The 2002 European registry of cardiac catheter interventions.

OBJECTIVE: The tradition of yearly reports on cardiac catheter interventions in Europe has been initiated in 1992. This 11th report presents aggregated data on cardiac catheter procedures in 30 European countries in the year 2002. DESIGN AND SETTING: A detailed questionnaire addressing summary data of all cardiac interventions was mailed to presidents or delegates of the national societies of cardiology in Europe. The questionnaire was distributed to all institutions with cardiac catheterisation programs. All questionnaires were compiled in a national summary data sheet, then entered into a c

[Complications in 1000 cardiac catheter examinations in childhood].

Complications of cardiac catheterization of 1 000 subsequent investigations of pediatric patients were prospectively recorded. 18.2% of all patients presented in the newborn period with usual distribution of cardiac malformations of this age group. Total mortality was 1.8% within the first 24 hours. Newborn babies who were commonly hypoxic and in cardiac failure at the time of catheterization had a mortality of 9.3%. Causes of death were cardiac failure, hypoxia and severe arrhythmias. Comparing the first and second five hundred patients, mortality in the newborn seems to decrease with anaesth

Recent Developments in Catheter-based Cardiac Procedures.

Since the first transcatheter aortic valve implantation in 2002, catheter-based cardiac procedures have increased significantly. This review provides an overview of CE approved devices, updated guidelines, and the key clinical findings for TAVR as well as for AV Valve interventions. Recent studies compare balloon-expanding and self-expanding valves, highlighting differences in prosthesis mismatch and paravalvular leaks. Long-term trials confirm TAVR's efficacy, especially in low-risk patients. Challenges include paravalvular regurgitation, vascular complications, and coronary obstruction, with

Catheter and Device Management of Inherited Cardiac Conditions.

This state-of-the art review discusses sudden cardiac death (SCD) risk stratification and prevention using implantable cardioverter defibrillator (ICD) therapy and the place of catheter ablation in the major inherited cardiomyopathies and primary arrhythmic syndromes. ICD therapy protects against SCD in many inherited cardiac conditions, particularly the cardiomyopathies in advanced stages, such as hypertrophic cardiomyopathy (HCM) and arrhythmogenic right ventricular cardiomyopathy (ARVC). However, they are not usually indicated in most patients with cardiac ion channelopathies, particularly

Catheter ablation to prevent sudden cardiac death.

Since ventricular arrhythmias are a common cause of sudden cardiac death (SCD), treatment for ventricular arrhythmias is the target area of interest in research field. Among different means to prevent ventricular arrhythmias, catheter ablation (CA) has emerged as an effective therapeutic method. CA can decrease the likelihood of SCD in the following arrhythmia categories: 1) idiopathic ventricular fibrillation (VF) that is usually triggered by premature ventricular beats originating in the Purkinje fibres; 2) VF in subjects with structural heart disease, especially after myocardial infarction,

Catheter ablation of atrial arrhythmias in cardiac sarcoidosis.

BACKGROUND: We previously reported on the incidence and clinical implications of supraventricular arrhythmia in patients with cardiac sarcoidosis (CS). The role of catheter ablation for the management of atrial arrhythmia (AA) in this patient population is unknown. METHODS AND RESULTS: One hundred consecutive patients with CS were monitored for the incidence of supraventricular arrhythmias. Those with persistent symptoms despite optimal medical therapy proceeded to catheter ablation. Following ablation, all patients were followed serially with Holter monitoring or device interrogation. Radiofrequency catheter ablation for the management of cardiac tachyarrhythmias.

Radiofrequency catheter ablation techniques allow for safe and highly effective curative therapy of a variety of cardiac dysrhythmias. The technique involves the delivery of a high-frequency, alternating electrical current through an intravascular catheter to sites of arrhythmogenic myocardium. This current induces resistive electrical heating of the tissue, resulting in discrete areas of myocardial destruction through coagulation and desiccation. Dysrhythmias most commonly treated with these techniques are atrioventricular nodal reentry and tachycardias related to accessory atrioventricular b

Cardiac tamponade from peripherally-inserted central venous catheters in neonates: Three case reports.

Pericardial effusion (PCE) and cardiac tamponade (CT) are rare but life-threatening complications of percutaneouslyinserted central catheter (PICC) use in neonates. There is often a lack in index of suspicion in a neonate with sudden deterioration, resulting in high mortality. We describe a series of three cases of PICC-related PCE/CT in low birth weight infants whose timely diagnosis with echocardiography and pericardiocentesis led to successful resuscitation and survival. We suggest that echocardiographic skills to determine tip position and pericardiocentesis be taught in neonatal resuscita

Venous Hemodialysis Catheters and Cardiac Implantable Electronic Devices: Avoiding a High-Risk Combination.

End-stage renal disease is frequently accompanied by cardiac comorbidity that warrants treatment with a cardiovascular implantable electronic device (permanent pacemaker or implantable cardioverter-defibrillator). In the United States, chronic hemodialysis (HD) population, cardiac implantable devices are present in up to 10.5% of patients; a venous HD catheter is utilized for blood access in 18% of prevalent patients. The concomitant presence of a venous HD catheter and cardiovascular implantable device creates a high-risk circumstance, with potential for causing symptomatic central venous ste

The transcervical cardiac catheter: its transverse pretracheal placement in the treatment of cardiac catheter complications with ventriculo-atrial shunts in infantile hydrocephalus.

In 40 hydrocephalic children with ventriculo-atrial shunts 58 operative revisions of the cardiac catheter were evaluated. The review of the patients with cardiac catheter complications showed that the majority (68%) had undergone the primary shunting procedure as newborn or as prematurely born within the first 6 months of life. Most frequently the revisions had become necessary because of obstruction of the cardiac catheter at its distal end (22 times) and the slipping of the catheter from the valve (21 times). Other revisions were carried out for too long (2 times) or too short (once) cardiac

Use of asymmetric bidirectional catheters with different curvature radius for catheter ablation of cardiac arrhythmias.

BACKGROUND: The impact of recently introduced asymmetric bidirectional ablation catheters on procedural parameters and acute success rates of ablation procedures is unknown. METHODS: We retrospectively analyzed data regarding ablations using a novel bidirectional catheter in a tertiary cardiac center and compared these in 1:5 ratio with a control group of procedures matched for age, gender, operator, and ablation type. RESULTS: A total of 50 cases and 250 controls of median age 60 (50-68) years were studied. Structural heart disease was equally prevalent in both groups (39%) while history of

Complex cardiac anatomy and catheter access: the role of imaging in patients referred for catheter ablation.

A 66-year-old man with Kartagener's syndrome, situs inversus totalis, and recurrent supraventricular tachycardia and a 49-year-old man with atrial fibrillation and drug-refractory rapid ventricular rate response were referred for catheter ablation. In the first case, the mirrored anatomy of the right atrium was reconstructed using three-dimensional electroanatomical mapping, which guided successful ablation of a typical atrioventricular nodal reentrant tachycardia. In the second case, computerized tomography showed the presence of left atrial isomerism with interruption of the inferior vena ca

Complications of epicutaneo-caval catheters: Pericardial effusion and cardiac tamponade in three preterm infants.

In the neonatal intensive care units (NICU), epicutaneo-caval catheters (ECCs) are common alternative vascular routes. Pericardial effusion (PCE) and cardiac tamponade (CT) are rare but serious complications in infants with ECCs. It may be asymptomatic or present with a variety of significant clinical signs, including dyspnea, bradycardia, sudden asystole, and hypotension. If untreated, PCE can be fatal. This report presents, three cases of ECC-associated PCE/CT during NICU stay. All three patients were born before 30 weeks of gestation and weighed less than 1500 g. Echocardiography was used f

Risk of infections related to endovascular catheters and cardiac implantable devices in hemodialysis patients.

Patients requiring dialysis are extremely vulnerable to infectious diseases. The high burden of comorbidities and weakened immune system due to uremia and previous immunosuppressive therapy expose the patient on dialysis to more infectious events than the general population. The infectious risk is further increased by the presence of endovascular catheters and implantable cardiologic devices. The former is generally placed as urgent vascular access for dialysis and in subjects requiring hemodialysis treatments without autogenous arteriovenous fistula. The high frequency of cardiovascular event

[The pulmonary artery catheter in the intensive cardiac care unit].

More than 50 years after its introduction in clinical practice, the increase in the intensity of care offered by the cardiac intensive care units, the shift in the population of patients treated and the wider availability of circulatory supports, still makes the pulmonary artery catheter (PAC) an essential tool for diagnosis, monitoring and prognosis in patients suffering from cardiogenic shock. In this review, we will discuss how to identify those patients who can benefit most from its use, the configuration and the correct insertion technique of a PAC. A pragmatic guide will also be provided
