Incentives for Access: Changes in Admissions and Care Delivery for Metastatic Cancer Patients in Medicare Skilled Nursing Facilities, After the Patient-Driven Payment Model.

BackgroundCancer patients experience suboptimal care in skilled nursing facilities (SNFs). Medicare's Patient-Driven Payment Model (PDPM) incentivized SNFs to admit complex patients, potentially promoting admissions without appropriate changes in care delivery.ObjectiveTo evaluate changes in SNF admissions, rehabilitation therapy volume, and chemotherapy/radiation around admission or discharge, following PDPM and COVID-19.MethodsAdjusted interrupted time series examining 1,125,782 hospital stays from metastatic cancer patients, using 100% Traditional Medicare claims (2018-2021).ResultsPDPM was

Distance to Health Care Facilities, Lifestyle Risk Factors, and Stage at Diagnosis in relation to Geographic Pattern of Esophageal Cancer in Tanzania, 2006-2016.

Esophageal cancer is an aggressive, often deadly disease globally that represents a significant health problem in Tanzania. The WHO reported 604,100 new esophageal cancer cases worldwide during 2020 and 544,076 deaths (Sung, 2021; World Health Organization, 2020). In Eastern Africa, 16,137 cases and 15,188 deaths were related to this disease in 2020. Esophageal cancer is associated with various etiologic risk factors, and access to the disease treatment is a major barrier to survival. This study examined associations between the prevalence of four geographically stratified, population-level, e

Three-year survival of breast cancer patients attending a one-stop breast care clinic nested within a primary care health facility in sub-Saharan Africa-Zambia.

In Zambia, women with breast symptoms travel through multiple levels of the healthcare system before obtaining a definitive diagnosis. To eradicate this critical barrier to care, we nested a novel breast specialty service platform inside a large public-sector primary healthcare facility in Lusaka, Zambia to offer clinical breast examination, breast ultrasound, and ultrasound-guided core needle biopsy in a one-stop format, tightly linked to referral for treatment. The objective of the study was to determine the life expectancy and survival outcomes of a prospective cohort of women diagnosed wit

Uptake of cervical cancer screening services among women of reproductive age enrolled in HIV care: a facility-based cross-sectional study in rural Northern Uganda.

BACKGROUND: Cervical cancer is a leading cause of morbidity and mortality among women with HIV, who are at higher risk due to immune suppression. Despite the integration of cervical cancer screening into HIV care, uptake remains suboptimal in Uganda. This study assessed the uptake of cervical cancer screening and associated factors among women of reproductive age enrolled in HIV care in Northern Uganda. METHODS: A facility-based cross-sectional study was conducted among women with HIV (WWHIV) aged 15–49 years attending antiretroviral therapy (ART) clinics at 11 health centres in Northern Ugand

Assessing and Listening to Individual Goals and Needs (ALIGN) versus enhanced usual care for hospitalized older patients with cancer discharged to skilled nursing facilities: Protocol for a pilot randomized controlled trial.

INTRODUCTION: Hospitalized older adults with malignant solid tumor cancer discharged to skilled nursing facilities (SNFs) are at high risk of being "rehabbed to death." These patients have high 6-month mortality, experience burdensome care transitions near the end of life, and have limited access to palliative care. The Assessing and Listening to Individual Goals and Needs (ALIGN) intervention aims to address unmet palliative care needs and leverages the skills of palliative care social workers (PCSWs) to support serious illness communication, decision-making, and caregiver needs. We report th

Association between HIV Serostatus and premalignant cervical lesions among women attending a cervical cancer screening clinic at a tertiary care facility in southwestern Uganda: a comparative cross-sectional study.

BACKGROUND: Uganda has approximately 1.2 million people aged 15-64 years living with human immunodeficiency virus (HIV). Previous studies have shown a higher prevalence of premalignant cervical lesions among HIV-positive women than among HIV-negative women. Additionally, HIV-infected women are more likely to have human papilloma virus (HPV) infection progress to cancer than women not infected with HIV. We determined the prevalence of premalignant cervical lesions and their association with HIV infection among women attending a cervical cancer screening clinic at Mbarara Regional Referral Hospi

Determining the Cancer Center priorities at the Wilmot Cancer Institute: A proposed framework informed by an academic-community collaborative approach.

Current National Cancer Institute (NCI) Cancer Center Support Grant guidelines include a Community Outreach and Engagement merit descriptor related to the justification of Cancer Center priorities. Unfortunately, there is limited guidance and published literature on the process of determining the Cancer Center priorities. The purpose of this commentary is to propose a framework for Cancer Center prioritization, informed by the academic-community collaborative approach that the Wilmot Cancer Institute (Wilmot) at the University of Rochester implemented as part of a successful application for NC

[Organization of the health care of patients with breast cancer].

The paper discusses the 5-year experience of operation of cancer control service provided at a hospital department with a capacity of 50 beds as well as the results of two out-patient mammological screenings conducted by the Dispensary staff. Coordination of this work yielded a unified approach to the diagnosis and treatment of precancer and cancer of the breast. It also ensured continuity in application of procedures of therapy and prophylactic medical examination in the entire cancer control service of the Region. As a result, early diagnoses of breast cancer have become more reliable and tr

Employment-related Education and Support for Cancer Survivors: a Content Analysis of Employment Resources Offered on National Cancer Institute-Designated Cancer Center Websites.

High rates of employment changes and associated concerns among cancer survivors following diagnosis and treatment suggest a need to examine what employment-related educational resources and support are currently being offered to cancer survivors and what gaps exist in those resources. In 2023, we conducted a content analysis of employment resources described on the websites of the NCI-Designated Cancer Centers that provide clinical care (N = 64) through a systematic review procedure using predetermined search terms and a standardized process to examine the availability and accessibility of suc

Occupational therapy, cancer, and occupation-centred practice: impact of training in the model of human occupation.

INTRODUCTION: This study examined the influence of training in an occupation-centred model on the practice of occupational therapists working in a cancer hospital. There is an increased need for occupation-based rehabilitation services for individuals with and surviving cancer. Incorporating an occupation-centred model into practice has unique challenges for occupational therapists working in oncology settings. Utilizing an occupation-centred model of practice may influence the therapeutic reasoning of occupational therapists. METHODS: A generic qualitative inquiry (Patton, 2015) was used to

[Completeness of the cancer registry].

In 1985 the regional cancer registry in the IKMN area started. Pathology reports are used as the primary source of information for the registry. In order to evaluate the completeness of the registry, the registry database was compared with the discharge diagnosis register of the participating hospitals (LMR). In 1986 a total of 3546 tumours were registered by the registry, using the pathology reports as the primary source. The comparison with the LMR revealed that 616 patients were included in the cancer registry but not in the LMR. On the other hand 376 patients, in 180 of whom the diagnosis

[Specialised treatment of colorectal cancer in certified cancer centres: Do patients really have to travel further?].

INTRODUCTION: In Germany, many cancer patients are treated outside of cancer centres certified by the German Cancer Society (DKG) resulting in underuse of these facilities and inferior oncological treatment. One way to address this issue would be to restructure the healthcare landscape by following the Danish approach that limits cancer treatment to specialized hospitals. Such an approach would have an impact on the travelling times to treatment centers. The present study determines the impact on patient travel times using the example of colorectal cancer. METHODS: For the present analysis, d

Strategic governance of integrated nuclear medicine, diagnostic radiology, and radiation therapy in comprehensive cancer centres: A balanced scorecard framework.

Comprehensive cancer centres depend on nuclear medicine, diagnostic radiology and radiation therapy for diagnosis, staging, treatment planning, image guidance, treatment delivery, response assessment and follow-up. Although these services are clinically interdependent, they are often governed through separate departmental structures, budgets, information systems, quality routines and performance reports. This fragmentation can limit the ability of cancer-centre leaders to balance access, clinical quality, patient safety, workforce capacity, equipment sustainability, innovation and data steward
