Traumatic internal carotid artery dissection.

Traumatic internal carotid artery dissection is a serious condition that may cause ischemic stroke in young patients. It has been under-diagnosed in the past. We present three cases of traumatic internal carotid artery dissection. The clinical manifestations include hemicrania, hemiparesis, partial Horner's syndrome and cranial nerve palsy. Diagnosis is with carotid color Doppler ultrasound, CT angiography of the neck and conventional angiography. The outcome may be poor with hemiparesis, persistent vegetative state and death. We review the literature and discuss the clinical presentation, dia

[Spontaneous internal carotid artery dissection].

Spontaneous carotid artery dissection is one of the main causes of stroke in young adults. We report two cases occurring after non-traumatic hyperextension of the head. Most of these events result from minor head trauma. Some family history cases would suggest the possibility of an underlying artery wall disorder. The favorable natural history of carotid dissection emphasizes the need for a noninvasive approach for diagnosis. Delayed diagnosis may lead to poor prognosis. Ultrasonography and magnetic resonance angiography are adequate for diagnosis.

Congenital internal carotid artery aneurysm.

Congenital carotid aneurysms are frequently found in the intracranial arteries but rarely seen in extracranial part of the internal carotid artery. A case of a congenital internal carotid artery aneurysm at the skull base was treated in a 13-year-old female child. This case is being presented for its rarity and also to highlight the ease of exposing the upper end of the aneurysm via lateral skull base approach. Infratemporal fossa approach was adopted in this case. The carotid canal was drilled off and the upper end of the aneurysm in the intrapetrous part was mobilized and vascular clip appli

Carotid cave aneurysms of the internal carotid artery.

In a series of 32 surgical cases of carotid-ophthalmic artery aneurysm, seven of the lesions were located in the "carotid cave." This special type of aneurysm is usually small and projects medially on the anteroposterior view of the angiogram. At surgery, it is located intradurally at the dural penetration of the internal carotid artery (ICA) on the ventromedial side, appears to be buried in the dural pouch (carotid cave), and is often difficult to find, dissect, and clip. The aneurysm extends into the cavernous sinus space, and the parent ICA penetrates the dural ring obliquely. An ipsilatera

Fate of External Carotid Artery Following Carotid Artery Stenting for Internal Carotid Artery near Occlusion.

BACKGROUND: The external carotid artery (ECA) plays a vital role in facial perfusion and acts as a collateral pathway for cerebral blood flow during internal carotid artery (ICA) stenosis. In cases of carotid near-occlusion (CNO), characterized by severe ICA stenosis with hemodynamic changes, carotid artery stenting (CAS) is rising as a potential new treatment. During CAS, the stent is deployed in ICA, covering the ECA orifice. Therefore, this study aims to evaluate the effects of CAS on ECA. MATERIALS AND METHODS: This retrospective study included 159 patients diagnosed with CNO and treated

Myxoma in the carotid artery. Myxomatous occlusion of internal carotid artery.

BACKGROUND: We describe a patient with an unusual cause of internal carotid artery occlusion resulting in a stroke. CASE DESCRIPTION: A 41-year-old woman presented with a typical acute right middle cerebral artery territory infarct. Her hematological and cardiological status was assessed, including all extracranial vessels. Carotid angiography and a biopsy were performed of the occluded right internal carotid artery and demonstrated a myxoma. Cardiac investigations to determine the source of the myxoma, including transthoracic and transesophageal echocardiograms, CT and yo-yo CT scans, and MR

Non-bifurcating cervical carotid artery with ipsilateral internal carotid artery hypoplasia.

PURPOSE: A non-bifurcating cervical carotid artery (NBCCA) is a rare anomaly. Hypoplasia of the internal carotid artery (ICA) is also rare. To the best of our knowledge, their simultaneous coexistence has not been previously reported. METHODS: We report a case of NBCCA associated with ipsilateral ICA hypoplasia diagnosed by magnetic resonance angiography (MRA). RESULTS: A 52-year-old woman was transported to our hospital by ambulance with severe headache. Magnetic resonance imaging revealed a large suprasellar mass. MRA showed impaired delineation of the right ICA and a 2-mm left ICA aneurys

Carotid artery stenting in patients with chronic internal carotid artery occlusion.

BACKGROUND: The risk of ischemic stroke in patients with chronic total occlusion (CTO) of the internal carotid artery (ICA) on best medical treatment has been estimated to be 5.5% per year. The purpose of this study was to assess early and mid-term outcome of patients who underwent an attempt at transfemoral carotid artery stenting (CAS) for CTO of the ICA. METHODS: Clinical data of symptomatic patients who underwent attempt at CAS for CTO of the ICA between January 1, 2010 and July 1, 2020 were retrospectively reviewed. Clinical success, perioperative and mid-term stroke and death rates were

Spontaneous thrombosis of a giant intracavernous internal carotid artery aneurysm and ipsilateral internal carotid artery occlusion.

A 75-year-old man who had suffered from right visual disturbance for 10 years suddenly experienced right cavernous sinus syndrome. Magnetic resonance imaging revealed a giant thrombosed aneurysm in the right cavernous sinus extending to the right middle cranial fossa. Digital subtraction angiography disclosed occlusion of the right internal carotid artery at the petrous portion and good cross filling in the right-sided circulation through the anterior communicating artery. There was no filling of the aneurysm. In this case, the mechanism of parent artery occlusion is unclear, but direct compre

External carotid artery to ophthalmic artery flow associated with internal carotid artery stenosis.

PURPOSE: One of the most devastating complications of facial filler injection is sudden ischemic blindness. However, its mechanisms and predisposing factors are poorly understood. The purpose of this study was to investigate the prevalence of external carotid artery (ECA) to ophthalmic artery (OA) anterograde flow in patients with internal carotid artery (ICA) stenosis and in a control population without carotid disease. METHODS: In this cross-sectional cohort study, two groups of patients who underwent catheter cerebral angiography over a 5-year period were identified: patients with symptoma

Duplicated Cervical Internal Carotid Artery with Ipsilateral Persistent Stapedial Artery and Contralateral Aberrant Internal Carotid Artery: A Case Report.

OBJECTIVE: Duplication of extracranial cerebral arteries is a rare anatomical variant, most commonly observed in the vertebral artery. However, duplication of the cervical internal carotid artery (ICA) is extremely rare. We present a unique case of a patient diagnosed with a right facial artery aneurysm, in whom a right duplicated ICA, ipsilateral persistent stapedial artery (PSA), and contralateral aberrant ICA were incidentally identified. CASE PRESENTATION: A 56-year-old woman presented with a painless mass in the right lower jaw. Physical examination revealed a round, pulsatile mass. Cont

A Treated Case of Internal Carotid Artery Stenosis Associated with an Occipital Artery Originating from the Internal Carotid Artery.

OBJECTIVE: The occipital artery (OA) typically originates from the external carotid artery; however, rare anatomical variants exist in which it arises from the internal carotid artery (ICA). Because the OA generally has anastomoses with the deep cervical and vertebral arteries, the OA in the present case, which originated from the ICA, functioned as a collateral pathway in the setting of ICA stenosis due to this unique anatomical variant. Furthermore, it may represent a potential embolic route during carotid artery stenting (CAS). We report the hemodynamic characteristics of and treatment stra

Spontaneous bilateral internal carotid artery dissection.

Spontaneous internal carotid artery dissection is not an uncommon cause of ischaemic stroke in younger patients, but multiple cervical arterial dissections at presentation are uncommon. Recurrence of dissection in a previously normal artery is common. In this case report we review the history, clinical findings and management of a 42-year-old woman who presented with stroke and Horner syndrome and was found to have spontaneous bilateral internal carotid artery dissection. She was not anticoagulated due to concerns relating to the size of her infarct. She was treated with a combination of aspir
