Stress in cardiovascular diseases.

INTRODUCTION: Minor oral surgeries done under local anesthesia can be quite traumatic to the patient; thus, the use of conscious sedation can improve patient comfort, reduce stress levels, provide better pain management, smoother surgical experiences, and better recovery. Combinations of ketamine-dexmedetomidine and midazolam-pentazocine were used as conscious sedation regimens, and they have unique strengths and weaknesses. Evaluating the sedation effects of ketamine-dexmedetomidine versus midazolam-pentazocine in maxillofacial surgeries is crucial for refining anesthetic practices. Therefore

Cardiovascular diseases in India.

India is undergoing an epidemiological transition and is on the threshold of an epidemic of cardiovascular disease. Cause-specific mortality data indicate that cardiovascular disease is already an important contributor to mortality. Demographic projections suggest a major increase in cardiovascular disease mortality as life expectancy increases and the age structure of the growing population changes. Surveys in urban areas suggest that coronary risk factors are already widespread and that urgent action is needed to prevent a further rise as socioeconomic development proceeds. It is vital to ob

Thyroid and cardiovascular diseases.

The thyroid gland is one of the major regulator organs of hemostasis in the human body, controlling the functioning of numerous systems. Thyroid hormones exert a modulating effect on the cardiovascular system in particular, ensuring optimal functioning within the normal range. Triiodothyronine (T3), as an active form of thyroid hormone, is mainly responsible for this effect via both genomic and nongenomic mechanisms. It has been reported that overt thyroid disorders are associated with a number of cardiovascular diseases and cardiac mortality. While hyperthyroidism appears to be related to atr

Lipoprotection in cardiovascular diseases.

Cardioprotection is a well-established term in the scientific world. It describes the protection of various mediators on the cardiovascular system. These protective effects can also be provided by certain lipids. Since lipids are a very specific and clearly definable class of substances, we define the term lipoprotection as lipid-mediated cardioprotection. In this review, we highlight high-density lipoprotein (HDL), sphingosine-1-phosphate (S1P) and omega-3 polyunsaturated fatty acids (n-3 PUFA) as the most important lipoprotective mediators and show their beneficial impact on coronary artery

Hyperhomocysteinemia and cardiovascular diseases.

Homocysteine (Hcy) is a sulfhydryl-containing amino acid, which is not acquired through the diet, but rather synthesized as an intermediate metabolite in the methionine cycle. Hcy is present in plasma, with normal levels between 5 and 15 μmol/L, a slightly elevated level between 15 to 30 μmol/L, moderate from 30 to 100 μmol/L and a value > 100 μmol/L classified as severe hyperhomocysteinemia (HHcy). HHcy has been associated with inflammation and atherosclerosis and is considered an independent risk factor for cardiovascular diseases (CVD). Here, we review the main evidence showing the associat

Cardiovascular Diseases and Zinc.

Zinc is structurally and functionally essential for more than 300 enzymes and 2000 transcription factors in human body. Intracellular labile zinc is the metabolically effective zinc and tiny changes in its concentrations significantly affect the intracellular signaling and enzymatic responses. Zinc is crucial for the embrionic and fetal development of heart. Therefore, it is shown to be related with a variety of congenital heart defects. It is involved in epithelial-to-mesenchymal transformation including endocardial cushion development, which is necessary for atrioventricular septation as wel

Iron and cardiovascular diseases.

Iron is a necessary element for life; however, excess iron leads to oxidative stress by the Fenton reaction. Iron deficiency is prevalent in patients with heart failure, while iron overload is associated in the pathogenesis of atherosclerosis. These findings suggest the "iron paradox" in cardiovascular diseases. Iron metabolism in cardiovascular diseases is complex, and the mechanisms regulating systemic and cellular iron metabolism in cardiovascular diseases remain completely unknown. In this review, we focus on the role of iron in cardiovascular diseases.

Lipoxidation in cardiovascular diseases.

Lipids can go through lipid peroxidation, an endogenous chain reaction that consists in the oxidative degradation of lipids leading to the generation of a wide variety of highly reactive carbonyl species (RCS), such as short-chain carbonyl derivatives and oxidized truncated phospholipids. RCS exert a wide range of biological effects due to their ability to interact and covalently bind to nucleophilic groups on other macromolecules, such as nucleic acids, phospholipids, and proteins, forming reversible and/or irreversible modifications and generating the so-called advanced lipoxidation end-prod

AMPK in Cardiovascular Diseases.

This chapter summarizes the implication of AMP-activated protein kinase (AMPK) in the regulation of various physiological and pathological cellular events of great importance for the maintenance of cardiac function. These include the control of both metabolic and non-metabolic elements targeting the different cellular components of the cardiac tissue, i.e., cardiomyocytes, fibroblasts, and vascular cells. The description of the multifaceted action of the two AMPK catalytic isoforms, α1 and α2, emphasizes the general protective action of this protein kinase against the development of critical p

[Cardiovascular diseases in childhood].

Cardiac imaging in childhood poses significant challenges for the instrumentation and the operator. Small cardiac structures, high heart beat rates, non-compliant or sedated patients and in addition to the challenge of detecting acquired heart diseases, a broad spectrum of congenital heart defects (CHD) needs to be addressed, preferably without the burden of ionizing radiation. These challenges require not only accurate anatomical imaging but also a comprehensive assessment of hemodynamic and functional parameters for prognosis and risk stratification. With an incidence of 1 % among all live b

Adrenomedullin and cardiovascular diseases.

The cardiovascular system is regulated by the autonomic nervous system, the renin-angiotensin-aldosterone system, nitric oxide (NO) and other factors including neuropeptides. Research in neurohumoral factors has led to the development of many cardiovascular drugs. Adrenomedullin (ADM), initially isolated from the adrenal gland, has diverse physiological and pathophysiological functions in the cardiovascular system. It is produced in many organs and tissues including the vasculature. ADM has numerous actions, including vasodilation, natriuresis, antiapoptosis and stimulation of NO production. I

MicroRNAs and cardiovascular diseases.

MicroRNAs (miRNAs) are a class of small noncoding RNAs that have gained status as important regulators of gene expression. Recent studies have demonstrated that miRNAs are aberrantly expressed in the cardiovascular system under some pathological conditions. Gain- and loss-of-function studies using in vitro and in vivo models have revealed distinct roles for specific miRNAs in cardiovascular development and physiological function. The implications of miRNAs in cardiovascular disease have recently been recognized, representing the most rapidly evolving research field. In the present minireview,

Leptin and cardiovascular diseases.

1. Leptin is a 16-kDa hormone, synthesized primarily by adipocyte, which acts as a key factor for maintenance of energy homeostasis in central and peripheral tissues. In most obese individuals, serum leptin levels are increased and correlate with the individual's body mass index. 2. Abundant investigations ranging from clinical and animal model studies to in vitro analyses show that leptin plays a pivotal role in obesity-related cardiovascular diseases (CVD). Hyperleptinaemia has been confirmed to be a predictor of acute cardiovascular events. However, some studies have shown that leptin has a

Cardiovascular diseases in pregnancy.

BACKGROUND: Cardiovascular diseases arise during 0,2% to 4% of all pregnancies in the industrialized world. In Germany, this type of complication, which is sometimes lethal, affects approximately 30 000 pregnant women per year. METHODS: We performed a simple literature search in the NCBI databases for publications that appeared from 2008 to 2010 and that contained the search terms "pregnancy" and one of the following: "valvular disease," "endocarditis," "coronary heart disease," "cardiomyopathy," "hypertension," "anticoagulation." We also took consideration of the relevant international medic

[Statins and cardiovascular diseases].

Statins are utilised in the primary and secondary prevention of coronary heart disease, due to their efficacy at lowering lipid levels. However, statins may also prevent atherosclerosis disease by non-lipid or pleiotropic effects, for example, improving endothelial function by promoting the production of NO. By increasing NO production, statins may interfere with atherosclerosis lesion developement, stabilise plaque, inhibit platlet aggregation, improve blood flow and protect against ischaemia. Therofore, the ability of statins to improve endothelial function through the release of NO may part

Kinins and cardiovascular diseases.

Kinins are synthesized from their precursors by different enzymes and participate in the regulation of cardiovascular function through bradykinin (BK) B1 and B2 receptors. They modulate blood coagulation by exerting antithrombotic and profibrinolytic actions. By activating B2 receptors that results in the release of nitric oxide and prostacyclin, kinins inhibit vascular smooth muscle growth and neointima formation, which may play an inhibitory role on the atherosclerosis development, while through the activation of B1 receptors, they may play a deleterious role in this disease. Kinins are pote
