[Orthostatic hypotension after space flight].

Cardiovascular changes occur during spaceflight. Because of the loss of hydrostatic pressure there is, during weightlessness, a fluid shift of extra- and intra-vascular volumes to the upper part of the body. This provokes objective and subjective symptoms, especially in the first days of spaceflight. The fluid shift is perceived by baroreceptors as a relative central hypervolaemia and induces neuro-hormonal mechanisms which provoke a real hypovolaemia. Cardiovascular deconditioning exists during spaceflight with the following symptoms when astronauts return to earth: decreased orthostatic tole

Evidence of physical deconditioning during psychiatric hospitalization in a Veteran sample.

Prior observations of low physical activity in psychiatric inpatient care suggest a risk of physical deconditioning, but to date no studies have explicitly investigated this possibility by measuring change in cardiovascular fitness over the course of hospitalization. The present study used mattress actigraphy to obtain a passive measure of sleep heart rate (sHR) among a sample of 111 male Veterans receiving treatment for PTSD at the VA Palo Alto Health Care System Trauma Recovery Program (TRP). A linear mixed-effect regression model indicated that sHR significantly increased over nights hospit

[Microgravity and autonomic nervous system].

How microgravity influences autonomic function is still under investigation. Microgravity induces neuro-vestibular alterations and body fluid shift, and these two changes cause "space motion sickness(SMS)" and cardiovascular deconditioning. "Space motion sickness" is a autonomic syndrome that exhibits nausea, vomiting, headache, anorexia, pallor etc., whose incidence in Space Shuttle mission reaches 67.1%. There are several hypotheses for SMS mechanism: 1) sensory conflict, 2) fluid shift, 3) otolith asymmetry, 4) space orientation readaptation, 5) otolith tilt-translation reinterpretation, an

Centrifugation as a countermeasure during actual and simulated microgravity: a review.

This paper summarizes what has been learned from studies of the effects of artificial gravity generated by centrifugation in actual and simulated weightless conditions. The experience of artificial gravity during actual space flight in animals and humans are discussed. Studies using intermittent centrifugation during bed rest and water immersion, as a way to maintain orthostatic tolerance and exercise capacity, are reviewed; their results indicate that intermittent centrifugation is a potential countermeasure for maintaining the integrity of these physiological functions in extended space miss

["Doctor, I am short of breath" - a primary care perspective].

Breathing difficulty can be the first symptom of many diseases. Frequent causes of dyspnoea in patients under out-patient care are of pulmonary and cardiovascular origin. Deconditioning is often a contributing factor in patients with chronic lung or heart disease. Key elements in the evaluation of patients with chronic and acute dyspnoea will be reviewed in this article.

Integrative cardiovascular dose-response to graded lower-body negative pressure.

Lower-body negative pressure (LBNP) has been posited as a potential spaceflight countermeasure to counteract the physiological deconditioning related to fluid shifts in microgravity. However, open questions remain regarding the magnitude of LBNP that should be applied. We systematically characterized the cardiovascular effects of LBNP and quantified the effect size of varied LBNP doses across different parts of the cardiovascular system. Twenty-four subjects (12 male and 12 female) were exposed to graded LBNP, increasing from 0 to -50 mmHg in 10 mmHg increments, in both supine (0°) and 15° hea

Active LifestyLe Rehabilitation interventions in aging spinal cord injury (ALLRISC): a multicentre research program.

BACKGROUND: With today's specialized medical care, life expectancy of persons with a spinal cord injury (SCI) has considerably improved. With increasing age and time since injury, many individuals with SCI, however, show a serious inactive lifestyle, associated with deconditioning and secondary health conditions (SHCs) (e.g. pressure sores, urinary and respiratory tract infections, osteoporosis, upper-extremity pain, obesity, diabetes, cardiovascular disease) and resulting in reduced participation and quality of life (QoL). Avoiding this downward spiral, is crucial. OBJECTIVES: To understand

Artificial gravity in Space: vestibular tolerance assessed by human centrifuge spinning on Earth.

Artificial gravity created by the astronauts themselves, without any external power supply, by pedalling on coupled counterrotating bicycles along the inner wall of the space module (Twin Bikes System, TBS), was previously suggested (Antonutto et al., 1991) to prevent musculo-skeletal decay and cardiovascular deconditioning during long term space flights. To investigate whether this unusual rotating environment would determine abnormal stimulations of the vestibular system due to Coriolis cross coupled accelerations, thus leading to acute motion sickness (AMS), the conditions of a rotating env

Acute cardiovascular and muscular response to rowing ergometer exercise in artificial gravity - a pilot trial.

Prolonged immobilization and spaceflight cause cardiovascular and musculoskeletal deconditioning. Combining artificial gravity through short-arm centrifugation with rowing exercise may serve as a countermeasure. We aimed to compare the tolerability, muscle force production, cardiovascular response, and power output of rowing on a short-arm centrifuge and under terrestrial gravity. Twelve rowing athletes (4 women, aged 27.2 ± 7.4 years, height 179 ± 0.1 cm, mass 73.7 ± 9.4 kg) participated in two rowing sessions, spaced at least six weeks apart. One session used a short-arm centrifuge with +0.5

Autonomic nervous system profile in fibromyalgia patients and its modulation by exercise: a mini review.

This review imparts an impressionistic tone to our current understanding of autonomic nervous system abnormalities in fibromyalgia. In the wake of symptoms present in patients with fibromyalgia (FM), autonomic dysfunction seems plausible in fibromyalgia. A popular notion is that of a relentless sympathetic hyperactivity and hyporeactivity based on heart rate variability (HRV) analyses and responses to various physiological stimuli. However, some exactly opposite findings suggesting normal/hypersympathetic reactivity in patients with fibromyalgia do exist. This heterogeneous picture along with

Evolution of depression during rehabilitation program in patients with cardiovascular diseases.

Background: Cardiovascular diseases represent the main cause of death in the world. Rehabilitation through exercise is more and more used in cardiac patients. Given that these patients suffer from depressive symptoms, the risk of having recurrent cardiovascular problems increases. Thus, the aim of this study is to identify the effects of a rehabilitation program on the physiological and psychological parameters; with a particular attention on the depression scores between the scales. Methods: Twenty-eight cardiac patients participated in this study during a cardiovascular rehabilitation program

ESA VIVALDI Dry Immersion Microgravity Simulations Induce Increases in Immune Biomarkers Associated With Physical and Psychological Stress, and Sex-Specific Factors.

With future manned space projects involving missions of unprecedented duration, multisystem deconditioning induced by spaceflight could seriously affect the well-being and health of astronauts. Safe and easily determined in-flight biomarkers are therefore needed to monitor health status. In this study, we simulated space deconditioning with a 5-day dry immersion (DI) of 18 healthy women and 19 healthy men and evaluated the effects of this protocol on three biomarkers: the neutrophil-to-lymphocyte ratio (NLR), the granulocyte-to-lymphocyte ratio (GLR) and the platelet-to-lymphocyte ratio (PLR).

Effectiveness of Functional Resistance Training in Hospital to Prevent Deconditioning and Improve Discharge Pathways: A Service Evaluation.

INTRODUCTION: Deconditioning is a complex process of physiological change following a period of inactivity. This can be observed in a clinical setting when hospital patients have prolonged periods of bed rest. Prolonged bed rest negatively affects health-related outcomes such as cardiovascular, respiratory and musculoskeletal function, and is linked to increased mortality. To combat deconditioning in patients, a functional resistance exercise intervention may be suitable to employ. Exercise interventions can successfully prevent and reverse muscle loss and functional decline, the main componen

Gravity, microgravity, and artificial gravity: physiological effects, implementation, and applications.

Gravity, the force that structures the cosmos, also shapes human physiology. It influences skeletal, muscular, cardiovascular, respiratory, and neurological systems, sustaining balance, blood circulation, and functional capacity. Unlike other senses, the brain lacks a dedicated gravity-sensing region and instead relies on a distributed vestibular network, graviception, to interpret gravitational cues. On Earth, gravity-driven blood pooling in the legs triggers compensatory responses that preserve cerebral perfusion. In microgravity, these mechanisms are altered, leading to fluid shifts toward

[Cardiopulmonary exercise testing for the diagnosis of unexplained dyspnea: a review of 194 cases].

INTRODUCTION: Chronic dyspnoea that remains unexplained after resting pulmonary function and cardiovascular testing is a common problem in clinical practice. The aim of this study was to determine the utility of cardiopulmonary exercise testing (CPET) in the diagnosis of unexplained dyspnoea. METHODS: This retrospective single-centre study included consecutive patients with dyspnoea who had normal resting cardiopulmonary examinations (including chest X-ray, electrocardiography, pulmonary function tests [PFTs], and cardiac ultrasound). CPET was performed using a cycle ergometer with analysis o

Exercise and Health-Related Risks of Physical Deconditioning After Spinal Cord Injury.

A sedentary lifestyle occurring soon after spinal cord injury (SCI) may be in contrast to a preinjury history of active physical engagement and is thereafter associated with profound physical deconditioning sustained throughout the lifespan. This physical deconditioning contributes in varying degrees to lifelong medical complications, including accelerated cardiovascular disease, insulin resistance, osteopenia, and visceral obesity. Unlike persons without disability for whom exercise is readily available and easily accomplished, exercise options for persons with SCI are more limited. Depending

Simulated microgravity alters rat mesenteric artery vasoconstrictor dynamics through an intracellular Ca(2+) release mechanism.

Previous work has shown that orthostatic hypotension associated with cardiovascular deconditioning results from inadequate peripheral vasoconstriction. We used the hindlimb-unloaded (HU) rat in this study as a model to induce cardiovascular deconditioning. The purpose of this study was to test the hypothesis that 14 days of HU diminishes vasoconstrictor responsiveness of mesenteric resistance arteries. Mesenteric resistance arteries from control (n = 43) and HU (n = 44) rats were isolated, cannulated, and pressurized to 108 cm H(2)O for in vitro experimentation. Myogenic (intralumenal pressure
