OBJECTIVE: To study the changes in peak systolic velocities of the ipsilateral external carotid artery (ECA) following carotid revascularization.
METHODS: All patients randomized to carotid artery stenting (CAS) or carotid endarterectomy (CEA) in the International Carotid Stenting Study (ICSS; ISRCTN25337470) in our center were included. Peak systolic velocities (PSV) were assessed with duplex ultrasound (DUS) at baseline, at 30 days, and at 12 and 24 months after treatment. Our primary outcome measure was the change in blood flow velocities in the ECA (ΔPSVECA). Secondary outcome measure was

This study analyzes the results of carotid endarterectomy with a uniform technique of external carotid artery management. Aggressive blind instrument and eversion endarterectomy of the distal external carotid and its branches above the superior thyroid artery was performed during 211 standard carotid endarterectomies. This technique allows isolated external carotid artery repair if necessary after re-establishing carotid blood flow. Of these, 196 (92.9%) had normal intraoperative continuous wave Doppler ultrasonography in the external carotid. The 15 (7.1%) abnormal external carotid arteries u

PURPOSE: To review the literature on the endovascular management of external carotid artery (ECA) disease.
METHODS: A review was conducted of the English-language medical literature from 1985 to 2011 using PubMed, OVID, and EMBASE databases to find all studies involving endovascular management of ECA stenosis. The search identified 21 reports describing endovascular repair of ECA lesions encompassing 56 patients (35 men; mean age 62.6±10.5 years) and 58 arteries.
RESULTS: The majority of patients (91.1%) were reported to have ipsilateral ICA occlusion, while 55.6% of patients were symptomati

A clinical series of five patients operated on for symptomatic cerebral blood flow deterioration is discussed. They presented with either hemispheral symptoms (hemiparesis, amaurosis fugax) or signs of diffuse cerebral hypoxia. In the presence of unilateral or bilateral internal carotid artery (ICA) occlusion a critical stenosis of the external carotid artery (ECA) was confirmed either by duplex ultrasound or angiography. In all these cases ECA was reconstructed by means of common vascular surgical techniques. The use of intraluminal (i.l.) shunt was mandatory. There were neither early nor lat

From January 1985 to October 1992 ten patients were submitted to reconstruction of the external carotid artery (ECA). Nine were males and one female with age that ranged from 64 to 74 years, mean 68. All were symptomatic due to TIAs in seven and amaurosis fugax in four of this group, previous completed stroke plus TIAs in two and chronic low perfusion in one. Associated risk factors were smoking (8 pts: 80%), coronary disease (5 pts: 50%), hypertension (4 pts: 40%), diabetes (4 pts: 40%) and peripheral arterial obstructive disease (2 pts: 20%). All patients were submitted to non invasive (Dopp

We report a patient with absence of the external carotid artery. The major external branches originate from the internal carotid artery which is really a common arterial trunk. The embryology and clinical significance of this rare vascular anomaly are discussed.

A retrospective study of 910 patients surviving at least five years after cervical irradiation for Hodgkin's disease, non-Hodgkin's lymphoma, or primary head an neck neoplasms showed the incidence of stroke following cervical irradiation was 63 of 910 patients (6.3%) during a mean period of observation of nine years. This represents a trend toward an increased risk for this population observed over the same period of time (p = 0.39). A prospective study of 118 similar patients currently living five years after cervical radiotherapy was performed to determine the incidence of carotid artery dis

The external carotid artery is an important collateral pathway for cerebral perfusion when the internal carotid artery is occluded. After internal carotid artery occlusion, there is a definite risk of ipsilateral neurological events. The authors retrospectively examined their experience with endarterectomy of the external carotid artery for symptomatic internal carotid artery occlusion. Results based on the authors' experience and on historical data show external carotid endarterectomy to be a safe procedure. Obliteration of the cul-de-sac appears to be a very important factor in the preventio

Authors present their own experience with the embolization of the external carotid artery branches. The main indications to embolization included well vascularized and hemorrhagic brain tumors in facial part of the skull, mainly meningiomas, juvenile angiofibromas, and angioneuromyomas. Embolization of external carotid artery was performed in 15 patients. Complete impatience of blood vessel supplying the tumor was achieved in 12 cases, but it was incomplete in 3 cases. Single serious complication in the form of hemiplegia was noted. There were also mild complications in 5 patients, which did n

BACKGROUND: The external carotid artery (ECA) is a major artery of the head and neck. Although numerous studies describe the variability of ECA branches, the course variation of the ECA was seemingly overlooked. It was aimed to study the morphological possibilities of the retromandibular segment of the ECA.
METHOD: A retrospective study was performed on 60 computed tomography angiograms of 26 male and 31 female adult patients. Three types of retromandibular ECA were defined: type 1 - the ECA has a straight course deep to the anatomical plane of the ramus of the mandible; type 2 - the ECA make

This patient had longstanding hypercalcaemia and hyperphosphataemia owing to chronic renal disease, then finally failure, inducing tertiary hyperparathyroidism. He also had long histories of diabetes mellitus type II, hypertension and hypercholesterolaemia. He then reported a painful expansile swelling of the anterior mandible which was diagnosed as a "brown tumour". Subsequent review of the CT data set by an oral and maxillofacial radiologist revealed two patterns of calcification of the carotid arteries. A pipestem pattern was observed bilaterally along almost the entire lengths of the exter

Transient ischemic attacks have been attributed to emboli from the external carotid artery in patients with complete occlusion of the internal carotid artery. We report a patient with an external carotid stenosis and normal internal carotid who developed ipsilateral transient ischemic attacks. Real time B-mode ultrasonography demonstrated that the plaque obstructing the external carotid artery extended into the carotid sinus. This may have served as a source of emboli into the internal carotid circulation.

Two angiographic instances of anomalous external carotid artery (ECA) and internal carotid artery (ICA) anastomosis are described, each occurring at the C2-3 level and bearing remnants of proximal ICA. The ICA remnant of one patient (identifiable immediately upon bifurcation of the common carotid artery) was hypoplastic, and that of the other patient was an occluded arterial stump. These features are not typical of non-bifurcating ICA. The occipital artery originated from an anomalous connection in one instance and from the main trunk of the ECA (just past the ECA-ICA connection) in the other.

The author describes the surgical treatment of 14 patients in whom angiography demonstrated total occlusion of the internal carotid artery (ICA) associated with stenosis at the origin of the ipsilateral external carotid artery (ECA). Each patient underwent ECA endarterectomy followed by an angioplasty to enlarge the ECA outflow tract by incorporating the ICA stump into the proximal ECA. Nine patients subsequently underwent superficial temporal-middle cerebral artery anastomosis. There was no mortality or permanent neurological morbidity. This method of treating ECA stenosis is recommended beca

The perfusion territory of the external carotid artery is often underestimated; however, this blood vessel forms abundant "dangerous anastomoses" with the internal carotid and vertebral arteries. An understanding of these vascular anastomoses is crucial to ensure safe cerebrovascular interventions. There are several important aspects of the middle meningeal artery that should be considered, including anastomoses with the anterior(frontal)branch and ophthalmic artery through the sphenoidal artery(recurrent meningeal artery)and meningolacrimal artery. Additionally, the blood supply to the facial

The ophthalmic artery arises from the internal carotid at the level of the carotid siphon; in 3.5-5% of the population, it arises from the middle meningeal artery, a branch of the internal maxillary artery, terminal of the external carotid artery. We report a rare case of bilateral malformation in a 28-year-old patient. Our study covers the possible dual origin of orbital blood vascularization. This variation in the anatomic distribution is important to know, particularly if embolization is intended as inadvertent occlusion of the ophthalmic artery, because it may lead to permanent loss of vis

The utility of angiography and embolisation of selected branches of the external carotid artery is occasionally helpful in the management of recurrent epistaxis, pre-operative devascularisation of tumours such as angiofibromas, and other head and neck conditions. The use of embolisation for recalcitrant post-tonsillectomy bleeding due to the formation of an aneurysm or pseudoaneurysm of branches of the external carotid artery has been described [P. Simoni, J. Bello, B. Kent, Pseudoaneurysm of the lingual artery secondary to tonsillectomy treated with selective embolization, Int. J. Pediatr. Ot

Surgical treatment of vertebrobasilar insufficiency has long lagged behind that of carotid disease. However, as more aggressive microsurgical techniques have been developed, surgical intervention in vertebrobasilar occlusive disease is seen with increasing frequency. The authors describe three cases of external carotid vertebral artery anastomosis, a rarely reported operative procedure, in which blood flow was successfully restored to the diseased vertebral artery. Awareness of this new technique will be useful to the radiologist servicing an active neurosurgery practice. The clinical, radiolo
