With magnetic resonance (MR) microscopy, high-resolution volumetric imaging (3DFT) of small animals is possible. Although these techniques are suitable for imaging the head and other small stationary objects, breathing and cardiac motion degrade the quality of body images. Scan synchronous ventilation and cardiac gating methods have been developed that permit acquisition of high-resolution images from anywhere in the body of small animals (150 to 400 g). Anesthetized rats were ventilated in synchrony with three-dimensional Fourier spin warp (3DFT) sequence (TR = 400 to 1000 ms, TE = 20 ms). Ei

Based on experimental and clinical studies, the technical aspects (choice of projection, injection technique), experimental and clinical results, and the indications for computerized digital angiography in congenital heart disease are reviewed. To obtain high-quality diagnostic images consistently, ECG-gating, suspended or gated respiration, and short pulsewidth acquisition are required. Clinically useful diagnostic information has been acquired in children with suspected functional murmurs, cardiac surgery that is believed to be inadequate, and in cyanotic newborns after intravenous injection

Cardiac computed tomography angiography (CTA) is a valuable tool in the assessment of congenital and acquired cardiac disease in children. The goal of cardiac CTA is to produce images that are free of motion and provide sufficient characterization of the anatomy in question. Given the complexity of pediatric patient characteristics, including patient size, heart rate, breath-holding capability, and variant anatomy, cardiac CTA technique must be individualized to the patient as well as the indication to answer the clinical question while also minimizing radiation exposure. The purpose of this a

Development of electrocardiographically (ECG) gated multisection computed tomography (CT) has had a significant, immediate impact in cardiovascular imaging. The capabilities of this new technique have become particularly important in the preoperative assessment of the cardiac surgery patient. Cardiac surgery in the 21st century has become increasingly complex because of an aging population needing multiple procedures. As patients live longer, reoperative surgery is often needed, requiring further complicated intervention. Recent research in cardiac surgery patients has linked atherosclerotic d

BACKGROUND: Fetal cardiovascular magnetic resonance (CMR) imaging may provide a valuable adjunct to fetal echocardiography in the evaluation of congenital cardiovascular pathologies. However, dynamic fetal CMR is difficult due to the lack of direct in-utero cardiac gating. The aim of this study was to investigate the effectiveness of a newly developed Doppler ultrasound (DUS) device in humans for fetal CMR gating.
METHODS: Fifteen fetuses (gestational age 30-39 weeks) were examined using 1.5 T CMR scanners at three different imaging sites. A newly developed CMR-compatible DUS device was used

Myocardial perfusion imaging with thallium-201 and gated cardiac blood pool scanning are finding increasing use in clinical cardiology. These noninvasive techniques have been found useful in detecting myocardial infarction independent of the electrocardiogram and determining the site and extent of the infarct as well as its effect on left ventricular function. These studies provide important prognostic data and are proving to be of value in evaluating patients with cardiogenic shock. Neither the thallium-201 myocardial perfusion image nor the gated cardiac blood pool scan can distinguish betwe

Cardiac stereotactic body radiotherapy is a promising noninvasive treatment for patients with refractory ventricular tachycardia. With the aim to prove feasibility of a novel image guided radiotherapy and heart motion gating device, cardiac proton radiotherapy was performed using a porcine model. Using a novel adaptation of γ - H2AX tissue staining techniques, we have been able to localize a radiation beam in large animal tissue to assess targeting accuracy within a defined field. Cardiorespiratory-gated irradiations of the animals were successfully completed and analysis of the γ-H2AX stainin

PURPOSE: To compare two blind source separation (BSS) techniques to principal component analysis and the electrocardiogram for the identification of cardiac triggers in self-gated free-running 5D whole-heart MRI. To ascertain the precision and robustness of the techniques, they were compared in three different noise and contrast regimes.
METHODS: The repeated superior-inferior (SI) projections of a 3D radial trajectory were used to extract the physiological signals in three cardiac MRI cohorts: (1) 9 healthy volunteers without contrast agent injection at 1.5T, (2) 30 ferumoxytol-injected cong

OBJECTIVES: Risk factors are similar for the development of both thoracic aortic aneurysms (TAA) and other cardiovascular diseases. Coronary artery disease is highly prevalent in patients with TAA, with a reported prevalence of 30% to 70%. Knowledge of the underlying cardiac pathology can minimize perioperative risk and improve patient selection. This study investigated the feasibility of simultaneous assessment of thoracic aortic pathology and cardiac structures and function, including the coronary arteries, using electrocardiogram-gated 64-slice computed tomography (CT) angiography.

In this report, the diagnostic value of magnetic resonance imaging (MRI) was compared with that of two-dimensional echocardiography, computed tomography and cardiac catheterization in a patient with pentalogy of Fallot who survived to her fortieth year. The advantages and disadvantages of MRI in diagnosing the present case were as follows: The cardiovascular system, with the exception of atrial septal defect, was evaluated precisely. Collateral vessels were detected using MRI, but impossible with other non-invasive methods. MRI was particularly suitable for imaging the cardiovascular system be

There have been numerous advances in the field of cardiac imaging. The advent of multidetector computed tomography coronary angiography (MDCT-CA) and in particular electrocardiographic (ECG)-gated acquisition has revolutionized the investigation of the complete spectrum of complications of common cardiothoracic surgical procedures. Generic complications, such as mediastinitis, pericardial effusion, sternal osteomyelitis, and mediastinal fibrosis, are discussed with illustrative examples of multiplanar and volume-rendered three-dimensional reconstructions. Graft-related complications of both co

BACKGROUND: The value of late-gadolinium-enhancement (LGE) imaging in the diagnosis and management of pediatric and congenital heart disease is clear; however current acquisition techniques are susceptible to error and artifacts when performed in children because of children's higher heart rates, higher prevalence of sinus arrhythmia, and inability to breath-hold. Commonly used techniques in pediatric LGE imaging include breath-held segmented FLASH (segFLASH) and steady-state free precession-based (segSSFP) imaging. More recently, single-shot SSFP techniques with respiratory motion-corrected a

OBJECTIVE: Thoracic aneurysm preoperative imaging is performed using static techniques without consideration of normal aortic dynamics. Improved understanding of the native aortic environment into which thoracic endografts are placed may aid in device selection. It is unclear what comprises normal thoracic aortic pulsatility. We studied these phenomena dynamically using ECG-gated 64-slice CTA.
METHODS: Maximum diameter and area change per cardiac cycle was measured at surgically relevant anatomic thoracic landmarks in ten patients; 1.0 cm proximal and distal to the subclavian artery, 3.0 cm d

Cine magnetic resonance imaging (cine MRI) was used to evaluate the cardiac structures and blood flow in congenital heart diseases with left-to-right shunts. Fifteen children with left-to-right shunts which were confirmed by echocardiography or angiography were investigated in the present study. Five children each had atrial septal defect, ventricular septal defect, and complete endocardial cushion defect. Their ages ranged from 4 months to 13 years (mean 5.5 years). Prior to cine MRI, the ECG-gated cardiac imaging using multi-slice acquisition was performed in all the children to localize the

Fast MR imaging techniques have multiple applications for evaluation of cardiac disease. Cine MRI and MR tagging have been shown to be highly accurate and reproducible in evaluating regional and global myocardial function. Segmented k-space cine MRI and echo-planar imaging (EPI) can considerably improve time efficiency and thereby the clinical utility of these techniques. Double IR fast spin-echo sequences enable breath-hold acquisition of T2 weighted MRI with good suppression of the blood signal. Myocardial perfusion can be assessed with fast dynamic MRI after administration of contrast media

PURPOSE OF REVIEW: Given the low occurrence of clinically important paravalvular leak (PVL), there are no large registries or trials in this space to investigate management strategies. This review integrates newer evidence, particularly in imaging guidance for these complex procedures, novel techniques and approaches that our group has taken, as well as approaches to more complex PVL plugging reported in case reports.
RECENT FINDINGS: Perhaps the largest area of growth in the management of PVL is the use of advanced imaging in both pre-procedure evaluation and intra-procedural guidance with g

Rationale and Objective: Thoracic CT angiography (CTA) for ascending aortic dissection, a life-threatening emergency, is performed routinely without Electrocardiographic (ECG) gating, therefore allowing the apparition of a pulsation artefact. We aimed to evaluate and compare the diagnostic performance, the inter and intra-reporter agreement of ECG gated CTA and non-ECG gated CTA for detecting ascending aortic dissection, considering their training level. Our hypothesis is that ECG gated CTA has superior diagnostic accuracy for ascending aortic dissection compared to non-gated CTA. Materials an
