[Historical review from beginning of CPR to CPCR (cardiopulmonary resuscitation to cardiopulmonary-cerebral resuscitation)].

This 2010 is a memorial year for CPR, as in 1960 closed chest cardiac massage was first introduced in clinical setting. Jude, Kouwenhoven are the pioneer of this method. However, Prof. Peter Safar was also distinguished pioneer of articial respiration and airway management. Manual chest compression is not efficient to maintain oxygenation and impossible to airway patent. Applying both respiration by mouth-to-mouth breathing and closed chest cardiac massage, it started the new era of CPR. CPR has been expanded to every countries, however, it has not been successful to obtain a good survival rat

Ultrasonographic Assessment During Cardiopulmonary Resuscitation.

The US-CAB (Ultrasound, Circulation/Airway/Breathing) protocol integrates several sonographic techniques into a structured assessment of the circulation, airway, and breathing status of a patient during cardiopulmonary resuscitation (CPR) in an advanced life support-compliant manner. US-C provides a subxiphoid view of the heart, to look for potentially reversible causes of disease, such as pericardial effusion, pulmonary embolism, hypovolemia, and acute coronary thrombosis. Sonographic cardiac activity during CPR not only helps differentiate pseudo-pulseless electrical activity (PEA) from true

Mitigating hyperventilation during cardiopulmonary resuscitation.

Although multiple airway management and ventilation strategies have been proposed during cardiac arrest, the ideal strategy is unknown. Current strategies call for advanced airways, such as endotracheal intubation and supraglottic airways. These may facilitate hyperventilation which is known to adversely affect cardiopulmonary physiology. We provide a summary of conceptual models linking hyperventilation to patient outcomes and identify methods for mitigating hyperventilation during cardiac arrest.

Airway management in cardiopulmonary resuscitation.

PURPOSE OF REVIEW: The optimal method for airway management during cardiac arrest is unknown. This review summarizes recent evidence comparing the use of basic and advanced airway interventions [insertion of supraglottic airway (SGA) devices and tracheal intubation], including the role of capnography during cardiac arrest. RECENT FINDINGS: A large observational study [649,359 out-of-hospital cardiac arrests (OHCAs)] has shown that the risk-adjusted odds of neurologically favourable survival were higher for those treated with bag-mask ventilation compared with SGA insertion or tracheal intubat

Pediatric cardiopulmonary resuscitation and stabilization.

Cardiopulmonary arrest refers to cessation of clinically detectable cardiac activity. In children, it usually results from progression of shock, respiratory failure or cardiac dysrhythmia. Early recognition and timely interventions in above group of patients is the key to prevent progression to cardiac arrest. The goal of resuscitation is to urgently re-establish oxygenation of vital organs by attention to Airway, Breathing and Circulation. Measures to restore airway patency include positioning, suctioning, continuous positive airway pressure, relieving a foreign-body airway obstruction and, e

Cardiopulmonary resuscitation: a retrospective review.

The outcome of patients admitted to intensive care after a cardiac arrest was determined by reviewing intensive care unit records at four hospitals for 1993 and 1994. Of the 112 patients identified, 49 survived intensive care of whom 28 were discharged from hospital. In January 1996, 26 of the 28 patients could be traced; 22 of these were still alive. Seven factors were significantly different between survivors and nonsurvivors. At the cardiac arrest these were the number of direct current shocks (p < 0.05) and adrenaline doses (p < 0.01) given. In intensive care the factors were the presence

New developments in cardiopulmonary resuscitation.

Since the last revision of the American Heart Association's guidelines in 1985, several new developments of clinical importance have occurred in the field of cardiopulmonary resuscitation. These include enhanced access to and earlier use of defibrillation, the use of high-dose epinephrine when standard doses fail, the assessment of resuscitative efforts with end-tidal CO2 monitoring and the addition of two new drugs, amiodarone (for refractory ventricular fibrillation) and adenosine (for paroxysmal supraventricular tachycardia). Time will determine the ultimate role of these advancements in th

Tension pneumoperitoneum after cardiopulmonary resuscitation.

OBJECTIVE: To increase awareness of the unusual complication of pneumoperitoneum after cardiopulmonary resuscitation. CLINICAL FEATURES: A 57-year-old male farmer with a history of chronic renal failure and heart disease, as well as severe oesophageal reflux for which fundoplication had been performed, developed a tension pneumoperitoneum after cardiopulmonary resuscitation. This resulted in lower limb cyanosis and an erection, a previously unreported complication. INTERVENTION AND OUTCOME: The tension was relieved by uncapping a peritoneal dialysis catheter that was in situ. The cyanosis an

Emergency medicine updates: Cardiopulmonary resuscitation.

INTRODUCTION: Cardiac arrest is the loss of functional cardiac activity; emergency clinicians are integral in the management of this condition. OBJECTIVE: This paper evaluates key evidence-based updates concerning cardiopulmonary resuscitation (CPR). DISCUSSION: Cardiac arrest includes shockable rhythms (i.e., pulseless ventricular tachycardia and ventricular fibrillation) and non-shockable rhythms (i.e., asystole and pulseless electrical activity). The goal of cardiac arrest management is to achieve survival with a good neurologic outcome, in part by restoring systemic perfusion and obtaini

Cardiopulmonary resuscitation in special circumstances.

Cardiopulmonary resuscitation prioritises treatment for cardiac arrests from a primary cardiac cause, which make up the majority of treated cardiac arrests. Early chest compressions and, when indicated, a defibrillation shock from a bystander give the best chance of survival with a good neurological status. Cardiac arrest can also be caused by special circumstances, such as asphyxia, trauma, pulmonary embolism, accidental hypothermia, anaphylaxis, or COVID-19, and during pregnancy or perioperatively. Cardiac arrests in these circumstances represent an increasing proportion of all treated cardi

Cardiopulmonary resuscitation in hospitalized infants.

BACKGROUND: Hospitalized infants requiring cardiopulmonary resuscitation (CPR) represent a high-risk group. Recent data on risk factors for mortality following CPR in this population are lacking. AIMS: We hypothesized that infant demographic characteristics, diagnoses, and levels of cardiopulmonary support at the time of CPR requirement would be associated with survival to hospital discharge following CPR. STUDY DESIGN: Retrospective cohort study. SUBJECTS: All infants receiving CPR on day of life 2 to 120 admitted to 348 Pediatrix Medical Group neonatal intensive care units from 1997 to 20

Current approaches to cardiopulmonary resuscitation.

Cardiac arrest occurs when there is abrupt cessation of effective pumping activity of the heart. Among the likely causes are ventricular asystole (electrical or mechanical), pulseless ventricular tachycardia or ventricular fibrillation. Clinically this may present as 'sudden cardiac death'.

[Drug therapy in cardiopulmonary resuscitation].

In the year 2000, new international guidelines for cardiopulmonary resuscitation (CPR) were published by the American Heart Association, and the European Resuscitation Council. These guidelines are evidence-based, indicating that these recommendations are based primarily on interpretation of data from clinical studies. Levels of recommendation range from class I (proven safe and useful), class IIa (intervention of choice), IIb (alternative intervention), indeterminate (research stage), and class III (unacceptable, no benefit). Administration of drugs during CPR should be performed intravenousl
