Engagement with services in Black African and Caribbean people with psychosis: The role of social networks, illness perceptions, internalized stigma, and perceived discrimination.

OBJECTIVES: Research and policies in the United Kingdom have repeatedly highlighted the need to reduce ethnic disparities and improve engagement with mental health services among Black African and Caribbean people with psychosis. The aim of this study was to examine the role of social network characteristics and psychological factors in engagement with services in Black people with psychosis. METHODS: A cross-sectional study was conducted with 51 Black African and Caribbean adults with non-affective psychosis and currently receiving care from mental health services in England. Measures were c

Diabetogenic elevated childhood total fat in South Asian and Black African/Caribbean people relates to adverse early life growth and low socioeconomic position compared with White people in the UK.

AIMS/HYPOTHESIS: Excess type 2 diabetes mellitus in minority ethnic groups remains unexplained, although greater fat mass makes a strong contribution. We hypothesised that height and weight through infancy in South Asian and Black African/Caribbean subgroups is more adverse than in White populations. These, allied to poor socioeconomic position, determine greater fat mass at age 7 years. METHODS: We report a secondary analysis from the UK Millennium Cohort Study, including 12,280 births of White ethnicity, and 358 of Indian, 650 of Pakistani, 268 of Bangladeshi, 163 of Black Caribbean and 277

'Find Your Float': A Public Health Approach to Addressing Drowning Inequities in People of African, Caribbean and Asian Heritage: A Cross-Sectional Study.

Drowning is a largely preventable global public health problem. Many unintended drownings are linked to the initial responses to cold-water immersion (cold shock), which includes a rapid involuntary cardio-respiratory response that increases the risk of aspirating water within the first two minutes of immersion. Floating rather than struggling to swim can help survival in this circumstance. In light of early research and subsequent changes in anthropometry, we are re-examining whether bone mineral density (BMD) in people from African, Caribbean, and Asian communities influences their float. Social determinants of health and long-term conditions in people of Black African and Black Caribbean ethnicity living with HIV in London: A qualitative study.

BACKGROUND: People living with human immunodeficiency virus (HIV) are disproportionately impacted by socioeconomic deprivation and are at increased risk of developing other long-term conditions (LTCs). These illnesses require transformative action to tackle the adverse effects on their health. Data on lived experiences of LTCs among people living with HIV of Black African and Black Caribbean ethnicities are sparse, and how people with LTCs are impacted by social determinants of health (SDoH). METHODS: Through a phenomenological study design this qualitative study, conducted in 2022, comprised

A qualitative study of attitudes towards, typologies, and drivers of concurrent partnerships among people of black Caribbean ethnicity in England and their implications for STI prevention.

BACKGROUND: Partner concurrency, (having sexual partnerships overlapping in time), especially when condoms are not used, can facilitate sexually transmitted infections (STI) transmission. In Britain, STI diagnoses rates and the reporting of concurrency are higher among black Caribbeans than other ethnic groups. We explored attitudes towards, drivers, characteristics, and contexts of concurrent partnerships, and their implications for STI risk among black Caribbeans in England. METHODS: Purposive sampling, by sex and age-groups, was used to recruit participants (overall n = 59) from five sexua

The association between region of birth and sexually transmitted infections among people of black Caribbean ethnicity attending sexual health services in England, 2015.

BACKGROUND/INTRODUCTION: In England, people of Black Caribbean (BC) ethnicity are disproportionately affected by sexually transmitted infections (STIs), but it is unclear whether this varies by their region of birth. AIM(S)/OBJECTIVES: To examine differences in STI diagnoses among UK- and Caribbean-born BC people. METHODS: Data on STI diagnoses in BC people attending specialist sexual health services (SHSs) during 2015 and living in England were obtained from the GUMCAD STI surveillance system, the national surveillance system for STIs in England. Associations between being UK- or Caribbean-

Variations in adverse pregnancy and birth outcomes among Latin American and Caribbean-Born birthing people by region of origin, California birth cohort, 2007-2020.

BACKGROUND: Although many studies have highlighted better pregnancy and birth outcomes among foreign-born Latinas than among U.S.-born people, few have assessed heterogeneity in outcomes disaggregated by region of origin. We examined adverse pregnancy and birth outcomes among birthing people born in Latin America and the Caribbean (LAC) compared to people born in the U.S. METHODS: We used a retrospective cohort from the Study of Outcomes in Mothers and Infants compiled from California births (2007-2020). We examined descriptive statistics, unadjusted, and adjusted odds ratios for the associat

Social and Proximate Determinants of the Frequency of Condom Use Among African, Caribbean, and Other Black People in a Canadian City: Results from the BLACCH Study.

African, Caribbean, and other Black (ACB) people are a priority group for HIV prevention in Canada, but little is known about condom use in this population. This exploratory community-based research project addresses this gap in knowledge. 125 sexually active ACB people completed a questionnaire covering condom use and social determinants of health. The data were analyzed using ordinal logistic regression and mediation analyses. 20.5 % of sexually active ACB adults used condoms consistently. Male gender, wealth, unstable immigration classes, and unsecure employment statuses were independently

Harm reduction services and interventions for People Who Use Drugs (PWUD) in Latin America and the Caribbean (LAC) between 2013-2024: A scoping review protocol.

INTRODUCTION: In Latin America and the Caribbean (LAC) the response to substance use has primarily been abstinence-based, acute-care-oriented treatments. While harm reduction services (HRS) and interventions have expanded in LAC over the last decade, the research evidence on such programs has been sparse and disjointed. OBJECTIVE: This scoping review will map peer-reviewed literature on HRS and interventions in LAC, and synthesize gaps and opportunities for policy, practice, and research. INCLUSION CRITERIA: Studies conducted in LAC. The HRS that will be included in the search are opioid ago

Compliance with eye and foot preventive care in people with self-reported diabetes in Latin America and the Caribbean: Pooled, cross-sectional analysis of nine national surveys.

AIMS: To estimate the proportion of people with self-reported diabetes receiving eye and foot examinations in Latin America and the Caribbean (LAC). METHODS: Cross-sectional analysis of national health surveys in nine countries. Adults aged 25-64 years with self-reported diabetes. We quantified the proportion who reported having an eye examination in the last two years or a foot examination in the last year. We fitted multilevel Poisson regressions to assess socio-demographic (age and sex) and clinical (oral hypoglycemic medication and insulin treatment) variables associated with having had e

Co-Designing a Digital Platform to Support a Culturally Adapted Family Intervention (CaFI:Digital) for Psychosis Among People of Sub-Saharan African and Caribbean Descent: Agile Co-Design Approach.

BACKGROUND: People of sub-Saharan African and Caribbean descent are significantly more likely to be diagnosed with psychotic disorders than other ethnic groups in the United Kingdom. The National Institute for Health and Care Excellence in the United Kingdom recommends family therapy as a clinically effective treatment for the management of psychosis. The National Institute for Health and Care Excellence also recommends that family interventions should be culturally informed to meet the needs of an increasingly ethnically diverse population. People from minoritized backgrounds are rarely offer

Understanding support needs of African and African-Caribbean people living with dementia, their care partners and families, and impacts of delayed support: Identifying inclusive strategies to facilitate social care support: A study protocol.

African and African-Caribbean (AAC) people living with dementia (PLWD) are a population at high risk of inequitable access to health and social care services, and have poor health and wellbeing outcomes. Research suggests they are likely to experience failings in care, be recognised by services late and at points of crisis, and are at increased risk of institutionalisation. This research aims to examine the experiences of AAC PLWD, their care partners and families of accessing social care services and support. Employing an intersectionality theoretical framework, this study will use a multi-me

Fungal Infections in the Caribbean: A Review of the Literature to Date.

The most common fungal infections reported from the Caribbean include dermatophytosis, candidiasis, pneumocystis, aspergillosis, histoplasmosis, and cryptococcosis. The Caribbean is hyperendemic for histoplasmosis, with high population exposures. Fungal infections are a significant public health problem in the Caribbean, with rates varying depending on the specific country or region. In Trinidad and Tobago, the fungal burden accounts for 3.3% of the 1.4 million population, while in Jamaica, with a population of 2.9 million, over 57,600 people suffer from fungal infections each year. A study in

The effect of immigration policy reform on mental health in people from minoritised ethnic groups in England: an interrupted time series analysis of longitudinal data from the UK Household Longitudinal Study cohort.

BACKGROUND: In 2012, the UK Government announced a series of immigration policy reforms known as the hostile environment policy, culminating in the Windrush scandal. We aimed to investigate the effect of the hostile environment policy on mental health for people from minoritised ethnic backgrounds. We hypothesised that people from Black Caribbean backgrounds would have worse mental health relative to people from White ethnic backgrounds after the Immigration Act 2014 and the Windrush scandal media coverage in 2017, since they were particularly targeted. METHODS: Using data from the UK Househo

The influence of religion on organ donation and transplantation among the Black Caribbean and Black African population--a pilot study in the United Kingdom.

Currently the demand for transplant organs, particularly kidneys, far outstrips the supply in the United Kingdom. This problem is particularly severe for the Black African and Black Caribbean populations who have a higher predisposition to end-stage renal failure. Several commentators have suggested that religious and cultural traditions may be the major determinant preventing Black Africans and Black Caribbeans from donating organs. An exploratory qualitative study using focus groups was undertaken with the aim of examining the influence of religion, among other things, on the extent and dire

Control of iron and other micronutrient deficiencies in the English-speaking Caribbean.

Most micronutrient deficiencies affect relatively few people in the Caribbean; however, many Caribbean residents are affected by anemia that appears due primarily to a lack of dietary iron. While generally substantial, the prevalences of such anemia have differed a good deal from place to place and study to study, observed rates ranging from 27% to 75% in pregnant women, 19% to 55% in lactating women, and 15% to 80% in young children. Severe anemia, defined by a blood hemoglobin concentration below 8 g/dl, has been found in approximately 6% of the pregnant women and 11% of the preschool childr

Differential Pathophysiological Drivers of Susceptibility to Type 2 Diabetes and Metabolic Dysfunction-Associated Steatotic Liver Disease: Ethnic Differences in Insulin Dynamics, Whole-Body Fat Metabolism, and Organ-Specific Lipid Deposition.

INTRODUCTION: This narrative review explores the epidemiological evidence and potential underlying pathophysiological defects underlying the disproportionately greater risk of Type 2 diabetes (T2D) and cardiometabolic disease in people of South Asian and African Caribbean ancestry compared with White Europeans. Differences in (i) insulin dynamics, (ii) body composition and liver and pancreas triglyceride accumulation, and (iii) dysregulated fat metabolism likely contribute to this obesity-related susceptibility.
