in beta cells to determine secretion and calcium metabolism. Furthermore, Adcy8 knockout mice were characterised.
RESULTS: In clonal beta cells, inhibitors of adenylyl cyclases or
